Assumption of Risk: I acknowledge that I have voluntarily applied to participate in Hot Air Balloon flights and
associated activities to be conducted by Orlaado Balloon Rides LLC. I have been advised and am ful1y aware that a
Hot Air Balloon flight is a potentially hazardous activity with full knowledge of the risks and dangers involved, including
but not iimited to, the loss or damage of personal proper[/, bodily injuries or death due to eyents or accidents including
lhe capsizing of the basket, collisions with obstacles on the ground or in the air, accident or illness in remote places
without medical facilities, immersion in wateq hypothermia, falling from the basket while in the air or on the ground,
fire, flames, temperature extremes, and/or inclement weather. I am voluntarily participating in these activities with the
fulIknowledge and understanding of the risks and dangers involved, and I hereby agree to accept and assume full
responsibility for any and all the risks and dangers, including bodily injury or death, and veri$z this statement by my
signature below and by placing my initials here: jl:

that may prevent me from participating in any Hot Air Balloon activities. By my voluntary participation in the Hot Air
Balloon flight and associated activities, I hereby certify thal l do not have any physical or mental condition that would
prevent me from following instructions from the pilot, entering or exiting the baske! remaining in the basket during
flight or on the ground, withstanding inclement weathe! andlor withstanding a rough impact or rough landing. I hereby
agree to listen and follow the instructions of the pilo* throughout the entire course of my participation in the Hot Air
Balloon flight and associated activities. I further hereby agree to assist in making the flight safer by pointing out to the
pilot any potential hazards such as powerlelectrical lines, other HotAir Balloons, or aircraft in the immediate vicinity.
Passenger Responsibility: I hereby agree to advise the

pafiicipate in the Hot Air Balloon flight and associated activities, I hereby agree that I, rny assignees, heirs, guardians,
and legal representatives will not make any claim against, sus, or attach the property of the Releasee, its agents, pilots,

employees, and/or contractors for any bodily injury, death, loss or damage to personal propefiy, or other damages
resulting fiom the negligence or other acts or omissions of the Releasee , its agents, pilots, employees, andlor contractors,
resulting from my participation in the Hot Air Balloon flight and associated activities. Furthermore, I, my assignees,
heirs, guardians, and legal representatives hereby release, acquit and forever discharge the Releasee, its agents, pilots,
employees, and/or contractors of and &om any and all responsibility and liability for any bodily injury death, loss or
damage to personal property, or other damages resuldng from the negligence or other acts or omissions of the Releasee,
its agents, pilots, employees, and/or contractors, resulting from my participation in the HotAir Balloon flight and
associated activities. Furthermore, I agree to indemnify and hold harmless the Releasee, its agents, pilots, employees,
and/or contractors against any claim for losses, injuries, death, or damages which may be brought against the Releasee,
its agonts, pilots, employees, and/or aontractors for any reason, including any ciaim based on the negligence or other acts
or omissions of the Releasee, its agents, pilots, employees, and/or aontractors arising from the Hot Air Balloon flight
and associated activities.
