Directory for
Senior Adults in the Atlanta Region
The Atlanta region’s 10 counties are home to a third of all Georgians age 60 and older and
this population is growing. Today, one person in 10 is over age 60 and it is projected that
by 2030, one person in five will be over 60. This Resource Directory for Senior Adults and
Individuals with Disabilities in the Atlanta Region provides information to help individuals and
their caregivers identify opportunities and services aimed at enriching and supporting a long
and healthy lifestyle for our region’s expanding population. It also includes a listing of services
to help individuals that are frail remain in the community and maintain their independence
as long as possible.
As the Area Agency on Aging (AAA) and the Aging & Disability Resource Connection (ADRC) for
the 10-county Atlanta region, the Atlanta Regional Commission (ARC) is responsible for planning, developing, implementing and coordinating the delivery of services through a network
of locally based agencies and service providers. ARC serves senior adults, individuals with
disabilities, family members, caregivers, friends, professionals and the community at large.
AAAs were established under the Older Americans Act in 1973 to respond to the needs of adults
aged 60 and over and that charge has grown over the years, most notably with the creation
of ADRCs which were created to allow people of all ages, incomes and disabilities to go to
one location to get information and one-on-one counseling on the full range of long term care
supports and solutions. AAAs/ADRCs are part of a comprehensive, nationwide information and
referral/assessment system that responds to the aging, disabilities and long term care needs
of individuals throughout the country.
The Atlanta Regional Commission AAA/ADRC coordinates and maintains the ESP statewide
database containing more than 26,000 services and programs. Many of the resources listed
in this publication are from the ESP database. Information and Referral Specialists and others
throughout the state use ESP to find services for Georgia’s residents. The database provides
detailed information, customized for each service category, including hours of operation,
eligibility, cost, and services or programs offered by agencies and fee for service companies
in the community. For more information about ESP, call (404) 463-3232.
This directory is a tool to help connect you to local services and activities. Services offered may
be subsidized by government, provided for a fee, covered through insurance or offered at no
cost to the user. For additional detailed information on resources, contact the Atlanta Regional
Commission’s Aging & Disability Resource Connection. ARC’s trained and certified Information
and Referral Specialists can consult with you, help you assess your needs and provide you
contact information for appropriate resources. There is no charge for this Information and
Referral service.

Cherokee County Senior Services (CCSS), a unit of Cherokee County government, manages and

County. The agency provides information and assistance, care management, homemaker or
respite services, a senior center and three satellite centers (Waleska, Ball Ground and Bells
Ferry) that provide both meals and opportunities for recreation, education and socialization.
Additional services include transportation and volunteer opportunities. Cherokee County
Volunteer Aging Council, a 501C(3) organization, works in cooperation with CCSS, providing
supplemental funding for emergency financial assistance, a chore program, a fan drive and a

The Fulton County Office of Aging provides one-stop centralized information and assistance
services through its’ STARline unit. Through contract agreements, the county has expanded the
senior service network to include new and remodeled neighborhood centers, home-delivered
meals, care management, homemaker services, some respite and volunteer opportunities.
The Fulton County office also oversees four senior multipurpose facilities. These facilities all
have cafeterias, exercise facilities, therapeutic pools, computer training centers and adult
day programs. There are also 15 neighborhood senior centers which provide opportunities to
participate in life enrichment, nutrition education, fitness and leisure and recreation activities.
Coordinated transportation services are provided through a private contractor. In-home services, which include homemaker and personal care are contracted with an outside agency.

Medicare
Medicare is a federally funded health insurance program administered by the Centers for
Medicare and Medicaid Services that provides hospital, prescription and medical insurance
for individuals age 65 and older; for disabled individuals younger than age 65 who have been
entitled to Social Security disability benefits for at least 24 consecutive months; and for persons
with end-stage renal disease or Lou Gehrig’s Disease.

Medicaid is a state-operated medical assistance program that helps many people who cannot
afford to pay for some or all of their medical bills.
Medicare Saving Programs are for individuals with limited income and resources that pay
some or all of Medicare’s premiums. Some programs may also pay Medicare’s deductible
and co-insurance.

Medicaid waiver programs allow states to provide services to people who are elderly or
disabled and need help to live in their home or the community instead of an institution such as
a nursing home. Below are brief descriptions of several Medicaid Waiver programs in Georgia:
Community Care Services Program (CCSP)
The Community Care Services Program (CCSP) helps Medicaid eligible individuals of any age
who are functionally impaired and meet a nursing home intermediate level of care to continue
living in their homes and communities. CCSP offers community based care such as adult day
health, personal support, skilled nursing and placement in an alternative living arrangement
(licensed personal care home) as an alternative to nursing home placement.

SOURCE
The SOURCE Program (Service Options Using Resources in a Community Environment), serves
persons who meet program financial criteria, have a chronic health condition and meet an
intermediate nursing home level of care. Clients must reside in the SOURCE site’s designated
service area, and must, with assistance, be capable of safely residing in the community. The
Program is an alternative to nursing home placement and provides comprehensive preventative medical care with in-home support services, including adult day health, emergency
response units, home delivered meals, personal support services, and alternative living
services (24 hour care in a personal care home). Enrollment in the program is open to eligible
Individuals of any age. Participants must be willing to use a SOURCE physician as the primary
care physician. There are currently four case management agencies that arrange and coordinate services in the Atlanta area.

Independent Care Waiver Program (ICWP)
The Independent Care Waiver program provides in-home support to individuals between
the ages of 21 to 64 who are either physically disabled or have traumatic brain injury
with limitation(s) in one or more Activities of Daily Living and are currently at risk of institutionalization. Services under the program are determined on an individual basis depending on
the disability. Services may include personal support, environment modification, personalized

The COMP Waiver
The Comprehensive Supports Waiver (COMP Waiver) is funded through Medicaid and is

The New Options Waiver (NOW) Program, funded under Medicaid, provides home and community-based services for persons with intellectual/developmental disabilities. A diagnosis
of developmental disability includes conditions such as cerebral palsy, developmental delay,
epilepsy, autism or some neurological issues. The NOW program enables individuals with
less intense and urgent needs to live independently in the community. Services may include
support coordination, day programs, residential services, respite care and specialized medical
supplies and equipment.

In-home support services include chore services, homemaker services, and other services
provided in an individual’s home.
(Geriatric) Care Management Services
A (Geriatric) Care Manager is a health and human services specialist who helps families who
are caring for older relatives or persons with disabilities. They are trained and experienced
in any of several fields including nursing, gerontology, social work, counseling or psychology.
Services include assessing needs, developing a plan of service, arranging and monitoring the
delivery of services and evaluating the need for continuation of services.
Caregiver Support Services
Trained information and referral specialists can offer telephone consultation to individuals who
need help in making decisions related to caring for a vulnerable Individual in their charge.

The Aging & Disability Resource Connection is a telephone information and assistance service
through which persons 60 years of age and older, persons of any age with a disability or anyone
calling on their behalf, can be linked with appropriate service providers. Inquiries are handled
by trained and certified information and referral specialists. These professionals confidentially
and sensitively assess the person’s needs, provide helpful information, and answer questions
concerning many topics.

Sitter Services/Home Care Providers
Licensed Private Home Care Providers offer nursing care, personal care assistance and
companion/sitter services to older adults or individuals with a disability in the care recipient’s
home. The Georgia Department of Community Health, Healthcare Facility Regulation Division
licenses private home care providers. There are more than 500 licensed home care providers
serving the Atlanta region.

Adult Day Care Centers
Adult day care programs are designed to meet the needs of frail adults and provide oversight, education, social programs and meals. Effective January 7, 2015, the Department of
Community Health, Healthcare Facility Regulation Division implemented licensing for Adult
Day Care Programs.
Home-Delivered Meals Programs
Home-delivered meals programs provide nutritious noon-time meals, which meet one third
of the USDA nutritional daily requirements, to older individuals who are homebound and, due
to an impairment, are unable to prepare their own meals.

Hospice Care
Hospice programs provide a full range of palliative and supportive services for terminally ill
people who are in the final stages of their illness. Services may include medical care, pain and
symptom management, home nurse visitation, care management, counseling and bereavement services for the patient and members of the family. A hospice may be a free standing
facility, a unit in a hospital or residential institution or service delivered in the home. Hospice
services are typically covered by Medicare if ordered by a physician. In Georgia, the Georgia
Department of Community Health, Healthcare Facility Regulation Division, licenses hospice
programs.

Continuing Care Retirement Communities, also known as Life Care Communities, are a type
of retirement community where three levels of residential needs; independent living, assisted
living/licensed personal care home and nursing home care, may all be met on the same campus. Individuals are able to move from one housing choice to another as their needs change.
Most communities require an entrance fee based on the floor plan selected and also charge
monthly fees. In some communities, the entrance fee may be partially refundable under certain

These options provide housing in a group setting for older adults or individuals with a disability who, because of some type of impairment, are unable to live independently. The Georgia
Department of Community Health, Healthcare Facility Regulation Division, licenses personal
care homes and assisted living communities. These homes and communities provide 24-hour
watchful oversight and generally offer staffing and a level of care between independent living

The Long-Term Care Ombudsman Program
of Metropolitan Atlanta
The Long-Term Care Ombudsman Program advocates for residents in nursing homes, assisted
living communities and personal care homes by investigating and resolving complaints related
to quality of care issues such as health, safety and welfare, human and civil rights violations,
or administrative abuses. The Ombudsman also provides information on facilities and services

Nursing Homes
Nursing homes provide care to persons who are frail, chronically ill or recuperating from an
illness and need regular nursing care and other health services but not hospitalization. Many
facilities are certified for Medicaid and/or Medicare reimbursement. Nursing homes are
licensed by the State of Georgia Healthcare Facility Regulation Division (HFRD), Nursing Home
Section. HFRD conducts annual surveys of the nursing homes. The results of these surveys
must be posted in a public, readily accessible and visible place. If there are questions about

Supplemental Nutrition Assistance Program (SNAP)
(Formerly Known as Food Stamps)
The Supplemental Nutrition Assistance Program (SNAP) is a federal program that helps lowincome individuals and families who meet certain income and other eligibility criteria buy food.
Recipients are able to access their benefits through a card that can be used in stores as one
would use a debit card. Non-food items cannot be purchased with the SNAP card. Older persons may use part of their monthly allotment to contribute to some home delivered meals and
congregate meal programs. If you are a resident of Cherokee, Clayton, Cobb, DeKalb, Douglas,
Fayette, Fulton, Gwinnett, Henry or Rockdale counties and wish to apply for food stamps, contact the Department of Family and Children’s Services statewide hotline at (877) 423-4746.

The State of Georgia provides energy assistance through three programs: Regular Energy

The Georgia Crisis and Access Line
The Georgia Crisis and Access Line (GCAL) is a single point of contact for Behavioral Health,
including Addictive Diseases, and Developmental Disability services and is the Behavioral
Health Crisis Service contact statewide. The GCAL can schedule an appointment with Georgia’s
public behavioral health or substance abuse providers and refer to the appropriate organization

MARTA issues half-fare passes for senior citizens age 65 and older, individuals with a disability
or those with a Medicare card. The card allows travel at half the regular fare at all times and
the rider’s portion of the fare must be paid for in cash. Half-fare passes are good for both the
bus and rail systems. Half-fare cards can be obtained at the Lindbergh Center Headquarters
and Five Points Transit Station locations. Hours are Monday-Friday, 9 a.m.-12 noon and 2 p.m.4 p.m. and persons must bring a picture ID, Medicare card and proof of disability.

To obtain a temporary, permanent, or special permanent disabled person parking placard,
pick-up an application at the local tag office or visit http://dor.georgia.gov/sites/dor.georgia.

MV9D_0.pdf. The form must be completed by a Georgia licensed physician, notarized and
returned to the local county tag office. There are two types of permits; a temporary permit valid
for six months or less and a permanent permit that is valid for a four-year period.
Non-driver identification cards may be obtained at any location that issues driver licenses.

Adult Protective Services
Georgia law requires that anyone who believes an elderly person or a person with a disability is being abused, neglected or exploited report the occurrence. By law, reports remain
confidential. Adult Protective Services (APS) are available through the Georgia Department of
Human Services, Division of Aging Services to provide protection from exploitation, abuse or
neglect of individuals who, because of physical or behavioral health/cognitive limitations, are
unable to act in their own best interests. Contact the following to report suspected abuse or

Reporting is confidential and the law protects anyone from a lawsuit who reports abuse.
Long-Term Care Facilities or Residences
Long Term Care Facilities include nursing homes, personal care homes, assisted living
communities and community living arrangements. The Department of Community Health,
Healthcare Facility Regulation Division is responsible for investigating reports in these

to report concerns or suspected abuse.
Long-Term Care Ombudsman
The Long-Term Care Ombudsman Program advocates for residents in nursing homes, assisted
living communities and personal care homes by investigating and resolving complaints related
to quality of care issues such as health, safety and welfare, human and civil rights violations,
or administrative abuses. The Ombudsman also provides information on facilities and services
and in general advocates for quality institutional care.

Consumer Protection
The Governor’s Office of Consumer Protection (OCP), (404) 651-8600, enforces the Fair
Business Practices Act and other consumer protection laws. The office is charged with
­protecting consumers from unfair and deceptive acts and practices in the context of
­consumer transactions. The OCP investigates consumer problems, monitors the marketplace
to promote fair and honest competition, takes enforcement actions against violators and
publishes consumer education materials and alert warnings. The office also investigates
instances of identity theft, telemarketing scams and other types of consumer fraud.
