In September of 2006, paid-on-call staff changed from responding from home to in-station staffing on 12hour shifts. Our response force from the main station at 8505 Harbach Blvd consists of career and paid-on-call personnel, working together to provide an in-house staff of six personnel at all-times. This staffing allowed us to provide a more timely response of both an ambulance and a fire engine. Statistical data illustrates the improvements in our response. Ice Rescue Training –The Fire Department utilized Raccoon River Park to conduct hands-on training in the water for ice rescue. Over the past two years, donations from the Clive Firefighters Association, County Club Homeowners Association, and Clive Jaycees have allowed ice rescue equipment to be purchased and implemented at both the Harbach and Westside Stations. Paramedic Engine Development – Fire Department staff upgraded the capabilities of our first out engine at the Harbach Station to function as a paramedic engine. Life Saving Recognition – The Clive Fire Department was involved in a life saving situation that was recognized by the patient and covered by the Des Moines Register. The patient had a cardiac event while exercising at 7-Flags. Through a solid chain of events with by-stander CPR and quick response/intervention of Clive Police and Fire Department personnel, he was successfully revived. Residential Fire – Urbandale – January 7 - Clive assisted Windsor Heights with a residential fire in the 3300 block of Melanie. Residential Fire – Clive – January 7 – Clive Fire responded to a report of smoke in a residence in the 2200 block of 137th. Extrication of Patient – Urbandale – January 11 – Clive assisted Urbandale with extricating a patient from a mini-van versus straight truck accident. The accident was in the 7000 block of Hickman Road. Residential Fire – Urbandale – January 12 – Clive assisted Urbandale with a significant residential fire in the 4000 block of 81st. The fire was through the roof upon arrival. Crews worked throughout the night in bitter cold to extinguish the fire. Residential Fire – Clive – January 12 – Clive responded to the 13200 block of Pineview for a fire in an attached garage. The fire was contained to the attached garage with an estimated $60,000 in damage to the building and contents. Assisting Clive was one engine company from West Des Moines. Residential Smoke Scare – Clive – January 27 – Clive responded to a report of a home filling with smoke in the 3800 block of Warrior Lane. Crews determined the problem to be a faulty furnace. The Waukee Fire Department responded to assist. Home Safety Council’s National Conference – Fire Marshal Collins attended this conference in Washington D.C. Steve Bunce Clive Citizen of the Year – Recently retired Clive firefighter Steve Bunce was honored as the 2006 Clive Citizen of the Year. Steve was recognized for 30 years of fire department service, current involvement as a volunteer photographer for the fire department, longstanding participation in the Clive Citizen’s Budget Committee, and numerous other community contributions. Residential Fire – Urbandale – February 2 - Clive assisted Windsor Heights with a residential fire in the 1300 block of 66th. Residential Smoke Scare – Clive – February 4 – Clive Fire responded to a report of smoke in a residence in the 1500 block of 90th. Smoke was evident in the home and further investigation determined the source to be a faulty furnace fan motor. Semi Tractor Fire – Clive – February 8 – Clive Fire responded to a semi tractor fire behind Centennial Warehouse (104th and Hickman). Tractor damage was estimated at $15,000. Residential Fire – Urbandale – February 15 – Clive assisted Urbandale with a significant residential fire in the 4500 block of 100th Street. Residential Fire – Johnston – February 25 – Clive assisted Johnston with a house fire in the 5800 Block of Beaver. National Fire Academy – Fire Marshal Collins attended the National Fire Academy as a grant evaluator. EMS Refresher – Clive collaborated with the Urbandale Fire Department and West Des Moines EMS in the delivery of a 48-hour EMS Refresher course. Page 11 of 16 Clive Fire Department 2007 Annual Report March Blizzard – Although Clive’s weather wasn’t severe, Flying J overflowed and created gridlock. Fire Department staff assisted with this event, particularly on Friday night and Saturday morning when the crisis peaked. We secured satellite parking areas for semis and created maps for WestCom agencies. EMT-B Class – The Clive Fire Department hosted a regional EMT-Basic course, in conjunction with Mary Greeley Hospital. Eighteen students from metro area departments attended. The course ran from April thru June, meeting twice a week and some weekends. Assisting with instruction were the WDM, Urbandale, and Waukee Fire Departments. Students included the above mentioned departments, plus Windsor Heights, Johnston, and Saylor. The class certification rate was above national averages. Residential Fire – Clive – March 2 - Clive responded to a residential fire in the 9000 block of Carpenter. The single family home was charged with smoke. Crews found a small smoldering fire in the basement, resulting from a shorted 220-volt outlet providing power to a clothes dryer. Although the direct fire damage was minimal, the smoke caused extensive damage. Total damage is estimated at $130,000. Providing assistance was the Windsor Heights Fire Department. HazMat Release – Clive – March 5 – Clive Fire responded to Flying J and the Walnut Creek area for a petroleum product release. The Fire Department contained the product in Walnut Creek by deploying absorbent booms at 114th Street and 86th Street. The Windsor Heights Fire Department deployed booms at 73rd Street. Working closely with Clive Public Works, Clive Parks, and the DNR, the situation spanned a number of days until extensive emergency maintenance was completed on the retention pond on Flying J property. Residential Fire – Urbandale – March 22 – Clive Fire assisted Urbandale with a residential fire in the 4100 Block of 76th Street. Clive provided an engine company and a duty officer. We assisted with ventilation, fire control, and salvage/overhaul. April Annual Emergency Operations Plan Overview Training – Overview training of the Clive All-Hazards Emergency Operations Plan was conducted on April 3rd for twenty personnel. Annual Clive Fire Fighter’s Association Easter Egg Hunt – On April 7th, the Clive Fire Fighter’s Association hosted the 41st Annual Easter Egg Hunt at Linnan Park. The weather was cool, but a nice crowd enjoyed the activities. Those children finding specially marked eggs were able to claim a small prize. FEMA Disaster Meeting – On April 9th, Fire Department staff participated in a meeting with FEMA to outline the necessary reporting information for reimbursement from the recent blizzard. Polk County MCI Drill – On April 17th, the Clive Fire Department participated in an county Mass Casualty Drill at Camp Dodge. The situation involved an explosion with subsequent petroleum fire and numerous injured. Chief Roe served as an evaluator at the unified command post. Clive also provided a foam trailer response. Flooding – On April 25th, flooding threatened Clive. The Emergency Operations Center at City Hall was activated and flood protection measures were implemented at three sites (Lincoln/93rd, Swanson Blvd, and University Blvd.). Technical Rescue – Urbandale – April 2 - Clive assisted Urbandale with a technical rescue, lowering a patient from an elevated platform in a grain elevator. The patient had fallen from one platform, landed on another platform and was found unconscious by coworkers. The patient was placed in a rescue basket, slid through an access hole, and then lowered to the ground. Vehicle Fire – Clive – April 2 – Clive Fire responded to vehicle fire at 10201 University. A small passenger car sustained minor fire and smoke damage from a fire in the passenger compartment. Damage is estimated at $1,000. Technical Rescue/EMS – Urbandale - April 11 – Clive assisted Urbandale with a rollover accident in the 4900 Block of I80/35. A Clive ambulance transported a patient who had been ejected from the vehicle during the rollover. Clive engine crews assisted with rope rescue to bring a patient up the embankment. Page 12 of 16 Clive Fire Department 2007 Annual Report City Hall for situation monitoring and resource coordination. May BP Vapor Control System – Fire Department staff were very involved in the evaluation and public communication regarding a temporary vapor control device located at the BP terminal. The vapor control device uses a pilot flame to efficiently remove excess vapors during truck loading. Holmes Safety Safari – Fire Department staff participated in the annual Holmes Safety Safari, providing a ladder truck for demonstration and assisting with the Blank Children’s Hospital Fire Safety House. EMS Week – May 13-19th was National EMS Week. This week was used to recognize and thank our staff for another year of excellence in EMS. A family BBQ and recognition event was held on Wednesday night. Residential Lightning Strike – Clive – May 23 – Clive Fire responded to a lightning strike in the 13800 Block of Hawthorne. A single family residence had been struck by lightning on a roof dormer. Circuit breakers in the home were tripped and a burning smell was evident on the upper floors. Further investigation found no active fire. HazMat/EMS – Urbandale – May 26 – Clive assisted Urbandale at a hazardous materials assignment. The location was a hotel in the 7600 Block of Hickman. An extremely volatile substance called toluene was in a room and the vapors were encountered by law enforcement officers. Clive Fire transported two law enforcement officers for evaluation. Rope Rescue – Urbandale – May 29 – Clive assisted Urbandale with a rope rescue in the 2700 Block of Carole Circle. A patient was removed from a residential roof using a rescue basket and rope lowering system. June Clive Festival – June 20-23 - Fire Department staff were very involved with the Clive Festival Activities, including; Emergency Incident Pre-Planning, Set-Up, Parade, Ambulance Stand-by, Fireworks Stand-by. Weather Event – June 22nd – A rain and wind event on Friday, June 22nd, brought both a potential for flooding and significant wind damage East of the Interstate. Flooding did not extend to the levels of the earlier April 2007 event. Wind damage uprooted large trees and took down some power lines. Wind gusts were measured at 78 mph with embedded downbursts. At the peak of the event, Clive had six roads closed due to flooding and debris. The EOC was activated at Fire and EMS Accreditation Workshop – June 13-15 – Chief Roe attended an accreditation workshop in Littleton, Colorado. We are researching and aligning our efforts to achieving accreditation through the Center for Public Safety Excellent/Commission on Fire Accreditation International. National Fire and EMS Safety Stand-down – The week of June 17-23 was the National Fire and EMS Safety Stand-down. The Clive Fire Department participated in this by focusing each shift on various preparedness efforts affecting their personal safety. HazMat Scare – Clive – June 7 - Clive responded to a report of a suspicious white powder at DeWaay Capital Management. Following safe protocol, the Des Moines Fire Department’s Hazardous Materials Team was requested to assist with the investigation. HazMat and other regional resources responded to assess the situation. There was not sufficient powder residue for testing. It is believed to be concrete dust from construction activities. Vehicle Fire – Urbandale – June 13 – Clive responded to Urbandale for a van on fire at a construction site. The cause was a bag of sawdust that also containing stain material. Heat from spontaneous combustion is the likely cause. Damage was estimated at $10,000. Residential Lightning Strike – Clive – June 21 – Clive responded to a reported residential fire in the 10800 Block of Forest. Neighbors witnessed a lightning strike and the homeowners could smell burning. The burning was determined to be from lightning damage to the alarm system. Residential Fire – West Des Moines – June 22 – Clive assisted West Des Moines with a house fire in the 4300 Block of Boulder. The fire is believed to have started from a lightning strike. Apartment Fire – West Des Moines – June 22 – Clive assisted West Des Moines with an apartment fire in the 3400 Block of Woodland. The fire is believed to have started from a lightning strike. Our crews assisted with ventilation of the structure. Hotel Fire – Urbandale – June 26 – Clive assisted Urbandale with a hotel fire in the 4800 Block of Merle Hay. A fire started in the pool chemical storage area. Our crews assisted with searching the structure and assisting with a hazardous materials team decontamination line. Page 13 of 16 Clive Fire Department 2007 Annual Report July Welcome Home Sgt. McGilvra – After an 18-month oversees deployment to Iraq, we welcomed home Sgt. Adam McGilvra from Operation Iraqi Freedom! Vehicle Crash with Extrication – Urbandale – July 17 – Clive assisted Urbandale with a single vehicle accident in the 4200 Block of Urbandale Avenue. An SUV was on it’s side requiring the extrication of one patient. August Training at Former Pizza Hut Building – During the month of August, staff utilized the former 86th Street Pizza Hut building for a variety of training opportunities. The most significant training was completed on Saturday, August 11th, with live fire evolutions. Burn cells were constructed in the building, allowing fires to be ignited using straw and pallets. Fire Department Fitness Challenge 2007 - The Fire Department initiated Fitness Challenge 2007, allowing employees to enter contests for improving their health. BP Terminal Table Top Drill/Facility Walk Thru The Clive Fire Department participated in a facility walk-thru and annual table top exercise at the BP terminal. A safety coordinator from BP facilitated the event. In depth discussions and scenarios were reviewed for both internal BP operations and external emergency response from the City of Clive. Townhome Fire – Urbandale – July 7 - Clive assisted Urbandale with a townhome fire in the 15600 Block of Monroe Ct. An engine from Clive arrived to find fire showing from a front deck. They quickly extinguished the fire and conducted overhaul operations. Residential Fire – Clive – July 7 – Clive responded to a residential fire in the 7800 Block of Drake. Crews arrived to find a rear deck on fire. The fire was quickly extinguished and overhaul operations conducted to insure that the fire did not enter the exterior walls of the home. The fire was caused by discarded smoking materials. Damage is estimated at $2,000. Vehicle Fire – Clive – July 12 – Clive responded to a vehicle fire at Dahl’s (15500 Hickman). Crews arrived to find significant engine compartment fire from a 1995 Dodge Stratus. The fire was quickly extinguished. Damage is estimated at $3,000. Vehicle Crash with Extrication – Clive – July 15 – Clive responded to a motor vehicle crash at 156th and Hickman. Additional ambulances were called from Urbandale and West Des Moines. One patient was unable to be removed from the vehicle and the door required the use of hydraulic extrication equipment for opening. Primary Response District Changes – Our primary response districts were modified in late August to provide a better balance of response times and area covered. We use a four-minute drive time to outline desired response areas. The outcome was shifting the zones from 104th to Interstate I35/80. Residential Fire – Urbandale – August 5 - Clive assisted Urbandale with a significant house fire in the 4600 Block of 153rd. The fire is believed to have originated from a lightning strike. Residential Fire – Urbandale – August 14 – Clive assisted Urbandale with a house fire in the 3900 Block of 92nd. A fire in the roof and walls was started from a lightning strike. Sign Fire – Clive – August 16 – Clive responded to a sign fire at Bett’s Cadillac. The elevated Cadillac sign had caught fire. No cars or buildings were damaged. Estimated fire loss is $40,000. Mass Casualty Incident – Woodward - August 17 – Clive provided one ambulance to respond/assist with a multiple patient incident at the Academy in Woodward. Clive transported three patients with minor complaints. In total, fifty-three patients were treated for nausea, vomiting, and respiratory problems. Page 14 of 16 Clive Fire Department 2007 Annual Report September October Training on 128th Street (LADCO Property) – LADCO provided us with a window of opportunity to conduct hands-on training at the house located at 1447 NW 128th Street. This house was slated for demolishment and made an excellent structure for training. We were able to conduct a department wide training on search and rescue drills. Fire Prevention Week – Included station tours, puppet shows, truck visits to schools and day cares, poster contest, live fire demonstrations, extinguisher training, fire drills, and the annual open house. This year’s theme was “Practice Your Escape Plan”. Apartment Fire – Clive – September 14 - Clive responded to a report of smoke in an apartment building at 1890 NW 82nd. Crews accessed an apartment and encountered heavy smoke. The fire was found to be contained to the top of a stove and resulted from unattended cooking. Crews assisted with ventilation. No damage was caused to the structure. Assisting was the Windsor Heights Fire Department. Technical Rescue – Urbandale – September 15 – Clive assisted Urbandale with a child stuck in mud at the bottom of a ravine in the 2500 Block of Canterbury. The mud was described by crews as “quick-sand” like which created a suction and was extremely difficult to displace. Removal of the child took two hours. Apartment Fire – Clive - September 26 – Clive responded to a fire at 1565 NW 84th Street. The fire originated from unattended cooking and extended into the kitchen cabinets. Damage is estimated at $33,000. Assisting was the Windsor Heights Fire Department. Firefighter/Paramedic Cross Named New Windsor Heights Fire Chief – Clive Firefighter/Paramedic Chris Cross was hired by the City of Windsor Heights as their new Fire Chief. Arrest of former Asst. Chief Brad Madsen – Although the formal arrest was completed on November 2nd, considerable time was spent during October by the Clive Police Department, Iowa Division of Narcotics Enforcement, and Clive Fire Department to complete the investigation on the allegations of missing narcotics. An internal audit revealed discrepancies and swift action was taken to address the situation. We reviewed our internal operations and control measures. This review resulted in a number of operational and control enhancements. Structure Fire – Urbandale – October 4 - Clive responded to a structure fire in the 4200 Block of 75th Street in Urbandale. Urbandale resources were previously committed to a significant fire on I80/35. The 75th Street fire was a small structure located in a back yard. The structure was heavily involved in fire. Residential Fire – Clive – October 19 – Clive responded to a reported grease fire in the 15000 Block of Summit. Crews arrived to find a small grease fire on top of a stove. The fire was quickly extinguished. Some smoke damage was present and estimated at $2,000. Apartment Fire – Clive - September 28 – Clive responded to a fire at 8424 Franklin. The fire originated in a bedroom and caused damage to the room of origin and the contents. Crews conducted overhaul operations to make sure the fire had not extended into the attic or walls. Damage is estimated at $13,500. Assisting were the Windsor Heights and Urbandale Fire Departments. Hazardous Materials Assignment – Clive – October 25 – Clive responded to multiple reports of anhydrous ammonia near the Iowa Quality Meats facility (2075 NW 92nd Court). This facility was in the process of shutting down and having the ammonia refrigerant removed. The Des Moines Fire Department’s Regional HazMat Team responded. Special detectors did not indicate any product in the air around the facility. The ammonia storage area inside of the building was accessed and readings confirmed no problem. Residential Fire – Waukee – October 28 – Clive assisted Waukee with a well involved house fire in the 1000 Block of Pinecrest. The operation was primarily defensive for safety reasons. Assessments/Promotions - The following promotions were made for the Clive Fire Department. • Neil Weglarz - Lieutenant Fire • Justin Powell - Lieutenant Safety Neil and Justin were chosen from seven candidates after an extensive and competitive selection process that tested their knowledge of department operating procedures, ability to effectively manage people, command of initial company operations and their ability to complete various tasks and projects that will be assigned to them as new lieutenants. 45th Annual Santa Day –December 15th – The Clive Firefighters Association has sponsored Santa Day every year since the inception of the Fire Department in 1962. We believe this is the longest standing tradition/community event in Clive. Five crews of Santa traveled the streets from 8:30 a.m. to 1:30 p.m. The weather was perfect with light snow and no winds. Commercial Fire – Urbandale – November 4 – Clive assisted Urbandale with a fire at the K-Mart located in the 7500 Block of Hickman. Fire was present on the outside of the building, consuming pallets and other combustibles. The exterior of the building was scorched and crews worked aggressively to make sure the fire did not extend into the roof and insulation materials. Commercial Fire – Clive – November 5 – Clive responded to a reported kitchen grease fire at the KMart located in the 10300 Block of University. The building was charged with smoke from the grease fire. K-Mart staff had extinguished the fire using six portable fire extinguishers prior to our arrival. Clive and Windsor Heights fire crews assisted with ventilating smoke from the building. Residential Fire – West Des Moines – November 7 – Clive assisted West Des Moines at a single family home fire in the 600 Block of 17th Street. Heavy fire was present in the basement. Residential Fire – West Des Moines – December 15 – Clive assisted West Des Moines with a significant residential basement fire in the 2900 Block of Orchard. Residential Fire – Johnston – December 25 – Clive assisted Johnston at a well-involved single family residential fire in the 4800 Block of Lovington. Residential Sprinkler System – The first residential sprinkler system in Clive was installed in a private single family house in County Club. The house is over 11,000 square feet. EMS Refresher – Clive collaborated with the Urbandale Fire Department and West Des Moines EMS in the delivery of a 48-hour EMS Refresher course. This was the second collaborative delivery of this course. Each city saved $130 per student through this local delivery. Page 16 of 16
