Excellus BlueCross BlueShield now offers you an online Health Risk Assessment. The Health Risk
Assessment is a simple and confidential
questionnaire that can help you identify health your
risk behaviors and healthy habits. The questionnaire
captures a range of information about
key lifestyle factors, personal medical history, gender
specific issues and readiness-to-change. Upon completion of the HRA you will receive a
Personal Wellness Report which outlines:

Step Up is designed to
assist individuals in
improving their daily activity level by taking more
steps each day. It also promotes healthy eating by
encouraging an increase in fruit and vegetable consumption.
Members are eligible for lifestyle benefit coverage upon
the effective date of
their Blue Choice Preference contract. Benefits renew
each calendar year if
continuously enrolled. In order to receive reimbursement for this benefit, the member must submit a claim
form for these services to Excellus BCBS. This claim
form is sent to the member upon enrollment and can
also be obtained through Customer Service.
