MKSAP 16 QUESTION
A 33-year-old woman is evaluated for a 2-day history of diarrhea. She describes the
stools as mucoid but without visible blood. Bowel movements are associated with mild
abdominal cramping. She has no nausea or vomiting. The patient works with infants in a
day care center, and several children have recently had a diarrheal illness.
On physical examination, temperature is 37.8 °C (100.0 °F); other vital signs are normal.
Abdominal examination discloses normal bowel sounds and diffuse mild tenderness to
palpation. A stool sample shows no gross blood, but results of guaiac testing and a test
for fecal leukocytes are positive.
The patient is advised to increase fluid intake and take over-the-counter antipyretic
agents for fever. No antibiotics are prescribed. Two days later, a stool culture is reported
to be growing Shigella sonnei. The patient is contacted and reports that her diarrhea and
fever have both resolved and that she wishes to return to work.
