General Information
Continuing Medical Education
The American Society of Dermatopathology is accredited by the Accreditation
Council for Continuing Medical Education to provide continuing medical education for physicians. The American Society of Dermatopathology designates
this live activity for a maximum of 35 AMA PRA Category 1 Credits™. Physicians
should claim only the credit commensurate with the extent of their participation
in the activity.
The American Society of Dermatopathology Annual Meeting (Program #254100)
is recognized by the American Academy of Dermatology for 35 AAD Recognized Credits and may be used toward the American Academy of Dermatology’s
Continuing Medical Education Award.
Continuing Medical Education Policy
The American Society of Dermatopathology (ASDP) is accredited by the Accreditation Council for Continuing Medical Education (ACCME). As such, we
are required to meet the ACCME’s expectations for our practice of continuing
medical education.
It is the policy of the ASDP to ensure balance, independence, objectivity and
scientific rigor in all of its educational activities. Faculty participating in and planning the Annual Meeting must comply with all procedures governing disclosure.
The ASDP has implemented a process where everyone in a position to control
the content of an educational activity will present evidence-based content,
disclose all relevant financial relationships with any commercial interest and discussion of unlabeled/investigational uses of a commercial product. In addition,
presentations must be free of commercial bias and any information regarding
commercial products/services must be based on scientific methods generally
accepted by the medical community. Conflicts were resolved prior to this educational activity. Faculty who refused to disclose relevant financial relationships
and discussion of unlabeled/investigational uses of a commercial product would
have been disqualified from being a part of the planning and implementation of
this CME activity.
Presenters who qualified for the implementation of this activity are required to
verbally disclose to the audience, immediately prior to the scientific presentation, any of the relationships mentioned above. Faculty relationships are also
disclosed to audiences in this program book.

Meeting Program
CME Claims and Verification of Attendance
New This Year! Attendees will be able to claim their CME credits online via
the ASDP website after the meeting. You must submit your CME credit online
in order to receive appropriate CME credit. The number of CME credit available for each course is indicated by the course title in this program and on the
online CME submission site. An official CME certificate will be available to print
immediately after completing the online CME claim form and evaluation. Paper
certificates will no longer be mailed.
Appropriate credit for attendance should be ascertained and reported by
individual physicians to the particular state or medical society to which he/she
belongs. For those registered in the AAD CME Transcript/Award Program, you
should log in to the AAD Online CME Transcript Program to enter credits for this
AAD Recognized program. ASDP’s AAD program number is 254100.
MOC Claims and Verification
MOC credit for the Self-Assessment in Dermatopathology sessions will not be
included on the Annual Meeting CME Certificates. Participants must complete
the post-course online self-assessment with a 50 percent pass rate or better in
order to claim MOC credit for Self-Assessment in Dermatopathology. A MOC
certificate will be available to print upon successful completion of the postcourse online self-assessment.
MOC credit for the ASDP Quality and Performance Improvement Program will
not be included on the ASDP Annual Meeting CME Certificates. Participants
must complete the follow-up survey six months after completion of the Quality
and Performance Improvement Program at the annual meeting. MOC certificates
will be available to print immediately upon completion of the follow-up survey.

Learning Objectives
The educational mission of the American Society of Dermatopathology is
to teach, aid in the dissemination of knowledge and encourage research to
improve the quality of the practice of dermatopathology. The annual meeting is
primarily an educational one, offering members, fellows, residents and medical
students lectures on current topics in dermatopathology, interactive sessions
at the microscope, a self-assessment course, and opportunities for poster and
abstract presentations.
After attending the 2014 Annual Meeting, participants should be able to:
• Develop overall diagnostic skills in dermatopathology following exposure
to clinical and histopathological aspects of a broad range of dermatologic
diseases.
• Describe recent advances in diagnosing soft tissue pathology and their
impact on the practice of dermatopathology.
• Diagnose a wide range of challenging areas of dermatopathology, such as
the pathology of the nail and hair follicle/alopecia, and develop a practical
approach to handling such cases.
• Evaluate relevance and appropriate utilization of innovative diagnostic
tools in the investigation of inflammatory and neoplastic dermatopathology
cases.
• Identify molecular tests for diagnosis and prognosis of cutaneous neoplasms with emphasis on melanocytic, hematopoietic and soft tissue
tumors.
• Utilize sophisticated technology in the realm of dermatopathology through
an informed and fiscally responsible approach.
• Outline new scientific dermatopathologic endeavors and discoveries, conveyed by junior and senior colleagues involved in research.
• Identify the challenges and rewards of an academic career in dermatopathology through exposure to relevant role models and establishment of
fruitful professional connections.
• Identify national policies relevant to the practice of dermatopathology and
gain awareness of how professionals in the field can influence decision
making.
• Demonstrate an approach toward diagnosis of various challenging skin
biopsies and generate relevant differential diagnoses.

Participants will earn .5 CME credit for each Poster Defense Session attended.
Basic research, practical techniques and new clinical or histopathologic entities
will be presented in this traditional and popular format. Posters will be on display
in two poster sessions Thursday afternoon through Sunday morning. Residents
and fellows-in-training are eligible for the ASDP Physician-in-Training Awards.
Awards will be presented on Sunday morning, 8:30 a.m. – 8:45 a.m., immediately before Short Course IV. See the Program-at-a-Glance for complete Poster
Session 1 and 2 schedules.
Poster viewing opens at 1 p.m. Thursday, November 6, until 11 a.m.
Sunday, November 9.
Poster Defense Sessions
The poster defense sessions will provide an opportunity for discussion between
poster presenters and attendees. Poster presenters are asked to make every
effort to be at their poster during the defense sessions.

The ASDP Slide Library opens Friday, November 7, at 4 p.m. and will be open
24-hours-a-day through Sunday, November 9, at 11 a.m.
Selected cases from the continuously growing slide library will be available for
microscopic examination. Cases are accompanied by the diagnoses and other
relevant written information and references. The 2014 Slide Library handout with
case summaries, diagnoses with discussion and references will be available to
download at www.asdp.org/AM14. Printed handouts will no longer be available.
Upon completion of study, participants should be able to:
• Explain histologic diagnoses of various skin conditions, including the use of
ancillary studies, where appropriate.
• Interpret histologic diagnoses of uncommon skin diseases.
• Critique cases that highlight diagnostic challenges.
• Apply morphological criteria to differential diagnoses of cutaneous lesions.
• Develop appropriate differential diagnoses for a wide spectrum of tumors
and dermatoses.

Dr. Thomas D. Horn attended Davidson College, graduating with High Honors
in Biology, magna cum laude. He completed medical school at the University of
Virginia, AOA. Dermatology training at the University of Maryland was followed
by fellowship training in dermatopathology at Johns Hopkins where he remained
on the faculty, becoming Associate Professor of Dermatology, Pathology and
Oncology as well as Director of the Division of Dermatopathology. He was
appointed Professor of Dermatology and Pathology at the University of Arkansas
for Medical Sciences where he served as chair of Dermatology. After several
years in community practice with a concomitant appointment as Professor of
Dermatology and Pathology at Tufts University, he subsequently joined the
Massachusetts General Hospital as Medical Director of the Mass General
Physicians Organization Dermatopathology Associates with an appointment as
Clinical Professor of Dermatology and Pathology at Harvard Medical School. He
is currently vice chair of the Department of Dermatology at MGH.
Dr. Horn has been continuously involved in the education of dermatopathology
fellows, dermatology residents and pathology residents for the past 26 years. He
helped to establish the training program in dermatopathology at Tufts University.
He has served the ASDP as secretary-treasurer and president. He has held
leadership positions in post-graduate medical education on the resident review
committee for dermatology, chairing the dermatopathology sub-committee as
well as with the American Board of Dermatology as director, associate executive
director and executive director.
The 2014 Founders’ Award will be presented to Dr. Horn at the President’s
Banquet on Friday, Nov. 7.

2014 Walter R. Nickel Award
Omar P. Sangueza, MD
Professor of Pathology and Dermatology and
Director of Dermatopathology
Wake Forest University School of Medicine
Winston Salem, North Carolina
Dr. Omar Sangueza is honored with the Walter
R. Nickel Award for his contributions in teaching
dermatopathology nationally and internationally. He
completed his residency in pathology at Oregon Health Sciences University in
Portland, Oregon, followed by a dermatopathology fellowship under the late Dr.
Bernard Ackerman at New York University Medical Center. He also completed
a one-year fellowship in dermatology at Provincial Hospital in Valencia, Spain.
Dr. Sangueza is board certified in clinical and anatomical pathology and
dermatopathology.
Dr. Sangueza served as assistant professor in dermatology and pathology at
Oregon Health Sciences University from 1992-1994, associate professor in
dermatology and pathology at the Medical College of Georgia from 1994-1999
and professor at the Medical College of Georgia from 1999-2000. In 2000, Dr.
Sangueza joined the faculty as a professor in the departments of dermatology
and pathology at Wake Forest University School of Medicine in Winston-Salem,
North Carolina. Since then he has served as the director of dermatopathology
and director of the dermatopathology fellowship.
Dr. Sangueza has presented at more than 200 meetings regionally, nationally
and internationally. He has authored or coauthored 180 articles, 60 abstracts,
eight book chapters, and two books. He served as the Secretary/Treasurer of
the International Society of Dermatopathology from 2002-2004 and the President
of the International Society of Dermatopathology from 2004-2007. He was
elected to the Board of Directors of the American Society of Dermatopathology
in 2010 and served in that position until 2013. He is the current President of
the International Committee for Dermatopathology and is the current editor in
chief of the American Journal of Dermatopathology. He is also on the editorial
board of several journals. He has served on the advisory test committee
of the American Board of Pathology and Dermatology and international
dermatopathology board examination. Dr. Sangueza has trained 15 ACGME
accredited fellows and more than 40 international fellows from all over the world.
The 2014 Walter R. Nickel Award will be presented to Dr. Sangueza at the
President’s Banquet on Friday, Nov. 7.

Case 1
An 80-year-old man presented with a skin nodule on the forehead.
Case 2
A 12-year-old male presented with a shoulder mass.
Case 3
A 55-year-old man presented with a subcutaneous nodule on the back.
Case 4
A 62-year-old male presented with a 10-day history of a non-pruritic eruption
of 3-15 mm red-brown papules and nodules on the trunk and bilateral upper
extremities with sparing of the face, mucosae, and palms/soles.
Case 5
A 57-year-old male was seen in consultation for suspected mastocytosis. He
complained of a 20-year history of an occasionally pruritic eruption of slightly
scaly erythematous papules and hyperpigmented macules in a reticulate pattern on the trunk and proximal extremities. The lesions on the extremities were
concentrated in flexural surfaces.
Case 6
A 73-year-old female presented with a several year history of friable lesions on
the palate, bilateral nares, and upper lip.
Case 7
A 93-year-old male presented with recurrence of a painless mass on the lateral/
posterior aspect of the right chest wall overlying the ribs. The mass first occurred a little over 2 years ago and has been resected twice. The surgeon’s op
note stated that the mass extended to fascia.
Case 8
A 55-year-old female presents with an erythematous papule on the upper back.
Case 9
A 41-year-old black male presented as a referral from an outside physician for a
2-year history of slowly progressive lower extremity edema that was previously
treated with antibiotics for presumed cellulitis. Physical examination revealed
non-pitting edema with woody induration and mild overlying hyperpigmentation
involving the majority of the right lower extremity and the distal portions of the
left lower extremity. Inguinal lymph nodes were enlarged. Review of systems was
essentially negative.
Case 10
This biopsy from a 1-year-old male was submitted with the clinical differential
diagnosis of Gianotti-Crosti, eczema, scabies, or Langerhans cell histiocytosis.
Case 11
This biopsy is from a 60-year-old female with hair loss.

An 85-year-old man being treated for psoriasis with a variety of medications,
including methotrexate, presented with refractory plaques and new lesions on
the extremities.

A 60-year-old Nigerian male presented with nail plate dystrophy that was present for 11 years and persisted after Mohs surgery.
Case 15
A biopsy was submitted from the right index finger of an 85-year-old woman with
the clinical diagnosis of “wart versus other”.
Case 16
An 18-year-old male presented with multiple firm nodules on the ears, arms,
chest, abdomen and back. He also had gingival hypertrophy and enlargement
of distal phalanges with osteolytic lesions. The patient had multiple surgeries
since childhood to remove similar lesions from the head and neck and extremities. An excision is performed.
Case 18
A 38-year-old male presented with an asymptomatic, slowly enlarging, soft
and mobile subcutaneous nodule that was first noticed about 1 year ago. On
examination, the lesion measures 1.3 cm and has a slightly translucent blue-violaceous hue.

NEW THIS YEAR! This course will focus on selected difficult areas in soft tissue
pathology. The course will predominantly focus on particular clinical groups of
soft tissue tumors. There will be a focus on soft tissue tumors of specific sites:
acral, head and neck, and genital soft tissue tumors, as well as pediatric soft
tissue tumors. The course will also discuss new developments in molecular
diagnosis of soft tissue tumors and the challenging area of myxoid soft tissue
tumors.

Self-Assessment in Dermatopathology

100 MOC SA Question Credits
Be on time – Self-Assessment sessions begin on time. Due to the timed
schedule of each session, doors will close promptly at start time. Late
arrivals will not be permitted. No refunds will be given for sessions missed
due to tardiness. No exceptions.
Participants will have the opportunity to study 50 challenging slides representing
a wide range of inflammatory and neoplastic diseases during one of four identical sessions (A, B, C or D). Subsequently, all participants will meet in a single
Self-Assessment Discussion session in which faculty members will review the
diagnostic features of each case, as well as the differential diagnosis. You must
be registered for one of the Thursday or Friday courses (A, B, C or D) to attend
the discussion session. Separate tickets are required to enter each course.
Post-course online self-assessment: A post-course online self-assessment will
be available after the meeting and will include 25 core knowledge cases in addition to the 50 advanced cases studied at the meeting. Registrants must take the
post-course online self-assessment in order to claim SAMs or MOC SA question
credits. Self-assessment registrants will receive a link to the post-course online
self-assessment by email after the Annual Meeting and will have up to 30 days
to complete. Self-Assessment participants must take the post-course online
self-assessment to receive MOC credit for this activity.

AAD Recognized Credits for one of the Self-Assessments A-D, 3.5 AMA PRA
Category 1 CME Credits™ and 3.5 AAD Recognized Credits for the Self-Assessment Discussion, and 1 AMA PRA Category 1 CME Credit™ and 1 AAD Recognized Credit for the post-course online self-assessment for a total of 6.5 AMA
PRA Category 1 CME Credits™ and 6.5 AAD Recognized Credits. Participants
must complete the post-course online self-assessment with a 50 percent pass
rate or better in order to claim CME credit for the post-course self-assessment.
MOC – The ASDP Annual Meeting Online Post-course Self-Assessment offers
MOC self-assessment credits to satisfy the American Board of Dermatology
(ABD) Component 2 and the American Board of Pathology (ABP) Part II MOC
requirements. An MOC certificate will be available to print upon successful completion of the online past-course self-assessment.
ABD SA – ASDP is a provider of Self-Assessments (SAs) to satisfy the American
Board of Dermatology (ABD) Maintenance of Certification program Component 2.
The ASDP Self-Assessment in Dermatopathology offers up to 100 MOC SA
question credits. Participants must take the post-course online self-assessment
in order to claim SA points.
ABP SAMs – ASDP is a provider of Self-Assessment Modules (SAMs) for Part II
of the Maintenance of Certification program of the American Board of Pathology
(ABP). The ASDP Self-Assessment in Dermatopathology offers six and a half
(6.5) SAMs. Participants must take and pass the post-course online self-assessment with a 50 percent pass rate or better in order to claim SAMs, and may take
the self-assessment as many times as needed to achieve a passing score.

Be on time – The Quality and Performance Improvement Program begins on

IMPORTANT! You must drop off your de-identified glass slides and
corresponding de-identified reports for 10 cases that you signed out
before 10 a.m., Friday, Nov. 7, at the ASDP Registration Desk, in order to
participate.
During the two-hour session, each participant will be given 10 de-identified
slides and corresponding reports of another participant and will independently
review each case at a provided microscope and complete a feedback form
about slide quality, accuracy, and report quality. The feedback forms will be
collected and re-distributed to the original participant during the session. Based
on review of these feedback forms, the original participant will complete a short
form describing what they have learned from peer review, what areas they may
want to improve and how they plan to implement improvements.
Approximately six months after completion of the Quality and Performance
Improvement Program at the annual meeting, participants will be asked to
complete a follow-up survey. MOC Certificates will be issued only to program
participants who complete both phases of the program (at the annual meeting
and follow-up survey).
