Magnet designation from the American Nurses Credentialing Center is the most prestigious distinction a health care organization can receive for nursing excellence and high quality patient care. 2 Letter From the President, CEO In many ways 2012 was a year that at once tested our organization’s determination and affirmed the unshakeable foundation of our commitment to caring for our community. Our clinical staff helped to uncover an individual whose alleged criminal diversion of narcotics incomprehensibly may have placed patients entrusted to us for their care at risk, and whom we later discovered may have undertaken similar activity in other hospitals across multiple states, possibly affecting even more patients. Despite the challenges arising from this event, the staff of the patient care affiliates of Exeter Health Resources Exeter Hospital, Core Physicians and Rockingham VNA and Hospice - maintained their deep, singular focus on providing care to thousands of people in need. That exemplary level of patient care was recognized when Exeter Hospital received the Healthgrades Distinguished Hospital Award for Clinical Excellence in 2012 and again in 2013. This recognition places Exeter among the top five percent of more than 4,500 hospitals nationwide assessed by Healthgrades. Our staff’s skill, compassion and expertise were again affirmed as Exeter Hospital completed the pursuit of its long-term goal of earning Magnet Hospital designation from the American Nurses Credentialing Center – an achievement that has only been accomplished by approximately seven percent of hospitals nationwide. Those efforts were rewarded when Exeter Hospital was officially designated as a Magnet Hospital – one of four such designated hospitals in New Hampshire. Exeter’s reputation for excellence also helped our organization form new relationships that further provide our community with access to the very best available advanced treatment options. Most notably, Exeter Hospital laid the groundwork in 2012 to further expand its relationship with the Massachusetts General Hospital Cancer Center, bringing MGH Cancer Center oncologists and hematologists to Exeter’s Center for Cancer Care. The expanded relationship builds upon the existing radiation oncology services provided at Exeter by MGH Cancer Center physicians, further enhancing the center’s capabilities to provide a level of care not normally found in a community setting. This past year brought numerous achievements as well for Core Physicians. During this time period, Core expanded its primary care physician services as well as its specialty physician services, including gastroenterology and general surgery. Further, in its ongoing efforts to more effectively deliver primary care, Core Physicians continued its development and expansion of the “medical home” model of primary care in its many ambulatory office locations. As a health system, Core Physicians, Exeter Hospital and Rockingham VNA & Hospice worked together to enhance the management of chronic diseases, focusing in particular on the care of diabetic and heart failure patients. This work involved deep collaboration among many disciplines covering various settings of care (hospital, physician office, home) and contributed to improved patient care. The nation continued to experience powerful forces of change across many different fronts in 2012 – economic, political and no less so, in the delivery of health care services. The ongoing implementation of national health care reform (“The Affordable Care Act”) influenced many different components of the nation’s health care delivery system with underlying themes of efficiency, affordability, improved quality and equitable access for all citizens. There is no question that as a country we can and must do better in the creation of a sustainable health care delivery system that benefits all citizens equally. For Exeter Health Resources our role in that effort is no less important to the communities we serve in the Seacoast Region of New Hampshire. Our success in that endeavor is significantly tied to the capabilities and commitment of our staff and this was never more true than in 2012. With their passion and expertise, we continue to further our effort to deliver efficient, integrated, patient centered care grounded in a tradition of innovation, high-quality and equitable access for those that we serve. We’ve become friends.” Addie Griset, Reception 7 ANNUAL REPORT 2012 For the fifth consecutive year, Exeter Health Resources was named one of the nation’s Most Wired health care organizations for its use of information technology. Exeter Health Resources received the highest level of recognition in the nationwide Most Wired ranking. The Most Wired survey focuses on how the nation’s hospitals use technology for quality, customer service, public health and safety, business processes and workforce issues. 8 “It makes a difference and you hold your head up a little higher at the end of the day when you know someone’s life might be better because of your care.” Rebecca Davis, RPh, Pharmacy 9 ANNUAL REPORT 2012 Epping Regional Health Center, scheduled to open in April 2013, will improve the greater Epping community’s access to health care. The new, 25,650 square-foot facility is directly aligned with Exeter Health Resources’ mission to continue providing innovative, progressive, patient-centered care to the communities we serve. Medical services to be offered in the new, eco-friendly center include family medicine, pediatrics, adult and pediatric rehabilitation, x-rays, laboratory services, and mobile digital mammography. 10 Community Benefits Report 2012 11 ANNUAL REPORT 2012 A NETWORK OF CARING 12 The mission of Exeter Health Resources and its affiliates is to improve the health of the community. This mission will be principally accomplished without compromising Exeter Health Resources’ sustainability, by supporting the provision of health services and information to the community by the affiliated companies of Exeter Health Resources. Throughout FY 2012 Exeter Hospital, Core Physicians, Exeter Healthcare and Rockingham VNA & Hospice have continued the pursuit of this mission and provided $30,582,651 in charity care and other community benefit programs and services to communities in the areas served. Since the inception of the community benefit reporting requirement in FY 2000, the affiliates of Exeter Health Resources have provided a total of $233,601,079 in community benefits. This addendum includes some of the affiliates’ many community benefit services and programs that address identified needs. 13 ANNUAL REPORT 2012 IMPROVING ACCESS TO CARE 14 Financial Assistance — Exeter Hospital has three components to its health care access program: – The Uninsured Care Discount/Hospital Access Plus Program extends a 26 percent discount off total charges to patients. – The Financial Assistance Program is a community-based program for uninsured and underinsured patients who meet specific income, geographical and other guidelines, and who do not otherwise qualify for any state or federal assistance. – Exeter Hospital’s Catastrophic Care Program provides financial relief for those patients who do not qualify for the Financial Assistance Program, but who are faced with a substantial debt due to a serious illness or injury. This program is calculated based on a percentage of the patient’s gross income. The operating affiliates actively promote these programs through direct patient education, paid advertising and community partnerships. Exeter Hospital The hospital provided $4,823,178 (calculated at cost) in charity care during FY 2012. In FY 2012, the charity care program served 2,173 people. Core Physicians The physician group offers a financial assistance program for patients who meet certain geographic and financial criteria. In FY 2012, Core provided $1,825,777 (calculated at cost) in charity care. Women’s Wellness Clinic: This clinic provides uninsured and underinsured women with recommended breast and cervical cancer screenings. In FY 2012, the Women’s Wellness Clinic served 77 women and Exeter Hospital provided $60,344 to continue this initiative. 15 ANNUAL REPORT 2012 OUR COMMUNITY PARTNERS 16 The operating affiliates of Exeter Health Resources work collaboratively with local non-profit agencies and organizations that strive to improve the health of the community. These relationships include: Lamprey Health Care: In FY 2012, Exeter Hospital continued its financial support of Lamprey Health Care with a community benefit grant in the amount of $440,000. Lamprey Health Care is New Hampshire’s oldest and largest private, non-profit community health center with locations in Newmarket, Raymond and Nashua. The organization serves over 16,500 patients of all ages each year. As a community health center, it provides primary care and health-related services to all individuals and families, regardless of insurance status or ability to pay. By focusing on prevention and health care management, Lamprey Health Care strives to keep insured and uninsured patients healthy. SeaCare Health Services: Exeter Hospital supported SeaCare Health Services in the amount of $267,750. SeaCare is the oldest volunteer provider network presently functioning in the U.S. Organized in 1989, SeaCare initially provided primary care for children in Portsmouth, but expanded in 1994 to include primary and specialty health care coordination services for people of all ages in 21 towns in the Seacoast Region. Families First Health and Support Center: In FY 2012, Exeter Hospital made a financial contribution to Families First in the amount of $35,000. Families First is a community health center offering a wide variety of health services and programs including primary care, prenatal care, dental care and mobile health care for the homeless. New Outlook Teen Center: Exeter Hospital continued its support of New Outlook Teen Center in the amount of $25,000. New Outlook is a recreational, educational and prevention-oriented program, located in Exeter, NH, offering after-school and summer adventure program settings for middle and high school youths. New Outlook serves more than 350 youths in the towns of Brentwood, East Kingston, Exeter, Kensington, Newfields and Stratham. 17 ANNUAL REPORT 2012 ADDITIONAL COMMUNITY PROGRAMS AND SERVICES 18 Seacoast Mental Health Center: Exeter Hospital partners with Seacoast Mental Health to offer mental health services to patients and their caregivers in the Emergency Department and Center for Cancer Care. In FY 2012, Exeter helped to underwrite mental health services in the amount of $386,589. Dental Care Access: During FY 2012, Core Physicians’ mobile dental van provided subsidized dental services in the amount of $120,446. The school-based Mobile Dental Program continued to provide pediatric oral health care to children in grades K-5 in 18 towns throughout the greater Exeter area. Services provided during the past year included oral screenings, dental examinations, radiographs (x-rays), preventive sealants, cleanings, restorations, oral health education and care coordination. During FY 2012, 5,327 children received education through the school mobile dental program; 555 children received an oral screening; 36 children received an examination and x-rays by a dentist; and 90 children received a dental cleaning. Transportation: Exeter Hospital’s transportation program is an important health care support service provided in response to an identified community need. Each year the program enhances access for hundreds of patients who otherwise would not be able to obtain needed health care and health-related support services. During FY 2012, Exeter Hospital provided 2,411 transports at a cost of $139,973. 19 ANNUAL REPORT 2012 PROFESSIONAL EDUCATION AND RESEARCH 20 Health Professions Education: The operating affiliates of Exeter Health Resources continue to support professional educational opportunities for those pursuing a future in health care. Through the Health Professions Education Program, students in various stages of their academic careers benefit from the experience of working with professional clinical staff. An agreement with DartmouthHitchcock Medical School allows medical students to pursue internships through Core Physicians’ Exeter Pediatrics practice. A partnership with the Seacoast School of Technology allows high school juniors and seniors to explore future health career options available to them. Each year a large number of nursing students are given the opportunity to work alongside the excellent clinical staff of Exeter Hospital, Core Physicians and Rockingham VNA & Hospice. During FY 2012, 159 students spent a total of 11,443 hours participating in training and internship programs provided by the affiliates of Exeter Health Resources at a total of $661,411. Research: The Center for Cancer Care at Exeter Hospital participates in several national research groups sponsored by the National Cancer Institute, which enables the center to offer clinical trials to patients undergoing treatment at Exeter Hospital. These offerings allow patients to voluntarily take part in leading-edge research that does not necessitate travel outside of the Seacoast area. During FY 2012, Exeter Hospital provided $63,383 for clinical trials and research that served 260 patients. 21 ANNUAL REPORT 2012 GIVING BACK TO THE COMMUNITY 22 The Affiliates of Exeter Health Resources: Exeter Hospital, Exeter Healthcare, Core Physicians and Rockingham Visiting Nurses Association & Hospice provided $30,582,651 in charity care and other community benefit programs and services to communities in the areas it serves during FY 2012. Over the last 12 years, Exeter Hospital and its affiliates have provided a total of $233,601,079 in community benefit programs and services. The second year of the Medicaid Enhancement Tax of $9.9 million and the hepatitis C outbreak negatively impacted the financial performance of the corporation. Exeter Health Resources reported consolidated operating revenues and other support of $267.5 million which is a $4.7 million or 1.7% decrease over the prior fiscal year. Gross revenue generated by Exeter Hospital increased $11.9 million or 3.2% predominately related to growth in outpatient ancillary services, particularly in the pharmacy, oncology, rehabilitation, CT scan and operating room areas. Gross revenue generated by Core Physicians increased $5.2 million or 5.3% due in part to growth in anesthesia, gastroenterology, orthopedics and urology services. While consolidated gross revenues increased 2% from FY 2011 to FY 2012, deductions from revenue increased $15.2 million or 6% to dampen the overall organization’s profitability. Primary contributors to this decline were overall reimbursement cuts particularly from some commercial and governmental payers. Exeter Hospital successfully qualified for the second year of Phase I Electronic Health Records (EHR) incentive program from the Centers of Medicare and Medicaid Services (CMS) during FY 2012. This program provides a financial incentive for the meaningful use of certified EHR technology to achieve health and efficiency goals. This significant organizational effort resulted in over $1.4 million in revenue to the organization. Consolidated operating expenses stayed flat increasing only $411,000 or 0.1% over 2011 levels. Supplies and other operating expenses increased nearly $1.5 million, but salary and employee benefit expenses stayed constant increasing $109,000 from FY 2011 to FY 2012 as the organization worked diligently to keep staffing in line with changing volumes and successfully controlled benefit costs. In addition, those increases were offset by combined decreases to depreciation and interest expenses of $1.7 million. Exeter Health Resources continued its commitment to the community by providing $13.5 million in charity care in 2012 which is consistent with 2011 levels. In addition, the organization provided approximately $30.6 million of community benefits, which included the costs of offering numerous community service programs, subsidized health services, providing direct cash contributions to other community agencies which support the mission of the organization and the costs of Medicaid services provided to patients that exceeded reimbursement which is an increase of $1.5 million or 5.3%. Exeter Health Resources incurred a $9.3 million loss from operations in FY 2012; however, this was completely offset by $12.3 million net non-operating gains. The most notable increase to net non-operating gains was favorable mark to market adjustments for securities held in our investment portfolio which resulted in net gains of $13.6 million versus net losses of $5.7 million in 2011. This favorable increase was slightly tempered by a $2.7 million decrease in realized gains on investments sold in 2012. Despite the challenging year, the overall liquidity of the organization remained strong as days cash on hand for 2012 was 264, which is an increase of 25 days over 2011. Total cash, short-term investments and investments increased approximately $19.8 million to $220 million, while the long-term debt-to-equity ratio favorably decreased from 25.9% to 23.6%. Although Exeter Hospital’s credit rating from Moody’s was changed from A2 to A3 during the fiscal year, it still retains one of the higher hospital ratings within New Hampshire. 24 Exeter Health Resources, Inc.
