Redistributor as proof-of-purchase.

Offer Details:
1. Offer limited to purchases by Authorized, General or Associate Distributors of listed Diversey
products ONLY during the promotion period April 1, 2012 – June 30, 2012.
2. Minimum rebate $25. Maximum rebate $1000.
3. Proof-of-purchase(s) form must be dated during the promotion period of April 1, 2012 –
June 30, 2012.
4. Rebate request must be postmarked by July 31, 2012 or it will not be accepted.
5. Upon receipt and approval of rebate request, Diversey will mail the rebate check directly to
Distributors. Do NOT deduct rebate from ANY Invoice(s). Please allow 6-8 weeks for receipt.
6. Diversey is not responsible for lost or damaged submissions.
7. Offer may not be used in conjunction with any other Diversey promotion, program, or
contract pricing. Stock transfers or internal sales among redistributor subsidiaries or
divisions do not qualify
8. Fraudulent submissions could result in prosecution under U.S. mail fraud statutes.
