Playtex Infant Carrier
The Hip Hammock
The Playtex Hip Hammock is an infant carrier
designed to strap the baby against the
caregiver’s body at the hip. It is made of a soft,
quilted fabric and intended for babies that are
from 15 to 35 pounds. The child seat is attached
with straps that wrap around the carrier’s hips
and shoulder.
“Playtex Hip Hammock” is printed on a label
sewn into the front of the carrier. All model
numbers are being recalled. Model numbers
05300, 05301, 05302, 05306, 05307 and 05308 are
sewn into the inside panel below the instructions
for use. They come in basic and deluxe styles.
The hip hammock’s fabric is suede or ultrasuede in black or navy colors on the outside,
and the inside lining is black, black and white
check or burgundy.
305,000 of the carriers have been recalled in the US, and 36,000 in Canada. They were
sold at Burlington Coat Factory, Target, Walmart, juvenile product, baby and discount
stores nationwide and online at Amazon.com from June 2004 through December 2008
and January 2010 in Canada for about $40 for the basic model and $60 for the deluxe
model.
Reason for the Recall
The buckles on the waist and shoulder straps can crack or break, creating a fall hazard.
Playtex has received 87 reports of the buckles cracking or breaking, including two reports
of injuries, where one infant required emergency room treatment.

cubetensils Spoons & Forks
This recall involves Edoche’s Cubetensils sets of children’s eating utensils. Each set consists
of a spoon and a matching fork with stainless steel heads and plastic
handles. The handle has three segments that rotate to create design
combinations.
The utensils are 5.75-inches long and were sold in seven different
models/designs: alphabet, moon and stars, nature, princess, rockets,
trucks and veggies. The sets come in a brown cardboard box with a
clear viewing window in the front.

“Edoche” is printed on the bottom of the utensils
and “Stainless Steel China” is embossed on the
metal heads. The phrase “Not for children under
3” appears on the back of the product’s
packaging.
They were sold at major retailers and specialty
stores nationwide and on Amazon.com and
ebay.com from May 2012 through November 2013
for about $8.
Reason for the Recall
The utensil’s handle can come off, creating a
choking hazard. Edoche has received one report
of the utensil’s handle detaching and a baby
putting it in his/her mouth. No injuries have been
reported.

Joovy Car Seat ADapter
for Graco, Chicco, & Peg Perego Strollers
The recalled adaptors are made out of gray metal with black plastic clips designed to
attach infant car seats to stroller frames. They are about 17” x 13” by 10”. The recalled
models are 00945 for Graco, 00946 for Chicco and 00947 for Peg Perego frames. “Joovy”
and the model numbers can be found on the label at the center of the end bar of the
adapter.

The adapters were sold at specialty juvenile retailers and online at Joovy.com between
May 2012 and August 2013 for around $25.
Reason for the Recall
The adapter clips can loosen on the stroller frame, creating a fall hazard. There have
been 9 reports of loose adapters, but fortunately no injuries have been reported.

Fred & Friends Pacifiers
Mustache, Volume, & Panic Button
This recall involves three styles of Fred & Friends Chill Baby pacifiers, including the Artiste
with a black plastic beard and mustache, Volume with a black volume control knob and
Panic with a red panic button. The pacifier’s name and UPC are printed on the
packaging. The Artiste’s UPC is 728987021282, the Volume style’s UPC is 728987020599
and the Panic style’s UPC is 728987020605. The pacifiers are plastic and silicone rubber
and measure about 2 inches wide and 1½ inches tall. They were sold pretty much
everywhere from April 2013 through December 2013 for about $10.
Reason for the Recall
The pacifier doesn’t meet federal safety standards. The beard on the Artiste (mustache)
and the knob on the Volume can detach, creating a choking hazard. Also, the
ventilation holes on the Volume and Panic Button pacifers are too small. There has been
one report of the knob on the volume pacifier detaching, but no injuries were reported.

Britax Strollers
B-Agile, B-Agile Double, & BOB Motion
This recall involves Britax B-Agile, B-Agile Double and BOB Motion strollers. The single and
double strollers were sold in various color schemes, including black, red, kiwi, sandstone,
navy and orange.
They were manufactured between March 2011 and June 2013 and have the following
model numbers: U341763, U341764, U341782 and U341783 for the B-Agile strollers; U361818
or U361819 for the B-Agile Double strollers; and U391820, U391821 and U391822 for the BOB
Motion strollers. The model number and the manufacture date in YYYY/MM/DD format
can be found on label located on the inside of the stroller’s metal frame near the right
rear wheel.
They were sold in stores and online from May 2011 through June 2013 for between $250
and $450.
Reason for the Recall
Brace for Nastiness: the hinge on the stroller’s folding mechanism can partially amputate
fingertips, break fingers, or cause severe lacerations, among other injuries, when they
press that release button while pulling on the release strap.
There have been 8 reports of incidents: including one partial finger amputation, one
broken finger, and several severe finger lacerations.

Graco Car Seats
Booster & Convertible Car Seats
Graco has voluntarily decided to recall 3.7 million baby convertible car seats and
boosters from the marketplace. Because they are doing this voluntarily, you won’t find
this announcement on the CPSC.gov website.
Which car seats are affected? The convertible car seats and harnessed booster seats
made from 2009 through July of 2013.
If you own a Graco SnugRide infant car seat, you are not impacted.
The products impacted by this recall are: Cozy Cline, Comfort Sport, Classic Ride 50, My
Ride 65, My Ride 70, My Ride 65 with Safety Surround, Size4Me 70, My Size 70, Head Wise
70, Smart Seat, Nautilus 3-in-1, Nautilus Elite and Argos.
Reason for the Recall
After extensive testing, they discovered that dried liquids and foods can make the
harnesses of the car seats much more difficult to unlatch.
The current buckle is safe to use. This is just to make the buckle easier to unlatch over
time.

To clean your buckle, turn the restraint over and push the retainer through the harness
strap slot. Place the buckle in a cup of warm water and gently agitate the buckle,
pressing the red button several times while it is in the water.

Do not submerge the harness webbing and do not use soaps or lubricants, only rinse the
harness buckle with warm water.

Shake out the excess water and allow the harness buckle to air dry.

Reattach the harness buckle into the same slot and re-check harness for correct
installation according to the car seat manual before use

Baby Walkers
The recalled includes all BebeLove walkers that were sold for babies age 6 months or older.
The walkers contain a plastic- covered foam padded seat with a plastic base and toy tray.
Model 358 walkers have a green, pink or orange musical tray with a white toy bar and solid
colored seats. Model 368 walkers have green, pink or white musical tray with a yellow toy
bar and printed patterned seats. Both models have white stoppers on the bottom of the
base of the walker and model numbers printed on a label on the rear bottom inside of the
base.“BEBELOVE" is printed on a label on the seat back and on the base of the walkers.

doorway and doesn’t have auto-stop features to prevent falling down stairs. Model #368

Ducky, Blowfish, & Splat
This recall involves miniature office and refrigerator magnets sold in the shape of a duck,
blowfish and a splat. A small magnet is affixed to the underside of the brightly colored plastic
objects which were sold in sets of four or six. Model number 3205121 (duck), 993205114
(duck), 3205122 (blowfish) or 3205078 (splat) is printed on the bottom of the packaging.
“Magnets” and the Design Ideas’ logo are printed on the front of the package.
Rubber ducky magnets were sold at Nordstrom’s Rack stores, novelty and gift stores, book
stores and art stores nationwide from March 2007 through September 2013 for about $10.
Blowfish magnets were sold at novelty and gift stores, book stores and art stores nationwide
from March 2007 through March 2011 for about $10. Splat magnets were sold at CB2 stores,
novelty and gift stores, office supply stores and art stores nationwide from November 2012 to
February 2014 for about $10.

The small magnets come off easily. Besides being a choking hazard, if ingested, magnets
can link together inside a child’s intestines and cause serious obstructions with potentially lifethreatening results.

Vera Bradley Toys
Bear Rattle & Stuffed Bunny
This recall involves all Vera Bradley Bear Ring Rattles and Bunny stuffed animal toys. The
products are made of cotton and fleece. The bear ring rattle has a white teddy bear head,
arms attached to an O-shaped body with a green, blue, brown and pink crisscross pattern
design rattle. The bear ring rattle measures about 4.25 inches in diameter.
The bunny is 10 inches tall from the top of its head to the bottom of its foot and was sold in
three printed patterns. The “Bunny in Lilli Bell” has green vines with pink and orange flowers
on the body, limbs and the back of the ears. The “Bunny in Lola” has a crisscross geometric
pattern on the arms, legs and ears of the white headed bunny with a floral print body. The
“Bunny in Tutti Frutti” has a green with a pink and yellow floral printed pattern fabric covering
whole body. All of the recalled rattles and bunnies have a white pom-pom tail on the back

Ganze Stuffed Toys
Grumpy Cat
This recall involves three styles of plush Grumpy Cat stuffed animal toys by Ganz. They include
an 8-inch Grumpy Cat in laying position, a 5-inch long sitting Grumpy Cat and a 4-inch
Grumpy Cat key clip. The toys are multi-colored in white with dark brown, light brown and
gray fur material. Grumpy Cat toys have blue crystal eyes with the eye lids half closed, a
down-turned mouth and white whiskers. This toy is labeled for ages 3 and older. The Grumpy
Cat key chain has a black plastic key clip at the top of the cat’s head. The recalled toys
have batch numbers 86754 or 224861, and model numbers printed on the sewn-in label
located near the tail of the cat.

Walmart Baby Doll
My Sweet Baby
The My Sweet Love / My Sweet Baby electronic baby doll comes in pink floral clothing and
matching knit hat. The 16 inch doll is packaged with a toy medical check-up kit including a
stethoscope, feeding spoon, thermometer and syringe. The doll’s electronics cause her to
babble when she gets “sick,” her cheeks turn red and she starts coughing. Using the medical
kit pieces cause the symptoms to stop. “My Sweet Baby” is printed on the front of the clear
plastic and cardboard packaging. The doll is identified by UPC 6-04576-16800-5 and a date
code which begins with WM. The date code is printed on the stuffed article label sewn into
the bottom of the doll.
The dolls were exclusively sold at Walmart from August 2012 through March 2014 for $20.
Reason for the Recall
The circuit board in the chest of the doll can overheat, causing the surface of the doll to get
really hot and creating a burn hazard.
Walmart has received 12 reports of overheating, 2 of those involved burns or blisters to the
thumb.
What Should You Do?
1. Immediately take away the defective dolls and remove the batteries.
2. Return the doll to any Walmart for a full refund.

Baby Monitor with Sensor Pad

This recall involves two models of the Tommee Tippee Movement and Sound Baby Monitors
with Sensor Pads. The product comes in three parts; the baby unit, parent unit and the
movement sensor pad. The pad is white with a 10 foot cord that runs from the pad to the
baby monitor unit. Both models come with a sensor pad that fits under the baby’s crib
mattress and sounds an alarm if no movement is detected for 20 seconds.

unit. The recalled products include models 1082S and 1094S. Model 1094S includes a nightvision video camera. Model numbers are located on the bottom of the base of the
recharger and also on the bottom of the camera.

$150.
Reason for the Recall
A child can pull the sensor pad cord into the crib and wrap around the neck, creating a
strangulation hazard.

Mason 4-in-1 Convertible Cribs

This recall includes Franklin & Ben Mason style 4-in-1 style cribs model number 5601 that were
manufactured from January 2012 through August 2012. The model number and the date of
manufacture are printed on a label on the inside of a side panel of the crib. Listed on the
label as a “serial number,” the following purchase order numbers are included in the recall:
H004522, H004546, H004548 or H004777.
The cribs have solid wood sideboards and wood rails on the front and back of the crib in
varying heights and can convert to a daybed, toddler bed or full-sized bed. There is a
storage drawer at the base of the crib with two metal knobs. The recalled cribs were sold in
weathered gray and rustic brown, and measure 57 inches long by 33 inches wide by 51
inches high.
The cribs were sold at juvenile specialty retailers from January 2012 – June 2014 for between
$600 and $700.
Reason for the Recall
There have been 14 reports of the front separating from the side panels of the crib. When
that happens, a gap is created between the mattress and the crib. This gap is a serious
safety hazard. Many babies have died when they accidentally get their heads stuck in
between them. It is a suffocation hazard.

Reason for the Recall
The opening between the recalled tray and the stroller seat bottom is too wide. It can allow

“Carta” and “Cotton” Styles
The recalled Livie & Luca children’s shoes include two styles: Carta and Cotton. The Carta
style Mary Jane shoes are canvas, solid color shoes stitched to a tan sole with a fabric strap
and a brown wooden button. The Carta shoes are blue, fuchsia or gray. The Cotton style
Mary Jane canvas shoes have a stitched tan rubber sole and a colored fabric strap with a
dandelion printed on a pink square button and a gingham or polka-dot cotton interior lining.
The cotton shoes are yellow, green or red. Both the Carta and the Cotton style shoes were

The suspension fittings can break, causing a child to fall from the swing. There have been
four reports worldwide including one in Germany, two in Austria and one in Canada of the

precautionary measure, Nutek believed it was a prudent decision to withdraw all its baby
wipe products.
B. cepacia poses little medical risk to healthy people. However, people who have certain
health problems like weakened immune systems or chronic lung diseases, particularly cystic
fibrosis, may be more susceptible to infections with B. cepacia. If you believe you have a
weakened immune system or chronic lung disease and you have used one of the affected
wipe products, you should call your doctor promptly for medical advice.
As of October 3, 2014, the date of the original withdrawal, the company had received only
one report of irritation. Numerous reports of complaints have since been received by the
company that include rash, irritation, infections, fever, gastro-intestinal issues, and respiratory
issues, though these reports have not been confirmed to be related to the use of these
products.
