ear Community Members,
You’ve undoubtedly heard me say how
excited and blessed I am to work shoulder-toshoulder with our employees and medical staff, as
we try our very best to improve our patients’ and
families’ quality of life, one patient at a time.
My staff and I feel extremely confident
about our ability to serve you, as is reflected by
our latest Joint Commission quality rating (see
article below), now hovering above 95 percent
out of 100 percent. This means we have met
more than 95 percent of nearly 600 best
practice requirements for the last 12 years,
according to the most credible healthcare
auditor in our business. This achievement
happened solely because everyone who works
and practices here wants every member of our
Northwest Connecticut community to have no
doubt in our ability to care for you in
Torrington, better than you might be cared for
elsewhere, for those services which we offer.
Very recently, I was faced with a decision
about where I should undergo major surgery,
something that was unexpected and very
important to deal with,
for the sake of my
family and my quality
of life, and the lives of
others who to some
measure depend on
me. In making this
decision, I found myself
feeling incredibly
positive about placing

The Accountability Composite Rate is the degree to
which a hospital performs in several clinical areas
including surgery, heart failure, outpatient care, heart
attack care, and pneumonia. One hundred percent
represents maximum performance. This rate is a reflection
of Charlotte Hungerford’s performance on 53 individual
measures and over 10,000 data points.
The rate is compiled as part of the Joint Commission’s
ORYX National Hospital Quality Measures Initiative and
integrates outcomes and other performance data into
the hospital accreditation process to support them in
their quality improvement efforts.

I know this sounds a bit “over-the-top,” but
I realized at that decisive moment, that my
staff had accomplished such terrific things
over time that I was actually looking forward
to having my own hospital family get me
through this major surgical event.
As you might guess, I was not able to
play “undercover boss,” chiefly because our
employees and medical staff have over time
developed personal and professional
interrelationships that now define us as a
family of heartful and knowledgeable
caregivers. As it turned out, my surgery and
recovery went extremely well!
Looking back at my inpatient experience,
I can only feel a debt of gratitude to our
employees, physicians and board members
who together have created a truly enviable
environment in which patients can be accurately diagnosed and can begin their healing
process, in a place filled with love and talent.
I have been leading hospital organizations
for over 33 years. During that time, my connection with the CHH family has been the
most unique and outstanding experience,
wherein a group of dedicated people have
made an unwavering commitment to work
successfully with one another in “bringing
hope and comfort to our ill and injured”!
I salute and thank our employees, medical
staff, board, volunteers, auxilians and corporators
for setting the “patient safety” bar high and for
being willing always to work together for this
most noble cause.

CHH AWARDED 3-YEAR ACCREDITATION FROM THE JOINT COMMISSION
For the 12th consecutive year, Charlotte
Hungerford Hospital (CHH) has once again
earned The Joint Commission’s Gold Seal of
Approval for accreditation by demonstrating
compliance with the Commission’s national
standards and best practices for healthcare
quality and safety in hospitals.
The accreditation award recognizes CHH’s
dedication to continuous compliance with 600
Joint Commission’s patient safety standards,
including infection prevention and control,
hospital and medical staff leadership, and
medication safety management. CHH underwent a rigorous unannounced on-site survey
in August 2013 when a team of Joint
Commission expert surveyors evaluated CHH
for compliance with standards of care specific
to the needs of patients.

The Joint Commission’s hospital standards
address important functions relating to the care
of patients and the management of hospitals.
The standards are developed in consultation with
healthcare experts, providers, measurement
experts and patients.

CHH IS CREATING HEALTHY HABITS IN THE NORTHWEST CORNER
The Community Outreach Team at Charlotte
Hungerford Hospital is dedicated to ensuring that
people young and old residing in northwest
Connecticut make good nutrition and hand hygiene
a healthy habit.
In 2013, CHH created and delivered a series of
presentations and outreach activities at local fairs,
school events, and day care centers designed to
educate and entertain the public and promote
preventative health and active lifestyles.
Addressing the issue of childhood obesity is one
of the top priorities identified by the staff and
caregivers at Charlotte through its community
assessment surveys. Using educational materials,
games and activities from myplate.gov and other
resources on good nutrition, the team promotes its
message by challenging and quizzing children and
adults on what foods are healthy and what choices
should be made when they are dining out or
preparing food at home.
Good hand hygiene is another very important
lesson taught by the team. Children have a chance to
discuss what germs are and how they affect a person’s
health. Pretend germs are viewed under a special
light to show children how they hide and can be

removed using hand sanitizer.
The team has been very pleased with the
response, especially from the youngest participants. Team members agree that the area’s local
schools and day care centers have done a great
job educating kids about the importance of hand
washing. As flu season approaches, the team’s goal
is to increase awareness of how this simple procedure a few times a day can keep flu away from
friends and family.
CHH and over 20 local community
organizations, local non-profits, area agencies and
schools formed a group called “Fit Together” and
adopted a mission “to build the healthiest kids,
families and communities through sustainable
strategies that foster healthy eating and active
living.” The hospital’s recent outreach and public
events have incorporated this mission, and reinforced a united effort among the region’s leaders
and caregivers to build healthier communities.
“An essential component of a successful
community-based system of services is the promotion of health through good nutrition and
physical activity,” said Tim LeBouthillier,
Director of CHH Community Relations and
Development. “We want to do all we can to
make sure our community members start off on
the right foot when it comes to health, nutrition
and exercise.”

CHH CREATES PINWHEELS FOR PREVENTION ART GALLERY
Child Abuse Investigation Team of Northwest Connecticut (CAIT NW) recognized Child
Abuse Prevention Month with the creation of the first annual Pinwheels for Prevention Art Gallery
at the Northwestern Connecticut Community College.
“We provide the community with information on how we can work together to keep kids safe while
highlighting the spirit of our children with their artwork,” said Bethany Ferreira, CAIT NW Coordinator.
CHH FORMS NEW OSTOMY SUPPORT GROUP
CHH founded a new Ostomy Support Group, the first group of its kind in northwest
Connecticut, to provide unify and strengthen its members for the benefit of people who have, or
will have intestinal or urinary diversions and their caregivers.
“I am very excited to offer this important resource to the community. The support group will allow
individuals with an ostomy or continent diversion, their families and caregivers the opportunity to share their
experiences and advice on related health and wellness issues,” said Suzanne Fox, RN, WOCN, the hospital’s

CHH OFFERS FREE SMOKING CESSATION CLASSES
CHH offered free admission to its Smoking Cessation Programs to assist residents
looking to kick the habit learn how to stop smoking and start building a healthier lifestyle
through education, relaxation techniques, and methods of preventing weight gain.
“The program features sessions led by a trained, certified clinic leader who understands the motivations
and rationalizations of smoking, and uses a positive behavior change approach that teaches people how to
become a permanent non-smoker,” said Sandy Markus, Program Facilitator.
CHH OFFERS NEW LUNG CANCER SCREENING SERVICE
CHH Early Detection Program’s offered a new Lung Cancer Screening Service utilizing a fast,
painless, low cost, low dose CT scan to assist current or former heavy smokers 55 or older that is
reviewed by a Lung Cancer Navigation Team of radiologists, pulmonologists, oncologists and a
nurse navigator.
“You can’t put a price tag on the peace of mind that can come with a lung cancer screening. This test
gives you access to a comprehensive program with the treatment and resources you may need to help you lead a
healthier life,” said Program Director Leslie Handelman, R.N.
CHH DEBUTS CHILD FIRST FAMILY CARE MODEL
Charlotte Hungerford Hospital’s Center for Youth and Families introduced “Child First,” an
innovative, home-based system of care designed to decrease serious emotional disturbance and
abuse and neglect among the region’s most vulnerable young children.
“Two teams, each consisting of a developmental clinician and a care coordinator, work with pediatricians, teachers and other community providers to identify, assess and intervene where necessary to make this
program possible. It’s a team effort,” said Joan Neveski, Director of the Center For Youth & Families.
CHH BREAST HEALTH CENTER RECEIVES FIRST KOMEN GRANTEE AWARD
Recognizing the efforts of the organization’s top sponsors, fundraisers and volunteers from
communities across Connecticut, Komen for the Cure CT choose the CHH Pink Rose Program as
its “Grantee of the Year” for 2013.
“It is the first time Komen for the Cure, CT, has officially recognized an outstanding program, and it is
an honor to be the first recipient of the award. We are very proud of all the work our program has done in
the community to educate women about the importance of breast cancer early detection,” said Project Director
Leslie Handelman, R.N.
CHH CREATES HEALTHCARE COALITION OF MEDICAL PROVIDERS
CHH formed the Greater Charlotte Hungerford Hospital Healthcare Coalition, a group that
brings together the hospital and a range of Northwest Connecticut healthcare organizations and
partners to develop and maintain critical emergency preparedness plans and strategies.
“The hospital and area healthcare stakeholders and agencies have been working together to improve
communications and planning during disasters for several years. This new partnership formalizes this
relationship and commitment ensuring we are all ready when needed most,” said Paul Rabeuf, CHH EMS
and Emergency Management Coordinator.
CHH PARTNERS WITH LOCAL AGENCIES TO OFFER CPR COURSES
Charlotte Hungerford Hospital along with Campion Ambulance Service, the Torrington Fire
Department, Valerie Manor Health Care Center and the Elks Lodge conducted free CPR classes
on Torrington’s Community Heart Saver CPR Day providing instruction on CPR and the use of
the Automatic External Defibrillator (AED).
“Less than one-third of sudden cardiac arrest victims receive immediate CPR from bystanders. This
course equips all people and communities to act in an emergency and ultimately save lives through early medical interventions,” said Tim LeBouthillier, Director of CHH Community Relations.
CHH CARDIOLOGY SERVICES RECEIVES ECHOCARDIOGRAPHY ACCREDITATION
CHH Cardiology Services and CHH Cardiovascular Medicine were granted a three-year term
of accreditation in Echocardiography in the area of Adult Transthoracic and Adult Transesophageal
by the Intersocietal Accreditation Commission (IAC).
“Early detection of life threatening heart disorders and other diseases is possible through the use of
Echocardiography procedures performed within hospitals,” said Patty Sullivan-Poletta, Director, Cardio
Pulmonary Services.
CHH OPENS NEW REHABILITATION & SPORTS MEDICINE CENTER
CHH opened a second location for its Outpatient Rehabilitation and Sports Medicine
Services at 1151 East Main Street in Torrington that offers the same physical, occupational and
speech therapy services as the hospital’s current location at 540 Litchfield Street.
“We are pleased to offer an additional new location and specialists we feel will provide greater accessibility
and flexibility to our patients in the northwest corner,” said Amanda S. Hill, OTR/L, MA, Director of
Rehabilitation Services.

became the first orthopedic surgeon in
Connecticut to use the HipSextant™
Navigation System during two hip
replacement surgeries.
“Patients want the results of their surgery
to last as long as possible and understand that
surgical precision is the key to long-term success.
This device helps us achieve this,” said Dr.
Freccero, who plans to use the navigation
system in the future hip surgeries.
CHH WOUND CARE CENTER CREATES NEW PHYSICIAN AFFILIATIONS
Charlotte Hungerford Hospital Wound Care & Hyperbaric Medicine added Podiatrists
Dennis D’Onofrio, D.P.M., and Darren Winkler, D.P.M. to its physician panel. The center
received recognition as a “Center for Excellence” achieving outstanding patient outcomes for the
last 12 consecutive months.
“Our patient relationships and programs set us apart from other wound care centers because our philosophy
is to consider the larger picture rather than just the patient’s wound itself,” said Michael Wildman, Program
Director, CHH Wound Care Center.
CHH CANCER CENTER UPGRADES RADIATION TREATMENT TECHNOLOGY
The CHH Radiation Oncology Department introduced upgraded Elekta VMAT technology as
their newest treatment option for cancer patients that works to significantly offers faster, more precise
cancer treatments by reducing the time required for radiotherapy sessions to five minutes or less.
“It provides less treatment time and more quality time for patients,” said Elizabeth Whalen, M.D., CHH
Radiation Oncologist. “We are committed to offering our patients a wider range of treatment options close to home.”
CHH OPENING NEW THOMASTON PRIMARY CARE PRACTICE
CHH and Roberta Meltzer, M.D., opened Thomaston-based “CHH
Primary Care” offering comprehensive primary care services committed to
personal and compassionate care through the diagnosis and treatment of
both acute and long-term health conditions with a focus on wellness and
lifestyle medicine.
“I’m so pleased to be able to serve the people of Thomaston and the surrounding
area. Our goal is to provide a welcoming presence to help our patients not only

2013 proved to be a very productive year for the
Charlotte Hungerford Hospital Auxiliary. Twenty-three
board members and many more sponsors gave their
support and time by raising money through the operation
of the CHH Gift Shop, membership dues, special vendor
sales, the annual fashion show, the Artist of the Month
Program and a holiday season greeting promotion.
The Auxiliary gave its fourth installment of its
$50,000 pledge to the hospital’s information technology
initiative. New chairs were purchased for the hospital
cafeteria and an infant warmer with a resuscitation system
for Maternity Services was also delivered for immediate
use. In addition, the Auxiliary awarded 12 $1,000
scholarships to students entering the medical field. One
of the scholarships honored Dr. Alfred Fabro and another
was given in memory of Dr. Joseph Curi.
The 29th annual fashion show was a huge success
with 180 women in attendance. Fashions were provided
by Dress Barn of Torrington. We are grateful to many
of our local and regional businesses that donate so
generously to our fashion show and raffle.

The CHH Gift Shop continues to serve the public and
hospital employees with a wide range of unique items
including jewelry, toys, clothing and accessories. Please stop

right gift.

Our Volunteers And Chaplains
The Charlotte Hungerford Hospital Volunteer Program
provides essential, supplementary and complementary services to
patients, families, visitors and staff. The program has over 160
volunteers in more than 30 non-clinical and clinical areas,
including at the Information Desk, Gift Shop, Financial Services
and as Patient Representatives. Our volunteers range in age from
16 to 94 and log over 18,000 hours every year.
We welcome volunteers from all backgrounds, as each
volunteer can make unique contributions to the hospital. The
Volunteer Program at Charlotte Hungerford Hospital has
approximately 130 traditional volunteers and over 30 junior
volunteers that make a profound and positive difference in the
hospital’s daily operations.
We are grateful to our many dedicated volunteers and
chaplains who work with us throughout the year. If you would
