Seasons Hospice has undergone a metamorphosis in recent years. From
humble beginnings with five employees, it has grown to become the vital and
effective organization it is today employing nearly ninety people.
It has been my privilege to serve as a member of the board of directors for
the past several years. It is a pleasure to work alongside the very talented,
accomplished and dedicated group of people who serve on the Seasons
Hospice board of directors. As Seasons Hospice has evolved, the members of
the board have risen to the challenge of taking a more active role in setting the
direction for the organization. In a constantly changing industry, against a
backdrop of economic uncertainty with the potential for cuts in Medicare, the
board is creating strategies to better position Seasons Hospice for the future.
With this in mind, the board was instrumental in identifying the need to
upgrade the administrative offices and in developing and implementing the
plan to accomplish that goal.

The product of this vision is the beautifully remodeled 12,000 square foot
office space located at 1696 Greenveiw Drive SW in Rochester, designed
specifically around the Seasons Hospice team approach. It provides the
physical plant to permit Seasons Hospice staff and volunteers to operate more
efficiently and effectively. Financially, this acquisition positions Seasons
Hospice to enter the future with the confidence that it will continue to grow
and serve its patients and the community with the high quality of services for
which it has become so well known.
Our ability to provide individualized and compassionate care, education and
support for the dying and those who have experienced loss is possible because
of the dedication and support of our staff, our families and our generous

– Highlights of 2011 –
•After extensive analysis by the Seasons Hospice Board of Directors and Executive Director, we purchased the building
formerly occupied by American Family Insurance at 1696
Greenview Drive SW in June 2011 as the site of our new administrative and homecare office and the Center for Grief
Education and Support.
•The number of patients served by Seasons Hospice staff and
volunteers at year end was 389, an increase of 13% over 2010.
This represents a total of 21,018 days patients received care,
either in their homes or at the Seasons Hospice House.
•Trader Joe’s began donating fresh flowers to Seasons Hospice
on a twice weekly basis. The flowers are rearranged by volunteers and our clinical staff presents them to patients on a
regular basis. Patients love receiving these bright, beautiful
bouquets.
•Seasons Hospice gala, “A Live & Love Affair’ raised over
$100,000 after expenses, making it the most successful fundraiser in our history. Seasons also joined the Directed Giving
Alliance of Olmsted County to support its efforts to increase
charitable giving in the workplace by payroll deduction.
•Continuing our commitment to serving veterans at the end
of life, we achieved the second level of Community Partnerships with We Honor Veterans. This is a national initiative
between the National Hospice and Palliative Care Organization and the Veterans Administration which provides educational tools and resources to hospices across the country that
serve veterans at the end of life. In 2011, Seasons Hospice
awarded $4,500 in financial support to veterans on our program to assist them with their hospice care and other expenses through our Veterans Fund. This fund was established in
2007 from donations from area veterans groups.
•Volunteers contribute significantly to Seasons Hospice with
the support and care they provide to patients and staff. In
2011, 267 individuals provided volunteer service to our organization through direct care to patients, bereavement support
and administrative and community activities. This represents
8,538 hours of donated time at a cost savings to Seasons Hospice of $182,382.
•Improvements were made to the Seasons Hospice House with
the recarpeting of the entire lower level, made possible in part
through a grant from the Rochester Area Foundation and the
purchase of new television sets and DVD players for each of
the eight patient rooms, made possible by a grant from the
Carl and Verna Schmidt Foundation.
•The Seasons Hospice Center for Grief Education and Support
continued to grow, especially in the number of community
participants to our grief support groups and one on one
counseling services. In 2011, over 200 individuals received
bereavement services from Seasons Hospice.

– Sustaining Seasons Hospice –
The Seasons Hospice staff is grateful to the thousands of individuals, businesses and organizations that have
contributed their time, talent and financial resources over the years to make this organization what it is today.
This continuous outpouring of support has enabled Seasons Hospice to expand and adapt our services to meet the
unique needs of our patients, their loved ones and others who have experienced loss.
There are many ways to give to Seasons Hospice where your contribution will make a profound impact on the lives
of the persons we serve.
Massage Therapy and Music Therapy Funds
Massage and music therapy are an important part
of the array of services we provide to our patients.
Neither of these services are reimbursable by
insurance or Medicare/Medicaid, yet we offer them
nonetheless because they add greatly to the quality of
life and comfort of our patients.
Center for Grief Education and Support
Seasons Hospice’s Center for Grief Education and
Support offers a broad range of services and programs
in bereavement support for our patients, their
families (including programs specifically designed for
children) and members of the general community
who have suffered the loss of a loved one, regardless
of whether their loved one was on the Seasons
Hospice program.
All bereavement programs are offered free of charge,
therefore our grief center is funded entirely through
charitable contributions.
Patient Care Fund
This fund provides grants to individuals who wish to
stay at our Hospice House, but cannot afford the cost
of room and board at $185 per day. Most insurance
providers/ Medicare/Medical Assistance do not cover
residential hospice care. The patient care grant
provides for up to 45 days of care.
Veterans Care Fund
The Veterans Care Fund was established in 2007
through generous grants from local and state veterans
groups. The Fund is used in a variety of ways to
assist dying veterans on our hospice program, from
subsidizing the cost of care at the Hospice House to
paying for travel expenses to fly active duty soldiers
home to visit a dying veteran on our program as well
as meeting other needs.
A Live & Love Affair Gala
Since its inception in 2006, the Seasons Hospice gala,
A Live & Love Affair has raised over $430,000 in
support of our hospice and bereavement programs.
Not only is the gala a great way to spend an evening,
it has become a significant fundraiser for Seasons.
Circle of Remembrance Granite Pavers
Circle of Remembrance granite pavers are available for
purchase year round and provide a lasting way to
remember your loved ones. The pavers are installed
around the pond on the grounds of our Hospice
House. For additional information on purchasing a
paver, please contact the Seasons Hospice office.
Endowment Fund
The purpose of the Seasons Hospice endowment is to
broaden our income base in order to secure the future
of the organization. Any revenue that is expended is
from the interest generated from the principal amount
and supports special needs that arise. The endowment
fund is overseen by a committee comprised of
community and Seasons Hospice board members.
Methods of Giving
There are many ways to make a tax-deductible
donation to Seasons Hospice. We gratefully accept
gifts of cash, check, credit card, stock transfers,
estate gifts, charitable remainder trusts and other
planned gifts. To ensure the long term growth and
viability of Seasons Hospice, we are appreciative
of such deferred gifts as bequests. For more
