First of all, we would like to thank God Almighty, for His blessings which allow us to complete this
qualitative study entitled ‘Operational Research on Family Planning Advocacy to increase method mix
contraceptive in East Java and West Nusa Tenggara Province: a quantitative study’ in 2013.
This report presents the information about Family Planning in three districts of East Java Province,
namely Tuban, Kediri, Lumajang District and three districts in West Nusa Tenggara Province,
namely West Lombok, East Lombok and Sumbawa District. Information collected in this study
included the socio-demographic characteristics, knowledge and attitude to long acting and
permanent methods, current used family planning methods, places to get the current family
planning methods, and community's satisfaction on the current family planning method and
services. Together with the qualitative study conducted earlier, the result of this quantitative study
is expected to provide baseline information to implement advocacy interventions aim at improving
contraceptive method mix in study areas.

EXECUTIVE SUMMARY
1.
Socio-demographic characteristics
The study was conducted in six districts located in East Java and West Nusa Tenggara Province. The
districts were Tuban, Kediri and Lumajang in East Java Province as well as Lombok Barat, Lombok
Timur and Sumbawa in West Nusa Tenggara Province. In total, information was collected from
13,162 ever-married women at reproductive age (15-49 years old), consisting of 6057 (46%)
respondents from East Java and 7105 (54%) respondents from West Nusa Tenggara Province. Total
households visited were 12,804, i.e. 5777 households in East Java and 7027 households in West
Nusa Tenggara Province.
The mean age of all ever-married women at reproductive age interviewed at the time of interview
was 33 years (34 years in East Java and 33 years in West Nusa Tenggara Province). Of the
households visited in this study, on average, there were four members in each household. Most
ever-married women at reproductive age were housewives (61%), followed by
trader/fishermen/farmer (20%).
Overall, 89.5% of ever-married women at reproductive age completed primary school; whereas
4.6% respondents never attended any school. More ever-married women at reproductive age in
East Java Province (93.4%) completed primary school than those in West Nusa Tenggara Province
(89.5%). District with the highest percentage of ever-married women at reproductive age
completing primary school was Kediri District (95.1%); whereas in West Nusa Tenggara, Sumbawa
District had the highest percentage of respondents completing at least primary school (90.7%).

Knowledge on Family Planning
Most of ever-married women at reproductive age in all six districts knew injection as a
contraceptive method. The second most commonly-mentioned contraceptive method was pills.
About half of the ever-married women at reproductive age knew about IUD and implant. Female
sterilization was known by 8-23% of ever-married women at reproductive age, the lowest was in
Lombok Barat and the highest was in Kediri. Male sterilization was only known by less than 10% of
the ever-married women at reproductive age.
When a more detailed knowledge was asked, about 60-70% of the women mentioned injection as
the appropriate method for spacing. The knowledge of appropriate contraceptive method for
limiting amongst ever-married women’s at reproductive age was lower than their knowledge of
contraceptive method for spacing. This also varied widely across districts. In Kediri and Sumbawa
District, about 40% ever-married women mentioned that female sterilization is the appropriate
contraceptive method for limiting. In Lumajang and Tuban, about 40% ever-married women at
reproductive age mentioned injection as the appropriate contraceptive method for limiting.
The married women’s at reproductive age knowledge of duration of contraceptive method was still
low. In all districts except for Kediri, less than half of women know that the duration of IUD is 5
years or more. Knowledge of the duration of implant was higher; between 50-64% of women knew
that the duration of implant is two years or more. For female sterilization, in all districts except for
Sumbawa, less than half of women knew that the duration of female sterilization is life-long.
Knowledge of male sterilization was the lowest; less than half of respondents in all districts knew
that the duration of male sterilization is life-long.

Use of Family Planning Contraceptive Methods
Eighty to ninety percent of ever-married women at reproductive age ever used contraceptive, with
the most common method was injection, followed by pills and IUD. Current user of contraceptive
method was between 70-80%. Three months injection and pills were the major contraceptive
method currently used. Current use of LAPM amongst ever-married women at reproductive age
varied between districts, from around 10% in Kediri to 20% in Sumbawa.
The unmet need of contraceptive also varies widely across districts, from as low as 8% in Lombok
Barat to 14% in Kediri. In general, unmet need for limiting was higher than unmet need for spacing,
except in Lombok Timur District. The higher unmet need for limiting might be due to a high
proportion of non-LAPM use among current user of contraceptive.
Contraception preference amongst ever-married women at reproductive age was still dominated by
injection in all districts. Amongst the LAPM, the most preferred method in Lumajang, Tuban, Kediri
and Sumbawa Districts was IUD. In Lombok Barat and Lombok Timur, implant for three years was
more preferable than others.

Decision Making on Health
Decisions related to maternal child health were mostly decided by both husband and wife, followed
by wife alone. However, the ratio of decision making on using contraception decided by women
alone was much higher compared to both husband and wife, such as in Kediri. Even in Tuban the
ratio was higher for women alone (55% for women vs. 35% for husband and wife). Only Lumajang
had a different pattern. Contraceptive used were mostly decided by both husband and wife (56.4%)
compared to women alone (38.3%). This pattern was almost the same as in West Nusa Tenggara.

Acceptance of non-LAPM compared to LAPM
Most of ever-married women at reproductive age preferred non-LAPM, particularly injection, and
followed by pills. This condition is similar with the overall condition in Indonesia. LAPM such as
IUD, implant and sterilization were less preferable compared to non-LAPM. The difference between
non-LAPM and LAPM acceptance among ever-married women at reproductive age were also found.
However comparing among districts, LAPM were relatively more acceptable in Kediri and
Sumbawa.
Further analysis shows that the average age of couple using non-LAPM (33.5 years) was relatively
younger than couples using LAPM (37 years). The difference was also quite significant. However,
the average age of couples using non-LAPM was still high since the average age of married was 20
years.
From various reasons of using contraceptive method, the effectiveness to prevent pregnancy, easy
to use, and easy to get are the most common reasons in selecting contraceptive methods, followed
by advice from midwives and husband. The reasons of selecting contraceptive methods amongst
districts were very similar.

Exposure of Family Planning Information
Between 36% and 50% of households were visited by health providers in the last six months for
maternal child health services. On these visits, health providers in West Nusa Tenggara (87%) used

these opportunities better in providing family planning information compared to those in East Java
Province (46%). In East Java, health providers in Lumajang provides more family planning
information compared to Tuban and Kediri.
Community was another source of information that was commonly mentioned by women. In East
Java, neighbors and friends followed by relatives are the most common source of information about
family planning. However, it is different for West Nusa Tenggara Province. Cadres in West Nusa
Tenggara were more active in disseminating family planning information compared to those in East
Java. However, it was found that neighbor was an important sources of family planning information
in both provinces.

CHAPTER I
INTRODUCTION

Background

The family planning program in Indonesia is known as one of the best in the developing world.
However, the use of mixed contraceptive methods remains a problem. The 1987-2012 Indonesian
Demography and Health Survey (IDHS) stated that the utilization of non-long acting and permanent
methods (non-LAPM) in Indonesia was much higher than the long acting and permanent methods
(LAPM) of contraceptive. Furthermore, most couples that intend to limit the number of their
children preferred pills and injection, which are actually designed for spacing births, instead of
limiting. The 2012 IDHS also showed that ever married women at reproductive age, 52% used nonLAPM and only 6% used LAPM (Figure 1.1). This indicated the need to improve the family planning
program in Indonesia.

Previous literatures has shown various determinants on the use of family planning, including their
partner’s approval, the quality of the services, the friendliness of family planning providers and the
women knowledge on family planning services. Additionally, the women’ level of income, proximity
to the provider and religion played important roles in the demand side of family planning services
(Okech, et. al., 2011). A study conducted by the National Population and Family Planning Board
(BKKBN) in Indonesia revealed that the age of the couple, the length of marriage, their education,
the economic condition of the family, their purpose of using contraceptive, and the quality of the
health provider influenced the usage of family planning services. Furthermore, it was found that

strong rumours related to the failure of LAPM such as Intra Uterine Device (IUDs) also became a
major obstacle in LAPM use in Indonesia (BKKBN, 2011).
Rigorous efforts have been made to increase use of family planning services including use of LAPM
in Indonesia. One of them is by strengthening family planning program at the health provider and
by creating demand. At the health provider, this included strengthening collaboration, ensuring the
availability and quality of contraceptives and their supporting equipment, as well as enhancing the
competency of human resources providing family planning services (Health Ministry of Indonesia,
2013). In creating demand, one of the programs carried out by BKKBN was promoting the family
planning motto, “2 children is enough” or Dua Anak Cukup to foster the mindset in order to achieve
the Prosperous and Happy Family (Keluarga Bahagia Sejahtera) (BKKBN, 2013). Regardless the
rigorous efforts to promote the coverage of family planning program, national surveys including
IDHS still showed that majority of community preferred non-LAPM, including amongst couples
intending to limit the number of children (Figure 1.1.)
To address this issue, the Center for Communication Program of Johns Hopkins University (JHUCCP) in collaboration with Yayasan Cipta Cara Padu, the Ministry of Health Republic of Indonesia,
BKKBN and the Center for Health Research Universitas Indonesia (CHR-UI) conducted a research to
demonstrate the current situation in the usage of family planning as well as the government’s
efforts in improving family planning services at various districts in Indonesia. This study was
conducted in six districts, namely Kediri, Tuban, and Lumajang District from East Java Province;
West Lombok, Lombok Timur, and Sumbawa from West Nusa Tenggara Province.
The baseline data was collected using qualitative and quantitative methods. This report only
discusses the results from qualitative study which will be used by Yayasan Cipta Cara Padu for
advocacy intervention in those study sites.

Objectives
a. General objective
This baseline survey aims to examine the knowledge, attitudes and practice of Long Acting and
Permanent Methods (LAPM) of contraception in East Java and West Nusa Tenggara Province.
b. Specific objective




To determine the prevalence of Long Acting and Permanent Methods (LAPM) of contraception
use in East Java and West Nusa Tenggara Province.
To determine the knowledge, attitude and practice of LAPM.
To determine reasons for non-use and the choice of methods among users.
To identify barriers to universal access to family planning services.

CHAPTER II
METHODOLOGY
2.1
Study design
The quantitative methods implements before and after proof of concept study, using repeated cross
sectional survey. This report only discusses the results of the quantitative study. The study was
performed after the qualitative study was completed.
1.3
Project location
The project was located at three districts in East Java and three districts in West Nusa Tenggara
(Figure 2.1). The districts selected at East Java Province were Kediri, Tuban, Lumajang; whereas the
districts at West Nusa Tenggara Province were West Lombok, Lombok Timur and Sumbawa.

Study methods
a. Population
The population for this study was ever-married women at reproductive age (15-49 years) living in
the three districts in East Java (Kediri, Tuban, Lumajang) and West Nusa Tenggara Provinces
(Lombok Barat, Lombok Timur, Sumbawa).
b. Sample size
The sample size was calculated to detect 15% change of the combined use of IUD, implants, male
and female sterilization. The sample size formula used for hypothesis testing was used to determine
the minimal sample size (Lemeshow, 1990) was:

intervention).

The significance level used was the 5% one tail test, the power of test used was 80% and design
effect of 2 for three stages cluster sample design. According to the Indonesian Demographic and
Health Survey of 2007, the proportion of married women who are currently using combined
methods of IUD, implants, male and female sterilization methods in East Java was 16% and in West

Nusa Tenggara it was 11.4%. These proportions were used as the baseline value (P1) for East Java
and West Nusa Tenggara.
The P2 for East Java was 16% + (15% * 16%) ≈ 18.5%, therefore the minimal sample size for East
Java was 5,648 married females between the ages of 15-49 years. And for West Nusa Tenggara, the
P2 was 11.4% + (15% * 11.4%) ≈ 13.5%. The minimal sample size for West Nusa Tenggara was
6,762 married females between the ages of 15-49 years.
c. Data collection method
This study used four stage cluster sampling method in each province. In East Java Province, the first
stage is district level. The 5,648 subjects were allocated at each district based on its population size.
The second stage took place at the village (kelurahan or desa) and in each district, 50 villages was
selected using the probability proportionate to size (PPS) method, based on its population size.
Then in the third stage took place in the hamlet (RW or dusun) and in each villages one hamlet will
be selected using the simple random sampling method. Then in fourth stage, 40 married women
between the ages of 15-49 years will be randomly selected from a list of eligible subjects in the
selected hamlets. Therefore the total sample size for East Java was 6,000 married women between
the ages of 15-49 years.
The same sampling procedure was used at West Nusa Tenggara Province. However, the total
sample was 6,762 respondents. The second stage took place in the village (kelurahan or desa), at
each district 50 villages was selected using the probability proportionate to size (PPS) method,
based on its population size. In each village (the third stage), one hamlet was selected. And in the
fourth stage, 50 married women between the ages of 15-49 years were randomly selected from a
list of eligible subjects. Therefore the total sample size for West Nusa Tenggara was 7,500 married
women between the ages of 15-49 years.

Listing activity
Before collecting the quantitative data on 50 randomly selected married women between the age of
15-49 years old, houses in the selected hamlet were listed. One hamlet per village was selected,
therefore there were 50 hamlets selected. The list comprised of all the houses and the number of
the house at the selected hamlet. A standard system was used in numbering the houses. Every
house in each selected hamlet was listed, visited, and numbered. No house was ignored, even empty
houses were registered and numbered. Every house that was visited was recorded and a sticker
was used as a sign that the particular house has been registered. A rough map of the hamlet was
made, including the conditions of the field, the first house of listing process, and the direction where
the process went.

Selection of respondent
After completing the listing process, houses to be included in the survey were selected using the
systematic random sampling method. This was established by determining an interval number, by
dividing the total number of houses available in the hamlet by number of required samples. After
determining the interval, the first house was selected by multiplying the interval number with a
random number. The second house was then selected by adding the interval number to the number
of the previous one. This process was continued until the desired number of houses was reached
for each cluster. Every selected house was visited and identified whether there were any ever
married women at reproductive age living in the house. Every eligible woman was asked to become
respondent and interview was performed using a questionnaire (Attachment A). If there are more
than one eligible respondent in one house, all of them will be interviewed. If no eligible respondent
was available, the process continued to the next house, until all selected houses were visited.
d. Instruments
The quantitative questionnaire (Attachment A) consists of several topics, including:
(1) The socio-demographic characteristics
(2) The knowledge and attitude to long acting and permanent methods
(3) The current used family planning methods
(4) The places to get the current family planning methods
(5) Their satisfaction on the current family planning method and services.
The instrument was designed and revised after obtaining the results of the qualitative study
conducted.

e. Data collection procedure
Only eligible respondents fulfilling the inclusion criteria were invited to participate in this study.
Data were collected by trained interviewers using structured questionnaires. The interviewers
were females, midwives/nurses or public health school graduates and are experienced in
household health survey. All interviewers were trained in a 3-day training program. Each
interviewer was responsible for interviewing 10 subjects per day and performed interviews for a
maximum of 12 days. The total number of interviewers was adjusted to the final (agreed) sample
size. Every 10 interviewers were supervised by a supervisor (field coordinators and assistant field
coordinator). The supervisor was responsible to check each questionnaire and made necessary
actions to ensure the completeness and correctness of the questionnaire.
f. Conceptual framework
In this study, we adapted the theory of Diffusion of Innovations (Rogers, 1962) and the Health
Belief Model (Rosenstock, 1966), which have been widely used to explain and predict health
behaviours. The adapted framework is shown in Figure 2.3.

g. Data analysis
The data entry was performed in CHR UI to ensure the quality of the data. A template was made for
data entry with range check and logical check. Epidata software was used for data entry.
The data cleaning was conducted before analysis. In data cleaning stage, two data files were
compared. If there were any inconsistencies between two files, questionnaires would be reviewed.
The consistency check was also carried out at this stage. Afterwards, the data was transferred to
Stata format and given variable and value labels.
Statistical analysis was performed in Stata/11 with consideration of complex survey design. The
main analysis was: (1) The proportion of women between the ages of 15-49 years currently using
long acting and permanent methods of contraception and; (2) The knowledge and attitude of
women between the ages of 15-49 years LAPM of contraception. The confidence interval used was
95%. A workshop with relevant stakeholders was established to gather input about the analysis.
h. Ethical clearance
Ethical clearance for using human as subjects for study was obtained from the Ethics Committee of
the Faculty of Public Health Universitas Indonesia. Additionally, permission to conduct the study
was obtained from the Ministry of Internal affairs from the central level, and local authorities
(provincial, district, sub-district and village level).
Prior to collect information from the respondents; enumerators explained study protocol to
respondent. Respondents were informed that their participation in this study is voluntary and the
information provided will be treated confidentially. Informed consent should be obtained prior to
the data collection.

CHAPTER III
RESULT
3.1 Socio-Demographic Characteristics
This study collected data from 13,162 married women in child-bearing years, i.e. 6,057 (46%)
respondents from East Java and 7,105 (54%) respondents from West Nusa Tenggara Province. The
number and percentage of respondents interviewed in this study of each district is presented in
Table 1. Total households visited were 12,804, i.e. 5777 households in East Java and 7027
households in West Nusa Tenggara Province.

The mean age of women at the time of interview was 33 years, i.e. 34 years in East Java and 33
years in West Nusa Tenggara Province. There were three married women under 15 years old
interviewed, one if East Java (Tuban District) and two in West Nusa Tenggara Provinces (Lombok
Barat and Lombok Timur District). The distribution of women by age at the time of the survey in
each district is shown in Figure 3.1.

On average, of the total 12,803 households visited, there are four members in each household
(Figure 3.2). The average number of household members in East Java Province was 4.4, ranging
from 1 to 14; whereas in West Nusa Tenggara Province the average was 4.2, ranging from and 1 to
16 members.

Most women interviewed in this study were housewives (61%). In East Java and West Nusa
Tenggara Province, the proportions of housewives were 61.3% and 60.5% respectively. The second
most common occupation was merchants, fishermen, and farmers, although it accounts for only less
than 20% of the total respondents (Figure 3.3).

More respondents in West Nusa Tenggara did not receive any schooling or did not graduate from
elementary school (13.7%), compared to East Java (6.6%) (Figure 3.4). In East Java, district with
the highest percentage of respondents who graduated from elementary school was Kediri (95.1%),
followed by Tuban (92.7%) and Lumajang (92.5%). In West Nusa Tenggara, district with the
highest percentage of respondents who completed elementary school was Sumbawa (90.7%),

followed by Lombok Timur (88.5%) and Lombok Barat (79.0%). Figure 3.4 shows the distribution
of women by their highest education level achieved.

e. Household wealth index
The wealth of the respondents was measured by creating index of ownership of valuable goods.
Principal Component Analysis with polychoric correlation was used to construct household wealth
index from the questions about: roof, wall, and floor material, main source of drinking water, place
for defecation, type of toilet, final waste disposal and availability of separate room for kitchen. The
constructed wealth index was a continuous score and then divided into five wealth quintiles, with
quintile 1 (Q1) is the poorest and quintile 5 (Q5) is the richest. The distribution of wealth quintile
by district is shown in table 3.2.

Overall, each respondent has approximately two living children (Figure 3). Almost 15.2% of
respondents in West Nusa Tenggara has three or more living children, whereas only 7.9%
respondents in East Java Province has three or more living children.

b. Reproductive history
Table 3.3 presents information about women's reproductive history. Overall, there were two
children lived with each women at the time of interview. On average, 5% of women were pregnant
at the time of interview, ranging from 3.5% in Lumajang to 7.8% in Lombok Barat.

c. Average age of first marriage
The average age of the women interviewed during their first marriage is 20 years. In East Java and
West Nusa Tenggara, the average age was 19.7 years and 20.3 years, respectively. In East Java
Province, amongst the three study sites, the highest average age of first marriage was Kediri (21
years), followed by Tuban (19.1 years) and Lumajang (18.9 years). In West Nusa Tenggara
Province, the highest average age of first marriage was in Sumbawa (21.4 years), followed by
Lombok Barat (19.7 years) and Lombok Timur (19.6 years). Detailed information on the age of first
marriage in all study sites is shown in Figure 3.6.

The average age of first pregnancy in both provinces was 20.9 years. Two districts with the highest
average age of first pregnancy were Sumbawa (22.1 years old) in West Nusa Tenggara and Kediri
(22.0 years old) in East Java. The age of the first pregnancy ranged from 10 to 42 years in East Java
and 10-45 years in West Nusa Tenggara. Distribution of respondents by age of first pregnancy is
shown in Figure 3.7.

The average age of first delivery in both provinces was 21.6 years. Two districts with the highest
average age of first pregnancy were Kediri (22.7 years old) in East Java and Sumbawa (22.6 years
old) in West Nusa Tenggara. The age of first pregnancy ranges from 10 to 45 years in East Java and

The mean age of women with their last childbirth was 26.8 years. The mean varies across districts,
i.e. 25.7 years in Lumajang, 25.9 years in Tuban, 28.0 years in Kediri, 26.3 years in Lombok Barat,
25.9 years in Lombok Timur and 28.6 years in Sumbawa. Figure 3.9 shows the distribution of

Table 3.5 shows the general knowledge of the respondents on type of contraceptive method. Most
of them knew pill and injection contraceptive methods. About half of them knew about IUD and
implant. More respondents knew about female than male sterilization. This pattern was almost
similar in all six districts.

When women were asked about the most appropriate contraceptive method for spacing, more than
half of their respondents mentioned injection and pills (Table 3.6). Some respondents (5-15%)
mentioned IUD. Implant was also mentioned by 2-11% of the respondents and sterilization was
also only mentioned by less than 1% of the respondents.

Table 3.7 shows respondents knowledge on contraceptive method for limiting. The knowledge was
widely varied between districts. In Kediri, almost half of the respondents knew that tubectomy as
the most appropriate method for limiting, whereas in Tuban, about 44% respondents still
mentioned injection. Injection was also mentioned by 40% of the respondents in Lumajang. There
were many respondents who said they were not aware of the most appropriate method for limiting,
(38% in Lombok Timur, 30% in Lombok Barat and 26% in Sumbawa). In general Table 3.6 shows
that knowledge of contraceptive method for limiting is still low.

Knowledge of places to get contraceptive method for birth spacing varies between districts (Table
3.8). In Lumajang, Tuban and Kediri, most of the respondents mentioned midwife private practice
as the places to get contraceptive method for spacing. While in Lombok Barat and Sumbawa, most
of the respondents mentioned primary health care center. In Lombok Timur, maternity post was
the most frequently mentioned facility. In Lombok Barat, 41% of the respondents mentioned village
maternity post as the place to get contraceptive method for spacing.

c. Knowledge of duration of contraceptive method
Respondents’ knowledge of duration of contraceptive method was still low as shown in Table 3.10.
In all districts except for Kediri, less than half of respondents knew that the duration of IUD is five
years or more. Knowledge of the duration of implant was better. Between 50-64% of the
respondents knew that implant duration is two years or more. For female sterilization, in all
districts except for Sumbawa, less than half of the respondents knew that female sterilization is lifelong. Knowledge of male sterilization was the lowest, less than half of respondents in all districts
knew that the duration of male sterilization is life-long.

Table 3.11 shows that the commonly known contraceptive method available in respondents’ area
were pill and injection in all districts. Among the LAPM, IUD was the most commonly known
method. Less than 10% of respondents mentioned that female sterilization were available in health

facility in their area, except in Sumbawa (16%) and Kediri (18%). Male sterilization was less
mentioned by respondents as available method in the health facility in their areas.

family planning field officer according to the respondents was injections, followed by IUD (Table

a. Past use of contraception
More than 75% of ever-married women at reproductive age in the six districts ever used at least
one method of modern contraceptive. Pills and injections (three months) were two most common
methods of contraceptives. LAPM varied between district, from less than 10% in Lombok Timur to
around 20% in Kediri, Lumajang and Sumbawa Districts (Table 3.13).

b. Current use of contraception
The current use of contraceptive methods varied from less than 72% in Lombok Timur to more
than 80% in Kediri District (Table 3.14). Pills and three months injections were two most common
methods used by women. The current use of LAPM varied between districts, from 10% in Tuban to
20% in Sumbawa.

Unmet need varied widely between districts, from 8% in Lombok Barat to 14% in Kediri (Figure
3.10). In general, unmet need for limiting births was higher than spacing, except for Lombok Timur
District. The higher unmet need for limiting might be due to high proportion of non LAPM use
among current users of contraception.

d. Reason for not using contraception
Table 3.15 shows the reasons of respondents who never used any contraceptive methods. The most
frequently mentioned reason was the desire of having another child, which ranged from 56% in
Kediri to 74% in Sumbawa District. The second most frequently mentioned reason was the
reluctance of husband, ranging from 4% in Kediri to 21% in Lombok Timur. The fear of side effects
was also emerged as another reason.

e. Plan to use contraceptive methods
Among respondents who did not use any contraceptive methods, between 40 to 61% planned to
use some method in the future (Figure 3.11). The percentage varied between districts, with the
lowest percentage is in Lumajang and the highest one is in Sumbawa. According to women, only a
small percentage (less than 1 percent) of their husband planned to use contraceptive methods in
the future. This condition is in accordance with the above finding that that contraception is the
responsibility of the wife.

f. Reason for not planning to use contraception
Table 3.17 shows the reason of women for not planning to use any contraceptive methods in the
future. It was found that the pattern is similar to reasons for not using any contraceptive methods.
The most commonly mentioned reasons were the desire of having more children, hesitation
husband and fear of side effects.

Among husbands, the most frequently mentioned reason for not planning to use any contraceptive
methods was contraceptive use was the responsibility of wife. This is the same reason for not using
any contraception at all. Other reasons were insignificant compared to this main reason (Table
3.18).

The method of contraception preferred in all the districts was the injection. Among the LAPM, the
most preferred method was IUD in Lumajang, Tuban, Kediri and Sumbawa; while in Lombok Barat
and Lombok Timur, the 3 year implant was preferable (Table 3.19).

condoms. Vasectomy was preferred in Kediri, Lombok Timur and Sumbawa, but not in other

The average age of first use of contraceptive among respondents was between 20-24 years. The
youngest age was found in Lumajang (20.9 years) whereas the oldest age was in Sumbawa (23.9
years), as shown in Table 3.21.

Three months injection was the most commonly used as first contraceptive method in all six
districts, followed by pills. LAPM was not popular for first contraceptive method since at the first
time, usually women used it for spacing, not limiting. For the husband, since there were not many
contraceptive choices for men, the most commonly used was condom (Table 3.22).

Decision Making
The decision making process in a marriage is an indicator to see the role of women in making
decisions related to family planning. Usually, the more dominant the husband, the weaker the role
of wife in decision making will be, including in family planning.
In this study, daily activities, such as choosing the daily menu, was dominated by women, more than
70% of housewives decided on what the family ate everyday and only less than 5% of male made
decision on daily menu (Table 3.23). This also happened for daily household expenditure in East
Java although the role of women in deciding daily expenditure was less than deciding daily food.
Around 60% of housewives decided on daily expenditure and around 20%-30% the decision made
together. This pattern in East Java was different from West Nusa Tenggara, since in Lombok (both
Lombok Timur and Barat) only 40% of housewives had autonomy for their daily expenditure; 44%50% mentioned that they had to decide it together. Only in Sumbawa the pattern was the same as in
East Java, even the autonomy of housewives was higher 77%, only 19% of them mention about
deciding the daily allowance together. The role of husband in this matters was low (less than 11%),
although it seemed to be rather high in Kediri (11.2%), Lombok Barat (10.7%) and Lombok Timur
(8.5%).
The decision of buying luxurious things was decided together (50%-80%). The pattern showed that
the second decision was dominated by husband (4%-24%). However, in Lumajang and Sumbawa
there was a slightly different pattern. The second decision was dominated by housewives compared
to husband (Lumajang 10.7% versus 6.7%; Sumbawa 13% versus 3.8%).

Concerning the decision for health of children, both husbands and wives decided together (67%80%), whereas the second decision was dominated by wives (11%-37%). Only less than 5% of the
decisions were made by husbands. This pattern was almost the same in deciding the place of
delivery. Almost two third of respondent decided it together with her husband and the second high
decision was the wives (9%-21%) followed, in almost same percentage, by husband alone (5%10%).
This pattern also applied in mother’s health care in West Nusa Tenggara, both husbands and wives
discussed and decided together (58%-62%), followed by respondents (18%-35%). However in
Tuban (43% versus 45.6%) and Kediri (49.6%-41.7%), the respondents decision was slightly more
dominant compared to both decisions.
The pattern of decision making in contraceptive use was similar except for Tuban in where
respondents decided what contraception she used (55.5% versus 35.4%) and in Kediri (47.4%
versus 44.7%). Lumajang and West Nusa Tenggara Province had a different pattern, almost two
thirds of the wives discussed with their husbands about contraception and only a third was able to
decide by themselves. Husbands alone were dominant in deciding the use of contraceptive; only
less than 4% husband alone dominated the decision.
From the three types of decisions related to health, it was demonstrated that the role of husband
alone were not fully dominant. Both husbands and wives decided together, even in some place, the
wives even decided by themselves.
Decision of having sexual intercourse is an indicator of non-consensual sex. This was measured by
the respondents’ acknowledgment that sexual intercourse was decided together. Almost in all sites,
more than 70% of respondents mentioned that both of them decided on having sex. Furthermore,
decision of giving permission to work is dominated by the husbands (38%-56%) followed by both
of them (33%-46%).
3.6
Acceptance of Contraceptive Methods using Diffusion of Innovation
Characteristics
a. Average acceptance score of contraceptive method
The acceptance for each type of contraceptive method was measured by asking several questions
that represented the various reasons and considerations to accept certain family planning methods
(Table 3.24). For a composite of each method, the Cronbach Alpha ranged between 0.63 and 0.76;
meaning that items were good enough to measure each method of contraceptive. For each item, the
respondents had to choose between the 5 scales of responses; strongly disagree (score= 5),
disagree (score= 4), indifference/neutral (score= 3), agree (score= 2) or strongly agree (score= 1)
with the statements.

From the table above, we can see that injections were more acceptable compared to other methods.
This is accordance with the data in the provincial level stating that women prefer to use injections,
and followed by pills. LAPM methods such as IUDs, implants and sterilizations were less preferable
compared to non-LAPM. The difference in acceptance between non-LAPM and LAPM were
significantly proven with p-value= 0.000. However comparing among districts, LAPM were
relatively more acceptable in Kediri and Sumbawa.
b. Correlation of contraception acceptance score with age
From further analysis, we can conclude that the average age of couple using non LAPM (33.5 years)
was relatively younger than couple using LAPM (37 years) and the difference was significant with p
value=0.000 (Table 3.25). However, the average age of couple using non LAPM was still high since
the average age of married was 20 years.

c. Reason for using certain contraception type
There were nine reasons asked to respondents for using certain type of contraceptive methods. For
each reason, respondents were asked to score from 1 to 10 it based on their opinions about its
importance. If respondents thought that it was very important then he/she gave a score of 10.
However, if they did not think that the reasons were not important, then they gave a score of 1.
Table 3.26 below shows us the respondents opinion about the importance of each reason in
selecting contraceptive methods.

From the mean score of each district, we conclude that ease of use, effectiveness in preventing
pregnancies and accessibility were the common reasons in selecting a contraceptive method,
followed by advice from midwives and husband in certain districts. Only respondents in Kediri
concerned with its side effects.

Exposure of Family Planning Information
a. Home visitation
Visitation by health providers and other personnel such as family planning field officer and cadre
provides an opportunity to persuade couples to use contraception. However, only less than 4% of
households visited by these personnel in East Java and 11% in West Nusa Tenggara (Table 3.27).
Midwives and nurses followed by cadres were the most common personnel who visited the family.
Only in Lombok Timur the cadres were more active compared to midwives and nurses.
Respondents visits concerning maternal or child health is another opportunity in disseminating
information on family planning. Between 36% to 50% of households were visited by health
providers in the last six months for Maternal Child Health (MCH). On average West Nusa Tenggara
was better in using the opportunity to provide family planning information (87%) compared to
East Java (46%).

Among districts in East Java, health providers in Lumajang (71%) provided more family planning
information, in comparison to in Tuban (39%) and Kediri (37%). In contrast, more than 80% of
health providers gave information about family planning in West Nusa Tenggara.
Topics that were mostly discussed in East Java were types of family planning and its side effects,
followed by the benefits of using family planning. It was shown in the table that types of
contraception was the most information that the respondents received in West Nusa Tenggara
(70%) compared to East Java (48%). The table above described that the family planning program in
East Java was disseminated earlier than West Nusa Tenggara.
b. Source of information
The sources of information were from the media, health providers and the community. Table 3.28
indicated that TV was the most common media (East Java: 21%, West Nusa Tenggara: 30%),
followed by radio (East Java 4%, West Nusa Tenggara 3%). Internet and short message service
were the two modes that the respondents were not exposed to.

More than 80% of information about family planning was disseminated through midwives. Doctors
were the second health providers who gave the most information about family planning. However,
only 4% to 14% respondents received information from them. Interestingly in West Nusa Tenggara,
the involvement of nurses in providing family planning information was rather high (6% in West
Lombok, 19% in Lombok Timur and 17% in Sumbawa).
The community was another source of information that commonly received by the respondents. In
East Java, neighbours and friends were the most common source of information about family
planning, followed by relatives. However, there was a slightly different pattern in West Nusa
Tenggara, since cadres were more common in disseminating information compared to neighbours,
indicating that cadres were more active in West Nusa Tenggara compared to East Java. Community
meetings, community and religious leader were included as channels that were less used by the
program in disseminating family planning information.

CHAPTER IV
CONCLUSIONS AND RECOMMENDATIONS
4.1 Knowledge and Use of Contraceptive of Family Planning Method
 Most of ever-married women at reproductive age already knew about non-LAPM,
nevertheless the knowledge of LAPM was still limited. Furthermore, they already knew
about appropriate method for spacing, but not for limiting.
 The proportion of contraceptive use among ever-married women at reproductive age was
70-80%. Most of them used three months injections and pills. The proportion of LAPM used
among ever-married women at reproductive age was between 10-20%.
 The unmet need of contraceptive method was between 8-14% which is still far from MDGs
target (5%) and the unmet need for limiting was generally higher than for spacing.
Recommendation
Education program should be more focused to differentiate contraceptive method for spacing
and limiting. For limiting, use of LAPM should be emphasized.
4.2 Decision Making
 In East Java Province, decision making on contraceptive use was dominated by women.
However, in West Nusa Tenggara Province, decision making on contraceptive use was
mostly made by both husband and wife.
Recommendation
Awareness should be also promoted not only for women, but also for their husband, especially
in West Nusa Tenggara. Education for husbands about the benefit of LAPM should be
encouraged.
4.3 Acceptance of non-LAPM compared to LAPM


Non-LAPM especially injection method was still accepted compared to LAPM, although the
most common reason of using contraceptive method was its effectiveness to prevent
pregnancy. The average age of couples using non-LAPM was also high (33.5 years), showing
that they were no longer young couples.
The most common reason for using contraceptive method is its effectiveness to prevent
pregnancy.
Recommendation
The average age of couples using LAPM should be reduced. family planning education should
focus on shifting the non LAPM to LAMP users amongst couples who already have two children.
4.4 Exposure to Family Planning Information


West Nusa Tenggara Province used the opportunity better in giving family planning
information during maternal child health household visit.
In all districts, neighbour was an important source of family planning information. However
in West Nusa Tenggara, cadres played an important role in disseminating family planning
information.

Recommendations
 Household visits related to maternal child health provides a good opportunity for health
providers to educate couples about the benefit and importance of LAPM, particularly for
couples with two children or more.
 Programs to change family planning norms that non-LAPM is more acceptable than LAPM,
is important to change community perception about LAPM. It is recommended to form
groups of couples using LAPM in the community to actively share their experience and
knowledge about the benefit of LAPM.

Sekarang saya akan menanyakan, seberapa jauh kemungkinan ibu untuk menggunakan berbagai macam
alat/cara KB yang akan saya sebutkan. Jika ibu pernah menggunakan alat/cara KB tersebut, maka
pertanyaannya adalah: “Apakah ibu mau mencoba menggunakannya kembali?”
Saya akan menunjukkan penggaris dengan ukuran dari 0 hingga 10. Jika ibu sangat tidak ingin mencoba
menggunakan alat/cara KB tersebut, tunjukkan angka 0 pada penggaris dengan menggunakan pensil.
Sebaliknya, jika ibu sangat ingin mencoba alat/cara KB tersebut, tunjukkan angka 10. Ibu juga dapat memilih
angka di antaranya, sesuai dengan pendapat ibu. Namun, jika benar-benar di pertengahan jalan (antara ingin

pertimbangkan ibu dalam memilih metode kontrasepsi.
Saya akan akan menunjukkan kembali penggaris dengan ukurannya dari 0 hingga 10. Jika ibu merasa bahwa hal
