Muscle, Pledge or Scrubbing Bubbles products on single invoice. Rebate equal to the lesser of the Windex and eligible fantastik,
Glade, Mr. Muscle, Pledge or Scrubbing Bubbles case price shown on a single distributor invoice. On purchases of multiple
cases of Windex product, rebates will be based on the lowest case prices of eligible fantastik, Glade, Mr. Muscle, Pledge or
Scrubbing Bubbles products shown on the invoice. Maximum three (3) free cases per end-user location. Maximum value of
$65.00 per case. Offer valid on purchases sold through U.S. distribution only, not valid on any purchases direct from Diversey,
any of its subsidiaries, or consumer retail locations. Offer valid on products purchased, while supplies last, during the promotion
period 8/23/2010 - 12/31/2010. Limit one rebate per U.S. customer location, per calendar year; rebates valid for companies
and institutions only, not for individuals or distributors. All rebates must be completed and signed by end-user operator,
and must be received in end-user operator’s envelope with return address. Original rebate certificate must be completed in
its entirety. Original invoice from distributor must be attached as proof-of-purchase and must be dated between 8/23/2010
and 12/31/2010. Purchases must be on a single invoice. Multiple invoices will not be accepted. Submissions by distributors
will NOT be accepted. Rebate offer must be postmarked by 01/31/2011. All rebates paid in U.S. dollars. Allow 8-12 weeks for
receipt of rebate. End-users may not deduct rebate amounts from distributor invoices. Reproduction, sale, trading or purchase
of this certificate or proofs-of-purchase is prohibited. Use of multiple addresses or P.O. Boxes to obtain additional rebates is
considered fraudulent and may result in federal prosecution under U.S. Mail fraud statutes. Diversey assumes no responsibility
