GRADE THE STRING MAN’S BOX LACROSSE TOURNAMENT 2015 RELEASE OF LIABILITY FOR MINOR PARTICIPANTS (READ BEFORE SIGNING) Insurance and Medical: All participants must be covered with their parent’s medical insurance. This player’s parental release form must be signed and submitted for the player and his/her parents/guardian by each teams coach prior to participating in The String Man’s Box Lacrosse Tournament. IN CONSIDERATION OF ___________________________________, my child/ward, being allowed to participate in any way in The String Man’s Box Lacrosse Tournament on January 2nd and 3rd 2015, being held at The Brewster Sports Center, the undersigned acknowledges, appreciates, and agrees that: The risk of injury to my child from the activities involved in this program is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 1) FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child's participation; and, 2) I willingly agree to comply with the program's stated and customary terms and conditions for participation. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. ___________________________________ __________________________________ (PARENT/GUARDIAN SIGNATURE) (PRINT NAME) Date Signed:___________________________________ UNDERSTANDING OR RISK I understand the seriousness of the risks involved in participating in this program, my personal responsibilities for adhering to rules and regulation, and accept them as a participant. ___________________________________ _______________________________ Date Signed :____________________ (PARTICIPANT SIGNATURE) (PRINT NAME) THE STRING MAN’S BOX LACROSSE TOURNAMENT RULES 2015 TOURNAMENTS DATES AND START TIMES Both the Bantam and Junior Divisions will be played over Friday January 2nd and January 3rd Tournament schedules will be emailed to coaches on December 31st and will be posted on The String Man Facebook page as well as www.brewstersportscenter.com Team Registration – Must register 45 minutes prior to first game TEAM ROSTERS and PARENTAL RELEASE FORMS MUST BE HANDED IN NO PLAYER WILL BE PERMITTED TO PLAY WITHOUT A RELEASE FORM Bantam divisional games and double elimination Championship games will be played on January 2nd and 3rd from 8:30 a.m. through 2:30 p.m. Junior divisional games and double elimination Championship games will be played on January 2nd and 3rd from 2:30 p.m. through 8:30 p.m. Tournament Champions receive a team trophy and individual trophies The rules used for The String Man Box Tournament Games will be the most current rules set forth by the US Lacrosse with the minor variations listed in this document. Divisional and Championship Bracket Play Each team is randomly place in a bracket with 3 other teams. Each team plays every team in their bracket. Top 2 winning teams in each bracket will play in a double elimination Championship round Selection of top 2 is determined by: 1. Record 2. Face to face competition 3. Goals scores 4. Goals Allowed 5. Braveheart Number of Players—Each team will have a total of 6 players (five field players and one goalie) on the field. There must be a legally-equipped goalie on the field at all times. Head Coach—Each team must have a designated head coach. The head coach must be an adult. Failure to have a Head Coach prior to game time will result in a delay of game penalty. If one is not available by 10 minutes after the scheduled start time, the game will be declared a forfeit. Length of Game—All levels will play two 20-minute running time halves. There shall be 5-minute halftime between the first and second halves. Ties will played in a 5 minute overtime sudden death format; followed by a Braveheart if needed. Note: The game clock shall begin at the scheduled starting time whether or not teams are ready to play. Pre-game procedures—Time permitting each team will get 10 minutes to warm up. If the preceding game goes into overtime the following game’s team may not be able to warm-up Time-outs— Each team is permitted one 1-minute time-outs per game. Officials may call additional timeouts for injuries. Substitution—All substitutions will be done on the fly or during a dead ball situation. Players will enter through the net areas, with one player exiting the field before the next player enters the field. Face-offs—There will be one face-off at the beginning of each half only. Following each goal, the goalie who was scored upon will put the ball back into play. Each team must have 3 players behind its defensive restraining line, 2 players behind its offensive restraining line, and one at midfield for the face-off. All players are released from behind the restraining lines on the whistle to start play. Penalties Time—All penalties will result in a 3 vs. 2 fast break situation. Overtime— Ties will be played in a 5 minute overtime sudden death format; followed by a Braveheart if needed.
