Baltimore MEDICAL SYSTEM Annual Report 2006 “I have been a patient of BMS for 14 years and would never leave. Coming here is like the theme song from Cheers: ‘where everybody knows your name.’” Juanita Sirbaugh, Highlandtown Community Health Center patient Message from the Chair of the Board of Trustees As I reflect on my five years as a BMS Board Member, I am amazed at the transformation that our organization has undergone. We have emerged from being an organization reliant on government grants and focused on cost containment to become a vibrant, diverse network of healthcare sites with expanding patient services and community programs. This transformation results from the implementation of our strategic plan which focused on expansion of services, investments in information technology, and changes in operations. The results during this five year period are astounding and include the following: Vision Baltimore Medical System shall become the provider of choice for primary care services and will improve the quality of life for all we serve in the Baltimore metropolitan area. • Patients increase from 30,000 to 45,000, a 50% increase • Visits increase from 106,789 to 140,301, a 31% increase • The addition of a new site at St. Agnes Hospital and the acquisition of patients from GBMC’s Weinberg Family Health Center • Implementation of an Electronic Health Record, a tool used by only 8% of community health centers nationwide Mission Baltimore Medical System is deeply committed to improving health and wellness in Baltimore area communities by providing safe, high quality, accessible and affordable health care. • Implementation of a centralized Call Center • Implementation of a 340B pharmacy program which provides our patients access to affordably priced medications • Expansion of services to our Latino and international patients Essential during this transformation has been a continued commitment to the delivery of high quality patient care. “I love working at BMS because it gives me the opportunity to reach out to people in high need communities helping them improve their quality of life, and perhaps even live out a dream.” Earline Washington, Outreach and Access Manager Employee of the Quarter: Earline Washington If you are a new patient, have attended a community event or have been offered the chance to receive a free cancer screening, chances are high that you have already become acquainted with Earline Washington. A BMS employee since 2001, Earline is the Outreach and Access Manager for BMS, leading a team that handles patient recruitment, admittance and retention. Earline is the face of BMS in the community and is known for treating everyone with warmth and respect. In order to continue expanding services for our patients and their communities, BMS leadership, physicians and staff will be vigilant in their commitment to meeting the needs of the underserved. I am confident we are up to the challenge and am proud to be a part of the BMS family, truly at the Heart of Community Health. This hard-working team participates in and organizes neighborhood health fairs and events, attends community and business meetings, and acts as a primary point of contact for patient concerns and questions. Continuously going above and beyond the call of duty, Outreach also provides free educational programs and health screening services for patients. “It is because of Baltimore Medical System and wonderful staff members like Ophelia, that I am certain that the rest of my years will be the best of years.” Clarice Pope, Patient at BelairEdison Family Health Center Reaching Out to the Underserved Clarice Pope In July 2006, Clarice Pope was diagnosed with breast cancer and found herself in a difficult situation. A patient at BelairEdison Family Health Center for 7 years, Clarice received a free mammogram through a partnership between BMS and American Radiology Services. “The interpreters in Deaf Services show a genuine concern and care for their deaf and hard of hearing patients. They are reliable and dependable; I feel safe having my family cared for by them and the doctors they work with.” Ruth Crosby, Deaf Services patient GBMC Patients Find New Medical Home When the GBMC Weinberg Family Health Center closed in the fall of 2006, they called BMS. BMS affiliated with GBMC and provided on-site counseling to almost 1,000 patients. Not only did BMS welcome many new patients to our sites, but many Weinberg staff also moved to BMS. We are proud to be the health system selected by GBMC to insure continuity of care for its patients. Although she received the mammogram at no charge, she now had to find affordable treatment for her disease, despite her lack of health insurance. Ophelia Montague, the BMS Outreach Worker who persuaded Clarice to participate in the mammography program, made the dedicated promise to help her find low-cost cancer treatments. Thanks to Ophelia’s hard work, Clarice recently had her sixth and final surgery for cancer treatment and breast reconstruction—all at no charge. Message from the President & CEO I am proud to share with you Baltimore Medical System’s 2006 Annual Report. This was a year of significant accomplishment in the face of change. After 22 years of operating under the Municipal Health Services Program which provided life-extending benefits to many senior citizens of Baltimore, we saw the Medicare Waiver program (and its resulting revenue) finally come to an end. Fortunately, quick adaptation to a changing health care environment has always been one of Baltimore Medical System’s keys to success. We successfully implemented new programs to support our older patients so that they would not be too heavily impacted by the loss of this program. The ranks of the uninsured and under-insured continue to grow. Our awareness of unmet needs always seems to outstrip our resources and our ability to develop new programs. Baltimore Medical System—its staff and Board—continue to work tirelessly to protect and promote the health of our communities. Growing and diversifying is an important part of how we respond to the rapid shifts in community demographics related to age, new immigration, ethnicity and insurance status. You will see in this report that we have highlighted our operational accomplishments as well as the clinicians and front-line staff who make it all possible. However, we do not do it alone. A service representative for BMS since 2003, Rayiniel understands the importance of broadening her knowledge through in-house trainings and education. Her co-workers appreciate her desire to continuously improve her ability to provide patients with timely information and services. Jay Wolvovsky, President & CEO Electronic Health Record Improves Patient Care Investing in Quality As medical science continues to advance, so does a doctor’s need for up-to-date information about his or her patient. Baltimore Medical System knew the need for instant and remote access to patient records was essential in our growth as an organization, and so in 2006 we began the implementation of our Electronic Health Record (EHR) system. Electronic Health Records allow BMS clinicians to track outcomes for individuals and groups of patients. BMS will now be able to know where more resources or support services should be placed for the best clinical staffing and center-based programming. The EHR is also beneficial to patients, who can now have their prescriptions faxed directly to on-site NeighborCare pharmacies for pick up when they leave. BMS’ investment in EHR technology means better outcomes for everyone. “Being able to see an updated chart is a powerful tool. I can see all of my patients’ medications at a glance, whether I am in the office or at home. He is always very thorough and willing to take the time to discuss important matters with me.” Beverly Fortman, Highlandtown Community Health Center patient Message from the Chief Medical Officer At BMS, we want to be the “Provider of Choice,” the “Employer of Choice,” and the “Partner of Choice.” For our patients, employees and partners, we want BMS to stand for “The Best Medical System.” Provider of Choice Our 11 sites located throughout Baltimore City and County enable us to be the health care home for more than 46,000 individuals. We make appointments available on the same day a patient calls. And we provide extraordinary access to medications, laboratory, and imaging services—all at discounted rates. Our emphasis on quality is demonstrated by our recent investment in an Electronic Health Record (EHR). Employer of Choice BMS has a 22 year tradition of employing excellent staff from the communities that we serve and clinicians from some of the finest training programs in the country. We believe in a transdisciplinary team approach to care meaning that members of the health care team work collaboratively towards meeting a patient’s needs. Partner of Choice At BMS, we understand that health depends on many factors besides health care – what are often called the social determinants of health. Thus, we rely on community partners to help us develop and deliver important programs that address the social, cultural, financial and educational barriers to achieving good health. We are proud of our many diverse partners who work with us to improve breast health, promote smoking cessation, deliver cancer screenings, and provide health care for refugees, the deaf, and women with high risk pregnancies. “We believe in a trans-disciplinary team approach to care meaning that members of the health care team work collaboratively towards meeting a patient’s needs.” Dr. Kyu Rhee, Chief Medical Officer “We are the best Community Health Center around [and] we constantly strive to be and do better.” Dr. David Silver, Center Medical Director for the Highlandtown Community Health Center Employee of the Quarter: Dr. David Silver Dr. David Silver is a man who respects his patients as much as they respect him. For over 15 years he has focused his practice on geriatrics and also serves as the Medical Director for BMS’ largest and busiest site, the Highlandtown Community Health Center. He is known for the attention he gives each patient, many of whom are over the age of 80. Dr. Silver is proud of the growth of BMS. Dr. Kyu Rhee, Chief Medical Officer Baltimore Resettlement Center Adelante Familia Community Partners To meet the health care needs of the growing Hispanic community in East Baltimore, BMS has learned to also focus on education, outreach, and family support services. Adelante Familia has been our partner in this effort for four years. A program of St. Vincent de Paul, Adelante Familia helps us reach many women and children who might otherwise never come to our health centers. Bayview Care-A-Van Parked in front of Patterson Park or along Erdman Avenue, the Bayview Care-A-Van brings the health care system right out on the street. Every week, CareA-Van staff send patients to the BMS Highlandtown Community Health Center for follow up. Care-A-Van services are often the critical first step in moving a needy patient into a more comprehensive setting. Refugees who arrive in Baltimore receive their first experience of American health care at BMS. Thanks to our partnership with the Baltimore Resettlement Center, we have the privilege of providing initial health assessments to people from over 50 countries. The waiting rooms are full of patients waiting to be seen by a clinician. Senior citizens who grew up in Highlandtown sit next to women in full African garb and families who speak only Spanish. Staff are communicating in English, French, Spanish, Russian and American Sign Language. Patients are passing through on their way to see the pharmacist, to get advice from the benefits advisor, or to go to the lab. Looking to the Future The patient population at Highlandtown has grown to over 14,000. But this growth has brought new challenges to the aging building. By industry standards, a patient base of this size should be in a 25,000 square foot building. The site at 3509 Eastern Avenue is only 15,000 square feet. Standards also recommend an average of 2 exam rooms per clinician. Currently, each Highlandtown exam room is used by 2 or even 3 doctors, often a cause for long patient wait times. To address these critical constraints on our ability to provide quality health care, BMS has begun to plan for a new Highlandtown center. All photos on this page: Ira Wexler Focusing on Patients The Call Center staff handled 403,734 calls in the first year of operations. Call Center In 2006, BMS implemented a centralized Call Center with 23 staff members handling telephone inquiries and appointments for all BMS Health Centers. This major accomplishment resulted in reducing the wait time for patients on hold and increasing the number of same day visits scheduled.
