Participation & Liability Waiver: I know that participating in a road race is a potentially
hazardous activity. I should not enter and participate unless I am medically able to do so and
properly trained. I assume all risks associated with participating in this event, including, but not
limited to: falls; contact with other participants; and the effects of weather, traffic, and course
conditions, all such risks, being known and understood by me. I am aware that medical support
for this event will be provided by volunteer personnel who may be called upon to provide
assistance, including first aid, to me during the event. I authorize any such volunteer to assist
me or to perform such assistance as, in the opinion of such person, may be necessary or
appropriate. I understand that Mission Possible, assumes no responsibility or liability with
respect to my participation in this event. I agree, however, to abide by any decision of any race
official relative to my ability to safely complete the event. I hereby grant permission to Mission
Possible, and its sponsors to use any photographs, motion pictures, videos, recordings, or any
record for this event for legitimate purposes. I agree to abide by the rules of this event as
stated in all official event information. Having read this waiver and knowing these facts and in
consideration of your accepting my entry, I, for myself and anyone entitled to act on my behalf,
do hereby release and hold harmless Mission Possible, Inc. and all sponsors and their
respective employees, directors, officers, agents, and volunteers, from all claims or liabilities of
any kind arising out of my participation in this event even though that liability may arise out of
negligence, omission, or carelessness.

Sponsor a Mission Possible student for an entire year of school as you run. The most exciting part of Mission
Possible is our students in our 8 schools in Haiti. Through Mission Possible, each student is able to get the
education they deserve. We want you to be able to take part in their education by raising funds to run. We
will provide you with everything you need to find donors to sponsor you for the 2015 Run4Haiti. It can be 1
donor who donates $360 for you to sponsor a child for an entire year, or 10 different people who donate $36
each. And you don’t have to stop there! Raise as much as you can to sponsor even more students. If you hit
$360 in donations through your sponsors, you get a Heart4Haiti goodie bag, which includes a Run4Haiti pullover

Overall Male & Female winners will
receive a $50 gift card to House of
Awards & Shoes, and second place
male & female winners receive a $25
gift card. Medals are also awarded to
the overall winners and winners of each
age group. There will also be a variety
of door prizes given out after the race is
finished. T-shirts will be given to
participants upon availability
