How to Access your “Status of Benefit Entitlement” Letter
1. Login to you MiWam (MARVIN) account with your Username and Password:

2. Go the “LETTERS” tab and click on it:

Find the “Status of Benefits Entitlement” letter that states you have exhausted your unemployment
benefits. You will need to click on the blue “Letter Id” number in order to view:

workplace Kiosk or a saved as a screen shot to your personal computer or hand held device:

Click on the “Request SUB Card” tab:

Read the Supplemental Unemployment Benefits (SUB) Agreement and if you agree with the terms,
click “I Agree:”

Click on the calendar to enter your “Benefit Week Ending Date” & select
“Reason for No State System Benefits.” Answer the remaining questions, then click “Next:”

A) Select the Document Title “Proof of State System Benefit” from the drop down menu.
B) Select browse to access your saved “Status of Benefit Entitlement” letter (must be in an approved
format).
C) To receive a copy of the request, add your email to the “Send a copy to the following email box”
D) Click “Submit:”
