the NVNA FOUNDATION A Banner Year in Review ANNUAL REPORT 2013 NVNA WORKS PAT ROCH E H OSPICE H OM E A Message from Leadership NVNA and Hospice experienced a very successful 2013 reflected by a number of magnificent events: • The opening of the Pat Roche Hospice Home on Turkey Hill, Hingham – the only hospice residence from Needham to Cape Cod and the first to serve the South Shore • For the 8th straight year, received the Home Care Elite Award™ designating NVNA and Hospice in the top 25% of home care agencies in the nation (8500 agencies participating) and this year within the Top 500 • Continued high Medicare patient outcome measures, most notably, Patient Satisfaction and reduced hospital re-admission scores • Award for Innovation in Patient Care from the Home Care Alliance of MA for Advanced Practice Project in Alzheimer’s Specialty Care • Steady growth with our private services division, NVNA Works • Cancer Support Community continues to flourish in both financial support and program offerings, free of charge to our community members touched by cancer • Strong financials Our work remained focused on reducing unnecessary acute care re-hospitalizations of patients through the use of telemonitoring as a standard of care as well as developing our palliative care program and growing our hospice services. Developing community partnerships with local long term care and assisted living facilities as well as physician groups is an ongoing imperative. Despite a healthcare environment that presents new challenges every single day, NVNA and Hospice continues to be an important provider of post-acute and advanced illness care on the South Shore. Newly developed organization and reimbursement models continue to evolve and include homecare as a major player in improving patient outcomes and decreasing the costs of care. Our involvement with the Visiting Nurses of New England, particularly with education and advocacy, remains a priority as well. We no longer simply treat an illness at home and discharge a patient. Our responsibility is much greater, delivering preventative care and programs to maximize quality of life and enhance wellness. We embrace this new model of care and are confident we will continue to perform at the top of our class leading our patients to better health. We provide care in 27 communities south of Boston with talented and dedicated field clinicians who regularly receive high praise from patients for their expertise and compassionate care. Without the support of a loyal volunteer board of directors, as well as our marketing team, clinical and quality managers, and development and office staff, achieving our clinical and business goals would not be possible. We are pleased to share our year’s accomplishments for 2013 as well as acknowledge the many generous individuals, businesses and foundations that help make our success a reality. We are deeply thankful to all who support our work and our mission. HIGHLIGHTS Hospice Home Meg Doherty honored Top 500 Agencies Innovation Award 2013 Highlights of the Year • Our Hospice House opened in September of 2013, after many years of planning, development, reconstruction and renovation. • CEO Meg Doherty was appointed by Governor Deval Patrick to the Massachusetts Public Health Council. • For the 8th Year in a Row, NVNA and Hospice named to the HomeCare Elite™ this year once again being chosen within the Top 500 Agencies in the Nation!!! NVNA and Hospice is the only Massachusetts agency selected for Home Care Elite every year the award has been given. NVNA WORKS Two new programs were designed to expand upon these specific needs: • At Your Aide Program Provides care when you can’t be there...whether it’s overnight at a loved one’s hospital bedside or day and night during a short-term rehab stay...our trained and certified aides can provide the extra companionship and assistance your loved one may need during difficult times in an unfamiliar setting. • Transitions Program Assists clients transitioning from their present home to a new home (e.g. an assisted living community). Our caregivers meet clients while they are still in their home and spend time with them there getting ready for the move. Once relocated, the caregiver continues to visit in the new setting until the clients become comfortable with their new surroundings.
