parking is available. We are a short walk north from
the Red Line on Clybourn.

The program is a discount program, not a dental
insurance plan.
It Cannot Be Used:
The Annual Program Fees are based on family members
living in the same household.
With our Dental Discount Plan there are:

• In conjunction with another other form of dental

• For injuries covered under any other insurance plan
• For treatment, which, in sole opinion of the treating
dentist, lies outside the realm of their capability
• For referrals to specialists
• For hospitalization or hospital charges of any kind
• Immediate eligibility
• At any other dental office

• This plan is only honored at Pure Design Dentistry
• Discounts cannot be used in conjunction with any
other dental plan or discount program
• These fees are Non-Refundable
• No refunds offered for underutilization of the

• Patient’s portion of bill is due day of service
