Eskaton launched Live Well at Home’s dementia home care services for
the Greater Sacramento region, including Placer and El Dorado Counties.
(See page 17.)
Live Well at Home Care Management and Dignity Health’s Mercy San Juan
Medical Center are collaborating on a project focused on increased care
coordination, prevention of falls and mitigation of acute care visits.
Construction is underway for The Trousdale, assisted living and memory
care in Burlingame, CA. Scheduled to open in 2018, The Trousdale is
owned by Peninsula Health Care District and managed by Eskaton.

The following communities are celebrating their 30th
Anniversaries: Eskaton Care Center Fair Oaks, Eskaton Care
Center Greenhaven and Eskaton Hazel Shirley Manor.
Eskaton Kids Connection is now in 28 Eskaton communities.
Six years ago we had one class in one community. With the
expansion of classrooms and communities, the camaraderie and
engagement of the “buddies” exceed 1,000 participants per year.
Eight Eskaton communities received awards for being best
among their competitors for services provided. (See page 33.)
Eskaton extended support services to three stand-alone nonprofit
organizations: O’Connor Woods, St. John’s Village and Paradise
Valley Estates.
Eskaton Village Placerville had 100% occupancy for a consecutive
20+ months.
Eskaton residents are sparking new connections using innovative
therapeutic music programs. Music can shift mood, manage stress,
stimulate positive interactions and support brain health. Trained facilitators
have lead over 835 musical journeys that include trips across the globe,
American cinema, show tunes and more.
Eskaton Care Center Fair Oaks received a payment award from the
Centers for Medicare and Medicaid Services (CMS) in recognition of its
high quality standards and outcomes. This exemplifies the quality care
and services provided to residents and patients.
Eskaton Adult Day Health Center Carmichael received a zero deficiency
survey by the Department of Healthcare Services.
President’s Message
I think it is commonly accepted that good organizations change with the times, and
that is exactly what Eskaton has done throughout its nearly 50-year history. There are
few who remember that the Eskaton board of directors repositioned the organization
from hospitals to housing and services for seniors in the mid-1980s. Since that time,
Eskaton has been expanding its offerings for older adults. Today, in response to the
rapidly aging population, healthcare reform, technology, for-profit expansion, and
consumer demands, Eskaton is once again repositioning for the future. Our commitment
to the needs of the aging will remain; however, our approach is evolving to communitywide coverage as opposed to a narrower focus on property addresses. What this simply
means is that we will strive to serve older adults in their homes and in our residences,
and we will expand our array of services to bridge the gaps between. Our philosophical
approach to services is to place a premium on choice, control and flexibility for the
seniors and families in our local communities.

There are a few things I would like to point out regarding our Annual Report. First, all
of the photos are of Eskaton residents, families, employees and volunteers, and we
have a lot of fun selecting which are included in the annual report. Second, I hope it is
evident that Eskaton has a uniquely broad spectrum of housing and services, and that
we are continuously exploring ways to enrich lives and support all aspects of aging
in its various stages. Finally, please understand that as a community based nonprofit
organization, our mission to serve older adults is why we exist, and we aspire to do it
in a compassionate, transparent, and responsible manner. After all, we intend to be
serving seniors for a very long time to come.

What We Offer
Eskaton’s diverse spectrum of services is what sets us apart. Eskaton communities feature everything from independent living to skilled
nursing and rehabilitation. Remain in your own home as long as possible with our Home Support Services. Our professional care advisors are
available 24/ 7 to help find solutions.
RESIDENTIAL LIVING COMMUNITIES
Continuing Care Retirement Community (CCRC) offers a
continuum of lifestyle options on one campus. Independent living
with services is offered exclusively through membership. Assisted
living, memory care, rehabilitation, skilled nursing care and shortterm stays are available to the greater community. Village residents
create the lifestyle they want — now and for the future — with an
array of life-enriching amenities.
Multi-Level Retirement Communities (MLRC) offer
independent living with services, assisted living, memory care and
short-term stays are available. Social programs, recreation and
hospitality services enhance the experience. Home ownership and
rental options are available.
Independent Living with Services offers the options needed
to lead an independent lifestyle along with the convenience and
assurance of support. Residents appreciate a full complement of
hospitality-style amenities, housekeeping and transportation services.
Assisted Living provides enhanced personalized support with
activities of daily living in the comfort of the resident’s apartment.
A full spectrum of convenient services, including fine dining,
recreation and cultural pursuits assures an engaging life.
Short-term (respite) stays are available.
Memory Care and Pre-Memory Care are designed for persons
living with Alzheimer’s disease or other forms of dementia. The
empathic approach focuses on comfort, personal dignity and selfworth. Residents engage in meaningful activities that promote selfexpression, empowerment and mutual respect.
REHABILITATION AND SKILLED NURSING CARE
CENTERS
Rehabilitation offers 24/7 skilled nursing support combined with
short-term physical, occupational and speech therapies.
Long-Term Care provides 24/7 nursing care and support with
activities of daily living. Residents are encouraged to participate
in recreational and social activities to support their independence
whenever possible. In 2015, nearly 85% of Eskaton Care Center
residents were discharged home or to a lower level of care.
AFFORDABLE HOUSING
For older adults with limited incomes, rental assistance is available
through the U.S. Department of Housing and Urban Development
(HUD) at Eskaton’s affordable living apartment communities.
Professional on-site staff members provide support, activities,
maintenance, security and social service coordination in an
independent setting.

What We Offer
Adult Day Healthcare located in Carmichael offers a day program,
including nursing, personal care, social services, therapy and a
vibrant activity program. This program greatly benefits participants
and provides respite for caregivers.
Home Healthcare offers a combination of clinical services,
rehabilitation, education and encouragement in the comfort
of one’s own home, through our team of nurses, therapists and
home health aides.
Live Well at Home by Eskaton launched an expanded network
of services in the Greater Sacramento region to include home care,
care navigation and care management.

Eskaton provides choice and flexibility by offering a broad spectrum
of services designed to retain independence and connection.
Information and Assistance. Navigating age-related concerns
and service options can be confusing. Our resource specialists
provide information on senior housing and healthcare, dementia
and Alzheimer resources, geriatric and care management referrals,
home maintenance, transportation and much more.
Telephone Reassurance. Trained volunteers provide daily check-in
calls to older adults who live alone or are isolated and would benefit
from regular connection to our friendly network of volunteers.

companionship and personal care. Services include assistance
with bathing, dressing, meal preparation and transportation to
appointments and shopping. A hallmark of Live Well at Home
is a new program called Transforming Dementia Care, providing
enriched training for staff members and families of persons with
dementia and Alzheimer’s disease.
• Care navigators provide older adults and families seamless support,
information and resources throughout the discharge process from
the hospital, rehabilitation or skilled nursing community.
• Care managers are professional social workers and nurses with
a primary goal of helping older adults and families understand their
options, create a personalized plan and help coordinate
care and services that improve and maintain well-being.

Consulting Services. Eskaton provides a wide range of short-term,
project-specific consulting services in the areas of:

Livable Design. The Livable Design Seal of
Approval program promotes innovative home
design with built-in function, flexibility and

owners now and is prepared for life changes that

Management Services. Eskaton provides a turn-key, high-quality

The aging experience is unique for everyone, so too are the creative solutions to assure that life is as independent and purposeful as
possible. Our commitment to the well-being of our residents and program participants is demonstrated by our rich variety of signature programs.
SMART-SENSOR TECHNOLOGY
State-of-the-art motion-sensor technology is one of the examples of
Eskaton’s commitment to prompt personalized service, prevention and
support. Discreet “smart sensors,” seamlessly integrated throughout
each apartment, identify changes in routine to help the care team
anticipate and respond with appropriate, timely assistance.
ESKATON KIDS CONNECTION, our intergenerational program,
brings students from 28 local elementary school classrooms together
with Eskaton residents to read, write, laugh and enjoy one another’s
company. These relationships are a win-win situation: increased
socialization, engagement, insight, sense of purpose and happiness
for all.
LIFE ENRICHMENT INITIATIVES include art and music therapies,
veterans’ appreciation, Thrill of a Lifetime events and Eskaton
Celebrates 100+ to recognize birthdays of centenarians. Eskaton
is advancing Keep Connected, which uses online video services to
keep residents and families in touch. The Community Medical Partner
Program offers residents “house calls” at their community — the
convenience of an in-house physician and professional clinical team.

Longevity Explorers Club Introduced New Technology
The Longevity Explorers Club led by Dr. Caro, co-founder of TechEnhanced Life, leverages the insight, wisdom and enthusiasm of older
adults to help evaluate and accelerate the development of technologyenabled products and services to improve quality of life. Explorer and
resident, Marilyn Hoffman, wanted to see a hip protective belt on the
market because she believed it would reduce the fear of falling. Through
Aging2.0, Eskaton found Active Protective, a start-up from Pennsylvania.
Marilyn and her Longevity Explorer peers participated in a study to
evaluate the functionality of this prototype. Participants will receive a
complimentary smart protective belt upon its release in fall 2016.

Eskaton Village Carmichael
Eskaton Village Carmichael is advancing on the next phase of the
master plan to build an additional 20 assisted living apartments
located on acreage near the entrance of the Village. Spacious
one- and two-bedroom apartments, common spaces and beautiful
indoor and outdoor spaces are part of the plan. Expected to open
late 2017.

Assisted Living and Memory Care

Live Well at Home by Eskaton
The first year launch of Eskaton’s non-medical homecare was a
success, providing over 43,000 hours of home care in the first year
and achieving over 90% client and staff satisfaction. Live Well
recently launched “Transforming Dementia Care”, personalized and
engaging memory care at home.
The Trousdale, Burlingame, California
A 124-apartment assisted living and memory care community
is scheduled to open in early 2018. The community is owned by
Peninsula Health Care District and will be managed by Eskaton.

Music and Memory Program
Due to overwhelming resident response to the therapeutic benefits
of music, the Eskaton Care Centers have adopted the Music and
Memory Program, which reduces depression, increases resident
happiness and socialization, and overall improves resident quality
of life. Care Center staff have been certified to provide this valuable
program in our communities.
Fall Prevention Class Participants
Fall Prevention Classes
Eskaton Live Well at Home and Dignity Health launched a care
coordination demonstration pilot in July 2015 and since inception has
enrolled over 100 clients. 96% of clients who completed a six-week
fall prevention class saw significant improvement in overall strength,
flexibility and balance. 61% of clients who have completed the program
saw an improvement in quality of life. Participants also saw a 36%
reduction in number of falls as well as a 38% reduction in number of
emergency room visits at post-assessment.
Testing the Next Generation of Smart-Sensor Technology
Eskaton Village Carmichael, is piloting new technology designed
to allow staff and residents to engage more proactively in their
own health decisions. Installed in 72 apartments, the system gives
staff access to resident daily trends and receive alerts when there
are significant changes. This next generation technology brings
enhancements to the consumer experience.
Eskaton – Kaiser Partnerships
Whether it is piloting a C-Diff program in our Care Centers,
evaluating sub-acute care services or collaborating to keep hospital
readmissions to a minimum, the Kaiser – Eskaton partnership is
integral to the ongoing continuum of services provided to our
patients, clients and residents.
Eskaton Foundation’s Resident Care Program
Over 1,000 hours of nonmedical home care was provided to low-income
seniors who with “just a little help” can remain independent and safe in
their own homes longer. This new program can literally be a life-saver
and all made possible through the generosity of philanthropy.

Everything we do is because of you –
I’m pleased with how Eskaton Foundation is serving more seniors each year. Whether
you are a donor, volunteer, or board member, the work we do for seniors is made
possible because of you.
Because of you, last year we provided 2,208 round trip rides to necessary doctor
and grocery store visits. Read what one senior couple had to say –
Joanne, 79, and her husband Bernie, 92, use Transportation Plus to get back
and forth to doctor appointments. Joanne, who has macular degeneration, says
the program has been a huge help because she and her husband can no longer
drive. “If we ever lost it, we’d be in a lot of trouble,” Joanne says. “We don’t have
children and you can’t rely on your friends and neighbors all the time.”
Because of you, we are able to expand our Therapeutic Music Program into
Memory Care. Read what a therapist shared –
“One day I played in a room where the resident was very ill and surrounded by
visitors. The visitors acknowledged my presence, but did not speak. Later one of
the visitors approached me with tears in her eyes, hugged me and thanked me
for playing. Her mother had passed away shortly after I left.” Because of you, low-income seniors can stay in their own homes longer with our
Resident Care Fund. Read about Fred (there are thousands like him.) –

Because of you – Don Bromley
When asked, “Why do you give?”Don Bromley’s body language said it all — loud and
clear. Sitting at a card table finishing up a game of Rummikub, Don slowly pumped his
fist up and down. His face crinkled up in a smile and he bounced up in his chair. When
Don gives, it makes him feel real good!
Although active in the Chamber of Commerce and Rotary while running the busy
Best Western Garden Inn in Santa Rosa, Don didn’t consider himself a philanthropist
until getting involved with Eskaton Foundation. He is the largest donor to Eskaton
Foundation’s Adult Day Health Center Assistance Program — making it possible for
seniors with varying disabilities to attend. “I got to thinking about it, and it clicked,”
he said. “The program is necessary and there are many who can’t afford it. I am in a
position to support those who can’t. It’s a cause that oftentimes is overlooked. Helping
seniors is not always popular.”

Philanthropy and giving back are values that Brian
and Joan Unter live by. As an executive at Hewlett
Packard, Brian worked in an environment where
giving was important. Joan, a third grade teacher and
piano teacher, learned at an early age the pleasure

Not long after moving to Eskaton Village Carmichael
Brian and Joan got involved. Brian volunteers as
assistant treasurer for the Resident Council, is on
the Financial Review and Building and Grounds
Committees. Joan plays the piano. When they heard
how Eskaton Foundation helps our local community,
they eagerly supported. “Giving locally rather than
nationally is important to us, and helping seniors is
a cause that gives us a good feeling,” said Brian.
Making a significant deferred gift is one way that
Brian and Joan make an impact in the lives of seniors
today and well into the future.

Giving Back
We passionately believe in the significance of being “part”
of the community. We do that by practicing good corporate
citizenship including engaging in our local chambers, walking
for Alzheimer’s research, providing expertise on nonprofit
boards, and sponsoring events that uplift our region. Giving
back to build a strong and vital region is the cornerstone
of Eskaton.
As the largest community-based nonprofit organization
serving seniors in the area, we are proud to be a caring
community partner and aspire to spreading our mission
beyond our Eskaton walls.
Volunteerism

The power of volunteerism is strong at Eskaton. Whether
you lend your expertise by serving on a board or committee,
make a check-up call to a homebound senior, lend a hand at
an event, or sing to those with memory loss, volunteers are
welcomed and appreciated. At Eskaton we wholeheartedly
believe in the saying, “Volunteers are the life-blood of an
organization.” In any given year, nearly 3,000 volunteers
make a difference because Seniors Matter.

In Memory
Wes Justyn
1946 – 2016
Wes Justyn was an integral part of Eskaton for over 20
years. His compassion for helping others in need was
only surpassed by his determination and desire to further
Eskaton’s mission to enhance the lives of older adults.
Wes aimed high, whether it was shooting down clay
targets or raising funds for the Eskaton Golf Tournament.
His tireless commitment to providing needed services to
those less fortunate will be deeply missed.
Wes served on the Eskaton Foundation Board from
1994 to 2014 and was instrumental in increasing golf
tournament sponsorships to $200,000 annually. He
served for two terms on the Eskaton Board of Directors
beginning in 2003 where his leadership and fortitude
paved the way to a policy governance leadership board.
Wes’ foresight and personal experience played a key role
in the design and building of Eskaton’s Demonstration
Home, a universal design home that has received
numerous accolades and awards. He was chair of the
Eskaton Board from 2014 – 2016.
Wes will be remembered for his devotion to his family,
friends and Eskaton. We thank him for all that he has
done to make a positive impact in our community.

Joanne Handy, RN, MS
Gay Marie Bone has been involved with
Eskaton for over 15 years. She began as
a volunteer at Eskaton’s Golf Tournaments
and served as chair of the tournament for
several years. Gay Marie also participated on
the Eskaton Foundation Board of Directors.

President of External Affairs for AT&T, Tim Ray
epitomizes the consummate board member.
Because of his stellar leadership skills and
passion for our mission, Tim plays an integral
role in moving our mission forward.
Joanne Handy has served as President and CEO
of LeadingAge California for the past seven
years, representing over 400 nonprofit members
that provide aging services throughout the state.
Since 2003, she has served on the Eskaton
Board of Directors and held the position of
chair from 2012 – 2014.
We thank Gay Marie for her generosity of
time and talent in forwarding Eskaton’s
mission to enhance the quality of life of
older adults through innovative health,
housing and social services.
Tim has been with AT&T for nearly 25
years. He is very involved in the Sacramento
community and over the years has
provided his expertise to many nonprofit
and community boards. He is an American
Leadership Forum Class 13 Fellow and is well
known for his charismatic personality.
Tim has earned this special recognition and
we are continually grateful for his leadership,
fervor, and belief that Seniors Matter.
During her tenure, Joanne led the organization
through the “great recession” without a loss
in membership and led the rebranding process
to conform to the repositioning of the national
affiliate. Additionally, the association bifurcated
itself into a trade organization and a foundation
to elevate the effectiveness of its advocacy work.
Joanne’s leadership has been visionary and
will leave a lasting effect on the future of the
association’s mission.

her leadership and contributions to the field of

Eskaton Village Grass Valley was voted “Best Assisted
Living” and “Best Alzheimer’s Care” Community by
readers of The Union for the 12th consecutive year.
Eskaton Village Roseville was voted “Favorite Senior
Living Center”for the 7th year in a row by readers of
Roseville Style Magazine. Eskaton Village Placerville
was voted a “Favorite Senior Living Center” by
readers of Foothill Style Magazine.
Eskaton Lodge Cameron Park and Eskaton
FountainWood Lodge were selected 2016 “Caring
Stars” by Caring.com. Eskaton Lodge Gold River,
Eskaton Village Placerville and Eskaton Village Roseville
received their second “Caring Star” designation.
Eskaton was voted “Best Assisted Living” by readers
of Mountain Democrat and Village Life.
Eskaton was recognized for the third consecutive year
as the Sacramento region’s most health-conscious
employer in the medium-size organization category by
The Sacramento Business Journal.
Eskaton was voted “Best of the Best Senior
Community” and “Best of the Best Assisted Living”
by readers of Roseville Press Tribune and Granite Bay
Press Tribune.
Eskaton won two AVA Digital gold awards and a
National Council on Aging honorable mention for its

Live Well at Home by Eskaton received The Best of
Home Care® Employer of Choice Award, putting Live
Well at Home in the top 3% of home care agencies in
California for caregiver satisfaction.
Eskaton Care Center Fair Oaks was awarded payments
under the Quality and Accountability Supplemental
Payment Program for Freestanding Skilled Nursing
Facilities.
All four Eskaton Care Centers received 4-star
ratings from Centers for Medicare and Medicaid
Services (CMS).
