One Patient at a Time - Joliet Area Community Hospice
1 History Joliet Area Community Hospice (JACH) has been providing compassionate, professional care to terminally ill patients and their families since 1982. We are a not-for-profit hospice, community based, state licensed and Medicare/Medicaid certified. Over 20,000 patients have received our world class care and service since our incorporation in 1982. JACH is a long term United Way partner of Will and Grundy Counties. Our Mission The mission of Joliet Area Community Hospice is to provide comprehensive, holistic, community based support services and care for the terminally ill persons, their caregivers and loved ones, without regard to economic status, to enable the dying person to live peacefully, in comfort, and with dignity to the last moment of life. From day one, we have continued to deliver the quality, compassionate end-of-life care, one patient at a time, over 20,000 times. A need of the community created the vision and from there we have grown to the community resource that we are today, one patient at a time! In this inaugural Annual Report, we honor all those who worked feverishly to sow the vision, the donors who have nurtured this vision and the many employees and volunteers BOARD OF who have worn the Joliet Area Community Hospice name badge, for making the vision a reality, one patient at a time. DIRECTORS Today Joliet Area Community Hospice operates in a six county “community” serving over 1,200 patients a year, employing 140 professionals, and is privileged to have over 250 volunteers. Over 96% of our employees and 100% of our volunteers live in our service area. They are part of the community, they are neighbors, friends, attend church with you, send their children to school with your children, and shop where you shop. They are tuned into the needs of the community and are very much invested in it. Donald L. Cordano, President George Rydman, Vice President Bill Aeschliman, Treasurer Carolyn Dystrup, Secretary James Galligan Duane Krieger Laura Malinowski In this Annual Report, we will share a few observations, and a few testimonials on the impact this agency has upon the community. As our name suggests, Joliet Area Community Hospice views our relationship with the community as a sacred trust. As your not-for-profit community based hospice, our mandate is to listen to the community and respond to its changing needs. John Mezera Terrence Ruettiger Nicholas Sakellariou Cathy Schley Richard Yudzentis What will the next 30 years bring? Many more challenges, many more successes, of that we are certain. We stand ready to serve in the same compassionate manner as we have done for the past 30 years, one patient at a time! That we promise. Don Cordano Rick Kasper Board President Chief Executive Officer 3 Meeting the Needs of the Communit y One veteran at a time “ Hospice is about shifting mindset. With a focus on quality of life, JACH made it clear that we were making the best choices for my Dad. From admission to Dad’s death the entire JACH staff was fantastic. My family has a warm feeling of gratitude for the Pride, Honor, and Dignity Veteran’s pinning ceremony. We knew Dad was a veteran but discovered so much through this program. It was amazing that hospice recognized Dad for his services to our country EXACTLY 66 years to the day of a battle he fought and won in France,” stated Sharon Sullivan daughter of Anthony Kayfish. “Joliet Area Community Hospice will always be a special part of my family. I will treasure these fond memories. We love and thank you all. ” Sharon Sullivan Woodstock, GA van) n Sulli Kayfish, Sharo ny ho nt A h, is : Joe Kayf (Left to Right 4 One family at a time “ When I hear the name "Joliet Area Community Hospice," my thoughts get immediately flooded with specific memories of those who took care of my family during a very difficult time. My wife Lisa, diagnosed with non-smoker's lung cancer in 2006, was admitted to Joliet Area Community Hospice in 2009. As her condition worsened, the field staff who took care of her at our house, and then the Hospice Home staff, were all fantastic. Brooke, our Hospice Volunteer, instantly connected with Lisa and was a huge help to me. Jody, the (Left to Right: Mark, Matthew Car linsky) Hospice Home Social Worker, spent a great amount of time with our son Matthew (five years old at the time). I felt really good about that, she had such a calming effect on him. Nine days after entering the Hospice Home, Lisa passed away comfortably and with dignity, as was her wish. In addition, Joliet Area Community Hospice, and particularly Mary Ann Burns, the Bereavement Coordinator, supported Matthew and me immensely following Lisa’s death. We've come a long way together over these last three years. I was very pleased to have met and participated with Mary Ann’s new Young Adult support group, recently sharing my experiences and offering my help to those suffering recent losses. Joliet Area Community Hospice means a lot to me and now it is my time to give back. ” Mark Carlinsky Naperville, IL One physician at a time “ Joliet Area Community Hospice, along with the Hospice Home, is an extremely valuable asset to our community. Providing patients and families with comfort, dignity, and warm family support at the end of life is truly a blessing many have benefitted from. I have greatly enjoyed working with Joliet Area Community Hospice and look forward to the same in the future. ” Dr. John M. Walsh Midwest Respiratory, LTD Joliet, IL 5 Volunteers JACH Volunteers recognize that dying is not just a medical event but a personal one as well. They demonstrate that one patient at a time! We have many roles for the volunteers: Vigil, Veteran, Direct Care, Hospice Home, Community Ambassadors, Gardeners, and Administrative Volunteers. There are volunteers that share their expertise with us from making Memorial Bears to baking and photography to name a few. Talents are diverse and yet the common thread remains the same: They are friends and neighbors wanting to help their friends and neighbors and to serve the community. Veteran Volunteer “ After 20 years in the U.S. Army, including two stints in Vietnam, I have developed a sense of understanding of today’s veteran. When I found out about Joliet Area Community Hospice’s Veteran’s Program I jumped at the chance to give back to my fellow veterans. This program recognizes the veteran patients and thanks them for their service by presenting them with certificates of gratitude and a Pride, Honor and Dignity lapel pin. I am honored to be a part of this. The look of gratitude and expressions of joy as these veterans are recognized is very emotional to me. My heart goes out to them and I am moved by the experience. God bless them all. ” Cliff Lauderdale Joliet Area Community Hospice Volunteer Shorewood, IL Mr. Lauderdale also volunteers at the Hospice Home, Provena Saint Joseph Medical Center, Abraham Lincoln National Cemetery, and he is a member of the Shorewood Glen Veteran Group, AmVets, and the American Legion. 6 Vigil Volunteer “ Being a Vigil Volunteer at Joliet Area Community Hospice is a passion for me. Sitting bedside with patients that have no family or no family in the area is especially rewarding. Someone needs to be with these individuals, no one deserves to die alone. JACH offers out of state families a great sense of “peace of mind” knowing that someone is caring for their loved one. Over the years, I have seen the JACH staff offering compassionate care and I have come to learn that it truly does take a team to help a patient through this journey. I have come to consider JACH part of my family. Ted Bellos Joliet Area Community Hospice Volunteer Crest Hill, IL Ted Bellos is a Doctor of Naprapathy, and a Licensed Dietician ” Total volunteers: 263 Volunteer hours: 21,743 Avg. volunteer hours per week: Administrative Volunteer 418 “ I refer to Joliet Area Community Hospice as My Hospice because I feel part of the essence of the place even as a volunteer,” states volunteer Donna Galligan. “I have been volunteering at Joliet Area Community Hospice for over 18 years. During this time it certainly has been a mutually rewarding endeavor. Volunteering here makes me feel good. I started answering telephones to now working with the Memorial Bear Program and various fundraising events for the agency. I always tell people considering volunteering for hospice what a great job the agency does in matching up positions with talent. It seems it is always a good fit. ” Donna Galligan Joliet Area Community Hospice Volunteer Joliet, IL Donna also volunteers as an active member of Childerguild benefitting Silver Cross Hospital 7 Specialty Care.. Meeting the Needs of the Community JACH has diligently and compassionately served our community for 30 years, one patient at a time. JACH has and will always respond to the ever changing needs of the community. The JACH Hospice Home answered the need for a free standing inpatient unit. The JACH Pediatric Hospice and Pediatric Palliative Care Program filled a community void for this much needed specialized care. The Pride, Honor, and Dignity Veteran’s Program met the increased needs of the dying veterans and the unique challenges facing them at end-of-life. Complementary Therapies were found to aid in bringing a calmness and peacefulness to patients and was quickly incorporated into services as a specialty care. Hospice Home The JACH Hospice Home is a home like facility designed to provide short term care to terminally ill patients who are experiencing pain or symptoms related to their terminal illness that is not manageable at home. Hospice patients can also be admitted to the Hospice Home for short term respite care. The Hospice Home has 16 private patient bedrooms, all with private bathrooms, with sleeping accommodations for family members. The Hospice Home also has a family room, living room, dining room, kitchen, spa room, children's playroom and a solace room for patient and family use. Although we understand that there truly is "no place like home" for our patients and their families, we strive to provide the next best thing. PR DE H NOR DIGNITY T H E V E T ER A N ’ S P RO G R A M of Joliet Area Community Hospice 8 Pride, Honor and Dignity Veteran’s Program Joliet Area Community Hospice has been honored to provide services for thousands of veterans since 1982 and now enhances that care with Joliet Area Community Hospice’s Pride, Honor & Dignity Veteran’s Program. This comprehensive program focuses on respectfully celebrating veterans and providing care that recognizes the unique challenges that may exist in military families. This program provides care for those who served with Pride, will Honor their lives and service to country, and offers the Dignity so richly deserved. Pediatric Hospice and Palliative Care The children we care for are aged 0 to 21 years old, who have been diagnosed with an illness or condition that is life limiting or life threatening. Through the use of our two programs, Pediatric Palliative Care and Pediatric Hospice Care, we care for children throughout their disease stage. • Pediatric Palliative Care - The JACH Palliative Care Program provides comprehensive and specialized care for children coping with a life limiting illness. Working as a team, we are committed to providing unique individual care for every child, along with care and support for the entire family. This program is specifically designed for the children undergoing curative treatment for their disease. • Pediatric Hospice Care - The JACH Pediatric Hospice Care Program provides comprehensive holistic, community based support, services and care for terminally ill children, caregivers, and loved ones. We are committed to providing unique individual care for every child to assist them to live peacefully, in comfort, and with dignity to the last moment of life. “Reassurance is what the Joliet Area Community Hospice Pediatric staff gave to my family.” Denise Goyack Plano, IL Complementary Therapies The term complementary medicine is widely used to describe health care and individual therapies that people use as adjuncts to their conventional health care and are understood as complementing conventional medicine. These therapies include: • Music Therapy- Music Therapy involves interaction of therapist, patient and music developing relationships that enhance the quality of life. • Massage Therapy- Massage Therapy is both an art and a science, fostering peace of mind and a sense of well being. It promotes relaxation and satisfies the need for caring nurturing touch. • Pet Therapy- Pet Therapy reduces stress and brings smiles to patient faces. These pets allow the people to focus on something other than themselves. 9 Employer of Choice “ For the past 17 years Joliet Area Community Hospice has been my second home. I began working here as a Field Nurse and moved into my current position about a year before the opening of the Hospice Home. Opening the very first Hospice Home in Illinois was no easy undertaking. At that time Carol DeBenedetti was VP of Operations. We proceeded, with her mentoring, to make our vision a reality with a lot of assistance from many people. Patient and family needs were always the priority and that still remains the constant today. Our work here is very rewarding. The Hospice Home has evolved over the years. Many patients and families have come through these doors and have been served by compassionate caring professionals. I am very proud of our staff and our meeting the needs of the community. Just yesterday a sister of a Hospice Home patient, that had died minutes before, asked staff for the name of the administrator. I identified myself and was told what a wonderful compassionate staff we have here at the Hospice Home and how they were so caring and thoughtful to her brother and his family. “I just wanted to thank whoever hired them.” That is what makes me look forward to working here every day. ” Lori Senior, RN Director of Hospice Home Lori has been with JACH for 17 years. Wilmington, IL “ I am very proud to be a Joliet Area Community Hospice employee. We offer great care, for families and patients, in a very compassionate and very consistent manner. It truly is an agency that is family and goal oriented and a great place to work. ” Bonnie Leto, LSW Social Worker Bonnie has been with JACH for 5 years. Plainfield, IL 10 “ I feel that being an employee at Joliet Area Community Hospice fills a need not only for the community but for me. I enjoy going to work on a daily basis because I know, in my heart, that I can help families during their difficult times as I was once helped by hospice. I work with very talented, dedicated, and compassionate co-workers. ” Kathy Brockhaus Intake Coordinator Kathy B, as she is known, has been with JACH for 9 years. Employees: 140 Employees living in service area: 96% Employees with 5 years or more service: 41% Plainfield, IL “ After 22 years at another job, I came to Joliet Area Community Hospice and have never looked back. I thoroughly enjoy working side by side with compassionate co-workers, helping one another as a team but more importantly, helping patients and families get through these difficult times. ” Greta Jones, RN Greta has been with JACH for 12 years. Joliet, IL 11 Community Partners Lights of Love The Rialto Square Theatre, Joliet IL As part of the Rialto Square Theatre’s Festival of Trees, the Lights of Love Memorial Service drew almost 1,000 people on the eve of Thanksgiving. “ The Rialto’s Festival of Trees is a Holiday tradition in the Joliet area, allowing families to begin their Holiday season with family and friends in a beautiful setting with all sorts of special events throughout the week. The Hospice Lights of Love Memorial Service has been a wonderful addition to the Festival for the past 2 years now. It’s such a natural fit, being the night before Thanksgiving as families are gathering to celebrate each other as well as remembering those who are no longer with us. The Rialto Stage has many wonderful stories hidden behind its curtain, now it has also felt the love of hundreds of Hospice’s families. We are already looking forward to next year! ” Mary Beth Clausen Director of Development, Rialto Square Theatre Joliet, IL 12 Journey Children’s Grief Camp “ Hosting Joliet Area Community Hospice’s Journey Children’s Camp was a great experience for the Greater Joliet Area YMCA. We are very excited to continue the partnership and cannot wait for the 2012 camp to begin. ” Meg Schroeder Youth and Family Director Jerald S. Galowich Family YMCA Joliet, IL Veterans Breakfast Held on November 10th in celebration of Veterans Day, veterans throughout the service area were honored. Attendees included veterans in area health care centers from Marian Village in Homer Glen, The Park in Plainfield, Paradise Park in New Lenox, Rosewood Care Center, Our Lady of Angels and Sunny Hill Nursing Home of Will County in Joliet, and Walnut Grove in Morris. Abraham Lincoln National Cemetery Memorial Squad and the John Whiteside Ceremonial Color Guard presented the colors. Over 115 veterans were recognized for their service! “ Your recent JACH Veterans Day Breakfast was a huge hit with our veteran residents in attendance. They returned here to Rosewood Care Center eagerly showing off their American Flag Pins and could not stop talking about the ceremony and the honor bestowed upon them. Thank you so much. You really made their day! ” William Matjasich Rosewood Care Center, Administrator Joliet, IL 13 Building for the Future One Patient at a Time For 30 years Joliet Area Community Hospice has grown to become a vital resource to the community. As a not-for-profit hospice, your support has helped us achieve that status. Through your help we pioneered the first free standing inpatient unit in the State of Illinois meeting a growing community need. Your continued support will help us meet the ever changing needs of the community for the next 30 years and beyond. We envision many new programs, services, facilities and funds to meet the challenges of the future. Some may come to fruition in a year or two, others are dreams of a decade or two out; but all are being discussed, studied, and prioritized right now in some shape or fashion. Many may not be deemed feasible or needed at the time and we are certain that others will replace a few of them; but we continue to look at growing Joliet Area Community Hospice one patient at a time.
