Introduction
Advanced Waste Services, Inc. is an environmental services firm specializing in non‐hazardous and hazardous waste collection, transportation, disposal and on‐site cleaning services to commercial and manufacturing clients located throughout the Midwest states of Illinois, Indiana, Wisconsin, Iowa, Ohio and Pennsylvania. The company is headquartered in West Allis, Wisconsin, a western suburb of Milwaukee, and operates full‐service waste collection and non‐hazardous waste processing facilities in Milwaukee, Ripon, Kimberly and Winneconne, Wisconsin; Portage, Indiana; Cedar Rapids, Iowa; and New Castle, Pennsylvania. Each facility maintains full‐service laboratories where lab technicians are able to analyze incoming wastes. Hazardous wastes are delivered to third party TSDFs for proper disposal. Company History
The company was started as a non‐hazardous waste transportation company on January 1, 1993 by Mike Malatesta. The company finished the year 1993 with gross sales of $673,000, a small net profit and four employees. Since then, the company has grown through a combination of internal sales and marketing efforts, targeted acquisitions and the internal development of processing plants. The company has experienced strong and steady growth, consistently growing sales and cash flow while maintaining a consecutive history of positive net income. Now in its 24th year in business, the company has about 175 employees and serves in excess of 1,000 commercial and industrial clients, generating over $50 million in annual revenue. In May 2015, Advanced Waste Services was acquired by Covanta Holding Company, the leading energy‐from‐waste company in the US. AWS became the anchor for Covanta Environmental Solutions, a wholly owned subsidiary focused on expanding Covanta’s portfolio of services in the profiled waste, environmental and industrial materials management and waste solutions arenas. Becoming a Covanta Company allows us to improve and broaden our services, our geography and our financial resources. Business Description
Advanced Waste Services, Inc. provides non‐hazardous and hazardous waste collection, transportation and disposal services as well as on‐site cleaning and waste‐related maintenance services to commercial and manufacturing clients in all industries. The Company operates a fleet of specialized waste collection equipment including vacuum tanker, Hi Vac, SuperSucker, Roll Off, and Van Trucks. In addition, the Company maintains specialized cleaning equipment for on‐site services that includes its patented VacSimizers, water blasters, pressure washers, confined space entry equipment, roll off boxes, vacuum boxes, drum loaders, etc. Additionally, each location maintains a full‐service, on‐site laboratory with personnel to provide analytical and testing services. The Company employs ten Solutions Sales Managers located in Wisconsin, Illinois, Indiana, Iowa, Ohio and Pennsylvania, who are in the field day‐to‐day meeting with new and potential clients to promote the Company and create opportunities. The majority of the Company’s sales practices center around face‐to‐face personal meetings with clients. RCRA and DOT Training Annually, AWS invests over $250,000 into employee training. Our instructors are our employees. The instructor‐led course we design for our clients will be led by the same team that trains our experts. We designed our RCRA and DOT training to establish and/or improve our clients’ regulatory compliance, produce best practices for a safe work environment and reduce their insurance costs and compensation claims. Emergency Spill Response AWS’ Spill Response and Spill Contingency Planning protect the reputations of our clients. By providing our clients with 24/7 365 Spill Response for diesel fuel and regulated materials, they know exactly what to do and who to call when a spill occurs. Advanced Waste Services recently invested $1MM into its Milwaukee wastewater treatment plant to process used oil and oily wastewater. It’s another way we help clients eliminate wastes and risks. This process helps clients get more value for their oil and helps them comply with the EPA regulations of used oil and the rebuttable presumption. All AWS facilities have the capacity for processing used oil. By testing the used oil and confirming it is non‐hazardous, our clients greatly reduce the cost to process the used oil. More importantly, it greatly reduces the risk of fines, superfund fingerprinting and criminal penalties for not complying with environmental regulations Lab Pack Services (Get Rid of It Program – GRIP) Advanced Waste Services created an environmentally friendly lab pack program called G.R.I.P, also called the Get Rid of It Program. Whatever “It” is ‐‐ any amount, any size ‐‐ Advanced Waste Services can dispose and recycle hazardous, non‐
hazardous and universal wastes properly and quickly. The turnkey lab pack G.R.I.P program includes onsite inventory, labeling, packing, transportation and options for disposal and/or recycling. Laboratory & Analytical Services Advanced Waste Services maintains a full‐service, on‐site laboratory at each of its wastewater treatment facilities that is staffed with technicians to provide analytical and testing services. At right, a senior chemist analyzes an industrial wastewater sample to confirm it matches the Waste Certification Statement (profile) prior to the treatment process. UnWrapp – Using New Ways to Recycle All Packaged Products UnWrapp is a de‐packaging program to help clients achieve zero landfill product destruction or beneficial reuse of package products that are recalled, expired, off‐
spec, damaged – really anything deemed non‐performing inventory or “not to be sold.” Products are separated from the packaging inside our UnWrapp machine. Packaging is shredded and baled for recycling. The product can be sorted and reused as raw material ingredients or recovered for commercial reuse. It can also be beneficially reused in compost programs, anaerobic digestion facilities, our energy‐from‐waste operations or recycled back into our wastewater treatment operations. With a fleet of semi‐trailer trucks to help coordinate transportation, we can “UnWrapp” any amount and any size personal care products, cleaning products and food and beverages. Besides easy truck access, the facility is also accessible by rail. Within 50 miles of our treatment facility and the proposed storage tank impoundment, all of Beaver, Butler, Lawrence, and Mercer Counties, and portions of Alleghany, Armstrong, Clarion, Crawford, and Venango, in Pennsylvania rests. Michael T. Malatesta ‐ Vice President & General Manager, West Region A co‐founder of the Company, Mr. Malatesta has been involved in all areas of company management. Mr. Malatesta is a graduate of Dickinson College, Carlisle, Pennsylvania, with a BA in English. Mr. Malatesta began his career at Browning‐Ferris Industries gaining exposure to a variety of solid waste operations and gaining experience in routing, residential collection and transfer station operations. In 1989, Mr. Malatesta moved into the company’s “Special Services” division that concentrated on a wide variety of non‐solid waste activities that included the collection, transportation and disposal of liquid industrial wastes. In 1990, Mr. Malatesta was one of the youngest employees in BFI history to be promoted to the position of District Manager and Vice President for Midwest Special Services. Following the May 2015 acquisition by Covanta, Mr. Malatesta became the Vice President & General Manager of Covanta Environmental Services, West Region, which stretches from Western Pennsylvania to California. Karen Bleach ‐‐ General Manager‐‐ Wisconsin & Iowa Business Units Ms. Bleach serves as the General Manager for the Wisconsin business units located in Milwaukee & Fox Valley, as well as the business unit located in Cedar Rapids, IA; she is a CPA and graduated Magna Cum Laude from the University Wisconsin‐Oshkosh with a BBA in Accounting. Ms. Bleach has been with the Company since 1998, served as the Chief Financial Officer for numerous years before becoming General Manager, and is a valued member of AWS’ Executive Committee. Bill Dwyer – General Manager – Indiana Business Unit Mr. Dwyer joined the Company earlier in 2015 and is responsible for the overall operation of the Indiana business unit. Bill is a very accomplished and experienced industrial waste professional, with more than 20 years of field and management experience. He is a results‐ oriented, Global Supply Chain/Logistics/Transportation professional with a track record of reducing costs and facilitating new multi‐industry business revenue and profitability. Throughout his career, Bill has worn many hats, including lab pack tech, lab pack / de‐pack plant manager, Regional Services Manager, Hazardous Waste Specialist, General Manager and more. He’s worked for big and small, private and public companies. Mr. Dwyer has a broad knowledge of customer service, regulatory requirement, vendor selection and negotiation skills that support breakthrough achievements in target areas such as rail transportation, warehousing and waste management. His versatile career is complimented by a BS in Chemistry from the University of Illinois in Chicago. Mr. Dwyer serves on AWS’ Executive Committee. Tom Wendler – General Manager – Pennsylvania Business Unit Mr. Wendler has served AWS for more than 20 years in varied positions and been headquartered in Rockford, Milwaukee and now New Castle, PA. Prior to being named General Manager of the Pennsylvania Business Unit, he led the Industrial Services Division, responsible for all Company industrial cleaning operations in Wisconsin, Illinois, Indiana, Iowa and Pennsylvania. Under Mr. Wendler’s management, the Industrial Services Division grew from next to nothing to an integral and profitable part of AWS’ business, generating nearly 1/3 of overall revenue. As General Manager, Mr. Wendler is responsible for the overall operation of the Pennsylvania Business Unit and serves on AWS’ Executive Committee. He has been with the Company since 1993. William B. Christel ‐ Vice President, Environmental Health & Safety / Human Resources Mr. Christel joined the Company in August 2005. He is responsible for the oversight of employee and environmental safety protocol as well as Human Resources at the all AWS facilities. Previously, Mr. Christel held the position of Environmental, Health and Safety Manager and Human Resources Manager for ITT, a global manufacturer of precision products. He graduated from the University of Wisconsin‐Whitewater with a Bachelor of Science degree in Education/Occupational Health and Safety. He received his MBA from the University of Wisconsin‐Parkside. He is a Value Based Six Sigma change agent ‐‐ Green Belt and Black Belt certified. Mr. Christel serves on AWS’ Executive Committee. Joseph Pawlak – Vice President, Sales Mr. Pawlak joined the Company in 2010. He is responsible for the development of significant new client relationships throughout AWS’ operating region; improving and integrating existing and new hazardous waste opportunities and operations in all locations. Mr. Pawlak has a lengthy history of sales management strategies and developing territories into record‐breaking teams with Waste Management and other national companies. He received a BS degree from Murray State University in Murray, Kentucky. Mr. Pawlak also serves on AWS’ Executive Committee. Our Mission is to Ensure No Waste is Ever Wasted P a g e | 19
KC Wulf – Director of Operations As the Director of Operations, Mr. Wulf is responsible for collaborating with General Managers on their operational directives and Business Unit plans to maximize their accomplishments. Additionally, he is responsible for leading special and business‐targeted growth projects to revenue‐producing status, as well as ensuring that elite standards, SOPs, etc. are in place. Mr. Wulf has been with the Company since 2007 when he was hired to direct AWS’ GRIP operations at our Rockford facility. Mr. Wulf graduated from Western Illinois University with a Master’s Degree in City Planning where he wrote his thesis on Recycling and Waste Management. His undergraduate work was in the fields of Earth and Environmental Sciences. The majority of his career has been spent in the hazardous waste industry including lab packing, routing waste streams for disposal, and operating a TSDF (Transfer, Storage and Disposal Facility). Other professional experiences include, managing a drilling operation, leading an emergency response HAZMAT division, overseeing a pump and treat location, health, safety and environmental director, and developing sales and fostering business with environmental brokers. Mr. Wulf is a member of AWS’ Executive Committee. Ann Marie Trapp – Controller Ms. Trapp serves as the Controller for AWS and is the conduit to the finance arm of Covanta Environmental Services. She is responsible for managing all of the AWS’s financial and accounting functions. Her duties include preparing monthly financial statements, working with lenders to secure corporate financing, working with auditors to prepare year‐end audited financial statements and corporate tax return and implementing new systems to streamline workflow. Ms. Trapp received a BA in Accounting from the University of Wisconsin‐Milwaukee and earned her CPA from the State of Wisconsin. She is a member of WICPA. William Fowler, CHMM – Waste Treatment Laboratory Manager Mr. Fowler is responsible for managing the laboratory operations not only in Milwaukee at the Company’s largest managed centralized waste treatment facility, but he also oversees laboratory operations in Indiana, Iowa and Pennsylvania. Mr. Fowler’s duties include maintaining the facility’s State of Wisconsin Laboratory Certification, monitoring incoming and treated wastewaters to ensure compliance with permit requirements and preparing required reports for various AWS operates a state‐of‐

General Waste Treatment Process Description for AWS Facilities
Bulk Waste Processing 1) Bulk Receiving – Tank trucks hauling pre‐
approved loads back into the unloading area. The contents of the truck are sampled, and the sample is taken to the on‐site laboratory maintained at each location. The lab tests the sample for pollutants and for treatability (how well the waste will treat in our system or what mixture of chemicals will need to be added to the waste to treat it successfully). When approved by the lab for receipt, the contents of the truck are dumped via gravity into the underground unloading pit where it is screened (to remove debris) and then pumped to primary treatment tanks. Free oil is pumped into oil treatment tanks. 2) Primary Treatment – Primary treatment tanks are all equipped with mixers and automated chemical dispensing systems. To treat the waste, the mixers turn the waste and blend in the chemicals, which are required for treatment. The operator adds the chemicals in the proper ratio to generate a flocculation in the tank. The goal is to generate a flocculation (floc) that does two things. First, precipitate the pollutants present in the waste (mostly inorganic metal contamination. Second, create a floc that we can dewater easily. Once the floc is generated, the sludge in the tank settles by gravity so that we are left with a distinct water phase and a distinct sludge phase in the tank. 3) Oil Treatment – Free oil is treated by introducing acid, chemicals and compressed air into the tank. This combination helps split the oil from the water. Once the split is complete, the water is drained from the bottom of the tank and transferred into the primary treatment tanks, where it is treated as described in #2 above. The oil phase from these tanks is then pumped through a filter and into a holding tank, where it is stored and subsequently hauled away for reuse as a supplemental fuel. Every used oil shipment we receive is analytically tested before processing into recycled oil. Our Mission is to Ensure No Waste is Ever Wasted P a g e | 23
4) Secondary Treatment – After primary treatment, the distinct water phase is removed (decanted) from the tank and pumped to the secondary treatment tanks. Here the water is tested in the lab to determine the level of pollutants that are still present. If the levels exceed the maximum levels prescribed in our sewer discharge permit, the water must be re‐treated. When this is the case, a chemical formula is worked up that will achieve the desired result. To treat this water, the mixers turn the water and blend in the chemicals, which are required for treatment. The operator adds the chemicals in the proper ratio to generate a flocculation in the tank. Once the water is re‐treated, it is again tested in the full‐service, on‐site laboratory AWS maintains at each location for permit compliance. If the water meets the discharge parameters, it is then discharged to the sewer. The sludge from the secondary treatment tanks is then pumped to sludge treatment. 5) Sludge Dewatering – Flocculated sludge from the primary and secondary treatment tanks is pumped to the sludge holding tanks. Depending on the AWS facility, rotary vacuums or plate and frame filter presses dewater the sludge. The dried sludge is conveyed away. The water from dewatering is then pumped to the secondary treatment tanks, where it is combined with the decanted water from the primary treatment tanks. Non‐Bulk Waste Receipt and Processing  Non‐Bulk Receiving – Pre‐approved non‐bulk containers, which include 55‐gallon drums, 275‐gallon totes, 5‐gallon pails, etc. are unloaded on the west side of the plant by fork truck. All non‐bulk containers are labeled non‐hazardous and include the name and address of the generator, the name of the waste and the approval profile number.  Liquid Removal and Treatment – Non‐bulk containers containing wastewaters and waste oils that can be treated are pumped via a unique vacuum system at the plant. These liquids are then screened and pumped into the treatment plant for processing. Empty containers are crushed and landfilled.  Solids Removal and Processing – Non‐bulk containers containing sludges, solids and non‐treatable liquid products are transitioned to the solidification unloading station. At this station, the drums are conveyed into a 17,000‐gallon solidification pit. Here, a 60,000‐pound excavator opens the drums. Solidification media is conveyed into the solidification pit. The excavator operator then solidifies all the waste from the non‐bulk containers. The solidification sludge is then loaded into dump trailers for transport to the landfill. I have reviewed your February 8, 1998 letter of application and related
information for discharging pretreated industrial wastewater to the City of Ripon sanitary
sewerage system. It is my understanding that your new company called Chief Waste
Treatment Corporation will be located at 625 South Douglas Street and is co-owned by
Mr. Pat Wittlinger, who will be the licensed operator of the treatment processes.
Based on the specific information submitted in your application and subsequent
meeting and verbal communications with you and Mr. Wittlinger, I believe your plan for
discharge to the Ripon sanitary sewers will be in compliance with the City's sewer usage
code. However, to clarify issues raised, discussed and tentatively agreed upon; I will
briefly highlight those issues as follows:

An appropriate and accurate flow proportional automated monitoring
system of your wastewater discharge to the City will be planned, installed,
and approved under the authority and guidance of a registered Professional
Engineer. It is my understanding that you have retained Mr. Joe Kraemer,
P.E., of Badger Labs and Engineering for this service. As agreed, please
provide me a copy of the monitoring system drawings and specifications
as soon as they are available.
Sampling and testing frequency of your wastewater discharge will be on a
24-hour basis until an adequate history of volume and pollutants, as
determined by me, is established. 48-hour testing frequency will be
allowed after adequate history is determined.

In addition to daily flow volume chart recordings, a daily printed or typed
log of volume totals will be maintained by your company and submitted to
my office on a monthly basis.
Testing will be performed for Biochemical Oxygen Demand (BOD), Total
Suspended Solids (TSS), Phosphorous (P), and Nitrogen Ammonia NHJ).
A log of these test results will be submitted to my office at the same
frequency and time as the flow volume reports. Your company will be
responsible for testing requirements to be performed by a laboratory
registered and certified by the Wisconsin Department ofNatural
Resources (WDNR).
Metals will be tested in accordance with WDNR regulations for metal
platers and finishers. Metal discharge limits will be the same as those
required by WDNR unless total metal discharge loadings to the City
become a problem as determined by the City.
The Hydrogen Ion concentration (PH) will be tested on each equalization
tank «batch" discharged by your operator and logged for reference. A pH
neutralization feed line will be installed for adjusting the pH as needed.
Flow equalization tanks used for discharge to the City will include two
each at 5,000 gallons capacity each. Predicted average discharge volume
is4,Oee gallons per.day. IO,OrJO
The automated discharge sampler and refrigerated sample bottle
compartment will be of the locking variety to provide discreet sampling by
the City whenever desired.
Biochemical Oxygen Demand (BOD) loadings discharged to the City will
be limited to 200 po~s per day. Pre-treatment of BOD by your
company to maintain this level will be required if any exceedences of 200
pounds per day is experienced.
In addition to the above specific issues addressed, your company will also be subject to
City of Ripon sewer use codes Chapter 17.04 and Chapter 17.16. While you have already
requested, received, and reviewed a copy of these two Chapters, a copy of each is
enclosed with this letter for your use as an extra copy.
With the understanding that your company will be in compliance with the requirements
stated in this letter and the Ripon sewer usage codes, permission is hereby granted to
begin discharge ofyoUI company's wastewater to the Ripon sewers as soon as all
requirements are in place for discharge compliance. However, no discharge may occur
until I have an opportunity to physically visit your facility to help determine the
probability for discharge compliance. Please contact me 10 days or more prior to your
planned start up of the facility.

In accordance with Section 260.395 of the "Missouri Hazardous Waste Management Law", the
Missouri Department of Transportation approves the application of the above-named transporter.
Beginnning on the effective date shown above, until the expiration date, this transporter may transport
hazardous waste within this state. This permit is not transferable, and does not apply to other
environmentally regulated areas.
All equipment used for the transport of hazardous waste shall meet and be operated in accordance with
the provisions of the Missouri Hazardous Waste Management Law (Section 260.350 to 260.430,
RSMo) and rules made under its authority. The Department does not inspect each vehicle to be used for
hauling waste, and the issuance of this permit does not imply approval of that equipment.
The Department may suspend, revoke, or modify this permit if it finds that the hauler is in violation of
the chapter, rules, or any terms or conditions thereof, or has created a public nuisance, or posed a threat
to the health of humans or other living organisms.
This license must be carried in the vehicle at all times when transporting hazardous waste. The license
must be shown torepresentatives of the Department, Officers of Missouri Highway Patrol and other law
enforcement officials on demand.

permit provides no relief from the Hazardous Materials
Regulations (HMR) other than as specifically stated herein.

associated with transportation in commerce.

SAFETY CONTROL MEASURES:
a. PACKAGING - Prescribed packaging is:
(1) A 55-gallon UN 1A2 steel drum certified to the PG I
performance level for solids and the PG II performance level
for liquids and dual marked to a minimum of UN1A2 X/400/S
and UN1A2 Y/1.2/150; or
(2) A 30-gallon UN 1A2 steel drum certified to the PG I
performance level for solids and the PG II performance level
for liquids and dual marked to a minimum of UN1A2 X/235/S
and UN1A2 Y/1.2/150.
b.
OPERATIONAL CONTROLS:
(1) Transportation is authorized by private or
contract carrier only.
(2) Transportation is authorized one-time, one-way,
only from the generator of the waste material to the
hazardous waste treatment facility where it must be
unloaded by the consignee for disposal.
(3) Sufficient water must be present in each drum to
ensure that the waste phosphorous is covered during
transportation, in any orientation of the drum.
(4) Drums must be held and observed for a minimum of
24-hours before transportation. Any leaking or

otherwise unsuitable drums must be replaced prior to
transportation.
(5) Packages must be destroyed at the disposal site
and may not be reused.
(6) The net mass of the waste material and water, in
kilograms, must not exceed the mass that would be
permitted by calculating the volume of the packaging in
liters multiplied by the specific gravity indicated on
the package certification.
8.
SPECIAL PROVISIONS:
(1) A current copy of this special permit must be
maintained at each facility where the package is offered or
reoffered for transportation.
(2) A person who is not a holder of this special permit who
receives a package covered by this special permit may
reoffer it for transportation provided no modification or
change is made to the package and it is reoffered for
transportation in conformance with this special permit and
the HMR.
9.
MODES OF TRANSPORTATION AUTHORIZED: Motor vehicle.
10.
MODAL REQUIREMENTS: A current copy of this special permit
must be carried aboard each motor vehicle used to transport
packages covered by this special permit.
11.
COMPLIANCE: Failure by a person to comply with any of the
following may result in suspension or revocation of this
special permit and penalties prescribed by the Federal
hazardous materials transportation law, 49 U.S.C. 5101 et
seq:

Under Title VII of the Safe, Accountable, Flexible,
Efficient Transportation Equity Act: A Legacy for Users
(SAFETEA-LU)- 'The Hazardous Materials Safety and Security
Reauthorization Act of 2005' (Pub. L. 109-59), 119 Stat.
1144 (August 10, 2005), amended the Federal hazardous
materials transportation law by changing the term
“exemption” to “special permit” and authorizes a special
permit to be granted up to two years for new special permits
and up to four years for renewals.

REPORTING REQUIREMENTS: Shipments or operations conducted
under this exemption are subject to the Hazardous Materials
Incident Reporting requirements specified in 49 CFR
171.15 B Immediate notice of certain hazardous materials
incidents, and 171.16 B Detailed hazardous materials
incident reports. In addition, the grantee(s) of this
exemption must notify the Associate Administrator for
Hazardous Materials Safety, in writing, of any incident
involving a package, shipment or operation conducted under
terms of this exemption.

This special permit authorizes the transportation in
commerce of lead acid batteries and packages of battery acid
(with two different UN numbers) on the same vehicle. This
special permit provides no relief from the Hazardous
Materials Regulations (HMR) other than as specifically
stated herein. The most recent revision supersedes all
previous revisions.
b.
The safety analyses performed in development of this
special permit only considered the hazards and risks
associated with transportation in commerce. The safety
analyses did not consider the hazards and risks associated
with consumer use, use as a component of a transport vehicle
or other device, or other uses not associated with
transportation in commerce.
c.
Unless otherwise stated herein, this special permit
consists of the special permit authorization letter issued
to the grantee together with this document

that acid batteries and packages of the same acid contained

editorial review under § 107.121 initiated on December 4,

SAFETY CONTROL MEASURES:
a. PACKAGING – As required by 49 CFR, Part 173, for the
materials to be shipped.
b. Packages containing the Class 8 materials, as listed in
Paragraph 6, may be transported on the same vehicle.
c. Packages must be secured against movement within the
transport vehicle in accordance with § 177.834.
8.
SPECIAL PROVISIONS:
a. A person who is not a holder of this special permit who
receives a package covered by this special permit may
reoffer it for transportation provided no modification or
change is made to the package and it is reoffered for
transportation in conformance with this special permit and
the HMR.
b. A current copy of this special permit must be maintained
at each facility where the package is offered or reoffered
for transportation.
c. The marking requirements of 49 CFR 172.301(c) are
waived.

MODAL REQUIREMENTS: A current copy of this special permit
must be carried aboard each motor vehicle used to transport
packages covered by this special permit.

COMPLIANCE: Failure by a person to comply with any of the
following may result in suspension or revocation of this
special permit and penalties prescribed by the Federal
hazardous materials transportation law, 49 U.S.C. 5101 et
seq:

All terms and conditions prescribed in this special
permit and the Hazardous Materials Regulations, 49 CFR
Parts 171-180.
o
Persons operating under the terms of this special
permit must comply with the security plan requirement
in Subpart I of Part 172 of the HMR, when applicable.
o
Registration required by § 107.601 et seq., when
applicable.

permit to be granted up to two years for new special permits
and up to four years for renewals.

REPORTING REQUIREMENTS: Shipments or operations conducted
under this special permit are subject to the Hazardous
Materials Incident Reporting requirements specified in 49
CFR '' 171.15 B Immediate notice of certain hazardous
materials incidents, and 171.16 B Detailed hazardous
materials incident reports. In addition, the grantee(s) of
