Please see the “Benefits Guide” which lists the services, limits, conditions and procedures. MANUTENCOOP PROTECTS YOUR HEALTH. The Manutencoop Group believes in company welfare benefits. IT BELIEVES that it is important to combine the company’s development with its employees’ ambitions of personal and professional growth. IT BELIEVES that to do this, it is important to be able to meet the real needs of its workers. For this reason, it provides its employees with a completely free supplementary healthcare plan through enrolment with the IPRASS Fund: so, Manutencoop Facility Management workers can take advantage of supplementary healthcare services above and beyond those provided by the National Health Service (NHS). With this initiative, Manutencoop anticipates and supplements the requirements of the Cleaning and Multi-services national collective labour agreement starting from July 2013. To support households in an economically difficult moment such as the present, Manutencoop has chosen to immediately activate a healthcare plan, investing more resources than the compulsory contributions required under the national collective labour agreement to ensure services and reimbursements that can better meet the real needs of its employees. The supplementary healthcare is provided to employees at no cost. The healthcare services cover illnesses and accidents and are provided to all employees with unlimited term contracts as well as their households. The healthcare is provided by the IPRASS Assistance Fund, a Supplementary Health Care Fund, in partnership with Unisalute, a specialised health insurance company. 3 Iprass I P R A S S AS S I S TA N CE F U N D DO YOU NEED A CARDIOLOGIST, AN ORTHOPAEDIST OR, IN GENERAL SPECIALIST EXAMINATIONS? > If you use National Health Service facilities, you are responsible for paying only 5 EUROS for each co-payment, and you will be reimbursed for the rest by Unisalute after submitting expense records (co-payment and the prescription from your general practitioner which indicates the assumed or confirmed illness) twice per year (see page 14). Book an appointment at www.unisalute.it IT’S FAST AND EASY! > If you use private healthcare facilities approved by Unisalute and approved doctors, you pay 25 EUROS for each examination, and Unisalute will pay the remaining amount due directly to the facility you have chosen. DO YOU NEED DIAGNOSTIC TESTS FOR EXAMPLE BLOOD TESTS? > If you use National Health Service facilities, you are responsible for paying only 5 EUROS for each co-payment, and you will be reimbursed for the rest by Unisalute after submitting expense records (co-payment and the prescription from your general practitioner which indicates the assumed or confirmed illness) twice per year (see page 14). Each household has an annual expense limit of 1,500 EUROS for specialist examinations or diagnostic tests. 5 Iprass I P R A S S AS S I S TA N CE F U N D HAVE YOU BEEN HOSPITALISED FOR AN ACCIDENT OR ILLNESS AND NEED HOME HEALTHCARE? For 120 DAYS after the discharge date, you have home hospitalisation and medical, rehabilitation, nursing and pharmacological assistance services available to you through healthcare facilities approved by Unisalute. > If you use private healthcare facilities NOT approved by Unisalute, you will be reimbursed for 80% of the expense incurred, up to the established expense limit. Unisalute pays for one specialist dental examination and one professional cleaning per year (the latter for up to a maximum cost of 60 EUROS) in approved healthcare facilities indicated by the Operational Centre, which must also make the appointment***. Iprass I P R A S S AS S I S TA N CE F U N D WHAT TO DO... > BEFORE THE HEALTHCARE SERVICE For all services covered by the Healthcare Plan PLEASE GO TO THE Customers section on the website www.unisalute.it or, alternatively, CALL the Operational Centre’s toll free number 800-822470 (8:30 a.m. - 7:30 p.m. Monday to Friday) CHOOSE the type of healthcare facility you prefer: > National Health Service (NHS) > Private Healthcare Facilities approved by Unisalute (you can check which facilities are approved at www.unisalute.it or by calling the toll free number) > Unapproved private facilities (covered by healthcare assistance only for rehabilitation treatments after accidents and gynaecological examinations during pregnancy) BOOK AN APPOINTMENT and OBTAIN the service you need. If you choose facilities approved by Unisalute booking an appointment is fast and easy! Just use the medical visit/exam appointment tool on the website www.unisalute.it in the Customers section. This tool is useful because you can immediately see if the required service is covered by the healthcare Plan and understand which approved healthcare facility best meets your needs. Zanardi 30 - 40131 - BOLOGNA Remember that the healthcare co-payment reimbursement request may be made only twice per year, once in June and once in December. Therefore, keep the co-payment tickets for the half-year and send them in one reimbursement request. Without prejudice to disputes or requests for clarification, Unisalute pays amounts due by bank transfer (and reserves the right to request the original records if necessary). DID YOU CONTACT A FACILITY APPROVED BY UNISALUTE? Unisalute directly pays the facility based on what is established by the individual coverage.
