To Team and/or Club Representatives,
Welcome to the 2015 SCVA Junior Girls’ Volleyball Season! We will begin our season with the Qualifying
tournaments in January. Please look over the tournament registration forms included in the packet for
tournament dates and registration deadline dates.
Please read over the manual carefully. There are recent changes regarding the registration process,
including the commitment letter, tryout dates, and Application to Change Clubs procedure. There are also
new divisions for the 2015 USAV Junior Olympics. We also ask for your continued support in helping
keep the facilities clean during tournaments. It is your responsibility as club representatives to make sure
that all staff, coaches, and parents associated with your club are aware of all the rules and regulations.
Please inform all parents and all outside parties of the rules specific to the junior girls’ season. Please
discuss, explain, and emphasize the rules specific to eligibility, registration, and facilities. Parent
handbooks are available through the SCVA office and website.
The USA Volleyball Waiver and Release of Liability Form has been drastically changed over the past
few years. There is a new Individual membership form that is formatted to use for all membership types;
i.e. junior player, adult player, coaches, chaperones, officials, etc. Please indicate on the form which
membership type is being submitted. This is the only form that will be accepted—no old forms will be
accepted! All registration material is available on our website under the blue link “Registration” across the
top of our home page.
The Parent Handbooks are available this year in pdf format only. Please provide your club parents with
the link to the document under the blue link “Registration” across the top of our home page. If you need
additional information, please call the SCVA office at (714) 917-3595 weekdays from 10:00 AM to 2:00
PM or email the office at [email protected].
If you need to use Express Mail (US Postal Service) please sign the signature release waiver located on

I. GETTING STARTED
A. Registration
All members (coaches, players, chaperones, team representatives, and club directors) MUST register
with the SCVA in order to participate in any SCVA activity. All adults involved in the club (Including club
directors, coaches, team representatives, and chaperones) must pass a USA Volleyball Background
Screening test prior to the registration process. The background clearance is good for a two year period.
The background screening process will take at least 14 days for processing. Please incorporate this
time frame when registering adults. Please refer to the registration packet that has a copy of the waiver
and release of liability form for all members, the team registration form, the medical release form, and
the consent and waiver form for the background screening of all adults associated with the club. Please
refer to the directions in the registration packet for specific instructions regarding online registration and
additional registration information. If you have any questions regarding registration call the SCVA office.
DUE TO THE USA VOLLEYBALL INSURANCE REGULATIONS, REGISTRATION FOR ALL CLUBS
IS DUE TWO WEEKS PRIOR TO THE FIRST TOURNAMENT IN WHICH THEY ARE ENTERED
INCLUDING THE COMPLETED BACKGROUND SCREEINIG PROCESS.
A 2014/2015 Individual Membership Form for each player, coach, and team representative must be
completed. This waiver and release form is two sided and requires signatures on both sides. All sections
must be filled out completely and signed on both sides before the registration form will be accepted.
Along with the 2014/2015 Individual Membership Form a completed Letter of Commitment must be
submitted for each player. All clubs must collect a copy of the Medical Release Form from every
player. It is mandatory that every coach carries a copy of the Medical Release Form for each player in
case of an injury. Do not submit these forms to the SCVA office. They are to remain on file with your
club.
As part of registration, it will be mandatory for each coach to sign a CODE OF ETHICS and return the
signed code along with their registration form and consent and waiver form in order to be eligible to
coach in any SCVA event.
A TEAM REGISTRATION FORM must be filled out for each team and submitted with the individual
registration forms. The team registration form is not mandatory to use. You may elect to use a form of
your own choosing (i.e., off of your computer). However, if you do use your own form, the information
must be in the same format and in the same order as the attached SCVA form.
Online individual registration, the completed team registration form, the completed and signed individual
registration forms, and the appropriate money must be received in the SCVA office before registration is
complete. Birthdates must be submitted for each registered member. Please check the forms
carefully and make sure they are complete.
All clubs must collect a copy of the Medical Release Form from every player. It is mandatory that every
coach carries a copy of the Medical Release Form for each player in case of an injury. Do not submit
these forms to the SCVA office. They are to remain on file with your club.
The registration fees for the season are as follows: $60.00 for each junior player, $60.00 for each adult,
plus an additional $20.00 for background screening. The fees have been increased due to the increase
of USAV registration fees. For each junior team registered the fee is $15.00. Club registration is
$150.00 which includes registration for both girls and boys clubs using the same name as well as for
facility insurance. Note: No club with an outstanding balance will be eligible to register with the SCVA
until the balance is paid in full.
Registration forms filled out and signed at club tryouts are not considered commitment to play by the
players.

You may register new members throughout the season as long as: 1) the player is registered online, 2)
the completed paperwork and appropriate money is received in the SCVA office ONE WEEK prior to
participation, and 3) all eligibility requirements are met. Please note that a form to add new players to
your club is enclosed in the registration packet. Registration of new players will not be accepted unless
the add form is enclosed along with the waiver and release form.
No USAV registration cards will be distributed this season. Each club has the ability within their online
database to keep track of every player and staff and their USAV #. Upon request we can provide a
detailed description of how this is done.
NO REGISTRATION (EITHER PAPERWORK OR MONEY) WILL BE ACCEPTED AT ANY
TOURNAMENT SITE. It must be sent to the SCVA office. Please keep copies of ALL submitted
registration forms (both sides), and have your coaches carry copies of the medical release forms.
B. Insurance
1. Individual coverage
Once registered with the SCVA, members are covered by the USA Volleyball’s sport accident
policy. Please note that the liability insurance as provided by the SCVA will not cover the club
and/or its administrators during practice, tournaments, or tryouts unless the club and/or the SCVA
has on file the signed waiver and release forms. If an injury occurs during a tournament an Incident
Report Form must be completed (see Supplements) and returned to the SCVA office. Coaches
must carry a copy of the medical release forms with them at all times!
2. Certificates of Insurance
Certificate of Insurance, naming facilities as additional insured, are available upon request from the
SCVA office. The certificates will be issued within a three to four week period upon request.
Request should be made in writing listing the club, specific facilities required along with their
addresses, and sent by mail to the SCVA office. Please direct any questions regarding insurance to
Ann at the SCVA office.
C. Entries
The 2015 schedule is enclosed. The completed entry form and appropriate entry fee must be in the
SCVA office by the indicated deadline date before acceptance into any tournament. The entry deadline
will be FRIDAY, three weeks PRIOR to the tournament date. Phone and/or fax entries will be
accepted, only if there is available room. The entry fee will be $250.00 per team for each day of each
tournament, unless otherwise specified. One entry form can be used to enter any one or all of the
tournaments you wish. One entry form per club is enclosed—please make copies. The exceptions to
this entry form are the Las Vegas Classic, the Junior National Qualifiers, and the Summer Soiree, and a
tournament to be determined at a later date. These tournaments will have their own separate entry
forms. No entry will be accepted unless accompanied by the entry fee.
If a team is unable to participate, the SCVA office must be notified by 12:00 PM of the Friday
preceding the tournament date. If there is a problem after 12:00 PM the Friday preceding the
tournament date, you must leave a message at the SCVA office at (714) 917-3595. There will be
no refunds issued if a team drops from a tournament 10 days or less prior to the date of the
tournament.
Confirmation of the site and time will be posted on our website at www.scvavolleyball.org the week prior
to the tournament date. Please double check the playing sites on the Friday before the tournament date
to make sure there were no last minute changes. Please take the time to inform your coaches and
parents of the site locations for your teams, as it is your responsibility.

D. Age Divisions
USA VOLLEYBALL JUNIOR PLAYER AGE DEFINITION
For use during the 2014-2015 Season
Once a player participates (including but not limited to practicing, training, attending workouts and/or
competition) in a club or varsity program for any university, college, community college, or junior college,
he/she is ineligible to take part in any regional and national programming, which includes but is not
limited to national JNC qualifying and championship events.

Players who were born on or after September 1, 1996
or
Players who were born on or after September 1, 1995 and a high school student in the twelfth (12th)
grade or below during some part of the current academic year
17 and Under Division:
Players who were born on or after September 1, 1997
16 and Under Division:
Players who were born on or after September 1, 1998
15 and Under Division:
Players who were born on or after September 1, 1999
14 and Under Division:
Players who were born on or after September 1, 2000
13 and Under Division:
Players who were born on or after September 1, 2001
12 and Under Division:
Players who were born on or after September 1, 2002
11 and Under Division:
Players who were born on or after September 1, 2003
Note: These age definitions are used for domestic club programs and are not the same as the age
definitions for the International and/or Domestic High Performance programs for 2015.
No waivers will be granted for the 2015 season.
Please note that a junior athlete or team may NOT play in two age divisions during the same
season. An individual player may play in an older age division but must remain to play in that division
for the entire season (exception: optional tournaments). If you cannot field a team for a mandatory
tournament, you must consult the SCVA office in order to obtain a one-time waiver for the particular
tournament. If a waiver is not obtained, the team involved will forfeit all games for that day and will not
be allowed to participate in the next scheduled mandatory tournament. If more than one waiver is
granted during the course of the season due to a shortage of players, a different player must be
utilized each time.
II. ELIGIBILITY
A. Beginning of Season—Junior athletes are eligible to play for any club or team up until the beginning of
the sanctioned season. At the time of her first court appearance during the sanctioned season, the
athlete is to remain a registered member of that specific club throughout the season. Please note, that
for a club to be eligible to participate in regional events, or to be eligible for entry into the Las Vegas
Classic, the club must abide by the tryout and commitment dates set forth by the SCVA. However,
abiding by the tryout and commitment dates does not guarantee entry into these events.

B. Tryouts, Commitment Dates, and Transfer Policy
For the 2014/2015 season, all junior girls clubs are eligible to host tryouts for the various age divisions
on the following dates:
1. Girls 14 and Under and 12 and Under Divisions - October 9th, 2014
2. Girls 18 and Under through 15 and Under Divisions - October 2nd, 2014
SCVA Tryout Procedures should be posted at all club tryouts.
SCVA Tryout Procedures for San Diego Areas should also be posted at all tryouts.
For the 2014/2015 season, no junior player/parent/guardian may commit to a club prior to the following
commitment dates: 4:00PM on Monday, October 6th for 18 and under through 15 and under
division, and 4:00PM on Tuesday, October 14th for 14 and under and 12 and under
divisions. No deposits may be given during the tryout period. Once the letter of commitment is
signed and turned into one club, the player is then committed to that club.
Please note, the SCVA recognizes that San Diego area high school players are bound to the CIF
rules. The SCVA asks that all San Diego clubs and players abide by those rules, so as to not
jeopardize the eligibility of girls during their high school season. Thus the following tryout dates and
commitment dates will be used for those clubs in the San Diego areas:
1. Girls 14 and Under - tryout date of October 18, 2014 with a
commitment date of October 27, 2014.
2. Girls 16 and Under and 15 and Under - tryout date of November 8, 2014
with a commitment date of November 17, 2014.
3. Girls 18 and Under - tryout date of November 15, 2014 with a
commitment date of November 24, 2014.
A Letter of Commitment must be signed by all participants.
After the tryouts and Letter of Commitment are signed and turned in, all teams in division 15 and
under and higher do not begin practices until December 1, 2014 or after.
The Transfer Period will be from December 13th, 2014 through February 20th, 2015. No transfer
between clubs will be allowed prior to or after the transfer period. In order to transfer between clubs
during the transfer period, one must complete an application for a change of clubs, and then wait for
the SCVA to approve the transfer.
If a player does not follow the tryout procedures, commitment date, and/or the transfer procedures,
she may be suspended for the 2014/2015 season.
C. Coaches Impact Certification
It is a requirement for all coaches be Impact Certified. There are webinars available through the USA
Volleyball website—www.usavolleyball.org—under Resources. Or look at the information regarding
Impact clinics on the SCVA website—www.scvavolleyball.org.
D. Inter-Regional Transfers
A player shall represent no more than one region, club, or team in sanctioned competition during the
same season. A family relocation due to a job or military transfer may receive special consideration.

E. New Players
Players may be registered at any time throughout the year as long as the proper paperwork and monies
is received prior to participation.
F. Team Changes Within Clubs
During the season, an athlete may change teams within her age division within her respective club.
However, rosters must remain the same for the Qualifying, Regional, or Special Event tournaments.
Please note that it is your responsibility as coaches to make sure that the players as well as the parents
are well informed of these eligibility rules. There is a section that the parents must sign on the
registration form which acknowledges that they have been informed of these rules. They are to consult
their coach or Club Director for further details. Any questions regarding eligibility please call Ann
Davenport at the SCVA office. Please take the time to inform the parents of the involved athletes prior to
the beginning of the season as this will help alleviate misunderstandings.
G. Clearinghouse Procedures—College Bound Athletes
Prospective student-athletes intending to participate in Division I or II athletics must register and be
certified by the NCAA Initial-Eligibility Clearinghouse. Please inform your athletes and their parents to
obtain registration materials from their high school guidance counselors.
III. NON-SANCTIONED COMPETITION
Any team or club desiring to conduct a volleyball event or local, regional, or zone competition must receive
a sanction/approval from the SCVA. SCVA-registered players and teams competing in non-sanctioned
competition may jeopardize their eligibility.
AT THIS TIME, the SCVA will only sanction the following events: tryouts, scrimmages, or friendship
tournaments (where no entry fees are charged).
USAV/SCVA insurance is not in force during non-sanctioned events. Please refer to the USAV Official
Rulebook and/or Guidebook for specific policies.
IV. RECRUITING POLICY
Although the SCVA is aware of certain illicit recruiting actions, we have an inability to enforce such a policy
because recruiting violations come to us in a “he said, she said” manner and are very difficult to ascertain
fault. Therefore, we are asking the cooperation of all coaches and directors in this matter. The following are
guidelines that we ask all clubs to abide by:

Refrain from making negative comments concerning other clubs and/or their coaches.

Allow no coach or club representative to make initial contact with an athlete or her parents.
We ask that these guidelines be followed in order to foster a positive experience for the players in the
pursuit of their volleyball goals.
V. FORMAT
The tournament format is designed to give maximum competition to each athlete in our program and to
allow them to achieve a healthy and satisfactory attitude toward themselves, their teammates and the sport
of volleyball. Following the qualifying tournaments, each team will be placed into their level of competition
(Divisions). There will be movement of teams following each Mandatory Tournament. Points will be

awarded to teams based on their order of finish in their respective divisions. No points will be given for
optional or special event tournaments. Please refer to the enclosed point system for the specific point
breakdown.
A. Seeding
The only information available for seeding teams is what is submitted by the club with their tournament
entry form. Please include the following information:





Team Name
Expected Division—what seed you expect; Division 7-8 would be equal to Silver Division, Seed
3 or 4.
Last year’s finish for the team
Strength comparison to last year’s team.
Comments helpful to understanding the team’s strength and weakness.
B. Qualifying Tournaments
Requests for morning and afternoon playing times for Day 1 will be considered only if noted on the
Qualifying entry form and received by the entry deadline. The Qualifying Tournaments will
determine the starting Division (and position within that Division) for each participating team in the
SCVA Mandatory Tournaments. Any non-participating team that wishes to enter future tournaments
will be placed in the appropriate position (excluding Divisions I, II, III, and IV). The Qualifying
Tournaments have their own separate entry forms that MUST be used to enter the teams. Teams
must enter ALL dates of the Qualifying Tournaments. A complete and detailed description for all the
age divisions will be issued when the number of teams in each division is known. Please refer to the
accompanied description. Remember, this is to be used as an example only; the exact format will be
issued later. Any non-qualifying teams wishing to enter future tournaments will be seeded at the
Program Director’s discretion.
18 & Under Division—3 Days (Estimated 275 teams)
Teams will be expected to identify themselves as Gold Division (top 64), Silver Division (next 64),
Bronze Division (next 64), Copper Division (next 64), and Brass Division (all remaining teams) on the
Qualifying entry. All teams wanting consideration for the Gold Division must submit a separate,
written statement to justify this consideration. Without this statement, seeding will be done at
the discretion of the Program Director. Teams will then be placed into the divisions based on their
own rankings and the number of total entries. All teams will compete on Day 1, Day 2, and Day 3 in
pool play and the final starting positions for the first Mandatory Tournament will be determined.
16 & Under Division—3 Days (Estimated 275 teams)
Teams will be expected to identify themselves as Gold Division (top 64), Silver Division (next 64),
Bronze Division (next 64), Copper Division (next 64), and Brass Division (all remaining teams) on the
Qualifying entry. All teams wanting consideration for the Gold Division must submit a separate,
written statement to justify this consideration. Without this statement, seeding will be done at
the discretion of the Program Director. Teams will then be placed into the divisions based on their
own rankings and the number of total entries. All teams will compete on Day 1, Day 2, and Day 3 in
pool play and the final starting positions for the first Mandatory Tournament will be determined.
15 & Under Division—3 Days (Estimated 200 teams)
Teams will be expected to identify themselves as Gold Division (top 64), Silver Division (next 64), and
Bronze Division (all remaining teams) on the Qualifying entry. All teams wanting consideration for
the Gold Division must submit a separate, written statement to justify this consideration.
Without this statement, seeding will be done at the discretion of the Program Director. Teams
will then be placed into the divisions based on their own rankings and the number of total entries. All
teams will compete on Day 1, Day 2, and Day 3 in pool play and the final starting positions for the first
Mandatory Tournament will be determined.

14 & Under Division—3 Days (Estimated 240 teams)
Teams will be expected to identify themselves as Gold Division (top 64) , Silver Division (upper middle
64), Bronze Division (lower middle 64), and Copper Division ( next 64 teams), and Flight 1 (all
remaining teams)on the Qualifying entry. All teams wanting consideration for the Gold Division
must submit a separate, written statement to justify this consideration. Without this statement,
seeding will be done at the discretion of the Program Director. Teams will then be placed into the
divisions based on their own rankings and the number of total entries. All teams will compete on Day
1, Day 2, and Day 3 in pool play and the final starting positions for the first Mandatory Tournament will
be determined.
13 & Under Division—3 Days (Estimated 160 teams)
Teams will be expected to identify themselves as Gold Division (top 32), Silver Division (next 32),
Bronze Division (next 32), and Copper Division (all remaining teams) on the Qualifying entry. All
teams wanting consideration for the Gold Division must submit a separate, written statement
to justify this consideration. Without this statement, seeding will be done at the discretion of
the Program Director. Teams will then be placed into the divisions based on their own rankings and
the number of total entries. All teams will compete on Day 1, Day 2, and Day 3 in pool play and the
final starting positions for the first Mandatory Tournament will be determined.
12 & Under Division—3 Days (Estimated 160 teams)
Teams will be expected to identify themselves as Gold Division (top 32), Silver Division (next 32),
Bronze Division (next 32), and Copper Division (all remaining teams) on the Qualifying entry. All
teams wanting consideration for the Gold Division must submit a separate, written statement
to justify this consideration. Without this statement, seeding will be done at the discretion of
the Program Director. Teams will then be placed into the divisions based on their own rankings and
the number of total entries. All teams will compete on Day 1, Day 2, and Day 3 in pool play and the
final starting positions for the first Mandatory Tournament will be determined.
NO POINTS WILL BE GIVEN FOR THE QUALIFYING TOURNAMENTS.
C. Mandatory Tournaments
Each team positioned in all Divisions must play in each Mandatory Tournament. The 18 and under
division will have 4 Mandatory Tournaments. All other divisions will have 5 Mandatory Tournaments. If a
team does not participate, the team will automatically drop a Division, but points will be given for last
place of that Division. If a team chooses not to participate in a SCVA Mandatory Tournament in order
to attend another tournament elsewhere, that team could drop a division. All Divisions will consist of 16
teams. Points will be given to each team participating in the Mandatory Tournaments.
Division I - IV:
The 16 teams will be seeded into 4 four team pools with match play. The top two
teams from each pool will play-off in a one game to 25 bracket format which will be played all the way
through. All four teams in each Division (with the exception of Division 1) will advance one Division for

the following Mandatory Tournament. The 13 , 14 , 15 , and 16 place teams in each Division will
move down one Division for the following Mandatory Tournament. See below for the playoff summary.
1. Girls 13s – 18s
a. Playoffs for Divisions 1 – 2
1) Upper and Lower Brackets – first round is 2 out of 3 games
2) Following rounds are 1 game to 25
b. Playoffs for Divisions 3 – 4
1) Upper Brackets -- First round is 2 out of 3
2) Lower Brackets – One round of playoffs – 2 out of 3 games to
determine which team drops a division.

a. Playoffs for divisions 1 and 2 only
b. All playoffs are 1 game to 25

Division V thru remaining Divisions: The teams will be seeded into four pools of four with match

play. The winner of each pool will move up one Division, and the 4 place team in each pool will move
down one Division for the next Mandatory Tournament. Play will be divided into Am and PM sessions
for Divisions V thru remaining Divisions. Starting times will alternate each Mandatory to ensure
fairness. Please note: if it becomes necessary to have a three team pool, match format will be the
best 3 of 5 games. Five team pools will play 2 out of 3 games on 2 courts, if possible.
D. Regional Tournament
The Regional Championships will be a one-day event for the 16 and under and 18 and under age
divisions. It will be a two-day event for all remaining divisions with pool play on Saturday and pool play
followed by single-elimination brackets on Sunday. The top 32 teams based on the overall point totals
will play in the Gold Division. If there is a tie on points for Regionals, the tie will be broken by using the
most current finish from the last Mandatory tournament. The remaining teams will be divided into groups
of 32, 48, or 64 to comprise the Silver, Bronze, Copper, and so forth. Double points will be awarded to
all participating teams.
E. Points: A point system based on a percentage multiplier will be implemented. It is under review and will
be distributed once the multiplier has been established.

A team must qualify for the Junior National Open Divisions. In order to qualify a team must enter
and play in one of the Junior National Qualifiers. For the inter-regional qualifier schedule, check at

a guaranteed number of slots into the Junior National Tournament.
The Girls’ Junior National Championships also offers three club divisions, the National, USA and the
American divisions. For the National division, the SCVA regional bids will be awarded to teams
based on the current regional rankings. There are 24 total USA bids all to be awarded at Junior
National Qualifiers. The 18, 17, 16, 15, 14, 13, and 12 and under divisions will be awarded at least
one American bid at each of the Junior National Qualifiers, providing that the division meets the
required number of teams per division. The American bid will also give one bid per division to the
SCVA region which will be awarded based on current standings. The Patriot division is a division for
which anyone can apply for entry. It is filled on a “first come first serve basis.” The SCVA highly
recommends that if a club and/or team is interested in the Junior National Volleyball Championships
to send all necessary paperwork and monies by the stated USA Volleyball deadlines. For further
information regarding the Junior National Championships, Junior National Qualifiers, and/or regional
bids please contact Ann Davenport in the SCVA office.
Once a team earns and accepts a bid in a Qualifier or applies for a bid in an at large process, the
roster of that team is frozen. All players listed on the roster of a team qualified for the Junior
National Championships may not transfer to any other club and/or team. In addition, players on a
frozen roster may not transfer to any other team even within the same club.

VI. TOURNAMENT REGULATIONS
A. General Rules
1. A maximum of 15 players and 5 coach/staff personnel are allowed.
NOTE: Will be enforced this season.
2. Net Serve—If the ball hits the net on a serve, the ball is alive (playable).
3. Substitutions—In addition to the unlimited Libero substitutions, each team will have 12 team
substitutions.
4. All matches are 2 out of 3 games, rally scoring to 25 points in games 1 and 2 with no cap. Game 3
will be rally scoring to 15 points with no cap. Because of the rally point scoring method, teams have
three options at the coin toss: to serve, to receive serve, or to take choice of court. Please note that
one game play-off will be one game to 25, rally scoring with no cap.
5. The service zone has been extended to the entire width of the end lines.
6. Ceilings and/or obstructions are now in play.
7. The ball can be played with any part of the body.
8. Each player receives only one toss per service attempt. Two tosses are allowed for 14 and 12 and
under divisions only.
9. If a back row player plays the ball above the height of the net while on or in front of the 3-meter line,
the next player to touch the ball must be a teammate. If the ball enters the vertical plane of the net
and is blocked by the opposing team, it is an illegal attack by the back row player.

B. Rules specific to the 12 and 10 and Under Divisions:
Net height will be 7 feet
The Mikasa Ultra Lite Volleyball will be used (8 ounces)
Two serving tosses will be allowed
All other USA Volleyball rules will prevail.
C. Tie-Breaking Procedure (for Qualifying, Mandatory, and Regional)
In the case of a tie, pool position will be determined by the following formula:
2 way tie: 1) match record, 2) head to head
3 way tie: 1) match record 2) game record 3) point ratio
Play off seeding (Divisions I-IV only): Seeding will be based on 1) pool finish 2) match record 3) game
record 4) point ratio. Teams will be seeded to avoid any “pool conflicts” until the finals. The Tournament
Director will make the final decision if there are any further complications.

1. The playing uniform shall consist of a jersey, shorts, socks, and light shoes (rubber or leather
soles without heels).
2. It is forbidden for Junior National age players to wear jewelry, including rope bracelets.
3. It is now required that all uniform numbers be centered on the chest and back. It will also be
required that all the numbers be 6” in height on the chest and 8” in height on the back. Numbers
must be in a color contrasting to that of the jersey and must be visibly located. Shirts may be
numbered from 1 through 99 inclusive. (0 or 00 is not allowed.)
4. Uniforms (jerseys and shorts) must be of the same color, style, and trim. If T-shirts, boxer shorts,
tights, leotards, body suits, bicycle shorts, etc are worn in such a manner that they are exposed,
they will be considered part of the uniform and must be worn by all the team members and must
be identical. For the purpose of identical uniforms, shoes, socks, and kneepads are not
considered part of the uniform and are not required to be identical for team members. During
cold weather, it is permissible for teams to wear identical training suits provided they are
numbered in accordance with the specifications of #3 above and are of the same color, style,
cut, and trim.
5. Uniform regulations will be strictly enforced. The Tournament Director will be responsible for
determining infractions and may make exceptions to this rule if circumstances warrant.

F. Officiating
Each team must provide a complete officiating team when scheduled. If the officiating team is late for
their assignment, that team will be responsible for officiating the remainder of that match and the next
non-pre-assigned match in that pool. Failure to officiate for an entire assigned match will result in the
forfeit of a game win from that team's overall record. A game win will become a game loss. If a team is
late for the last match of the day or misses the assignment, the penalty as described above will carry
over to the next tournament.
The officiating team will consist of SIX people:

A team that fails to have a complete officiating crew available for an officiating assignment including a rostered adult (preferably a team coach) for that team - at the time the court official
is ready to proceed can be penalized by losing points in the first set of its next match.
For every minute an officiating team is late to fulfill its assignment, one point will be awarded to
that team’s next opponent in the first set of the next match, up to a maximum of 25 points.
Penalty points begin to accumulate at the start of the receiving team’s warm-up.
An adult listed on the event roster must be present at the scorer’s table and easily identifiable
while any members of the team are officiating. Performing duties as the R2 on the match the
team is assigned to officiate fulfills this requirement.
A team may forfeit its entire next match for failing, a second time, to have an officiating crew
available on time for an officiating assignment.

Enclosed in the Supplements please find a USA Volleyball score sheet. Make sure your team
members are familiar with the score sheet and can use it correctly. If you have any problems with an
official or need a rule clarification, please call the office at 714-917-3595.
G. Supervision
A registered member of USA Volleyball is REQUIRED to supervise ALL team members during any
SCVA competition. This person, or another registered adult, is required to remain on site until all team
members have left the campus. This pertains to ALL age levels. Failure to follow this requirement will
result in a $50.00 penalty fee, due before any future participation by the entire club. If the designated
supervisor is someone other than a coach, the Tournament Director must be notified.
H. Warm-ups
A 10-minute warm-up (including ball-handling, hitting, and serving) will be allowed before your first
match of the day ONLY--2 minutes shared ball handling and 4 minutes per team for hitting and serving.
A 4-minute per team warm-up will be enforced throughout the remainder of pool play.

Enclosed in the Supplements please find a copy of a roster/line-up sheet. Prior to your first match of
the day you will need to complete and turn in this form to the official on your court. This must be done at
EACH TOURNAMENT. List only the players (First and Last Names) that will be participating on that
given day. You will also need to provide the official with a separate line-up sheet for each game played
during the tournament. The line-up sheet has on the reverse side the appropriate number of line-up
forms for use during the entire day. No verbal line-ups will be accepted.

Protest Procedure
A protest must be filed at the time of the incident—no later. Only the floor captain may file the protest
(the coach may intervene in 14,13, and 12 and under competition). The protest must be written on the
score sheet by the scorer or official and must be signed by both the floor captain and the referee. The
protested game will be completed but before the next game begins the protest must be ruled upon by
the site director. A protest cannot be filed after the completion of the match. JUDGEMENT CALLS MAY
NOT BE PROTESTED.
VII. FACILITY REGULATIONS
This season ALL facilities will follow the same rules. Please note that these are facility rules. A facility
encompasses the entire building: hallways, foyers, gym proper, and any rooms (unless otherwise
specified). The Tournament Director will make exceptions in case of inclement weather. The SCVA asks
your help in informing all parents, your players, and interested parties regarding these facility rules. If these
rules are not adhered to, we will lose the use of the gyms. (Please emphasize that NO food, drink, or
beach chairs are allowed.)
A. Chairs—NO chairs of any kind will be allowed INSIDE any facility. They may not be placed on
blankets, towels, or newspapers. The use of beach chairs is prohibited by campus administration.
B. Coolers—NO coolers will be allowed inside any facility unless the Site Director sets a specific area
aside.
C. Plastic squeeze water bottles—Plastic squeeze bottles with push/pull tops filled only with water will be
allowed courtside. Glass Containers of any kind are strictly prohibited.
D. Food and drink—NO food or drink will be allowed inside any facility unless the Site Director sets a
specified area aside. Any athlete SEEN with food or drink will not be allowed to participate in the
next game. If any person associated with a club (i.e., parent, coach, and/or other interested party) is
SEEN with food or drink in the gym all team and/or club members will remain until the conclusion of the
tournament on the same day of the infraction to thoroughly clean the facility. If a team, their members,
or supporters introduces and/or consumes alcoholic beverages in or about the playing site, the penalty
will be immediate disqualification.
E. Animals—No animals are allowed in any facility or any school campus, with the exception of service
dogs.
F. Skateboards and Rollerblades—There is NO skateboarding or rollerblading in any facility or on any
school campus, including parking lots.
G. Barbecues—NO barbecues that use briquettes, gas, or propane will be allowed on any campus.
** All water bottles must be removed from the court at the conclusion of your match. Water
bottles left after a match may be cause for your team’s loss of privilege to have water courtside.
The gym and outside eating and lounging areas must remain clean and picked up throughout the
tournament. Trash bags will be provided if necessary. Failure to comply with this will result in your
teams cleaning the facility.

COMMUNICABLE DISEASE PRECAUTIONS
……when a player is bleeding, has an open wound or has
blood on his/her uniform, the game shall be stopped
immediately. The player must leave the court and shall not
return until the proper procedure is followed. The
competition area and equipment must be completely
cleaned and disinfected before play resumes….
1. Routine use of glove or other precautions to prevent skin an mucousmembrane exposure when contact with blood or other body fluids is
anticipated.
2. Immediately wash hands and other skin surfaces if contaminated (in
contact) with blood or other body fluids. Wash hands immediately after
removing gloves.
3. The bloodied portion of the uniform must be properly disinfected or the
uniform changed before the athlete may continue participation.
4. Clean all blood contaminated surfaces and equipment with a solution
made from a 1 – 10 dilution of household bleach or other disinfectant
before competition resumes.
5. Practice proper disposal procedures to prevent injuries caused by
needles, scalpels and other sharp instruments or devices.
6. Although saliva has not been implicated in HIV transmission, to minimize
the need for emergency direct mouth-to-mouth resuscitation,
mouthpieces, resuscitation bags or other ventilation devices should be
available for use.
7. All personnel with bleeding or oozing skin should refrain from all direct
athletic are until the condition is healed.
8. Contaminated towels should be properly disposed of.
2012-2013 Season
INSURANCE PROGRAM
USA Volleyball administers a medical and liability insurance program customized
specifically for the sport of volleyball. It is designed to respond specifically to the
inherent hazards of the sport. Offered as part of the sanctioning program, it is one of
the most cost effective policies available.

GENERAL LIABILITY COVERAGE SUMMARY
The General Liability plan includes spectator and participant liability, sexual abuse
and harassment coverage. A $1,000,000 limit of liability for bodily injury and property
damage loss is provided with additional limits available on an excess basis. Coverage
is provided for officials, volunteers, coaches, trainers, sponsors and registered
participants. The policy covers liability from pre-event setup, the event itself and postevent activities at sanctioned events. The policy will respond to claims from spectators,
participants and the public in general. The USA Volleyball registration requirement is
a condition of the liability policy and a common practice among sports federations.
The general liability policy provides an extension of coverage to include non-owned
physical damage to volleyball related sports equipment which is under your care
custody and control. This extension is limited to a maximum of $5,000 and required
certified damage or proof of loss from payment.
NAMED INSURED: United States Volleyball Association, United States Volleyball, Inc. (“USAV”),
USAV Regional Volleyball Associations committee members and commissioners
while acting on the behalf of and under the direction of USA Volleyball,
USAV Member Clubs, but only with respects to activities sanctioned or approved
by USAV or its Regional Association Commissioner,
USAV coaches, trainers and “member officials”, while acting in their capacity as
such, but only with respect to activities sanctioned or approved by USAV or its
Regional Association Commissioners.
USAV “member officials” for non-USAV sanctioned volleyball competitions who
have paid the appropriate premium and whose names are recorded and on file
with USAV;
Validly-registered athletes participating in events sanctioned by USAV or its
Regional Volleyball Association (Validly-registered means registered with a
Regional Volleyball Association)
Event organizers/promoters/event managers of USAV-sanctioned events and
only with respects to USAV sanctioned events.
*No coverage will apply for regions and clubs for events conducted in which
all participants are not USAV members, except for non-sanctioned
fundraising activities.
It is further understood and agreed that USAV “member officials” are those who
have successfully completed the USAV officials’ certification program.

Definition: “Sanctioned or Approved” Event: An event USA Volleyball and a Regional
Association Commissioner has approved or sanctioned as a USA Volleyball event. Events shall
include, but may not be limited to, team competition, practices, sports clinics, or fundraisers
conducted or attended as a part of a sanctioned event.
ADDITIONAL INSUREDS: Certificates will be issued naming other interests as
additional insured in respect to sanctioned activities by the named insured.

A REVIEW OF GENERAL LIABILITY COVERAGE
Commercial General Liability insurance provides coverage for claims of bodily injury or
property damage made against the insured for which they become legally liable. The
insurance company will pay on behalf of USA Volleyball and other named insureds,
claims which the Insureds shall become “legally” obligated to pay as damages because
of bodily injury or property damage to which the insurance applies, caused by an
occurrence during the policy period, up to the policy limit.
The General Liability policy for USA Volleyball is an “occurrence” policy. A claim under
this policy shall be considered as being first made at the earliest of the following times:
(a) When USA Volleyball first notifies the Insurance Company in writing that a claim
has been made; or
(b) When USA Volleyball first notifies the Company in writing that a suit has been
brought; or
(c) When USA Volleyball first notifies the Company in writing of specific
circumstances, which may result in a claim being made or suit being brought.
All claims for damages because of bodily injury to the same person, including damages
claimed by any person or organization for care, loss of service, or death resulting at
any time from the bodily injury, will be deemed to have been made at the time the first
of those claims is made.

All claims for damages because of property damage causing loss to the same person or
organization as a result of an occurrence will be deemed to have been made at the
time of the first of those claims is made.
EXCLUSIONS
The USA Volleyball General Liability insurance policy does not apply to the following:
(a) Ownership, maintenance, operation, use, loading or unloading of any automobile
or aircraft owned or operated by or rented or loaned to any Insured or operated by
any person in the course of employment by any insured.
(b) Actual, alleged or threatened discharge, dispersal, release or escape of pollutants.
(c) Loss due to war, whether or not declared, civil war, insurrection, rebellion or
revolution.
(d) To any obligation for which USA Volleyball may be held liable under any workers’
compensation, unemployment compensation or disability benefits law.
(e) To bodily injury to any employee of USA Volleyball arising out of and in the course
of their employment or to any obligation of USA Volleyball to indemnify another
because of damages arising out of such injury.
(f) To loss arising out of asbestos
(g) To loss arising out of employment related practices
(h) Claims or actions brought by one player against another player
(i) Intentional Acts: Bodily injury or property damage expected or intended from the
standpoint of the insured.
(j) Bodily injury or property damage for which any insured may be held liable by
reason of:
(1) causing or contributing to the intoxication of a person
(2) the furnishing of alcoholic beverages to a person under the legal drinking age or
under the influence of alcohol or any statue, ordinance or regulation relating to the
sale, distribution or use of alcohol beverages.
The above exclusions are only a recap of the pertinent exclusions. This policy contains
additional exclusions not specifically listed here.

BROADENED COVERAGE
The General Liability policy has been broadened to include the following coverage:
(a) Contractual Liability – Covers oral and written contracts or agreements relating to
the conduct of USA Volleyball’s business.
(b) Personal Injury and Advertising Injury Liability – Covers USA Volleyball’s legal
obligations for injury to others arising from:
(1) False arrest, detention, imprisonment or malicious prosecution
(2) Libel, slander, defamation or violation of right of privacy and/or
(3) Wrongful entry or eviction or invasion of right of private occupancy
(c) Incidental Malpractice Liability – Covers USA Volleyball, Employees and Volunteers
for legal liability arising out of rendering or failure to render certain professional
health care services. Medical providers must be volunteers without compensation.
This coverage is limited to the terms and conditions of the actual policy.
(d) Host Liquor Liability – Covers against loss arising out of the giving or serving of
alcoholic beverages at functions incidental to USA Volleyball’s normal operations.
(e) Premises Damage Legal Liability - $1,000,000 for property damage to premises
insured that USA Volleyball rents from others, or premises temporarily occupied by
the named insured. This coverage is excess insurance only over any part of any
other insurance that provides coverage for property damage to said premises.
(f) Non-owned Watercraft (up to 58’ feet) – Covers loss arising out of the use of nonowned watercraft by USA Volleyball.
(g) Limited worldwide liability coverage for bodily injury, property damage, personal
injury and advertising injury liability subject to the terms and conditions of the
actual policy.
(h) Additional Persons Insured – Broadens the Named Insured to include any
employees of USA Volleyball while acting within the scope of their duties.
(i) Extended Bodily Injury coverage provides coverage for loss resulting from
intentional acts resulting in bodily injury if the use of reasonable force is used to
protect persons or property.

SPORT ACCIDENT EXCESS MEDICAL INSURANCE COVERAGE
The Sport Accident Excess Medical insurance program provides participant coverage
for loss resulting directly from members competing in an approved or sanctioned
event. Coverage does not include loss from pre-existing conditions or competing in
non-sanctioned events. The coverage extends from the start, through the completion of
the event, including direct designated group travel to and from the event.
The Medical policy provides up to $25,000 of excess accidental medical coverage for
expenses incurred within one year of the date of the accident. Written proof of loss by
the Insured is required within 90 days or as soon as is reasonably possible. Proof
must, however, be furnished no later than 12 months from the time it is otherwise
required, except in the absence of legal capacity. The policy provides coverage against
loss in excess of coverage provided under other valid and collectible medical insurance
and is subject to a $250 per claim deductible. If no other collectible medical insurance
is available, the loss is subject to a $1,000 deductible.
If injury to the member athlete requires treatment by a legally qualified physician or
confinement in a legally constituted hospital, or employment of a trained nurse, x-ray,
or ambulance services, and if the first expense of such treatment is incurred within 90
days of the date of the accident, the insurance company will pay the usual and
customary expense incurred up to $25,000, subject to the appropriate deductible and
any other collectible insurance.
DEFINITION OF PARTICIPANT: Athletes, coaches, trainers, volunteers, committee
members, officials and any other persons who are functioning on behalf of and/or
while participating in any event sanctioned or approved by USA Volleyball.
DEFINITION OF PARTICIPATING: Participating includes pre-event and post event
activities which are officially approved or sanctioned events. Pre-event activities can
include but are not limited to clinics, event dinners and pre-event practicing. Post
event activities can include but are not limited to award banquets, award ceremonies
and clinics that occur within one day after the event. Coverage is also included for
non-sanctioned volleyball related activities for certified officials who meet extended
coverage criteria.
ACKNOWLEDGEMENT WAIVER AND RELEASE FROM LIABILITY
As with most sports activities, a signed “Acknowledgement Waiver and Release from
Liability” (AWRL) form is required from all participants and from parents or guardians
in the case of minors. This requirement exists in virtually every sport. It serves to
document that the participants or parents of participating minors have acknowledged
the inherent risk and danger associated with participating in sporting events. It is
intended to serve as “appreciable warning” of these risks and the participants by
signing the waiver, are giving their informed consent to the acceptance of those risks.
It is important to remember that a signed waiver DOES NOT reduce the need for
insurance or effective safety practices. A signed waiver is USAV’s “first line of defense”
against a cause of action for negligence and is a very effective risk management tool.
The Regional Commissioner and others working under the direction of the Region
must make every effort to conduct an event with safety as the number one concern.

Secondary Sport Accident – Summary of Coverage for 2012-2013 Season
All Domestic Team Participants Competing in Sanctioned USA Volleyball Events

What is covered?
Accidental injury that occurs while participating in USA Volleyball sanctioned events.
Who is covered?
All members registered with USA Volleyball including but not limited to athletes, coaches, trainers,
volunteers, committee members, and officials while functioning on behalf of or while participating in a
sanctioned event.
What are the benefits?
Excess Accident Medical – $25,000 maximum per injury
Coverage will consider the usual and customary expense for medically necessary care received at a
hospital or provided by a licensed practitioner.
Accidental Death & Dismemberment - $10,000 principal sum
Coverage will consider $10,000 for the accidental loss of life and $2,500, $5,000 or $10,000 (depending
on loss type) for covered incidents resulting in accidental dismemberment. Loss must occur within 100
days after the date of accident.
Is there a deductible?
Yes. The deductible for USA Volleyball’s accident medical coverage is $250 for participants with primary
health insurance. This means that the injured person must pay the first $250 of the medical bill. If
primary health insurance is not carried, the deductible is $1,000.
Does the policy have any restrictions?
 For coverage to apply, the injury must be reported immediately to an official.
 The policy provides coverage against loss in excess of coverage provided under other
valid and collectible medical insurance.

I acknowledge that volleyball or any sporting event is an extreme test of a person’s physical and mental limits and that my participation in a volleyball event can cause potential death,
serious injury, or property damage. With a full understanding of the potential risks, I HEREBY ASSUME THE RISKS OF PARTICIPATING OR OFFICIATING IN A
VOLLEYBALL EVENT.
I hereby take the following action for myself, my executors, administrators, heirs, next of kin, successors and assigns: a) I WAIVE, RELEASE, AND DISCHARGE from any and all

THE FOLLOWING ACTIONS ARE PROHIBITED:
1. Violation of any anti-doping policies, protocols or procedures as defined by the International Olympic Committee (IOC), World Anti-Doping Agency (WADA), Federation
Internationale de Volleyball (FIVB), US Anti-Doping Agency (USADA) or the United States Olympic Committee (USOC). Violations of this provision will be adjudicated only by
USADA or the proper anti-doping authority, not USA Volleyball (USAV).
2. Possession, consumption or distribution of alcohol and/or tobacco if illegal or in violation of USA Volleyball (USAV) or Regional Volleyball Association (RVA) policy.
3. USAV policy prohibits the possession, consumption or distribution of alcohol and/or tobacco by anyone registered as a junior volleyball player at the event venue of any
USAV/RVA sanctioned junior event.
4. Use of a recognized identification card by anyone other than the individual described on the card.
5. Physical damage to a facility or theft of items from a room, dormitory, residence or other person. (Restitution will be part of any penalty imposed.)
6. Possession of fireworks, ammunition, firearms, or other weapons or any item or material which by commonly accepted practices and principles would be a hazard or harmful to
other persons at USAV/RVA sanctioned events.
7. Any action considered to be an offense under Federal, State or local law ordinances.
8. Violation of the specific policies, regulations, and/or procedures of the USAV, RVA or the facility used in conjunction with a sanctioned event. (It is the responsibility of the
individual to be familiar with applicable specific policies, regulations and procedures.)
9. Conduct which is inappropriate as determined by comparison to normally accepted behavior.
10. Physical or verbal intimidation of any individual.
11. Actions that will be detrimental to USAV or the RVA.

bylaws and operating codes of USA Volleyball and the RVA as printed in the current Official USA Volleyball Guide and RVA Handbook,
respectively.

Junior Club Eligibility:
Junior Girls: I agree that I will be affiliated with the above named club for the 2014/2015 sanctioned season, and that I am aware of the Junior Girls Eligibility
Requirements. I am aware that for a girls club to be eligible for regional competition the club must abide by the tryout dates and commitment dates and transfer
policies and procedures set forth by the SCVA. In addition, any transfers must be approved by the SCVA office. The transfer deadline for the Junior Girls
season is February 20, 2015. Under NO CIRCUMSTANCES will transfers be granted to any player who has competed at ANY JUNIOR NATIONAL
QUALIFIER.
Junior Boys: I agree that I will be affiliated with the above club 2014/2015 season. I am aware that for a boys club to be eligible for regional competition, the
club must abide by the tryout dates and commitment dates set forth by the SCVA. For the 2014/2015 season, clubs will be eligible to host tryouts after
September 4th with a commitment date of September 9th. In addition, any transfers must be approved by the SCVA office. No transfers will be awarded
UNDER ANY CIRCUMSTANCES after May 29, 2015.

Application to Change Clubs – Junior Girls
During the sanctioned season, a player is to remain a registered member of one specific club throughout the
season. However, during the transfer period, a player or parent/guardian may request a transfer in club affiliation.
The transfer period is from December 13, 2014 through February 20, 2015. The player or parent/guardian must
submit this form to the office of the Southern California Volleyball Association by 4:00PM on Friday, February 20,
2015. Under NO CIRCUMSTANCES will transfers be granted to any player who has competed at ANY JUNIOR
NATIONAL QUALIFIER. The form must be completed by the player or parent/guardian, current Club Director, and
new Club Director. The form will then be reviewed by the SCVA, who must ultimately approve the transfer. Please
allow up to 5 business days for the transfer to be approved.
The current club is under no obligation to release the player. If the club will not grant a release, the player or
parent/guardian may appeal to the SCVA. To appeal to the SCVA, the request must be made via email to the

BACKGROUND SCREENING POLICY OF USA VOLLEYBALL AND ITS REGIONAL
VOLLEYBALL ASSOCIATIONS
Last revised June 21, 2013
POLICY:
It is the policy of USA Volleyball (USAV) and its Regional Volleyball Associations (RVAs) that any
club/entity intending to hire or use registered individuals in any sanctioned junior volleyball events
and/or activities (some examples of events or activities that can be sanctioned with regional approval
are: tournaments, practices, clinics, tryouts, and fundraisers) will accept and abide by this
background screening policy. The following individuals, 18 years of age or older, will be screened:
Club directors, club administrators, team reps, coaches, chaperones, and trainers who intend to
register, affiliate and/or participate with a junior volleyball club or team in a USAV/RVA. Any Junior
Tournament Director/Site Director/On-site Tournament Administrator/Manager and official 18 or
older who intends to work a junior sanctioned event will also be screened with the exception of a
junior player. Each RVA may also choose to require other members of their organization to submit
to and pass a background screen in order to affiliate with their organization. Additionally, the
club/entity will enforce the penalties resulting from a negative background screening report. Failure
to do so is grounds for automatic suspension of membership privileges to participate in USAV/RVA
sanctioned junior events and/or activities. All disqualified individuals have the right to dispute the
findings of the background screening directly with the RVA’s approved Background Screen Vendor.
The RVAs and/or USAV will not register, or allow to be registered, any individual who refuses to
consent to a background screen if he/she intends to affiliate and/or participate with a junior club/team,
in the RVA or any other regional or national junior level programming. Junior members are any
members under the age of 18. A background screen will not be required for those individuals who
will be classified only as junior players or those individuals not registered, affiliated and/or
participating with a junior volleyball club or team in a RVA (other than those categories listed above).
For those regions that allow individuals under 18 to be an assistant coach, any individual who is not
yet 18 years old and who is in a non-player role affiliated with a junior club must be background
screened immediately upon reaching 18 years of age. A 30-day grace period shall apply from the
date of the 18th birthday in order to allow time for the background screening to be processed. During
the 30-day grace period, the same restrictions apply to the individual and should be enforced as are
in place for junior coaches regarding supervision by a qualified adult. It is the responsibility of the
individual, club and region to identify the individuals in this situation and to meet the background
screening requirement. Upon the conclusion of the 30-day grace period, the individual may not
participate in a non-player role affiliated with a junior club unless the background screening
requirement is met.
All screens will be good for two membership seasons (maximum of 26 months). Anyone that fails a
background screen cannot reapply for another screen until the following season. USAV and the
RVAs retain the right to require additional background screens at any time with the exception of
reissuing an application that was previously disqualified due to falsification.
PROCESS:
Every individual required to submit Background Screening must complete, sign and date the Consent
and Waiver Release Form. Electronic signatures are only accepted on the USAV Online Registration
System. The Background Screen Consent and Waiver Release form will be submitted and the
applicant cleared before the applicant may participate in RVA/USAV sanctioned junior events and/or
activities.
Upon receipt of the above described documents, the USAV/RVA will request that the USAV/RVA
approved Background Screen Vendor perform the background screen.

All information received as a result of a background check will be strictly confidential. Notice of
clearance or disqualification for all applicants will be provided to:
1. The designated contact of the RVA that submitted the application.
2. USA Volleyball National Office
A notice of automatic disqualification will be sent by email by the USAV/RVA approved Background
Screen Vendor to the RVA office. The RVA will provide the approved Background Screen Vendor a
contact e-mail for the Club Director or highest staff member for the hiring entity. The approved
Background Screen vendor will then contact the Club/entity to provide notice of the automatic
disqualification and request additional contact information for the disqualified individual.
The complete profile will be sent by the USAV/RVA approved Background Screen Vendor directly to
an automatically disqualified individual using the agreed upon method of delivery, along with a copy
of the “Summary of Your Rights under the Fair Credit Reporting Act” (FCRA), and a notification that
the individual is prohibited from participating in USAV/RVA sanctioned junior events and/or activities.
All disqualified individuals have the right to review and dispute the accuracy of the background
screening findings directly with the USAV/RVA approved Background Screen Vendor. A disqualified
individual MAY NOT appeal an automatic disqualification or the results of the findings of the
background screen vendor to the RVA and/or USAV. USAV and each RVA is required by the policy
to accept the findings of the approved background screen vendor.
Individuals automatically disqualified are excluded from participation in any USAV/RVA sanctioned
junior events and/or activities.
EFFECTIVE SEPTEMBER 1, 2013, AUTOMATIC DISQUALIFIERS FOR PARTICIPATION IN
SANCTIONED JUNIOR EVENTS AND/OR ACTIVITIES:
Anyone found guilty, entering a plea of guilty, or a plea of nolo contendere (no contest) regardless
of adjudication or received court directed programs and/or other sentencing directives in lieu of a
finding of guilt, for the following criminal offenses; All Sex offenses, Murder, and Homicide
regardless of time limit; Felony Violence and Felony Drug offenses in the past 10 years; any
misdemeanor violence offenses in the past 7 years; any multiple misdemeanor drug and alcohol
offenses within the past 7 year; or any other crimes (not listed) against children in the past 7 years
(the time frames associated with the categories of crime listed above are calculated based on the
date of the offense).
Individuals found to have pending court cases for any of the disqualifying offenses will be disqualified.
If the disposition of the pending case does not meet the criteria for disqualification as listed above,
the individual would then be cleared and reinstated.
Falsification of information on any membership application or the consent/release form is grounds
for membership revocation or restriction of membership.
Individuals that are automatically disqualified must wait one season before reapplying for affiliation
and/or participation with a junior club or team.
ENFORCEMENT:
The hiring entity is responsible for ensuring adherence to this policy, and ensuring that those
individuals who are disqualified do not participate in USAV/RVA sanctioned junior events and/or
activities.
PENALTY:
Failure of a club/entity to request background screening or enforce disqualification is cause for the
RVA or USAV to impose penalties. The minimum penalty shall be suspension of all members of the
offending club/entity until background screening and enforcement requirements are met. Additional
measures may include financial penalties and/or extended suspensions against disqualified
individuals and/or the club/entity.

2013 Season - BACKGROUND SCREENING PROCESS FOR NON-US CITIZENS
The organization will need to collect more information for non-US citizens who do not have a
social security number. An example would be a college student who is in the United States to
attend school. With the Visa/Green Card number, SSCI can still do a screening on the
information provided.
If the non-US citizen has a social security number, both fields (SSN and Visa/Green Card) must
be completed. If there is no social security number, this must be clearly indicated. That means
that the applicant must write “no SSN” in the social security field. If the SSN field is left blank, the
application will be considered incomplete. The Visa/Green Card field must be completed. If
neither field is completed, the application will be considered incomplete and more information will
be requested of the region.
Non-US citizens WITH a valid social security number may proceed by completing the application
on-line.
Background screening applications for non-US citizens WITHOUT a valid social security number
cannot be conducted on-line since the Visa/Green Card Number field is not an available field in
the online system at this time.
The non-US citizen without a social security number will be instructed to use the link to the PDF
form which is to be printed, completed using the Visa/Green Card Number, signed, and sent or
faxed to the region. The region will then forward that application to Kerstin Fracassini-Rieff at the
national office who will forward the form to SSCI for processing.

2015 Season - USA VOLLEYBALL JUNIOR CLUB PERSONNEL CODE OF ETHICS
It is the duty and obligation of USA Volleyball affiliated Junior Club Program administrators, directors, coaches
and other club personnel to assure the following Code of Ethics is followed and adhered to by all individuals who
have an active role in a USA Volleyball Junior Club Program in any Region of USA Volleyball.
In a continuing effort to promote safe, healthy and ethical communication, relationships and treatment of all
USA Volleyball players and personnel, all adults associated with a junior club program must read, accept and
submit this Code of Ethics before they are eligible to actively participate in a junior club program associated,
affiliated, or participating in USA Volleyball.
1.
All adult club personnel affiliated with a junior program must be a registered member with a Region of
USA Volleyball and USA Volleyball.
2.
All adult club personnel including coaches, chaperones, assistant coaches, trainers, etc. affiliated with a
junior program intending to participate in USA Volleyball must have an approved and current
background screen on file as per USA Volleyball policy. It is intended that the term “all adult club
personnel” be all inclusive and not limited to only those categories identified herein.
3.
A head coach or assistant coach affiliated with a junior program must also: be an adult (see Region
definition of an adult) and be IMPACT certified according to USA Volleyball and Region policies.
4.
If allowed by Region rules, an assistant coach who has not yet met the age of majority in the state of residence
must be supervised by a head coach recognized by the Region and must meet all applicable Region and
USA Volleyball requirements. Individuals who are registered as junior players and also have an interest in
coaching should contact their region regarding coaching eligibility.
5.
Responsibilities:
A. A head coach or other equally qualified club personnel must be present at all practices and
competitions. A head coach, adult club representative personnel or registered chaperone must be
present during team-supervised travel. This individual shall be responsible for the moral, legal and
ethical well-being for each participant during team/club activities.
B. Coaches shall understand the unique power of a coach-athlete relationship. Coaches and all other club
personnel shall not exploit athletes and shall avoid any relationships which could compromise the
integrity of the learning and participation process, impair their professional judgment and/or take
advantage of a situation for their own personal gain or gratification.
C. All club personnel must understand that all forms of sexual abuse, assault or harassment of a current or
former athlete are unethical and illegal even when an athlete invites or consents to such behavior or
involvement. Club personnel shall not engage in sexual/romantic relationships with current athletes or
other participants over whom there is/was authority. See B above.
D. All club personnel shall insure that all individuals have met all Regional Volleyball Association and USA
Volleyball membership requirements prior to participation in any club, team and/or Region/National
USA Volleyball activity.
E.
All club personnel must inform the players and their parent(s)/guardian(s) about any Region
and/or USA Volleyball transfer policy. This policy may restrict or prohibit a participant from
transferring to another club or team if specified criteria have been met. Likewise, all club personnel

must inform the players and their parent(s)/guardian(s) of any rules or policies regarding coaching
transfers during a particular season.
F.
All club personnel shall abide by and inform the players and their parent(s)/guardian(s) of applicable
regional recruiting policies.
G. All club personnel may not participate in, require another individual to participate in, or condone any
act considered to be illegal under federal, state or local laws and/or ordinances.
H. All club personnel shall strive to educate their athletes and personnel to respect, honor and adhere to
the rules of the facility being used during practices, tournaments or events. In this regard, the rules of
the facility shall have priority over the rules of the Regional Volleyball Association.
I.
All club personnel shall ensure that all activities are suitable for the age, experience and ability of their
athletes.
J.
All club personnel shall seek professional medical advice when making decisions regarding an injured
athlete's ability to continue training or playing.
K.
All club personnel shall, while serving in a professional capacity, avoid any drug, tobacco or alcohol use
while in the presence of athletes.
L.
All club personnel shall not supply or condone the use of drugs, alcohol, tobacco, fireworks,
ammunition, firearms, knives or any item or material that can be used as a weapon, to any of the
participants or athletes and shall report any athlete using or in the possession of the same.
M. All club personnel shall not allow, encourage, condone or require any behavior that threatens an athlete’s
amateur status or Regional, USA Volleyball, school and/or collegiate eligibility.
N. All club personnel shall maintain all relationships with other club personnel on a professional and
confidential basis.
O. All club personnel must be positive role models. This includes being courteous, respectful and polite to
players, parents, other coaches, club directors, event personnel, and officials.
P.
All club personnel will not engage in any physical, verbal or emotional harassment, abusive words or
actions, or coercion of current and/or former athletes.
Q. All club personnel will immediately report any suspected case of illegal activity, abuse, assault,
harassment, or ethical violations of this club personnel code of ethics to the appropriate authorities,
including Regional Volleyball Administrators.
Any violation of this Code may result in sanction being issued against the club representative, the individual(s)
and the club/team involved. These sanctions may extend to the loss of eligibility of the club representative,
the individual(s), the entire club and the team involved.

Thank you very much for volunteering to be a junior team chaperone. As a chaperone you are
assuming certain responsibilities for the welfare of the players under your care, custody and
control. To assist you in knowing what your responsibilities are we have created this information
sheet for you. Please read and discuss these responsibilities with the team coach or manager. If
you understand and accept these responsibilities please sign and date the bottom of the form and

and will not take any driving risks that will place the players or me in a harmful situation. All

automobile liability insurance in the amount of $300,000 or more covering the automobile I
will use to transport players. I agree not to transport more players than my automobile has
