This form must NOT be used for registering individual players.
Team registration requires you to provide an online internet banking payment receipt
number as evidence of your payment.
To do this you will need to:
1. calculate your payment amount for the number of players in your team
2. make payment to our account
Once you have completed this form and made payment, notify us of your payment.

Important points to simplify the registration process:
1. Use the form on page 4 to list the players in your team.
2. Email your team list as soon as available with all details completed.
3. Bring a Player Consent Form signed by a parent or guardian for each player –
a copy of this form is available for you to distribute to parents or guardians.
4. Hand in the consent forms to the registration desk on match day.
