Kami: LPKM 1/107/2/4 Tarikh: 28hb Mei 2013 Yang berkenaan, Tuan/Puan, SEBUTHARGA BIL : MTIB/SH/017/2013 SEBUTHARGA BAGI KERJA-KERJA PENYELENGGARAAN BANGUNAN GALERI GLULAM, TEBRAU, JOHOR BAHRU BAGI LEMBAGA PERINDUSTRIAN KAYU MALAYSIA (MTIB) Dengan hormatnya dimaklumkan bahawa syarikat tuan/puan telah dipilih untuk menyertai tawaran sebutharga yang tersebut di atas. Tawaran sebutharga di atas adalah tertakluk kepada syarat-syarat seperti berikut: 1. Pendaftaran dengan Kementerian Kewangan dalam kod bidang 220301, 220401 dan 220502. (Sila majukan sijil pendaftaran dengan Kementerian Kewangan yang masih sah tempohnya sekurang-kurangnya enam (6) bulan untuk rujukan MTIB). 2. Pendaftaran sebagai kontraktor bumiputera dengan Kementerian Kewangan dalam kod bidang 220301, 220401 dan 220502. (Sila majukan sijil pendaftaran dengan Kementerian Kewangan yang masih sah tempohnya sekurangkurangnya enam (6) bulan untuk rujukan MTIB). 3. Tempoh sahlaku minima tawaran sebutharga adalah selama sembilan puluh (90) hari dari tarikh tawaran sebutharga. 4. Tempoh serahan bekalan / tempoh siap kerja adalah dua (2) minggu maksimum dan kerja-kerja penyelenggaraan dijalankan secara rutin iaitu empat (4) bulan sekali (tiga (3) kali setahun) dari tarikh surat setuju terima ditandatangani. 5. Tawaran sebutharga hendaklah dibuat berdasarkan kepada spesifikasi kerja di Dokumen Sebutharga yang disertakan bersama surat ini. 6. Lawatan Tapak akan diadakan pada tarikh, masa dan tempat seperti berikut: 7. Sila majukan maklumat tentang pengalaman syarikat tuan/puan dalam kerja-kerja penyelenggaraan sebegini. (Nyatakan nama agensi/syarikat yang sedang melanggan atau pernah melanggan perkhidmatan tuan untuk rujukan kami). Dimaklumkan sekali lagi agar pihak tuan/puan meneliti dan memenuhi syarat-syarat yang telah ditetapkan seperti diatas bagi mengelakkan tawaran sebutharga tuan/puan ditolak kerana tidak memenuhi syarat-syarat tersebut. Sebutharga adalah dipelawa kepada pembekal-pembekal yang berdaftar dengan Kementerian Kewangan Malaysia dalam kod bidang 220301, 220401 dan 220502 serta bertaraf bumiputera untuk membuat tawaran bagi sebutharga seperti berikut : NO. Dokumen sebutharga boleh dimuat turun bermula 28hb Mei 2013 (Selasa) sehingga 6hb Jun 2013 (Khamis) menerusi Portal Korporat MTIB www.mtib.gov.my. Tarikh tutup sebutharga adalah pada 7hb Jun 2013 (Jumaat). 4. Dokumen tawaran sebutharga yang telah siap diisi hendaklah dimasukkan ke dalam Peti Sebutharga yang ditetapkan di Unit Perolehan, Bahagian Khidmat Pengurusan, Tingkat 14, Menara PGRM, No. 8, Jalan Pudu Ulu, 56100 Cheras, Kuala Lumpur tidak lewat dari jam 12.00 tengahari pada 7hb Jun 2013 (Jumaat). Sebutharga yang lewat tidak akan dipertimbangkan. 5. Lembaga Perindustrian Kayu Malaysia tidak bertanggungjawab di atas kelewatan dokumen sebutharga yang diposkan dan tidak terikat untuk menerima tawaran yang terendah atau mana-mana tawaran yang dikemukakan. 6. Keputusan tawaran sebutharga dan syarikat yang berjaya akan disiarkan di dalam Laman Web MTIB (www.mtib.gov.my). Penyebutharga boleh menyemak status tawaran dalam tempoh diantara satu (1) bulan hingga dua (2) bulan selepas tarikh tutup sebutharga. Tuan/Puan, SEBUTHARGA SEBUTHARGA BAGI KERJA-KERJA PENYELENGGARAAN BANGUNAN GALERI GLULAM, TEBRAU, JOHOR BAHRU BAGI LEMBAGA PERINDUSTRIAN KAYU MALAYSIA (MTIB) Nombor Sebutharga : MTIB/SH/017/2013 1.0 ARAHAN AM 1.1 Sebutharga adalah dipelawa dari syarikat-syarikat yang berkelayakan bagi kerja-kerja penyelenggaraan Bangunan Galeri Glulam, Tebrau, Johor Bahru bagi Lembaga Perindustrian Kayu Malaysia (MTIB). 1.2 Penyebutharga yang dijemput adalah tertakluk kepada syarat-syarat dan arahan-arahan yang terdapat dalam dokumen ini. Arahan-arahan dan syaratsyarat yang dinyatakan hendaklah disifatkan sebagai syarat kontrak. Sebutharga yang tidak mengikut syarat-syarat yang dinyatakan tidak akan dipertimbangkan. 10 2.0 BIDANG KERJA PENYEBUTHARGA 2.1 Kerja-kerja penyelenggaraan penyelenggaraan Bangunan Galeri Glulam, Tebrau, Johor Bahru bagi Lembaga Perindustrian Kayu Malaysia (MTIB). 3.0 KEPERLUAN KELAYAKAN PENYEBUTHARGA 3.1 Sebutharga adalah dipelawa kepada semua penyebutharga yang berdaftar dengan Kementerian Kewangan Malaysia dalam kod bidang kod bidang 220301, 220401 dan 220502. 3.2 Pendaftaran sebagai kontraktor bumiputera dengan Kementerian Kewangan Malaysia dalam kod bidang 220301, 220401 dan 220502. 4.0 BORANG SEBUTHARGA 4.1 Sebutharga hendaklah dikemukakan di atas Borang Sebutharga sepertimana yang ditetapkan pada borang seperti berikut: 4.2 i. Borang I, ii. Borang II, iii. Borang III, iv. Borang IV dan v. Borang V. Sebutharga-sebutharga yang tidak dikemukakan di atas borang sebutharga yang berkenaan tidak akan diterima. Walaubagaimanapun, penyebutharga -penyebutharga boleh mengemukakan borang atau dokumen tambahan mengikut format dinyatakan untuk memberi penerangan-penerangan. 5.0 PENYERAHAN SEBUTHARGA 5.1 Sebutharga hendaklah dikemukakan dengan sampul surat berlakri (“sealed” ) serta ditandakan seperti berikut: ”SEBUTHARGA BAGI KERJA-KERJA PENYELENGGARAAN BANGUNAN GALERI GLULAM, TEBRAU, JOHOR BAHRU BAGI 11 LEMBAGA PERINDUSTRIAN KAYU MALAYSIA (MTIB) (SEBUTHARGA MTIB/SH/017/2013)” 5.2 Penyebutharga hendaklah menghantar satu (1) salinan asal bagi dokumen di atas dan dilabel sebagai “ORIGINAL” dan satu (1) salinan lain yang dilabel sebagai “COPY”. 5.3 Sebutharga hendaklah dihantar secara serahan tangan atau pos dan dialamatkan kepada :Ketua Pengarah, Lembaga Perindustrian Kayu Malaysia, Tingkat 14, Menara PGRM, No. 8, Jalan Pudu Ulu, Cheras, 56100 Kuala Lumpur. (U.P: Unit Perolehan) 5.4 MTIB tidak akan bertanggungjawab ke atas kehilangan borang-borang penyertaan sebutharga akibat kesilapan alamat yang dihantar secara pos atau melalui tangan. 6.0 KOS SEBUTHARGA 6.1 Penyebutharga mestilah menanggung sendiri semua kos berkaitan dengan penyediaan dan penghantaran sebutharga. MTIB tidak akan bertanggungjawab ke atas kos-kos tersebut. 7.0 HARGA DAN TEMPOH SAH LAKU 7.1 Semua harga sebutharga mestilah di dalam RINGGIT MALAYSIA bagi setiap item dan berdasarkan kepada format yang disediakan seperti pada Borang I dalam dokumen ini. Jadual harga yang dikemukakan mestilah dinyatakan dalam angka yang tepat bagi unit harga dan jumlah harga. 7.2 Harga yang disebutkan dalam sebutharga hendaklah sah laku di dalam tempoh tidak kurang dari sembilan puluh (90) hari dari tarikh tutup sebutharga. 12 7.3 Dalam keadaan terkecuali, tempoh pengesahan sebutharga yang hampir tamat, MTIB akan meminta penyebutharga untuk menyambung tempoh pengesahan sebutharga tersebut. Permintaan ini dan maklumbalasnya mestilah dibuat secara bertulis. 7.4 Penyebutharga yang bersetuju dengan permintaan ini tidak akan dikehendaki atau dibenarkan mengubahsuai sebutharganya tetapi dikehendaki untuk melanjutkan tempoh pengesahan sebutharganya. 8.0 MTIB BERHAK UNTUK MENERIMA ATAU MENOLAK SEBAHAGIAN ATAU SEMUA SEBUTHARGA 8.1 MTIB tidak terikat untuk menerima tawaran sebutharga yang terendah atau mana-mana sebutharga yang diterima. 8.2 MTIB juga berhak untuk menerima sebahagian atau keseluruhan cadangan dalam tawaran sebutharga. 8.3 Keputusan MTIB adalah muktamad dan MTIB tidak akan teragak- agak untuk memberi apa-apa alasan bagi penolakan mana-mana sebutharga. 9.0 PERJANJIAN KONTRAK 9.1 Penyebutharga yang berjaya jika perlu, adalah dikehendaki mengikat kontrak dengan MTIB bagi kerja-kerja penyelenggaraan yang akan dijalankan seperti yang dinyatakan di dalam Dokumen Sebutharga. 9.2 Denda akan dikenakan dalam kontrak jika tidak memenuhi syarat-syarat kontrak supaya kerja-kerja dilaksanakan dengan baiknya. Berdasarkan perjanjian kontrak, penerimaan MTIB disertai dengan pengesahan penyebutharga yang berjaya akan terikat dengan kontrak di antara MTIB dengan penyebutharga tanpa mengambilkira perjanjian kontrak disediakan dilaksanakan atau tidak dilaksanakan. 9.3 Penyebutharga yang berjaya mesti menanggung sendiri risiko berkenaan kontrak mengikut syarat-syarat, spesifikasi dan jadual sebahagiannya 13 mestilah menurut status, peraturan atau melalui undang-undang dan keperluan mana-mana kebenaran. 10.0 PENERIMAAN SEBUTHARGA 10.1 MTIB akan memberi notis bertulis kepada penyebutharga yang berjaya apabila sebutharganya disetujuterima. Notis ini boleh mengandungi syaratsyarat kontrak sebagai tambahan kepada syarat-syarat yang dilampirkan bersama ini dan sekiranya demikian sebutharga ini tidaklah boleh dipertimbangkan sehingga syarat tambahan ini dipersetujui oleh penyebutharga yang berjaya. 11.0 KERJA-KERJA PENYELENGGARAAN DALAM TEMPOH JAMINAN 11.1 Penyebutharga adalah dikehendaki menyempurnakan kerja-kerja penyelenggaraan dan semua yang berkaitan dalam tempoh dua (2) minggu maksimum dan kerja-kerja penyelenggaraan dijalankan secara rutin iaitu empat (4) bulan sekali (tiga (3) kali setahun)daripada tarikh surat setuju terima ditandatangani. 11.2 Penyebutharga dikehendaki menyediakan satu jadual pelaksanaan yang terperinci dan lengkap bagi tujuan kerja-kerja penyelenggaraan yang dicadangkan kepada MTIB. 11.3 Penyebutharga hendaklah bersedia untuk dihubungi pada bila-bila masa sekiranya berlaku kerosakan dan jika terdapat pembaikan dan penggantian alat ganti bagi unit alat kelengkapan tersebut, pihak penyebutharga hendaklah memberikan tempoh jaminan sekurang-kurangnya selama enam (6) bulan bagi setiap barang diganti. 12.0. MAKLUMAT DAN PENGALAMAN SYARIKAT 12.1 Penyebutharga perlu menyediakan maklumat mengenai syarikat seperti pada Borang II dalam dokumen ini. 12.2 Penyebutharga perlu menyatakan pengalaman syarikat dalam mengendalikan projek-projek kerja-kerja penyelenggaraan yang berkaitan 14 dengan mengemukakan latarbelakang projek, nama-nama dan alamat syarikat atau agensi di mana projek tersebut dilaksanakan. Nyatakan tarikh projek tersebut selesai dan tarikh sebenar projek tesebut sepatutnya diselesaikan. Sila nyatakan projek-projek yang masih dikendalikan dan tarikh projek tersebut dijadualkan tamat seperti pada Borang III dalam dokumen ini. 13.0 JADUAL BAYARAN 13.1 Semua harga sebutharga mestilah di dalam Ringgit Malaysia dan berdasarkan kepada format yang disediakan. Tawaran Harga yang dikemukakan mestilah dinyatakan dalam angka yang tepat bagi unit harga dan jumlah harga (termasuk penghantaran, pemasangan, penyenggaraan dan lain-lain kerja). 13.2 Pembayaran tidak akan dikeluarkan sehingga kerja-kerja penyelenggaraan tersebut dilaksanakan dengan lengkap dan sempurna serta disahkan oleh pegawai MTIB yang diberi kuasa. 14.0 PENENTUAN KERJA-KERJA PENYELENGGARAAN 14.1 Perakuan penerimaan hanya akan dibuat setelah kerja-kerja penyelenggaraan tersebut telah siap dan diperiksa oleh pegawai MTIB yang diberi kuasa. 14.2 Pegawai tersebut berhak membuat penolakan terhadap kerja-kerja yang rendah kualiti atau tidak mengikut penentuan spesifikasi kerja. Penyebutharga hendaklah menggantikan kerja-kerja yang ditolak tersebut. Kos perbelanjaan yang berkaitan hendaklah ditanggung oleh penyebutharga. 14.3 Lokasi kerja-kerja penyelenggaraan tersebut adalah seperti berikut:GALERI GLULAM JOHOR BAHRU LOT 19851, JALAN TAMPOI 81200 JOHOR BAHRU JOHOR DARUL TAKZIM TEL: 07-2329048 / 9057 15 15.0 KEPUTUSAN TAWARAN SEBUTHARGA 15.1 Keputusan tawaran sebutharga dan syarikat yang berjaya akan disiarkan di dalam Laman Web MTIB (www.mtib.gov.my). Penyebutharga boleh menyemak status tawaran dalam tempoh diantara satu (1) bulan hingga dua (2) bulan selepas tarikh tutup sebutharga. Bagi penyebutharga yang namanya tidak disiarkan di Laman Web MTIB adalah dianggap tidak berjaya dalam sebutharga tersebut. 16.0 TARIKH TUTUP 16.1 Tarikh tutup sebutharga adalah jam 12.00 tengahari pada 7hb Jun 2013 (Jumaat). Sebutharga-sebutharga hendaklah dikemukakan kepada Lembaga Perindustrian Kayu Malaysia pada atau sebelum waktu dan tarikh tersebut. Sebutharga-sebutharga yang lewat tidak akan dipertimbangkan. 16 BAHAGIAN II PENENTUAN SPESIFIKASI LEMBAGA PERINDUSTRIAN KAYU MALAYSIA KEMENTERIAN PERUSAHAAN PERLADANGAN DAN KOMODITI 17 SPESIFIKASI SEBUTHARGA BAGI PENYELENGGARAAN BANGUNAN GALERI GLULAM, TEBRAU, JOHOR BAHRU BAGI LEMBAGA PERINDUSTRIAN KAYU MALAYSIA (MTIB) (SEBUTHARGA BIL : MTIB/SH/017/2013) A. PENENTUAN SKOP KERJA-KERJA PENYELENGGARAAN BANGUNAN GALERI GLULAM, TEBRAU, JOHOR BAHRU ELEKTIRK 1. Menggantikan / menukar barangan yang rosak seperti mentol lampu, soket, fius serta yang berkenaan elektrik, 18 KEKERAPAN 3 bulan sekali & jika perlu bersedia dengan panggilan kecemasan / harian Luar bangunan penghawa dingin dan lain-lain yang berkaitan & memastikan sistem elektrik berfungsi serta boleh digunakan dalam keadaan selamat dan mengikut sepertimana prosedur / peraturan dan arahan pematuhan agensi keselamatan bangunan seperti BOMBA, TNB, SAJ, IWK, MBJB & agensi lain yang berkenaan. 1. Memeriksa keseluruhan pendawaian elektrik & 3 bulan sekali & jika penghawa dingin termasuk perlu panggilan rumah penyelenggaraan kecemasan / harian penapisan IWK 4.2 TUGAS BULANAN TEMPAT PERKHIDMATAN 1. Menjalankan pemeriksaan & penyelenggaraan semua pengkabelan / wayar elektrik, hawa dingin, lampulampu, penebat kilat serta yang berkaitan dengan kuasa Semua ruang elektrik termasuk chiller room luar dan dalam bangunan bangunan pentadbiran, ruang 2. Menjalankan kerja-kerja pemeriksaan / penyelenggaraan legar, dan sekeliling paip-paip dalam & luar bangunan tepi bangunan laluan pejalan kaki & luar 3. Menggantikan / menukar / membaik pulih peralatan / bangunan. bahan yang rosak / pecah / mendatangkan bahaya dengan peralatan yang berkualiti tinggi serta mempunyai tempoh jaminan sekurang-kurangnya setahun daripada tempoh pemasangan. 19 B. Memeriksa struktur kayu dalam & luar bangunan samada mempunyai masalah pengeringan, perekatan, pecah, rekah dan lain-lain kecacatan kayu dan hendaklah dibaiki dengan coating yang bersesuaian supaya tidak menjejaskan kekuatannya, ketahanannya, kecantikannya serta lain-lain kecacatan yang boleh 20 KEKERAPAN Sebulan sekali & jika perlu bersedia dengan panggilan kecemasan / harian menjejaskan keupayaannya dalam mengenengahkan kekuatan dan kestabilannya kepada sifat-sifat kayu itu sendiri. 2. Memeriksa bumbung kayu Belian shingles & memastikan tiada kecacatan seperti pecah, retak atau kecacatan lain yang boleh menjejaskan fungsi bumbung sebagai menahan panas / hujan / kebocoran dan mengaplikasikan pendekatan dalam menggantikannya kepingan bumbung yang rosak. 3. Memeriksa dan membaiki peralatan perpaipan di semua tempat dalam & luar bangunan termasuk dalam bilik-bilik air. 1. Memeriksa & membaik pulih keseluruhan struktur kayu serta dekorasi kayu yang rosak termasuk pintupintu kayu, dinding kayu Kekatong, lantai komposit pada sekeliling bangunan. Luar bangunan 2. Membaiki kayu-kayu yang rosak serta membuat kerjakerja kemasiapan pada struktur kayu dan dekorasi kayu dengan menyapu bahan pelapis / coating bertujuan pengindahan dan kecantikan tanpa menjejaskan kualiti, mutu kayu dan meningkatkan jangka hayat kayu. 4.2 TUGAS BULANAN TEMPAT Semua ruang bangunan pentadbiran, ruang legar, dan sekeliling tepi bangunan laluan 3 bulan sekali & jika perlu panggilan kecemasan / harian PERKHIDMATAN 1. Menjalankan penyelenggaraan semua struktur dan bukan struktur kayu di dalam dan luar bangunan. 21 pejalan kaki & luar bangunan. 2. Menjalankan kerja-kerja pemeriksaan / penyelenggaraan paip-paip dalam & luar bangunan 3. Menggantikan / menukar paip / hos / singki / cermin / pam air / getah / tiles / yang rosak /pecah dan tidak berfungsi. Bahan yang diganti mestilah berkualiti tinggi dan mempunyai tempoh jaminan sekurangkurangnya setahun daripada tempoh pemasangan / penggantian barangan tersebut. 4. Memeriksa / membaik pulih struktur konkrit yang rosak / pecah / cacat serta menggantikannya jika perlu dan mendatangkan bahaya / mengurangkannilai kecantikan / pengindahan bangunan. 22 C.
