Florida Chapter Best Care Practices in the Geriatrics Continuum Annual Program Description This program is designed to provide a review and update of major geriatric diseases, illnesses, and risks found in nursing home patients, residents of assisted living facilities, and seniors living at home. This year’s conference will have an underlying theme of acute medical emergencies with a focus on delirium, mental health in older adults, management of persistent pain, acute cardiac emergencies, dehydration, etc. Other topics being covered include the impact of improved communications on transitions of care and the reduction of avoidable hospitalizations, Medicare reimbursement and billing, falls prevention, endocrinology, reducing patient cavities in SNF, annual national leadership forum, POLST, estimating prognoses, plus an AMDA intensive on the role of pain management in LTC from barriers to its legal aspects. Target Audience This conference is designed to educate physicians, physician assistants, nurse practitioners, directors of nursing in LTC, registered nurses, senior care pharmacists, consultant pharmacists, and long-term care administrators, as well as geriatricians, primary care and home care physicians, physicians considering becoming long-term care or home care medical directors, and others with an interest in geriatrics and its continuum of care. The faculty includes national and regional authorities in the fields of medical direction, senior care pharmacology, as well as long-term care and geriatric medicine. OVERVIEW: Statistics suggest that as many as 80% of elderly adults have pain. Pain is a common and major problem among nursing home residents. Unfortunately, the long-term care setting, and the systems within and without may contribute to under-diagnosis and under-treatment of pain in elderly patients. Perceived barriers to providing adequate pain management included lack of knowledge about pain management among nurses and physicians, lack of a standardized approach to treating pain, lack of diagnostic precision in treating pain, and difficulty in not only choosing but having access to the right analgesic. These issues create not only barriers in achieving appropriate goals for pain management but create legal issues as well. This session will present an overview of current legislative and regulatory initiatives, discuss the barriers and practical ways to deal with those barriers and then discuss the legal issues surrounding pain management in this setting. COURSE OBJECTIVES: 1. Assess the barriers and challenges to pain recognition and management in the elderly and comprehend the factors and causes associated with pain and pain experienced at the end-of-life. 2. Evaluate the principles of pain management in the environment of difficult access and discuss issues around transitions of care and pain management. 3. Discuss how to view facility quality assurance activities to assess how well pain is managed and how pain management issues affect facility Quality Measure Profiles. 4. Understand the fundamentals of regulatory and legal requirements as well as risk management in pain management.
