The UnitedHealthcare Children’s
Foundation (UHCCF) is a 501(c)(3)
nonprofit charity dedicated to
improving access to medically related
services that are not covered, or
not fully covered, by the available
commercial health benefit plan, and
that have the potential to significantly
enhance either the clinical condition
or the quality of life of the child.
It was a record-breaking year for the UnitedHealthcare
Children’s Foundation (UHCCF) in 2012 – and we are
thrilled and grateful to report that we raised more money
and awarded more medical grants to deserving children
than ever before. By focusing on the values of both
relationships and compassion, we reached new heights:
• UHCCF approved 1,366 medical grants, 9.5% more than
in 2011.
• UHCCF approved $4.1 million in grant dollars, 13.9%

more than in 2011.
• Since 2007, UHCCF has awarded almost 4,900 grants to
children and families in need, valued at nearly $15 million.
UHCCF continues to be committed to enhancing the
quality of life for children who have health care needs not
covered, or not fully covered, by their commercial health
benefit plan. The foundation helps pay for much-needed
medical services such as physical therapy and surgery,
and medical equipment such as wheelchairs and hearing
aids – often keeping families from incurring major debt.

In this Annual Report, we’re excited to share the stories of
four special children who have benefited from UHCCF. You
will hear about Abraham, Katelyn, Kiara and Levi, and how,
despite their medical conditions, they all live very full lives.
You will also hear about some new strategic partnerships
we’ve put in place to help the foundation grow.
We’re incredibly grateful to our generous donors. During
our 2012 annual Giving Campaign, UnitedHealth Group
employees gave $1.6 million, for a total of $3.2 million when
paired with the company match. In addition, some very
generous corporate sponsors are joining us to truly make
a difference. This means that, thanks to you, we’ll be able
to help thousands of children and their families this year.
We also want to thank our national and regional board
members, staff and numerous volunteers, who give
selflessly throughout the year to UHCCF in support of
these precious children and their families.
In 2013, we are poised to award our 5,000th grant. We have
increased the scale and scope of UHCCF, and have built
tremendous momentum to propel us into 2013. We’re just
getting started!

The Power of "I Love You"
Sioux Falls, South Dakota • Katelyn,
diagnosed with autism, receives 30
hours of applied behavior analysis
(ABA) each week. UHCCF provided
a grant to assist with the cost of this
much-needed therapy. Katelyn’s
mom, Crystal, took the time to send
us the following thank you letter:

known as “Hammy” to his family
and friends, is a very friendly,
outgoing 9-year-old boy.
Diagnosed with a brain tumor,
Hammy has undergone surgery
and radiation therapy. A UHCCF
grant helped with the expense of
his surgery and is now assisting
with his necessary treatments.
the school chorus. An inquisitive
child, he is not timid about asking
questions to understand what he
is being taught and loves to try
new things while learning new
concepts. Also a fan of arts and
crafts, Hammy creates his own
original artwork, using bits and
pieces of various materials he finds
around the house.
Hammy’s condition doesn’t slow
him down! He loves to swim, ride
his bike and play basketball,
football and soccer. Being
musically inclined, Hammy plays
the trumpet and cello, and sings in
Hammy’s mom, Gina, said, “I am
grateful that despite Abraham’s
health condition, he is still able to
be a child and not worry about his
health care needs all the time.”
Dear UnitedHealthcare Children’s
Foundation,
We have had a rough four years and this
grant has allowed our little girl to get closer
to our goal of helping her communicate
better and to build her confidence in
everyday activities.
Currently Katelyn has ABA therapy 30 hours
a week. This grant is helping to pay outof-pocket expenses. I think back to when
Katelyn started her therapy and it’s like she
is a totally different girl. She knows so many
things and is really growing a personality
of her own. This year she was a fairy for
Halloween and she was able to say “trick or
treat” without prompting and “thank you”
independently.
She is absolutely beautiful to me. I know in
my heart someday she will do something
absolutely amazing and you all will be a
part of this.
Every day we wait to hear any words
that come out of our little angel’s mouth
and Katelyn has learned to smile and say
“I love you.”
Thank you so much for the large gift you
provided us. With you, dreams do come true.

Healthy, Happy – and Special
Mt. Juliet, Tennessee • Levi, originally from
China, was born with a unilateral cleft lip and
cleft palate that made eating and speaking
difficult. Back in 2010, when Levi’s parents
were looking to adopt a child from China,
they were asked by the adoption agency if
they would be willing to adopt a child with
special needs. While they had room in their
hearts to adopt a child with medical needs,
they weren’t sure they had the resources.
But they did adopt Levi – and thanks to a
grant from UHCCF that paid for non-covered
expenses related to his successful surgery,
things have changed considerably for Levi
and his family.
A Shining Star
Mobile, Alabama • Kiara, a 12-yearold girl who likes to dance and
listen to music, was diagnosed
with both asthma and attention
deficit disorder (ADD). To control
Kiara’s asthma symptom flare-ups,
she’s been prescribed several
medications and treatments that
have allowed her lung specialist to
remove one of her asthma pumps.
Kiara also needs therapies to help
her ADD symptoms so that she
can better focus and improve her

school grades. Luckily, UHCCF
provided two grants to assist with
the expenses for her medications
and therapies.
“UHCCF was a huge partner in the recovery
and surgery, and we were able to focus on
getting care because the financial hurdles
were being met,” said Levi’s mom. Today,
Levi is a healthy, happy boy who completes
their family and is doing incredibly well.
Kiara is a little bit shy when you
first meet her, but it doesn’t take
long for her cheerful personality to
shine through. Now, in addition to
her dancing, Kiara is enjoying her
other hobbies – drawing flowers,
skating and playing in the park
with her friends.

Central Region
UnitedHealthcare Children’s Foundation has a
very special partner in the Central region, the
Dallas Cowboys. The team brought the holidays
to life for a few special children in the Dallas and
Fort Worth areas. During December, the Dallas
Cowboys football players and cheerleaders visited
the Texas Scottish Rite Hospital for Children,
Parkland Hospital Burn Unit, Cook Children’s
Medical Center, Children’s Medical Center Dallas
and Medical City Children’s Hospital.
More than 800 children at the hospitals received
a “game day sock” loaded with goodies from the
Cowboys and UHCCF! We were proud to be a
part of this event, along with another hospital visit
during 2012, which brought cheer and created
special memories for a few special children.
Northeast Region
On March 19, more than 175
UnitedHealthcare business partners
helped raise $55,000 during the 2012
UHCCF New Jersey Wine Tasting
Event at Liberty House Restaurant in
Jersey City, New Jersey.
The event was hosted by the New Jersey
health plan, with generous sponsorship
support from area corporations, health
systems and other businesses.

The special guest of honor was 6-year-old
grant recipient Chelsea. Diagnosed with
dysarthria, or slurred speech, the grant
helped with the expense of speech therapy.
“Parents want to provide their children
with the support they need to do well in
school and be happy, healthy children,”
said Chelsea’s mother, Lavern. “The grant
helped Chelsea get the care she needed
to become an even sunnier, more
outgoing person.”
West Region
Southeast Region
Arizona’s health plan hosted its third
Wings of Hope Golf Classic in support
of UHCCF in September. The Classic
included lunch, an auction and dinner at
the beautiful FireRock Country Club in
Fountain Hills, Arizona.
The Southeast region had a busy year raising
funds for UHCCF with golf tournaments all
over the Gulf States.
The special guest of honor was 7-yearold UHCCF grant recipient Emma.
Diagnosed with a spinal cord tumor, the
two grants she has received from the
foundation are helping cover the costs
associated with her chemotherapy, MRIs and
rehabilitative therapies. Emma’s mom gave
a heartwarming speech, sharing the impact
the grants have had not only on Emma but
the family as well.
This premier golf tournament included
76 golfers comprised of employees,
customers, brokers and provider partners
representing UnitedHealthcare of Arizona
and Utah. The day’s events were an
outstanding achievement, raising $113,000
for UHCCF. These funds will go toward
providing grants for children and families
in Arizona and Utah.
On September 10, golfers from greater New
Orleans and across the Gulf Coast gathered
under blue skies at the beautiful English
Turn Golf & Country Club for a day of golfing
and fundraising. The annual golf classic
raised nearly $30,000 for UHCCF.
On October 1, Gulf States golfers gathered at
the Dancing Rabbit Golf Club at Pearl River
Resort in Philadelphia, Mississippi, for the
2012 annual golf classic, where more than
$31,000 was raised for the foundation.
On November 5, the beautiful Greystone
Golf & Country Club in Birmingham,
Alabama, was the site for an annual
golf classic, with golfers coming from
the Birmingham area and all around
Georgia. More than $40,000 was raised
by generous supporters to help provide
grants for children.
The title sponsor, Cardiovascular
Consultants, along with all of the golfers and
volunteers, helped make this a phenomenal
event. Additional appreciation goes to
UHCCF’s regional board member Bob Hart
for securing the spectacular private course
and donation of auction items.

Wings of Hope Event
The foundation’s 2012 flagship Wings of Hope
fundraiser expanded its reach beyond the Golf
Classic at the prestigious Hazeltine National Golf
Course to include a Tuscan-themed extravaganza
at the Minnesota Landscape Arboretum. The two
events took place concurrently on August 13, and
participants from both events united for a social
hour, dinner and live auction resulting in more than
$500,000 being raised for UHCCF.
Four-year-old UHCCF grant recipient Isabella
and her family were guests of honor at the
Arboretum festivities, and the special guest for
the evening was 15-year-old grant recipient
Marissa, who has received two wheelchairs from
UHCCF. Marissa came up on stage to tell her story
and help the auctioneer raise funds during the
Direct Giving Moment. The event drew numerous
UnitedHealth Group business partners and senior
leaders, as well as UHCCF board members. The
foundation thanks the more than 300 participants
who took part in both events – vendors, partners,
employees and volunteers. It just gets better
every year!

UnitedHealth Group Giving Campaign
The UnitedHealth Group employee Giving
Campaign accounts for nearly two-thirds
of UHCCF grant funds. It is a vital part of
maintaining and growing the foundation –
and this year’s campaign was outstanding.
More than 10,000 UnitedHealth Group
employees pledged nearly $3.2 million to
UHCCF, which includes the UnitedHealth
Group match. Employees who gave to
UHCCF at certain thresholds received
wonderful promotional gifts, which included
a 2013 UHCCF calendar, teddy bear, jacket
or crystal butterfly. We are incredibly
grateful for the ongoing generosity of
UnitedHealth Group and its employees.
Tickets for Charity
In August 2012, UHCCF partnered
with Tickets for Charity, Charitable
Partners, Inc., using tickets from generous
UnitedHealth Group ticket donors to raise
funds. By purchasing sports or entertainment
tickets through the UHCCF microsite,
www.ticketsforcharity.com/uhccf, customers
are able to allocate the charitable donated
amount from their ticket price to UHCCF.
From August to December 2012, UHCCF
raised more than $41,000 and hosted 11
events, including exclusive seating to games
for teams such as the Dallas Cowboys and
New England Patriots and entertainers such
as Bruce Springsteen. We are grateful to our
friends at Tickets for Charity, our partners
at UnitedHealth Group and the many others
who financially or logistically supported the
2012 Tickets for Charity fundraisers.

In 2012, UHCCF was seen, heard and read about all across the nation. Television,
radio, newspaper, magazine and online stories featured UHCCF and the
availability of medical grants for eligible children. UnitedHealthcare executives
and grant recipients were featured in interviews and 300 news stories appeared
throughout the country. Additionally, a UHCCF public service announcement
(PSA) aired more than 164,000 times on radio stations and 9,400 times on
television stations from coast to coast. These stories helped spread the word
about UHCCF, including the availability of grants as well as our fundraising
efforts. Eligible families were directed to apply online at www.uhccf.org.
UnitedHealthcare Pro Cycling
Team Rides for UHCCF!
In 2012, UnitedHealthcare continued as the
title sponsor of the UnitedHealthcare Pro
Cycling Team. In addition to supporting
UnitedHealthcare’s mission of helping people
live healthier lives™ and increasing employee
engagement, this internationally ranked team
promoted and raised funds for UHCCF throughout
the year at numerous races and events. The team
also contributed a portion of its race winnings
and a percentage of the proceeds from apparel
purchased from its website to the foundation.
The Pro Cycling team was among the most
successful in North America in 2012, winning
19 races and finishing in the top three 51 times,
in addition to racing internationally as a Pro
Continental team. Even with the demands of
professional cycling, the team continues to
be UnitedHealthcare’s health and wellness
ambassadors to local communities, including
schools, children’s hospitals and YMCAs
across the nation. The team made more than
60 appearances for UHCCF in 2012, personally
reaching more than 18,000 employees, community
members and business partners. Over the course
of the 2013 racing season, the team will once again
ride to raise funds and awareness of UHCCF.

Ride With the Leaders
This year, we found yet another
way that cycling can benefit
UHCCF. The “Ride With the
Leaders” event took place
August 12 – the day before the
Wings of Hope Golf Classic.
The event consisted of eight
riders who cycled 100 miles
and raised $50,000 to support
the foundation.
