AT MILLHON CLINIC, WE UNDERSTAND THAT IT CAN BE OVERWHELMING TO LEARN YOU HAVE BEEN DIAGNOSED WITH A CHRONIC ILLNESS OR DISEASE. YOU LIKELY HAVE A LOT OF QUESTIONS ABOUT HOW TO MANAGE YOUR HEALTH, FEEL YOUR BEST AND GET THE MOST OUT OF LIFE. + Private physician visit, including: + brief, private medical exam + any needed tests will be ordered + prescription management + ask questions of your physician or discuss other health issues or concerns What is a Shared Medical Appointment (SMA)? The SMA is an interactive visit that includes both a private medical appointment with your Millhon Clinic physician and a group education and support session with other patients who have your same illness or disease. + T hese convenient 90-minute visits are scheduled once a month for six months and each focus on a different educational topic. + F or most SMA visits, you may bring a family member or friend who is involved with your care. + If you have private medical concerns, you may discuss those with your healthcare provider in an exam room. + T he SMA is voluntary and is designed to enhance, but not replace, your regular office visit. + Y our Shared Medical Appointment is no extra charge and will be billed to your health insurance plan the same as a normal physician visit. You will be asked for a co-payment, if required by your health plan. How will I benefit from a SMA?
