INTRODUCTION & OVERVIEW 2005-2006
Over the past year, there have been two major changes
related to Pathology and Laboratory Medicine, the
establishment of Laboratory Medicine and Infection Control
as a Mount Sinai Hospital Centre of Excellence, and the
establishment of MOUNT SINAI SERVICES, the commercial
vehicle for Pathology and Laboratory Medicine and Medical
Microbiology.
Mount Sinai Hospital is in the midst of substantial
expansion and transformational change. The Toronto
Centre for Phenogenomics and the Samuel Lunenfeld
Research Institute expansion at 11 Orde Street are under
construction and will soon be operational. Mount Sinai
has also received permission and funding from the Ontario
government towards building a patient care obstetrics
facility, an ambulatory surgery centre and refurbishing
its facilities. As well, the hospital has established a new
strategic plan. This new strategic plan includes five clinical
centres of excellence: Women’s and Infant’s Health,
Chronic diseases (medicine), Surgical specialties and
Oncology, the Samuel Lunenfeld Research Institute, and
most notably, Laboratory Medicine and Infection Control.
The establishment of Laboratory Medicine and Infection
Control as a Centre of Excellence acknowledges the
strengths and stature of both Pathology and Laboratory
Medicine and Medical Microbiology not only at Mount Sinai
Hospital but in the external community. That Laboratory
Medicine and Infection Control is considered a Centre
of Excellence is a major opportunity for the laboratories
at Mount Sinai Hospital, as this is a virtually unique
organizational structure in Canadian health care.
For many years, Pathology & Laboratory Medicine has
served clients beyond Mount Sinai. These clients include
other hospitals but also pharma and medical device
corporations in the private sector. This commercial activity
has brought many benefits including connectivity to the
community and access to advanced technology from
around the world to Mount Sinai Hospital. In recognition
of these benefits, a new company, MOUNT SINAI SERVICES,
has been established to expand future commercial
opportunities related to diagnostics.
This year, a substantial boost to laboratory services
infrastructure at Mount Sinai Hospital was the award
of $1,248,000 of federal/provincial Designated Medical
Equipment funding to laboratory services. For Pathology
and Laboratory Medicine this has facilitated several
important initiatives including an automated blood bank
system, renewal of our coagulation system, and flow
cytometer, and the establishment of more automated
histologic processing technology.
Over the past year, our collaborative activity with the
Hospital for Sick Children has increased greatly. These
collaborations include perinatal pathology, and genetics.
A notable clinical development has been the establishment
of the Fred Litwin Clinical Genetics Centre which will
amplify Mount Sinai’s strength in clinical genetics. This is
accompanied by further outreach of our laboratory genetics
services including a collaborative program with Lakeridge
Health Centre.
We welcome Dr. Patrick Shannon to our staff. Dr.
Shannon is a neuropathologist with a special interest in
developmental perinatology. Dr. Shannon will join Dr.
Sarah Keating and Dr. Sandra Viero in building the regional
program in Fetal and Perinatal Pathology.
All of us were deeply saddened by the death of our
beloved colleague, Dr. Yvan Bédard. Yvan was widely
regarded as one of Canada’s most outstanding diagnostic
cytopathologists and pathology educators. In his career
at Mount Sinai Hospital spanning 34 years, Dr. Bédard
educated over 60 residents and fellows in cytopathology.
The advancement of Pathology and Laboratory Medicine
at Mount Sinai Hospital is highly dependent on expertise
of both our medical/scientific and support staff and their
capacity to work together towards the common goals
of enhancing health care, education and research. We
acknowledge their continuing contributions with gratitude
and pride. Once again, we thank the administration of
Mount Sinai Hospital led by Joseph Mapa for continuing
support and encouragement of all our endeavours.

CLINICAL SERVICES
Patient Services
Patient Services is one of Mount Sinai’s front doors with over 90,000 patients seen each year by our dedicated staff.
Over the year, space was secured and plans were advanced for enlarging our ambulatory care laboratory at 700
University Avenue.
Clinical Biochemistry
Maternal Serum Screening Program:
The Program continued to grow in size and complexity with the addition of a new biomarker inhibin A, conversion of the triple screen to the quadruple version, generation of first trimester Trisomy 18 and 13 risk, and
introduction of the first and second trimester integrated screen option.
The ability of the screens to predict better Downs’ Syndrome and other Trisomies was demonstrated after
linking screen risks to the karyotype outcome in the Cytogenetics database.
In Vitro Fertilization
Hormone testing was consolidated on the new Roche Modular automated analyzer, thereby simplifying specimen handling and facilitating rapid delivery of test results to the clinical service monitoring progress of the
developing follicle.
Reference Ranges
With medical leadership from Dr. Irv Bromberg, the Division of Clinical Biochemistry is continuing its efforts to
establish meaningful reference ranges for all analytes. Significant progress on this project has been made and
approximately 100 normal volunteers are now participating. We are currently in the stage of analyzing the
distributions for deriving reference ranges. We are also participating in a similar effort, initiated by the College
of American Pathologists.

Hematopathology & Blood Transfusion
A new high speed flow cytometry cell sorter “BD FACSAria” was acquired. This will facilitate the research on
hemopoietic stem cells under the guidance of Dr. Wang.
Mr. Shahcheraghi successfully completed his MSc on the pathophysiology of anti-K haemolytic disease of the
newborn under the supervision of Dr. Denomme.
Dr. J. Pendergrast successfully completed the Royal College fellowship in Transfusion Medicine.
Dr. Hamid Jahanmahar is continuing his sabbatical working on computerized image analysis of malignant
lymphocytes. Dr. Jahanmahar is from Tehran University of Medicine Sciences, and assistant professor and director of
Hematology Laboratory in the Department of Clinical Laboratory Sciences.
Rapid Response Laboratory
This past year saw one of the biggest changes to date.
Newly acquired space was completely renovated to suit the needs of the biochemistry section. New supply cabinets
were built to specifically store the daily supplies required in the RRL. Items are clearly labeled and displayed resulting
in improved inventory control and management.
The entire middle section of the lab was renovated to accommodate the new Roche Modular platform which
went into operation in September. This system revolutionized how chemistry samples are now processed in that it
integrates all three phases – pre-analytics, analytics and post – analytics. The introduction of this system ensures
improved turnaround time for results reporting, less blood drawn on patients, backup and expansion capabilities.
The Roche Modular Analytical and Pre-Analytical Systems are now fully integrated into the clinical service and they
perform well. Additional analytical capabilities are realized by implementing automated ELISA platforms for specialty
testing and maternal serum screening programs.
The RRL was fortunate to also acquire additional equipment through the DME funding process. New coagulation
equipment is now operational and promises to be more cost effective and improve turn around times for results
reporting.
The transfusion medicine laboratory is seeing automation for the first time with the purchase of the Galileo Automated System, .scheduled to be implemented in November.
A new protein and urine electrophoresis analyzer introduced in May 2006 is more cost effective, more user friendly
and has cut down dramatically the time for results reporting.
The Auto Delphia replaces other platforms used for Prenatal Screening Testing and the NexGen has the ability to
automate some of our more labour intensive testing.
Pathology
A major thrust of activity in anatomical pathology was progress towards incorporation of synoptic reporting into
surgical pathology reports. As well, there were trends toward increased utilization of immunohistochemical
biomarkers related to expanded indicators for HER2 testing for breast cancer, as well as increased demands for
immunohistochemistry related to consultation cases.
Dr. Patrick Shannon, a neuropathologist with special expertise in perinatal pathology was welcomed to our
staff. As well as his other duties, Dr. Shannon will have responsibility for the adult autopsy service, as well as
administrative direction of ultrastructural pathology.
Cytopathology
Planning is underway to expand the capacity of the liquid-based cytopathology technology with more automated equipment.
Dr. Shabin Nanji, a cytopathologist with special interest in education and telepathology, has been recruited to
Pathology and Laboratory Medicine. Dr. Nanji joined Mount Sinai Hospital in October 2006.
Ultrastructural Pathology
Dr. Bédard initiated the Electron Microscopy Service at Mount Sinai Hospital in 1972. Over the past 34 years
under Dr. Bédard, this service has grown to be the most respected diagnostic electron microscopic service in
Canada. Many leading pathologists in Canada, who are experts in Electron Microscopy, trained as residents
with Dr. Bédard.
Dr. Patrick Shannon has assumed a role of Principal Consultant for this service, succeeding Dr. Yvan Bédard.
The Electron Microscopy Service continues to enjoy high utilization, particularly for examination of tumours,
kidney biopsies, connective tissues and crystal analysis.
Canadian Reference Centre for Cancer Pathology (CRCCP)
Under the direction of Dr. Theo van der Kwast, the CRCCP continues its role as a tumour reference centre
principally for pathologists in more remote communities in Canada. With Dr. Doug Tkachuk, Objective Pathology Services, discussion has started on a proposal to digitize the CRCCP cases to make these cases more
widely available throughout Canada.

Laboratory Genetics
An active collaboration in Genetics has been established with the Lakeridge Health Centre. In Cytogenetics,
this has resulted in rearrangement of workload between Lakeridge Health Centre and Mount Sinai Hospital to
enhance services at both sites. Dr. Elena Kolomietz has been cross-appointed to Lakeridge Health Centre to be the
Laboratory Director of Cytogenetics.
Planning is underway to upgrade the software workstations for Cytogenetics. As well, Dr. Elena Kolomietz has
started exploration on the deployment of gene-based microarrays in Cytogenetics. This technology, comparative
genomic hybridization (CGH), has promise to identify genetic diseases not detectable by conventional Cytogenetics.
Gene probes using fluorescence in situ (FISH techniques) are used to identify genes in both Antenatal Cytogenetics
and in tumour tissues. Mount Sinai Hospital has received a grant from Roche Diagnostics to advance this
technology, with the goal of making the technique more robust and less time consuming.
In Molecular Genetics, an ABI-3130XL automated DNA sequencer was deployed together with DNA sequence and
fragment analysis software from Applied Biosystems, complemented by the Mutation Surveyor and Gene Marker
software packages from SoftGenetics. This equipment has accelerated the diagnosis of genetic abnormalities,
particularly in the Predictive Genetics of Cancer Program.
Pathology and Laboratory Medicine is collaborating with the Hospital for Sick Children and with Solutions by
Sequence, a company associated with Dr. Brenda Gallie, to provide retinoblastoma gene testing throughout
Ontario.
The implementation of Laboratory Information System Module “SoftGene” for Molecular Genetics is underway
with a “go live” schedule for late 2007.
Andrology
Plans are underway for expanding the ambulatory lab, as well as the Andrology Diagnostic Laboratory. Dr. Sergey
Moskovtsev has been recruited as a staff scientist for the Andrology Laboratory. New tests for specimen function
are being implemented. As well, a flow cytometer has been installed to provide tests to detect DNA fragmentation.
Rachel and David Rubinoff Bone and Tissue Bank
Over the past year, the Rubinoff Bone and Tissue Bank strengthened its capacity to provide more tissues under
stringent accreditation standards. The Rubinoff Bank is working with the Trillium Gift of Life to provide bone tissue
for applications throughout Ontario and beyond. These services are provided to other hospitals on a cost recovery
basis.
The Jeffrey Rubinoff Stem Cell Bank
This is a new bank designed to retain autologous haematopoietic tissues for future placement in the original donor.
Work on development of the bank is under the direction of Dr. Chen Wang. A key issue is decisions related to the
disease indications for cell storage banking and the appropriate point in the disease progression to harvest the cells.
To address these questions, Dr. Wang is investigating a class of lymphoproliferatiove disease, which by the available
biomarkers can remain quiescent for at least a decade.

LABORATORY INFORMATICS
Laboratory Information System
In 1992 Pathology and Laboratory Medicine (PLM) made the strategic decision to find a single vendor solution for
its laboratory information systems for each laboratory section. Soft Computer Consultants (SCC) was chosen as a
vendor and in 1994 the first module (a Blood Bank system) was installed. Between 1996 and 2000 other modules
were installed for chemistry, hematology, microbiology and pathology. Between 2003 and 2004 the LIS underwent
a major upgrade (going live November 29, 2004) involving all areas of the laboratory. With this upgrade a beta
version of SoftGene was introduced to the LIS.
LIS updates have continued through 2005 / 2006, including a sequential upgrade of all the laboratory systems,
going LIVE June 15, 2006. SoftGene has been promoted from the beta product initially introduced, to a much
more robust information system for handling cytogenetics information.
The backend of the LIS was strengthened in 2005/2006, with an upgrade of the LIS servers (completed April 13,
2006) and the move of the LIS servers to the Markham data centre (March 13, 2006). The move the Markham data
centre ensures that the LIS information is stored and archived in a safe and reliable manner.
Expanded Interfaces
The main clinical laboratory underwent a major renovation in 2005/2006. With this renovation a number of new
analytical machines were introduced to the laboratory. The interface to the Roche analytics and per-analytics was
implemented in September, 2005 and the interface to the Biorad Variant II was implemented in May 2006.
Citrix Support
Pathology and Laboratory Medicine has worked to allow all the professional staff remote access to network drives
and the Laboratory Information System via a Citrix solution. Following corrections found in the pilot project, the
Citrix solution has allowed the access to crucial files and information from remote locations around the world.
The introduction of the Citrix solution has also allowed several pathologists and support staff to work off-site.
Web Site/Communications
Pathology and Laboratory Medicine has worked in connection with Mount Sinai Public relations to create a distinct
web site. Part of this project was the joint supervision of a communications student (Keefe Lee) for web site
development. In addition, PLM is working with the Mount Sinai library to assist in the development of web site
content and other departmental communications.
The initial web site went live in the spring of 2005. Further enhancements have been made to the site and the
content has been enriched since the initial site was launched.
Mr. Keefe Lee has continued to develop departmental communications media and is working on developing
products for PLM research services.
Paradigm Document Control System
Controlling the hundreds of documents within PLM is a monumental task. To assist with management of the
laboratory documentation, the Paradigm Document Control System was introduced June 1st, 2006. As part of
this system all members of PLM have access to necessary documents through a web interface. Using this system
it should be possible to ensure that all members of the staff are using the current and most up to date version
of Standard Operating Protocols. This program should also help assist with the continual document review and
updating that occurs within Pathology and Laboratory Medicine.
Digital Imaging
Pathology and Laboratory Medicine has continued to move from traditional photography to digital imaging
solutions. All gross images are taken with digital cameras and the vast majority of photomicrographs are taken
digitally (all pathologists have digital cameras and a shared digital camera is available for residents).
Pathology and Laboratory Medicine is also continuing to evaluate and consider a number of solutions to handle the
growing number of digital images. Finding the solution to handle all of PLM’s digital needs is not a trivial matter.
Investigation into possible solutions is ongoing.
Digital X-Rays
A system to capture digital X-Rays was introduced in 2006, moving PLM to essentially complete digital imaging. As a complete digital imaging management solution for the department has not yet been implemented,
an interim solution using a picture archiving and communications system (PACS) has been introduced for the
digital X-Rays.
Digital Slide Scanning
A complete slide scanner was introduced to PLM in 2006. This system incorporates technologies from a
variety of companies, including software from Visiopharm that has advanced capabilities in image analysis and
morphometry.
The digital slide scanner has also been introduced to assist with the interpretation of the tissue slide microarrays that are being produced within the department. Using the slide scanner and microarray software it
should be possible to capture individual images of all the cores within the microarray. These images will then
be incorporated with the data from the cores, allowing both local and distant interpretation and analysis of
our microarray data.

RENOVATIONS & RELOCATIONS
The expanded Rapid Response Laboratory was opened on December 19, 2005 by Mr. George Smitherman, Minister of Health of Ontario in conjunction with his announcement of the Designated Medical Equipment funding for
laboratory services. The highlight of the Rapid Response Laboratory expansion is the Roche chemistry system. This
fully automated system includes extensive pre-analytical automation as well as automated chemistry and immunochemistry analyzers all linked by an automated specimen conveyor system. The pre-analytical component consists
of centrifuges and specimen aloquoting system. This facility is amongst the most advanced in the world. Early
experience indicates that this instrumentation is fulfilling its promise of decreasing labour and decreasing turnaround time. With the relocation of the Sysmex hematology system to be immediately adjacent to the chemistry
system, the Rapid Response Laboratory is even more capable of providing fast, efficient service at all times. In the
next phases of this project, space will be made available for new specialized chemistry analyzers, new coagulation
instruments, an automated blood bank system and new refrigerated storage.
This project was accompanied by the completion of the HVAC renovation of this portion of the laboratories, and
the installation of a sprinkler system. These features have greatly enhanced the capacity and flexibility of the Rapid
Response Laboratory.
In June 2006 Dr. Eleftherios Diamandis relocated his research laboratories to the Koffler Centre at 60 Murray Street.
These laboratories will be adjacent to the clinical facilities for the Koffler Centre for Men’s Health as well as the
research genetics facilities associated with Drs. Bharati Bapat, Steve Gallinger and Hilmi Özçelik all of whom are
members of Pathology and Laboratory Medicine.
Over the current year Mount Sinai Hospital embarked on a plan for capital renewal which includes both short term
and long term capital planning.
In the short term, Pathology and Laboratory Medicine is planning facilities for its Andrology Ambulatory Laboratory,
renovation/expansion of the 3rd floor ambulatory lab at the Ontario Power Generation building to accommodate
the expansion of the Women’s Program. As well, facilities for flow cytometry, a specialized facility to enable protein
extractions, a facility to house an advanced, high resolution, specialized x-ray machine, space for morphometry
including tissue microarray, and expansion of laboratory genetics space are planned.
In the longer term a comprehensive plan for renovation/expansion of Histology and Autopsy facilities is planned as
well as more definitive and purpose built space for laboratory genetics.
QUALITY PROGRAMS

Harold G. Pritzker Memorial Lecture 2006
Harold Goldsmith Pritzker was the Pathologist-in-Chief at Mount Sinai Hospital from 1938 to 1963. Upon his
death, his colleagues in all disciplines established an Annual Memorial Lectureship Fund, which was transferred to the Academy of Medicine. A committee was created, which included representatives of the founding bodies, namely the Academy of Medicine, Mount Sinai Hospital and the University of Toronto. With the
demise of the Academy of Medicine in 1992, the Annual Memorial Lecture continued with the generous support of the hospital administration and the Department of Pathology and Laboratory Medicine of Mount Sinai
Hospital. The initial lecture was delivered in 1965. Throughout the years, this lecture has become one of
the most distinguished lecture series attracting some of the most celebrated names in Pathology and related
fields.
The guest lecturer for the 41st Annual Pritzker Memorial Lecture was Professor Olli Kallioniemi from Finland,
who presented “Translational Cancer Systems Biology” April 24th, 2006.

“Working Towards a Conflict Free Work Environment”, January 12th and 13th, 2006, attended by 23
staff members.
Volunteer Student Program
Several high school students spent up to five months at Pathology and Laboratory Medicine, predominately
in the Cytopathology and Surgical Pathology sections. This very successful co-operative programme exposes
these students to observe, at an early stage, the important role played by laboratory medicine in the diagnosis
and management of patients. After completion of the programme, the students usually receive a high school
credit or an Ontario Academic Credit (OAC).
With the Volunteer’s Office, volunteers are encouraged to observe and assist in the laboratories. Occasionally,
these volunteers are high school students but, more frequently, are university undergraduates seeking additional exposure to a medical environment.

SPONSORED RESEARCH AND INNOVATION
Research and Innovation continues principally in cancer diagnostics, articular skeletal diseases and bioengineering,
fetal and perinatal diseases and genetics. Much of this work is collaborative, internally with clinical colleagues
and with investigators at the Samuel Lunenfeld Research Institute and externally with colleagues in similar
fields worldwide. The activities, although in a few select areas, do range from fundamental research through
translational research, introduction of innovation and reduction to practice. This is reflected in the broad range of
public and corporate sponsors of the Research and Innovation activity within Pathology and Laboratory Medicine.
