THE MODERN ROLE of BETABLOCKERS IN
CARDIOVASCULAR MEDICINE
EVIDENCE-BASED
CARDIOLOGY PRACTICE
John Malcolm Cruickshank
Hu, Nguyen, Kim, Kwan, Saito, Grines
This pocket manual is a valuable tool for medical students as well as
cardiovascular practitioners and internists worldwide, and provides a
concise and much-needed update of beta-blocker use in the
cardiovascular area (particularly hypertension and heart failure).
Medical decisions are made on the basis of critical thinking, and
clinical judgments are based on the relevancy of the collected data.
Medical decisions and clinical judgments are the results of the long
thought process generated from formal learning, data from guidelines
and practice standards, experience, an understanding of the incidence
of disease in the population that the patient represents, and the
likelihood of a disease being present in a specific patient.
Beta-blockers (BBs) are a large and important family of therapeutic
agents with primary use is in cardiovascular medicine. The
therapeutic areas that BB covers include: Ischemic Heart Disease,
Hypertension, Heart Failure and Arrhythmias.
This book introduces and encourages the concept of evidence-based
patient care. This work presents the ideology and methodology of
critical thinking and also the available evidence, management and cost
effectives across the field. Evidence-Based Cardiology is aimed at
any clinician who encounters a cardiac patient from the Clerk to the
Internist to the Practicing Physician, encouraging disease management
decisions to be based on the highest level of evidence.
It also provides a current update of the vital role of beta-blockers in
the treatment of heart failure, and why some beta-blockers are
“ineffective”.
The book also uncovers the clinical inaccuracies of how the UK NICE
(National Institute for Clinical Excellence) Committee have “got it
wrong” in their recent pronouncements on BBs and hypertension.

HEART’S VORTEX:
INTRACARDIAC BLOOD FLOW
PHENOMENON
PHYSICAL EXAMINATION OF
THE HEART & CIRCULATION,
4th edition
Ares Pasipoularides
Joseph K. Perloff
This outstanding resource provides a
comprehensive guide to intracardiac blood
flow phenomena and cardiac hemodynamics, including the
developmental history, theoretical frameworks, computational fluid
dynamics, and practical applications for clinical cardiology, cardiac
imaging and embryology. It is not a mere compilation of the most upto-date scientific data and relevant concepts. Rather, it is an integrated
educational means to developing pluridisciplinary background,
knowledge, and understanding. Such understanding allows an
appreciation of the crucial, albeit heretofore generally unappreciated,
importance of intracardiac blood flow phenomena in a host of
multifaceted functional and morphogenetic cardiac adaptations.
This outstanding resource, now in its 4th
edition, emphasizes physical examination,
rather than physical diagnosis, of the heart
and circulation. The cardiovascular exam for
patients from birth to senescence is presented, including physical
appearance, the arterial pulse, the jugular venous pulse, the peripheral
veins, percussion, palpitation, and observation of the precordium,
cardiac auscultation, the thorax, lungs, and abdomen.
With medical costs in the spotlight, interest in this relatively
inexpensive, safe, and often entirely sufficient clinical examination
should enjoy a resurgence. This guide demonstrates that specialized
laboratory tests should be used to supplement, not supplant, a careful
clinical examination.

INGLE’s ENDODONTICS, 6th edition
Ingle
Tooth replacement should never be preferred over tooth rescue: if a tooth can be saved successfully, that approach should
always be the first intervention. Even implants, which represent a great advance, are mechanical replacements that should
not replace teeth with healthy roots. Unfortunately, “I’d rather have a root canal!” has become a rallying cry for those
faced with what they anticipate will be an unpleasant experience. In fact, the root canal is less costly, less painful, less
time-consuming, and more integral to the patient’s biological makeup than the alternative treatments.
The 6th edition of Endodontics is replete with new innovations and knowledge. Now, more than ever, it deserves its
reputation as the “Bible of Endodontics,” a name long applied to previous editions. It is an essential part of any
endodontist’s library, whether still in training or currently engaged in practice.

EVIDENCE-BASED DERMATOLOGY,
2 edition
Maibach, Garouhi
Evidence-based thinking in clinical
medicine has impacted greatly on
the physician's approach to clinical
care. In addition to promoting the
understanding of the evidencebased philosophy, the authors have
focused on some of the
fundamentals in dermatology that
need to be approached differently.
Issues such as the interpretation of
clinical research, disease-oriented
evidence
versus
patient-care
evidence, and the use of placebo
are
examined.
This
book
introduces and encourages the
concept of evidence-based patient care in dermatology. This is a
growing area in dermatology, and this work presents the ideology and
methodology of critical thinking and also the available evidence
across the field.

DISEASE
Rietschel, Fowler
Hall, Hall
The skin has much to tell us about where to look for illness, how grave
that illness may be, and what strategies for therapy might be most
advantageous.
The skin is also the organ most easily accessible for obtaining tissue for
pathological examination, culture, and examination of debris. These tools
will also be discussed and expanded upon. This book classifies the skin
conditions from the standpoint of organ involved as well as specific
disease and makes use of many illustrations and photographs.
Since its introduction in 1967, Alexander Fisher's Contact
Dermatitis has been the one essential text for dermatologists who
see patients with contact dermatitis. The sixth edition is written for
the third time by Robert L. Rietschel and Joseph F. Fowler.
An encyclopedic reference that discusses all aspects of contact
dermatitis, the updated edition will feature a full color atlas section
as well as color images for the first time.

EMS: A PRACTICAL GLOBAL
GUIDEBOOK
Kovacs, Law Tintinalli, Holliman, Cameron, Holliman, Cameron
EMS: A Practical Global Guidebook is the
first international resource for EMS
services edited by an international team of
experts. The World Health Organization
recognizes that each year more than 100
million people worldwide sustain injuries,
more than five million people die from
violence and injury, and that 90% of the
global burden of violence and injury
mortality occurs in low- and middleincome countries.
AIRWAY MANAGEMENT IN
EMERGENCIES, 2nd edition
Written by international experts in a
style that's concise, practical and to
the point, Airway Management in
Emergencies covers all the options - both medical and surgical -- for
managing any patient's airway in an
emergency. Here, you'll find the
core knowledge and accompanying
management protocols necessary to
assess, oxygenate, intubate, and
monitor
patients
requiring
emergency airway management.
The WHO also recognizes that improved
organization and planning for provision of trauma and emergency care
is an essential part of integrated health-care delivery that it plays an
important role in preparedness for, and response to, mass-casualty
incidents. EMS organization and planning lowers mortality, reduces
disability, and prevents other adverse health outcomes arising from the
burden of everyday injuries.
In each chapter, this high-yield coverage is supported by
evidence-based algorithms, synoptic tips, and real-world case
studies that show you how to resolve any difficult airway scenario
you would likely encounter in clinical practice.
In response to a growing need for coordination and communication
across nations and cultures, this book is designed to help the Global
EMS community come together to establish and develop coordinated
components of global and national EMS systems.

INSIDE PUBLIC
PSYCHIATRY
THE HOSPITALIST
MANUAL
Selby C. Jacobs
Mehta, Mathews
Given the current implementation of mental health parity in
insurance benefits and the enactment of healthcare reform, this book
is essential reading for anyone wanting to understand both the
historical context of public psychiatry and what the future might
hold.
This book is evidence-based and takes into account the modern
managed care model in which the current hospitalist practices. The
Hospitalist Physician is a relatively new concept within modern
healthcare. As this new field evolves, so too will the educational
needs of internists who have chosen to practice this discipline.
Most people with serious mental illness are seen in the public sector
of psychiatry, and the significance of psychiatric treatment in the
public sector is best understood one person at a time.
This book will be used as an “in-the-field” guide for new hospitalists,
focusing on two groups of readers: the new graduate from residency
and the office-based internist that wishes to retrain and certify as a
hospitalist physician. The Hospitalist will serve as a pocket guide for
those intending to obtain rapid information on practice management
principles unique to a hospitalist physician.
This book tells the story of public psychiatry with examples from the
author’s experience running Connecticut Mental Health Center
(CMHC), one of the country’s premier public mental health centers.

spectrum of renal cell carcinoma from screening, epidemiology, and pathology
to molecular genetics, surgical management, and therapy of metastatic disease.
Front and center is the biology of renal cell carcinoma, where researchers have
made great progress in expanding our understanding of mechanisms and
processes making therapeutic advances possible. Built around the practical
management of the continuum of kidney cancer, different treatment approaches
are discussed within the context of each clinical stage of the disease. In
addition, special populations such as patients requiring palliative care, those
with brain or bone metastasis, and those with inherited renal cell carcinoma are
discussed.
Recent therapeutic advances in cancer treatment indicate that
cancer is becoming a chronic disease rather than a killer. This
comprehensive text is the first to define and address the broad
spectrum of acute and chronic internal medicine disorders that
occur in cancer patients and cancer survivors as side-effects of the
disease itself, or of the treatment regimens. The authors cover
nononcologic aspects of internal medicine such as anorexia,
obesity, bone loss, diabetes, depression, pain, fatigue, congestive
heart failure, skin disorders, and pneumonia. This book is
conceived as a companion to standard internal medicine and
oncology texts--a comprehensive reference resource for internists
caring for cancer patients and oncologists in practice. The text is
extensively indexed for easy access and retrieval of information.

historical overview of those who
pioneered
the
early
surgical
interventions from 1950 to the present
and the influences on current clinical practice.
The third edition of Harold F.
Schuknecht’s Pathology of the
Ear has been reorganized and
consists of four sections,
including:
(I.)
“Anatomy,
physiology,and pathophysiology
of the ear;” (II.) “Disease
processes of the ear;” and (IV.)
“Histopathology following surgical treatment.” An entirely new
section (III) has been developed, entitled “Patterns of degeneration
of the ear with multiple etiologies” which hopefully will serve as a
helpful guide to the reader to recognize that multiple etiologies
may result in a similar histopathologic phenotype.

PDQ PUBLIC HEALTH
PDQ EPIDEMIOLOGY, 3rd edition
Streiner, MacPherson, Gushulak
Streiner, Norman
Clearly -- and often humorously -- written, PDQ
Public Health defines public health and covers the
basic concepts of public health policy, including its
history, local to international structure, and role in
protecting human health.
PDQ Epidemiology will help to fill the considerable gap
between the exact findings of the laboratory and the
uncertain world of clinical medicine. By translating the
terminology of epidemiology into easy-to-understand
language, the underlying scientific methods begin to
emerge and make sense. The third edition reflects the
maturation of the field, which now encompasses much more than the
“randomized, controlled trial.” New sections explain techniques that have
been introduced into the field since the previous edition, and updated
examples have been incorporated.
Concise, yet comprehensive, the text educates the
reader in the history and evolution of the concepts and practices of public
health on local, national, and international scales. Key concepts, such as
communicable diseases, vectors, hosts, and environments, are defined;
and how they and other factors interact to influence public health issues
is described. The statistical tools that are used to determine risk and
describe the interactions contributing to community health are presented.
The authors also address the impact of population mobility, economic
factors, law and ethics on the practice of public health.

of the interstitial lung diseases. The third section titled Clinical
Entities describes the clinical manifestations, radiologic patterns,
histopathological features, and management of the specific process.
Pulmonary Hypertension raises questions and discusses concepts which
will address the issue of etiology of pulmonary hypertension, its
pathobiology with its close proximity to cancer, and the consequences
of severe PH which is right heart failure and death.

MOLECULAR IMAGING:
PRINCIPLES & PRACTICE
Mundt, Roeske
Weissleder, Ross, Rehemtulla, Gambhir
Over the last decade, the field of molecular
imaging of living subjects has evolved
considerably and has seen spectacular
advances in chemistry, engineering, and
biomedical applications. In a relatively
short period of time, comprehensive
molecular imaging centers have been
established in the US, Europe, and Asia
and are increasingly integrated into basic
sciences and translational networks. New
investigators, collaborators, and students
drawn into this multidisciplinary field have
often expressed the desire and need for an authoritative textbook. This
textbook was designed precisely to fill this need.
IGRT covers nearly every aspect of
radiation treatment planning and
delivery in radiation therapy and
explains both the “why” and the
“how,” the art and the science of
IGRT.
Image Guided Radiation Therapy: A
Clinical Perspective provides an
overview of the basic concepts of
IGRT and includes a review of its
history and present status.
Additionally, Image-Guided
Target
Delineation Technologies presents an in-depth overview of the main
imaging modalities used for target delineation, namely computed
tomography (CT) including 4-dimensional (4D) CT, magnetic
resonance (MR) imaging and positron emission tomography
(PET). Image Guided Treatment Technologies systemically introduces
the reader to the major in-room IGRT treatment technologies available
today, including video, ultrasound, planar and volumetric-based
imaging systems.
Given the multidisciplinary nature of the field, the book is broken into
six different sections. Part 1 (Molecular Imaging Technologies), Part 2
(Chemistry of Molecular Imaging), Part 3 (Molecular Imaging in Cell
and Molecular Biology), Part 4 (Applications of Molecular Imaging),
Part 5 (Molecular Imaging in Drug Evaluation), Part 6 provides
chapters on computation, bioinformatics, and modeling.

EUS PATHOLOGY WITH DIGITAL ANATOMY CORRELATION: A Text and Atlas
Bhutani, Deutsch
Atlas of Pathologic lesions by Endoscopic Ultrasonography (EUS) is a dedicated text to learn pathologic images seen
during EUS. The digital anatomy correlation used in this work is the natural continuation of efforts to apply the University
of Colorado Visible Human data set to gastroenterology.
The Visible Human data set was created by Dr. Vic Spitzer and colleagues at the University of Colorado and is currently
housed at the university’s Center for Human Simulation. The data set consists of high resolution transaxial digital images
captured as cadavers were abraded away at 1 mm or less depths. These images are compiled into blocks of data and each
structure is identified. This information can be used to pull out and manipulate 3-D structures as well as allowing one to review planar anatomy in any
orientation. Using the Visible Human dataset, one should be able to find a normal anatomy correlate to any image found during a EUS examination.
Endosonographers are asked to define lumps, bumps, cysts to find correlates for symptoms and abnormal laboratory findings. Accuracy requires a
tremendous amount of skill and experience. To help in this task, we have assembled chapters from a world-wide group of expert
endosonographers. These authors have shared their insight and images to help the readers of this work better see and understand some of the
complexities uncovered during a EUS evaluation.

International and regional travel continues to grow; in recent
years, disaster relief teams have joined the traditional ranks of
travelers and face exposure to both commonplace and more exotic
travel-associated infectious agents as a consequence of
deteriorating and uncertain environmental conditions. The
prevention of infectious disease is a major goal of the clinical
practice of travel medicine, and the protection of travelers against
vaccine-preventable diseases remains one of the most effective
interventions in promoting travelers’ health.
This third edition of the Atlas of Travel Medicine and Health provides the latest
travel health information in a visual format that will aid the healthcare
professional when discussing travel health and educating the traveler regarding
the risks and precautions that should be considered before and during travel in
disease risk areas. The authors have extensive experience in travel medicine,
and both are engaged in clinical practices in the field.
The second edition has been updated to reflect the development of
new, more immunogenic and safer vaccines for rotavirus,
meningococcal disease, hepatitis B, and Japanese encephalitis.
Recent changes in vaccine recommendations are covered, and new
chapters have been added to address the emergence and spread of
infectious diseases such as avian influenza and tick-borne
encephalitis.

Clearly -- and often humorously -- written, PDQ Public Health
defines public health and covers the basic concepts of public health
policy, including its history, local to international structure, and role in
protecting human health.
PDQ Evidence-Based Principles and Practice addresses the concepts
of evidence based health care in a gentle, non-technical manner. One
of its two major purposes is to provide a background to understand
health care research and how best to evaluate and apply new research
findings in health. In addition the book outlines how best to identify
important studies in health care published in the large bibliographic
databases such as MEDLINE and the Internet.
Concise, yet comprehensive, PDQ Public Health educates the reader
in the history and evolution of the concepts and practices of public
health on local, national, and international scales. Key concepts, such
as communicable diseases, vectors, hosts, and environments, are
defined; and how they and other factors interact to influence public
health issues is described. The statistical tools that are used to
determine risk and describe the interactions contributing to community
health are presented. The authors also address the impact of
population mobility, economic factors, government (law), and ethics,
on the practice of public health. This is all done in an engaging style
that aids the reader’s comprehension of this complex subject.
This text adds 4 new chapters to the existing 9 that cover new areas of
understanding and producing health research and helps the reader
develop optimal, effective MEDLINE search strategies. It offers stepby-step suggestions for retrieving sound clinical studies on the
etiology, prognosis, diagnosis, prevention, and management of
disorders encountered in adult general medicine.

PDQ Epidemiology will help to fill the considerable gap between the
exact findings of the laboratory and the uncertain world of clinical
medicine. By translating the terminology of epidemiology into easyto-understand language, the underlying scientific methods begin to
emerge and make sense. The third edition reflects the maturation of
the field, which now encompasses much more than the “randomized,
controlled trial.” New sections explain techniques that have been
introduced into the field since the previous edition, the section on RCT
has been expanded, and updated examples have been incorporated.
The section on measurement has also been brought into line with
newer thinking on reliability and validity.
PDQ Endodontics, 2e is a revised, abridged version of the full text,
Endodontics, Sixth Edition. This concise text covers a wide range of
topics in endodontics. Specific sections of the text thoroughly address
the access, cleaning and shaping, and the filling of the root canal
system. Also examined are endodontic mishaps, endodontic
emergencies, and the restoration of endodontically-treated teeth.
Need-to-know information is well-supported by the extensive
inclusion of illustrations, many in full color, making this an atlas of
endodontic therapy.

PDQ Statistics, Third Edition offers an expert overview of all major
statistical methods, giving the reader a thorough understanding of
statistics and how they are used in research articles. The book covers
the major categories -- variable and descriptive statistics, parametric
statistics, non-parametric statistics, and multivariate statistics. The
explanations are clear, succinct, and loaded with practical examples.
Integrative Oncology is the synthesis of mainstream cancer treatment
and effective complementary therapies. PDQ Integrative Oncology:
Complementary Therapies in Cancer Care provides oncology
professionals with need-to-know information about the data-based
utility of complementary therapies. It thoroughly discusses useful noninvasive adjunctive therapies that effectively and safely relieve
symptoms associated with cancer and cancer treatments. Following
discussions of the source and practice of complementary therapies,
this manual summarizes therapies by major cancer diagnoses and
concludes with a section on the management of major symptoms. The
text examines botanical research, herb-drug interactions in oncology,
and the potential and the concerns about dietary supplements.
This third edition includes new chapters on hierarchical and logistic
regression, path analysis, and structural equation modeling. PDQ
Statistics, Third Edition also helps the reader identify those statistical
analyses with basic flaws in design or research. The book's attractive
design and humorous writing style make the subject matter accessible
and engaging.

PDQ Physiology offers a concise, up-to-date summary of human
physiology. It is an ideal resource for the more advanced physiology
specialist and health professional requiring a quick view or review. Its
perspective is one of integration while incorporating the current
understanding in molecular and cellular physiology and offering up-todate explanations of physiological phenomena. The reader will greatly
benefit from the coverage of topics not found in most comprehensive
physiology books. PDQ Physiology is more focused on significant
points; more current in cardiac, renal, and muscle functions; and is
more descriptive in sexuality, reproduction and birth.
PDQ Histology is a concise laboratory guide and handy study aid for
Histology students. The text deals with the basics of the subject,
presented in a clear, rational manner. The content covers the
fundamentals and important features of microscopic recognition. It
focuses the reader's attention on the main structural features found in
standard sections taken from the student's slide box and emphasizes
the particular combination of histological criteria that characterizes
each section. It represents a mini-review of the distinctive microscope
features that permit histological preparations of the various tissues and
organs of the body to be correctly identified. PDQ Histology is the
only introductory book on the subject that presents short selections of
reliable tissue recognition criteria suitable for revision purposes.
