people around the world are making the decision to blend their health needs with
their holiday.
Healthcare and tourism are the largest two industries in the world. Cost, waiting
times and quality are reasons why people seek healthcare abroad, as well as the
opportunity to recuperate in a relaxing environment. Our June issue of Healthmatters
takes a look at the growing trend towards medical tourism.
According to the Dubai Chamber of Commerce and Industry, the UAE health
care market is expected to grow from AED11.7 billion in 2005 to AED43.7 billion
in 2015. In Turning the tide (page 10) we look at how more people are choosing to
stay in Dubai for medical care as the quality is gaining international status along
with the benefits of being close to home and family.

Fruit and veg deficiency in children could
up cancer risk
Parents must ensure children are eating more fruit and vegetables than current
levels as it could help to reduce the risk of cancer in later life, a leading charity has
claimed.
The World Cancer Research Fund (WCRF) said that recent studies had shown
that as many as four in five children are not eating the required amount of fruit
and vegetables and other research showed that fewer than one in 10 teachers
were eating the recommended ‘five-a-day’.
One in 14 boys and one in 25 girls did not eat any fruit and vegetables at all
on the day of the research carried about by the UK’s Food Standards Agency, the
charity added.
Nathalie Winn, from WCRF, said: “It is important that we encourage children to
get into healthy habits as early in life as possible.
“This is because scientific research shows that eating a plant-based diet with
plenty of fruits and vegetables, wholegrains and pulses probably reduces the risk of
a number of types of cancer later in life.”
Winn said eating enough fruit and vegetables also helped people to stay at a
healthy weight.
Obesity has been linked to a greater risk of certain cancers.
Malaria eradication pursuit
‘counterproductive’
Attempts to eliminate malaria could actually be
counterproductive in the fight against the disease, experts
have warned.
Researchers from the Kenya Medical Research InstituteWellcome Trust Research Programme in Nairobi have
warned that the emphasis on elimination or eradication
in strategic plans for the next 10-20 years in Africa is “at
best irrelevant and at worst counterproductive”, raising
expectations that cannot be met.
Writing in the Lancet medical journal, the authors claim it
is possible to use currently available methods to transform
malaria from a major health priority to a fairly minor
burden.
“Reaching low endemic control is achievable, its
financing requirements are predictable and its effects
measureable,” said Professor Bob Snow.
“The result would be a huge public health success, a
legacy we’d all be proud of, but this isn’t elimination, which
seems to be a more attractive term to donors.”

UAE to raise awareness
over hepatitis C
More than 50,000 people across the United Arab Emirates are
being offered free hepatitis C tests after experts warned that
many sufferers are unaware they have the condition.
Dr Adnan Hammour, a liver specialist at Dubai Healthcare
City’s Abuhamour Medical Centre, said at least 30% of patients
with hepatitis C do not know they have the disease.
Hammour also told the National newspaper he believes
as many as 90% of people with the condition are not being
treated for it.
“Millions of people can reach their 40s and 50s, after having
had the ‘silent disease’ for 10 to 20 years with mild symptoms,
if any, and then find that it is so advanced that liver cancer or
sclerosis has set in,” he said.
“We don’t want to scare people and make them panic, but
early detection and treatment is important because it reduces the viral load until it can no longer be detected,” he added.
American Heart
Association points
to pollution link
The American Heart Association has claimed that evidence is
now “stronger than ever” that breathing in ‘smog’ is linked to
strokes and heart attacks.
The association said in a report that pollution from industry,
traffic and power generation was a leading cause of such
conditions and that fine particulate matter from burning fossil
fuels such as gasoline, coal and oil is the clearest offender.
“Particulate matter appears to directly increase risk by
triggering events in susceptible individuals within hours to
days of an increased level of exposure, even among those who
otherwise may have been healthy for years,” said lead author
Dr Robert Brook of the University of Michigan.
A review of six year’s worth of medical research also
presented strong evidence that pollution can help clog arteries,
and a “small yet consistent” association between short-term
exposure to air pollution and premature death.
Merck Serono
celebrates 50
years in the Gulf
Dubai-based pharmaceutical and bio-technology company
Merck Serono celebrated over 50-years of operations in the
Gulf region last month with a dinner at the Burj Al Arab.
Merck Serono commenced operations in the region in the
late 1950’s when the company was then known as Merck
Pharmaceuticals. The audience was told that Merck Serono
considers the region to be an important strategic market, and
detailed how the company has experienced rapid revenue as
well as market share growth in the medical sector over recent
years as the sector has expanded.
Karim Smaira, chief executive of Merck Serono UAE,
said: “This was a wonderful opportunity to gather together
colleagues, industry partners and associates to help us
celebrate the long and important relationships we have
formed with the UAE.”
Dubai Healthcare City’s German Medical Center last month celebrated its fifth anniversary.

Scientists hope for
insulin breakthrough
The side-effects of insulin, a common form of diabetes treatment,
could be limited after a breakthrough by scientists.
Diabetic patients often suffer long-term damage to the heart from
the vital hormone treatment as it can cause blocks in blood vessels,
potentially leaving patients at risk of a heart attack.
Researchers have now identified a natural process in the body which
points towards a method of protecting patients from this side effect.
A study found insulin caused some cells within blood vessels to grow
more than they should, but also discovered that a naturally occurring
molecule, called C-peptide, is released at the same time as insulin in
healthy people.
Scientists have now found that this molecule halts the growth of the
cells.
Dr Karen Porter, who led the research, said: “We found that
administering insulin with C-peptide – which is released naturally in
partnership with insulin in healthy people – appears to protect blood
vessels against this damage.”
Scientists hope to have made a vital breakthrough against
the virus that causes dengue fever, after identifying that it
could be using the body’s immune response to help it infect
more cells.
The study, published in the journal Science, could help
explain why those people who contract dengue fever more
than suffer more severe and dangerous symptoms the
second time around.

Eating just one small packet of peanuts a day can lead to a drop in
cholesterol levels and a reduction in the risk of heart disease, scientists
have claimed.
Researchers also found that those who ate plenty of nuts, of all
varieties, had lower cholesterol levels than those who rarely ate them.
Dr Joan Sabaté, from Loma Linda University, in California, who led
the research said that the research could be linked to the popularity of
nuts throughout human history.
“Increasing the consumption of nuts as part of an otherwise
prudent diet can be expected to favourably affect blood (cholesterol)
… and have the potential to lower coronary heart disease risk,” she
said.
“Dietary interventions to lower blood cholesterol concentrations...
are the cornerstone of prevention and treatment plans for coronary heart disease.”

Cost of cancer
care doubles
over 20 years
Financial analysis of a period covering almost two
decades show that the medical costs associated with
cancer care have almost doubled in real terms, according
to researchers.
The cost of treating cancer, however, remains
proportional to other medical costs.
The research from the US Centers for Disease Control
& Prevention showed that cancer costs have shifted
away from inpatient care to solution delivered in the
community.
The ageing population has driven much of the
increase in cancer care costs, researchers added.
Australian study

heart disease advice
Despite popular support,
the consumption of
chocolate, coffee or red
wine does not provide
any cardiovascular health
benefit, researchers in
Australia have claimed.
The Australian Heart
Foundation said a
review of more that
100 scientific papers found that even though the three
were commonly referred to as part of a heart healthy diet,
they were not linked to better health, although all of the
products did contain antioxidants.
“Chocolate, coffee and red wine are okay as part
of a balanced diet,” said Susan Anderson, the Heart
Foundation’s national director of healthy weight. “But
these findings confirm that if you’re consuming them
thinking you’re reducing your risk of heart disease then
think again.”
Genetic advance for breast cancer test
Genetic researchers believe they have identified five new regions of
the genome that increase a woman’s risk of developing breast cancer
by between 6% and 16%.
A study published in the journal Nature Genetics, funded by Cancer
Research and the Wellcome Trust, detailed the link, which could
potentially lead to earlier diagnosis.
The research takes the total number of common ‘low-risk’ genetic
sites associated with breast cancer to 18, and as more of these
‘low-risk’ sites are identified it is thought tests could be developed to
single out those particularly at risk.
Study author Professor Nazneen Rahman, said: “Our results now
take the total number of gene regions linked to the risk of breast
cancer to 18, but we still don’t know which genes are causing this
increased risk.
“Identifying the underlying genes and mechanisms behind breast
cancer development is essential to increasing our understanding of
the disease and ultimately finding new treatments.”
Lead author Professor Doug Easton, from the University of
Cambridge, said: “While each of these sites has a small impact on
breast cancer risk, by finding more of these genes we may be able to
develop a test that can predict more reliably a woman’s risk of developing breast cancer.”
Japanese study to
observe pine bark
extract
Researchers in Japan are studying pine bark as a treatment
for heart failure, previous studies suggested that pine bark
extract might help reduce blood pressure in people with mild
hypertension.
In a new study, patients aged over 40 are also to be prescribed
the extract to test its benefits on high cholesterol levels.
In the clinical study at Kyoto University graduate School of
medicine in Japan, patients will be given 200 mg daily for 12
weeks.
Pine bark extract has been linked to a variety of health
benefits, although there continues to be little scientific support
for its efficacy.

Medical tourism has many fans, but it also
has its critics. A recent newspaper report
suggested that as many as 70,000 patients
visited Thailand last year from the United
Arab Emirates. While this figure perhaps
needs to be taken with a pinch of salt, it is
clear that many people within Dubai and
the other emirates continue to travel
overseas to receive treatment. In the long
term it is probably unsustainable, but in the
short term it can also be dangerous,
according to one senior physician.
It is certainly a situation that Dr Klaus
Kallmayer, chairman of Dubai Healthcare
City’s German Heart Centre (GHC) feels
passionate about. “With a number of minor,
chronic or cosmetic conditions I see few
dangers in flying abroad for treatment,” says
Kallmayer. “However, when it comes to
serious medical disorders, especially those
that can deteriorate into an acute illness or
life-threatening emergency, flying abroad can
quickly turn into a menace.”
Kallmayer argues that every form of
suspected or proven heart disease requires
reliable and continuous quality care close to
home. All too often, he says, minor or
chronic conditions can suddenly turn critical,
and “people who appeared previously well
and stable had a heart attack ‘out of the blue’
or died of sudden cardiac arrest”.
In these scenarios, he claims, physicians in
the UAE are often hamstrung by a lack of
knowledge of a patient’s medical situation.
“From experience, many patients believe
that, even in the case of an emergency, they
can fly out to Thailand or Europe and
undergo treatment there,” he says. “This is a
deadly misconception. Proper, life-saving
treatment for a heart attack, serious
arrhythmia or stroke has a limited time
frame of very few hours only. By that time,
even a private plane will still not have been
fuelled up. All too often the time for a life
saving intervention is wasted altogether, or
they are transported at short notice to a
centre which may not be equipped or trained
to deal with their particular emergency at
all.”
While many facilities overseas are
undoubtedly of the upmost quality, there are
some problems that are an inevitable result of
travelling abroad for treatment. “We have
seen quite a few patients, often just days after
their return, who present thick files from
abroad,” he says. “This first struck us as
unusual, until we discovered many of the
patients are seeking out our help as they do
not understand the results of their foreign
examinations, which have not been clearly

explained to them.”
Moreover, Kallmayer argues that by
travelling overseas for treatment, patients in
the UAE are actually hindering the
development of services closer to home. “The
medical tourists are not only spoiling their
own chances of optimal emergency and
follow-up treatment, but effectively using
their money to improve the medical
infrastructure of foreign countries,” he says.
Fortunately, however, the tide is starting to
turn, says Kallmayer. “With the creation of
Dubai Healthcare City, Dubai has already
taken an important step to ensure a high
quality medical sector,” he says. “Only
institutions meeting the highest international
standards comparative to those in Europe or
the US are licensed there to ensure
confidence in its patients… [and] networks
of high quality medical providers from all
levels and specialties of care are being created
throughout the community.”

Considering healthcare and tourism are two
of the world’s largest industries, the medical
tourism phenomenon is hardly surprising.
Nevertheless, over the past 15 years the
healthcare sector has started coming to
terms with the globalization of care
provision as patients increasingly look to
the international market for affordable and
high quality medical treatment.
Indeed, in many ways the United Arab
Emirates has been at the forefront of the
emergence of medical tourism. As one of the
most affluent countries in the region, the
UAE government has long provided
excellent care for its population – it is only
relatively recently, though, that nationals have
been able to receive high level treatment in
the country itself. Prior to developments such
as Dubai Healthcare City (DHCC), many
patients were forced to travel overseas to
receive treatment.
The main drivers for the medical tourism
market are cost and quality. Leaving your
own healthcare system to receive medical
treatment might seem like a huge
inconvenience, but if patients believe their
local care to be of a poor standard, lacking in
a specialty, or simply unaffordable, then the
global health market suddenly becomes a
viable option. And while, in the past, Dubai
has seen a lot of patients leave its shores for
treatments, the long-term plan is to reverse
that trend. And the key to achieving this,
says David Hadley – the hospital director of
DHCC’s The City Hospital – is to aim to
compete on quality.
“Dubai is not cheaper than elsewhere in
the world where other quality healthcare
facilities are available – such as Thailand and
India,” he points out. “It will take many years
to build a reputation and we are still in the

Medical tourism through the ages
The medical tourism industry may appear like a new fangled term, but its roots can
be traced all the way back to antiquity, with evidence of its existence among the
ancient Greeks, Egyptians, and Japanese. The first medical tourists were believed to
be those who went in search of mineral-rich thermal springs, natural resources
which are still used for their therapeutic qualities today. As early as 4000 BC,
Sumerian societies were constructing health facilities around these hot springs. It
was the ancient Greeks, however, who built the first major medical tourism sites.
The Asclepia Temples were in fact erected to honor the Greek god of medicine and
could be said to represent the world’s first health centers. By the year 300 BC, these
temples were a feature of Greek society, with notable facilities including Epidaurus
(or Epidauria), which even boasted a gymnasium, snake farm, and a dream temple
as well as the usual hot baths. As the Roman Empire’s influence developed, people
were known to travel far and wide to seek treatment in medical hotspots. Likewise,
by the 13th century, Cairo had become famous for its Mansuri Hospital and the
facility’s promise to serve anyone, regardless of race, religion, or wealth, drew health
travelers from far and wide. In Japan, too, mineral springs called onsen have been
popular for their healing properties and have attracted guests from overseas for
more than 1,000 years.

potential of medical tourism for Dubai’s
economy. “I do not think it is a fad… [but]
we cannot honestly say that we are presently
seeing high medical tourism numbers,” he
says. “I think that medical tourism could be a
relatively large industry in the future,
however Dubai is still in the early stages of
developing a reputation in this industry.”
Considering Dubai is looking to compete
on quality, it is natural that the market will
take time to develop. If facilities like DHCC
are to be successful attracting international
patients – and the early signs are promising
– then it will have to remain committed to
documenting its clinical success rates. “A
quality reputation takes time to develop and
transparency in terms of clinical results is the
key driver here,” says Hadley. “We measure
our outcomes to international benchmarks
and actively participate in international
healthcare surveys – these results are already
published in our annual report.”
While much of medical tourism traffic
can be explained by cost and quality issues, a
significant factor is availability of specialized
treatment. Many countries have been able to
develop high quality healthcare provision in
some areas, but supporting a wide range of
specialities in a healthcare community can be
extremely challenging. It is this area that
somewhere like DHCC could prove very
successful, argues Dr Chris Canning from
Moorfields Eye Hospital Dubai. “We have
not targeted the high-volume ‘simple’ surgery
market but rather the sub specialist complex
one,” he says. “Patients come from abroad for
a number of reasons – for us it is mostly
because of the reputation of Moorfields as
one of the finest eye hospitals in the world or
it is because of personal recommendation
regarding one of the consultant surgeons
here.”

Canning agrees with Hadley that DHCC
“will always struggle to compete with the
sub-continent and Asia for price” but he
challenges the idea that it is right to consider
medical tourism as an industry per se. “[It’s
success] depends on the definition of
‘medical tourism’,” he reasons. “I think Dubai
is respected as a place where there is a good
regulatory environment, so most doctors are
good, where there is good access to the
country and its medical services and is a

destination in its own right,” he says. “I think
it will never compete in the price sensitive
commodity end of the market but may well
be able to attract some higher volume work
in the mid-price bracket… [but] I do not
recognize it as an industry. Will patients
become more mobile and more willing to
seek care abroad? – yes; will there be a
wholesale and frivolous rearrangement of the
human shape? – I hope not!”

Self-improvement was once the last thing
on people’s minds when they booked a
holiday. Whether travelling for pleasure,
excitement or any other type of edification,
holidays are usually a chance to let your hair
down and return to the daily grind
invigorated – or at least that is the plan. But
now, due to the increasing popularity of
cosmetic medical procedures, many people
are booking a flight and returning, quite
literally, a different person.
The rise of Dubai’s cosmetic surgery sector
has broadly mirrored the growth of the
emirate itself. As the towers on Sheikh
Zayed Road climbed skywards, so did
Dubai’s reputation as a destination for
aesthetic surgery. With the establishment of
Dubai Healthcare City and its tough clinical
and regulatory standards, however, cosmetic
surgery has only gone from strength to
strength, argues Sam Bahsoun, from
DHCC’s American Academy of Cosmetic
Surgery Hospital (AACSH).
The secret to its success, he believes, is that
medical tourism – and especially aesthetic
procedures – chimes with much of the Dubai
masterplan to grow its reputation as a major
tourist destination. “Medical tourism is a
global trend and many countries all over the
world have tried to develop it,” he says. “But
I am convinced that Dubai is the best place
for medical tourism – it is a crossroads for
cultures and is a very well established travel
destination, whether for business, leisure or
shopping,” he says. “Medical tourism comes
as a complement to the others types of
tourism already established and working in
Dubai and the region.”
That may well be the case, but Dubai, like
the rest of the world, has not been immune
to the global downturn. While tourism is
now picking up again, the strain on

are attracting patients to Dubai? The
AACSH operates as four different

reflects the type of invasive and

ascent of cosmetic surgery. Not so, says
Bahsoun. “Demand has actually increased
with the financial crisis, contrary to the
widespread opinion,” he says. “Surgical
procedures have increased since the financial
recession started – perhaps because after all
the stress and hard work people are thinking
about themselves again.”
Indeed, for those who are considering

contains a boutique hospital, are likely to
prove appealing – and convenient. After all,
Dubai certainly does luxury well. “AACSH is
very much similar to the other activities
Dubai has developed,” says Bahsoun. He
points to the sheer number of high-end
hotels and restaurants in the emirate, or
facilities like the Atlantis or Ski Dubai, as
evidence that Dubai continues to attract
high-spending tourists. “Dubai is a dream for
a lot of tourists to come and enjoy and this
has helped develop the concept of medical
tourism in parallel to all other activities of
the city.”
In the past, medical tourism was mostly a
one-way stream for Dubai. It may have been
able to attract small numbers to specialist
clinics, but in general more people were
leaving Dubai for medical treatment than
visiting. Not any longer, he says. “People
from Europe turned their eyes to Dubai for a
medical solution of their problems instead of
travelling to the US,” he says. “Same thing
has happened with people from the Middle
East – today we can see at the American
Academy of Cosmetic Surgery Hospital
more and more visitors choosing Dubai as
the destination for medical trip instead of
Lebanon.”
Of course, you don’t have to be a tourist to
enjoy the services on offer at DHCC.
Indeed, many Emiratis and long-term Dubai
residents have been medical tourists in the
past. Bahsoun hopes that the availability of
Joint Commission International-accredited
facilities within the emirate itself will keep
patients away from the airport. He also
believes Dubai has the potential to act as a
cosmetic surgery hub for the region. “More
and more citizens of the United Arab
Emirates and the surrounding region – such
as Saudi Arabia, Qatar, Kuwait and Bahrain
– can now find the specialists for their health
problems just next door here in Dubai,” he
says. “They no longer have to travel to other
countries for treatments.”
And it is not just major surgical
procedures that are proving popular, either.

Indeed, many people still consider the idea of
elective surgery somewhat beyond the pale.
While most of us will have issues with our
appearance, few will take the active steps to
seek – and pay – to have them attended to.
That however, is starting to change, says
Dr Antonio Armani, from the Alvi Armani
clinic. Armani’s clinic specializes in hair
transplants, and, as they are not covered by
national health plans anywhere, Armani says
they are proving extremely popular.
“As the intervention has become better in
quality with more natural results it has
become much more popular,” he says. “It is
likely that it is the number one procedure for
men in cosmetic surgery.” The procedure is
attracting a lot of attention from patients
from outside of the UAE, he adds. “50% of
our patients come from Europe, GCC states,
Saudi Arabia, India and other Asian
countries.”
And Armani agrees with Bahsoun that
many individuals are looking to combine
holiday plans with aesthetic surgery
concerns. “Dubai has truly become a new
tourist spot as our European patients, when
given a choice, prefer to come here than to
our North American clinics,” he says. “In
fact, we also see some North American
patients who prefer Dubai rather than our
own North American centers in Toronto and
Beverley Hills,” he says. Armani dismisses
the idea that Dubai is blooming as a
cosmetic surgery center as it is more
affordable. “The main driver is definitely
quality as our procedures range from $20,000
to over $100,000 per procedure.”
The change in patient-flow has also been
reflected in the clinical community. “Even
the direction of the physicians has changed,”
says AACSH’s Bahsoun. “Today Dubai and
the UAE gather all top specialists in the
medical field here on a regular basis as they
are attracted by conferences and workshops
and the opportunity to pursue continuous
medical education and medical research.”
It is clear, then, that Dubai looks set to
continue flourish as a destination for
cosmetic surgery – and will surely stem the
flow of patients leaving the emirate for
treatment. So whether you’re booking a flight
from overseas, or a taxi from down the road,
Dubai has plenty of options for a bit of nip
tuck.

Dubai is going from strength
to strength as a dental tourism
destination
If there is one field of medicine that can
make a strong case for the merits of medical
tourism then it is surely dentistry. Travelling
abroad for treatment can be an
overwhelming experience, especially if the
procedure involves a long period of recovery
or rehabilitation. In dentistry, however, an
operation has to be pretty serious if the
patient can’t walk out of the clinic and onto
a plane a matter of hours after a trip to the
dentists.
Traditionally, too, dentistry has been a
field of medicine where both price and
quality can vary hugely across the globe. But
with the emergence of stricter regulation,
online training, and a burgeoning
international community, the standard of
dentistry is starting to improve across the
globe – prompting a fierce battle for patients
from competing healthcare systems.
Even before the emergence of Dubai
Healthcare City, Dubai has been well known

for its dental community. The cluster of
clinics on Dubai’s Jumeira Beach Road has
been a destination for dental tourists for
more than a decade – providing both
aesthetic and traditional treatments.
Nevertheless, the number of residents in
Dubai who still leave the country for
treatments is still higher than it should be,
says Dr Sanya Mooraj from the North
Carolina Dental Practice at Dubai
Healthcare City. “Dubai comprises a largely
expatriate population and this is one of the
reasons people wait to go home to get their
treatment done,” she says. But patients rarely
recognise that, due to the advances in the
field of dentistry, they are often paying out
extra for the exact same procedures, using the
same equipment, that practices are carrying
out in Dubai. “Dentistry has evolved in
terms of modern dental care… and the
equipment and materials assure that patients
can expect a certain result,” she reasons.

One of the biggest drivers of dental
tourism – as opposed to general medical
tourism – has been aesthetic concerns.
People can have healthy teeth but still be
unhappy with their color, shape or other
aspect of appearance. It might be
prohibitively expensive to have addressed in
their own countries, or it might not be
available at the right quality, but aesthetic
dentistry is certainly proving popular with
North Carolina Dental Practice’s patients,
says Mooraj. “We provide patients with the
possibility of full mouth ‘smile makeover’,”
she explains. “This involves simple teeth laser
whitening to cosmetic veneers – we also
provide full mouth rehabilitation and
implants for people with missing teeth,” she
adds.
And yet, getting most patients to visit the
dentist when it is just down the road can be
difficult enough. So what is the key to
convincing patients to travel hundreds of
miles to visit a dentist? The answer, says Dr
Hans van der Elst, clinical director of the
German Dental Oasis, is trust. “You have to
trust somebody you don’t know and who is
going to take influence on an essential part
of your body – your mouth,” he reasons. This
is why, he argues, DHCC’s growing
reputation is so important to protect and to
continue to build. For instance, his native
country of Germany has long been a
destination for patients from the Middle
East and he believes DHCC has the
potential to provide the exact same level of
treatment.
But the big advantage Dubai has in the
dental tourism stakes is the tourism. Van der
Elst believes that, once all are equal in
clinical terms, Dubai has a real opportunity
here. “All patients, especially from Europe,
love to come to Dubai,” he says. “You can
relax at the beach, shop in the malls, play golf
on the most beautiful courses in the world
– and at the same time patients can have
their teeth fixed in high-end clinics, which
are equipped with the best equipment and
some of the best European doctors are

available to make the treatment as
comfortable as possible.”
Van der Elst suggests that Dubai might
want to think about having formal partner
clinics across the world, where dental records
can be shared and trips of this sort can be
arranged seamlessly. “This would be a
challenging project for the tourist industry,”
he admits. “But in my opinion this will be a
huge opportunity and travelling agents
should start thinking about programmes that
might be fitting to the European customers
and also can be adapted to the European

insurance market.”
There is an argument, too, that Dubai’s
dental community could work closer with its
medical colleagues to create these sort of
packages. Indeed Mooraj is keen to dispute
the idea that a distinction exists between
‘medical’ and ‘dental’ tourism. “It is my firm
belief that oral health should be considered a
part of an individual’s general health,” she
reasons. “If a person is suffering from anemia
it will manifest itself in the mouth in the
form of a pale mucosa and ulcers. In this way
many systemic conditions may be diagnosed
by looking at the health of the oral cavity.
Based on this we should consider it as ‘health
tourism’ as opposed to ‘dental tourism’.”
For developments such as Dubai
Healthcare City, this sort of collaboration
between medical professionals makes perfect
sense. The health city was created to foster
clusters of excellence, and Mooraj reckons
the development of future ‘heath tourism’
should be a shared goal of the health
disciplines in the freezone. And despite
Dubai’s relative abundance of dental clinics,
Mooraj believes the strict standards in
DHCC are starting to differentiate its clinics
from the rest. “There are a multitude of

dental clinics in Dubai,” she says. “[But]
being in DHCC there is great importance
placed on the qualification of the
practitioner. To get a license to practice in
the candidate has to go through a
comprehensive process of documentation to
be able to get a license. It is a tedious process
but one that ensures that only those doctors
who have met a certain criteria can practice.
It raises the bar for us and this serves to
motivate us to improve ourselves.”

Knee replacement surgery is proving one of
medical tourism’s largest industries, and Dubai is
well-placed to be a leading provider
When it comes to medical tourism, there is
a much more robust market for some
procedures than others. After all, no matter
in which country you travel to the cost of
high-end cancer care is unlikely to fluctuate
greatly. Modern techniques for treating
cancer patients can be enormously
expensive, and the level of expertise required
means neither the physician nor the facility
will come cheap. But for standardized,
routine operations like knee replacements,
there is a burgeoning global market
developing and Dubai plans to be right at
the center of it.
Mostly, this is because the cost of a total
knee replacement can vary tremendously
depending on where it is performed. For
instance, in the United States it is not
uncommon for the procedure to cost up to
$40,000. In other areas of the world, the
operation can cost as little as $6,000. The
result has been a true global market for knee
operations – one that Dubai, with its
growing reputation as a holiday destination
and a provider of high quality healthcare, is
tapping into.
The key to the competitive nature of this
aspect of medical tourism is its simplicity. “It
is a standardized type of surgery and
rehabilitation,” explains Dr Mike van Steijn,
a senior consultant orthopedic surgeon at the
Dubai Bone & Jone Center (DBAJ). Based
in Dubai Healthcare City, DBAJ is a
comprehensive musculoskeletal center and is
well qualified to compete in the global
market. “The key to success is if the surgeon
can do it right, then there is no need for a
second attempt,” says van Steijn. “The
aftercare and the type of implants used is
very standard and available throughout the
world now.”
Knee replacement surgery typically
involves a process designed to remove
damaged cartilage from a patient’s damaged
knee. The surgery involves the replacement of
an unhealthy surface of a knee joint with a
metal and plastic implant. Depending on the
nature of the complaint, knee surgery can
replace all or part of the knee. The operation
is usually done through an incision of around
six to 12 inches in the knee area. The
unhealthy surfaces of the lower end of the
thighbone and the upper end of the shinbone
are removed then replaced by artificial
surfaces known as ‘prostheses’. These artificial
products are then cemented to the bone, a
suction devise is used to remove excess blood
and the skin is closed with stitches.
It might sound a little gruesome, but it is
relatively straightforward as a procedure and
patients are typically able to leave hospital
facilities between five and seven days after
treatment. Following this period, patients are
usually able to resume most normal activities
with three to six weeks. Sometimes, though,
it is not that straightforward. Van Steijn
points out that often knee operations can be
a result of extreme obesity, so there are often
other factors to consider. “If a patient has
co-morbidity it can be different,” he says.
“The patient will have to depend more on
the quality of the hospital – you need to
know there are good facilities to deal with
possible complications. Also, they may have
to stay longer in the hospital or access a
specially-equipped rehab center linked to the
facility.”
So while van Steijn accepts that cost may
be a key driver for the global knee

the reasons expats went abroad was based on
a lack of trust with the doctor or the quality
of care in the hospitals in the past,” he says.
“This is clearly changing now. Doctors from
all over the world are more willing to stay in
Dubai and give a continuum of care and the
hospitals can provide better nursing care
replacement market – and that Dubai is
unlikely to be able to compete on price with
somewhere like India – there are other
concerns to bear in mind. Indeed, van Steijn
argues that even with a procedure that can
vary in pricing as much as knee replacement
surgery, patients should think first about
quality and leave others to worry about its
cost. “I think quality should be the main
driver for the patient,” he says. “Cost should
be an issue for the insurance or the
self-payers.”
Indeed, in a health economy such as
Dubai’s, it is unlikely that there is any longer
much need for patients to leave the country
for such a treatment. “In countries like the
US where healthcare is expensive, most
people cannot afford elective surgery and will

go to places like Thailand or Mexico because
it will be less expensive,” argues Dr Charles
Jones from the California Chiropractic &
Sports Medicine Center.
In an insurance-based society like Dubai,
with facilities like DHCC available, it is now
unnecessary to send patients abroad for
routine operations, adds van Steijn. Before he
came to Dubai full time, van Steijn used to
visit the emirate to conduct procedures on
the national population who were covered by
the government’s health policy – and if he
did not travel to Dubai to conduct the
procedure, patients tended to travel to
Germany or other healthcare destination to
receive treatment. That is no longer the case,
he says. “The health authorities can now try
and keep patients in Dubai now and they
can work together with, for example, me at
DBAJ.”
The same is true with the expatriate
population, he says. “Previously I think that
too.”
Dr Jones echoes van Steijn’s comments,
arguing that developments like DHCC have
gone a long way to creating confidence in
healthcare providers within the emirate.
“One of the primary goals of DHCC is to
have the best physicians who are specialized
to be able to handle the more difficult types
of surgeries required, so the residents of
Dubai do not have to travel abroad to receive
medical treatment,” he says. “And I have had
patients from Kuwait, Bahrain, Afghanistan,
Saudi Arabia, and Egypt come to visit the
clinic. As DHCC continues to build its
reputation, I believe people will come from
every country to receive the best medical
treatment by the best doctors.”
As a rehabilitation center, though, Jones
does stress that anyone who does have
treatment abroad will not have to fly back to
the relevant country every two weeks to
make sure their recovery is on track. Modern
medical records and communication
channels means that the aftercare can be
coordinated across borders and healthcare
facilities. “The internet gives surgeons the

Despite overwhelming evidence as to the
power of preventive medicine, the majority
of people do not seek out medical attention
until they have convinced themselves they
have a real problem. This can partly be
explained by a natural and understandable
desire to suppress the thought of bad news,
but it is also a result of modern lifestyles.
Building a career in today’s economy can
be a stressful experience and few of us take
the time out to think about our health until a
problem manifests itself. There is simply too
little time to schedule a check-up – especially
when job security is low and economic
uncertainty remains high. And yet, only the
most ardent workaholic will get through a
year without taking a holiday and with
Dubai’s reputation as a holiday and health
destination continuing to grow a new
phenomenon is starting to emerge: the
health MOT.
“They are general checkups that check for
medical conditions or ill health that a person
is most at risk for,” says Dr Rosalie Sant, an
obstetrician and gynecologist at Dubai
Healthcare City’s Primavera Medical Clinic.
“A friend of mine calls them ‘preventative
maintenance programmes’. Everyone should
go for them regularly as ill health can very
often be prevented, which is much easier
than curing it.”
The charm of the health MOT is that it
usually only takes around half a day, is
comprehensive enough to screen for the
most dangerous and serious of conditions,
but will also give you a good idea of your
general fitness. Due to its mix of brevity and
comprehensiveness, argues Sant, it is the
perfect fit for tourists who have come to
Dubai to relax and recuperate, but also for
Dubai residents who are enjoying a week or
so away from the office. “It is much easier to
attend to these things when one is relaxed
and not hassled,” she reasons. “The results are
likely to be a better reflection of the person’s
well being. Also if the patient needs to fast
before the checkup they can do so more
easily.”
Tests normally look for common
conditions like diabetes, cholesterol, kidney,
liver and eye problems, but will also screen
for specific tests like breast cancer and
cervical cancer prevention for women,
prostate cancer prevention for male, and
individual tests according to risk factors

age, some may require additional ECGs, X
Rays and ultrasound scans, and female

outpatient, so no admission is needed and
holiday activities can resume once you leave

for these,” she says. “People of varying
nationalities and ages present for these, men
and women both. There is more awareness
now regarding the importance of these
checks.”
And it is a positive sign, says Fatima. After
all, the more we move towards a society that
values preventive medicine the more can be
done to help reduce the burden of global
health problems. “I think every one above

Trying
in vein
Varicose veins can be embarrassing but they can also
be treated, says Dr Ali Jawas
Individuals suffering from varicose veins are
unlikely to encounter serious complications,
but that can be a small consolation. As well
as being extremely unsightly, varicose veins
can leave individuals in no small amount of
pain, says Dr Ali Jawas, a vascular surgeon
and owner of Dubai Healthcare City’s Vein
Care & Surgery Center. “Besides cosmetic
problems, varicose veins are often painful,
especially when standing or walking,” he
points out. “They often itch, and scratching
them can cause ulcers.”
But what exactly are varicose veins? There
will be few people who have not seen them
on others and – no doubt – winced. “Varicose
veins are veins that have become enlarged
and tortuous,” says Jawas. “The term
commonly refers to the veins on the leg,
although varicose veins can occur elsewhere.”
The veins become so engorged and
painful, he explains, as leg muscles naturally
pump the veins to return blood to the heart,
against the effects of gravity. “When veins
become varicose, the leaflets of the valves no
longer meet properly, and the valves do not
work,” he says. “This allows blood to flow
backwards and they enlarge even more.”
Jawas says that the condition typically
runs in family and they can be made worse in
certain circumstances such as pregnancy and
obesity. “Large varicose veins are more
common in men,” he says. “About 39% of
men and 32% of women develop large
varicose veins.”
For many people, though, varicose veins
are simply something to be suffered.
Unfortunate as they can be for individuals,
they are rarely life-threatening and patients
tend to put up with them. Which partly
explains Jawas’ decision to open a specialized
vein care service – the only one of its type in
the region. “I wanted to open a dedicated
vein care center that addresses the need of
the local and expatriate population in the
UAE, as well as visitors to the country,” he
says. Working in a community with other
specialists was an important factor in
choosing DHCC, he adds. “The best part is
feeling that you are part of a community of
like-minded medical professionals,” he says.
The clinic opened in March of this year
and Jawas hopes it is going some way to raise
awareness about the sort of options people
suffering from varicose veins have. “The
greatest issue has been the lack of patient
and public awareness of vascular problems
and the treatments available to them,” he
says. “Non-surgical treatments include
sclerotherapy, elastic stockings, elevating the
legs, and exercise,” he says. There are

currently considerable surgical developments
in the field, though. “The traditional surgical
treatment has been vein stripping to remove
the affected veins,” he explains. Newer, less
invasive treatments, such as ultrasoundguided foam sclerotherapy, radiofrequency
ablation and endovenous laser ablation, are
however slowly replacing traditional surgical
treatments.”
As well as raising awareness over the
options available to patients, Jawas is also
keen to plan a prominent role in
standardising the level of care available to
individuals. “There is also a great variation in
standards of vascular care… [but] as a
member of the executive committee of the
Gulf Vascular Society that is an issue being
addressed.”

For many people, cosmetic surgery is one of
the great unknowns. Almost everyone will
have elements of their appearance – or
figure – that they are uncomfortable with,
but the decision to go under the knife could
seem like an extreme reaction – after all, the
majority of people will not have experienced
going under anesthetic before, and will no
doubt have plenty of doubts about the
process.
And that is exactly how it should be,
argues Dr Carol Azzam, from Dubai
Healthcare City’s American Academy of
Cosmetic Surgery Hospital. But there is
plenty of information out there, she says, but
patients should know where to look. One of
the most popular procedures at AACSH is
the abdominoplasty and Azzam tells
Healthmatters what patients undergoing the
treatment can expect.
“An abdominoplasty, popularly referred to
as a ‘tummy tuck’ is to be considered when
you present a skin excess or sagging in your
abdomen,” she says. “This procedure is
designed to remove extra fat and skin and
tighten the muscles of your abdominal wall.

The idea is it will create a firmer, flatter and
smoother abdominal profile that you couldn’t
achieve with diet and exercise.”
So what is the experience actually like for
patients? “You will have to be prepared for
the surgery and will have received clear
instructions and some tests done prior to it,”
she explains. “Usually performed under
general anesthesia (you sleep totally), the
procedure requires a horizontal incision, at
the pubic hairline level, and a perumbilical
one (inside the navel). This access will allow
repairing the weakened muscles, and
removing the excess of fat and skin. Several
layers of sutures will then close the skin
incisions and a special dressing will be
applied together with a binder.”
It might sound extremely painful, but
Azzam insists the anesthetic will do its job.
And after that, it is only a relatively short
recovery period before you start getting used
to your new stomach. “Your surgeon will

smoking for at least a week.”
But no-one should expect the
abdominoplasty procedure to be a magic
bullet. “Your surgeon will make sure that you
have realistic expectations and will explain in
detail the possible risks associated with the
surgery,” explains Azzam. “Knowing that,
you’ll decide yourself if the benefits of the
procedure will balance the potential
complications.”
Indeed, as a procedure, it can literally
change the shape of your body overnight, but
its long term results owe more to your
attitude than the surgeon’s knife. “You’ll need
to make sure to eat well-balanced meals, as
proper nutrition plays a key role in the
healing process,” she says. “But provided that
you maintain a stable weight and general
fitness, the results are long-lasting. Your
slimmer and smoother abdominal contour
will also better balance your body type.”
But as with all aesthetic surgery, its success
is ultimately relative. “Be happy with your
self-image and you’ll more likely be
self-confident, effective in work and social
situations, and comfortable in your
relationships,” she says. “Be dissatisfied and
you’ll be self-conscious, inhibited, and less
effective in activities.”

A little effort goes a long way when it comes to brown
rice

Ingredients:

Preparation method:
Marinate chicken with 85g yoghurt and garam masala powder. Heat 1 tbsp tea oil in a saucepan. Add onion and fry until
golden. Remove and keep aside.
Heat the remaining team oil in the same pan. Add garlic, ginger powder, cumin, cardamom and cinnamon.
Sauté for a few seconds and then add the marinated chicken. Sauté for a few minutes and then add red chillies, tomato
paste and fried onion. Sauté for a minute.
Add water and salt and keep cooking until all the water evaporates and chicken is cooked through.
Heat a clay pot. Spoon cooked rice into the bottom and top it with chicken along with all its masala.
Garnish with almond and raisins – mix before serving.
