This directory provides a list of Kaiser Permanente’s network providers.
This directory is for the Hawaii Region service area which includes the county of Honolulu and
partial counties of Maui and Hawaii. If you live in the county of Maui or Hawaii, please see the
service area description inside for the ZIP codes that are in our service area.
This directory is current as of August 2016. Some network providers may have been added or
removed from our network after this directory was printed. We do not guarantee that each
provider is still accepting new members. To get the most up-to-date information about Kaiser
Permanente’s network providers in your area, you can visit kp.org/medicalstaff or call
Member Services at 1-800-805-2739, seven days a week, 8 a.m. to 8 p.m. TTY users should
call 711.
Kaiser Permanente is an HMO plan with a Medicare contract. Enrollment in Kaiser
Permanente depends on contract renewal. The provider network may change at any time.

Section 1 – Introduction
This directory provides a list of Kaiser Permanente’s network providers. To get detailed information about
your health care coverage, please see your Evidence of Coverage.
Your Primary Care Physician
As a member, you must choose a network provider
who is listed in this directory to be your Primary
Care Provider (PCP). The term “PCP” will be used
throughout this directory. Generally, you must get
your health care services from your PCP.
Your PCP will provide most of your care and will
arrange or coordinate the rest of the covered
services you get as a member of our plan, including
specialty care. To see a specialist, you usually
need to get your PCP’s approval first (this is called
getting a “referral” to a specialist). However, there
are specialty care services that do not need a
referral (self-referrals) to make an appointment.
You do not need a referral to make appointments
for the following Kaiser Permanente services
(please refer to the Evidence of Coverage for
a complete list):
• Routine women’s health care
(obstetrics and gynecology)
• Flu shots, Hepatitis B vaccinations,
and pneumonia vaccinations
• Emergency services
• Urgently needed care
• Kidney dialysis services at a Medicare-certified
dialysis facility when you are temporarily outside our service area
You must select a PCP from any of our available
primary care plan providers. You can change your
PCP at any time and for any reason. When you
make a selection, it is effective immediately. To
learn how to select or change a PCP, please call
Member Services toll free at 1-800-805-2739,
seven days a week from 8 a.m. to 8 p.m., or 711
(TTY/TDD for the hearing/speech impaired) or visit
us online at kp.org.
Getting Covered Care
The “network providers” listed in this directory have
agreed to provide you with your health care services.
You may go to any of our network providers listed
in this directory; however, some services may require
a referral. If you have been going to one network
provider, you are not required to continue to go to
that same provider. In some cases, you may get
covered services from non-network providers, for
example: emergency care, urgently needed care
when our network providers are temporarily
unavailable, out-of-area dialysis, and authorized
referrals for services not available from network
providers. For some types of referrals, your PCP
may need to get approval in advance from our Plan
(this is called getting “prior authorization”). It is very
important to get a referral (approval in advance)
from your PCP before you see a Plan specialist or
certain other providers.

Bills from out-of-network providers
If you receive a bill from an out-of-network provider
for covered care (for example, emergency care), you
should not pay the bill and instead send it to our
Claims Administration department so we can pay the
provider the amount they are owed and determine
your cost-sharing amount, if any.

What are
“urgently needed services”?
“Urgently needed services” are a nonemergency,
unforeseen medical illness, injury, or condition that
requires immediate medical care. Urgently needed
services may be furnished by network providers or
by out-of-network providers when network providers
are temporarily unavailable or inaccessible. The
unforeseen condition could, for example, be an
unforeseen flare-up of a known condition that
you have.
What if you are in our service area
when you have an urgent need
for care?
You should always try to obtain urgently needed
services from network providers. However, if
providers are temporarily unavailable or inaccessible,
and it is not reasonable to wait to obtain care from
your network provider when the network becomes
available, we will cover urgently needed services
that you get from an out-of-network provider.
We know that sometimes it’s difficult to know what
type of care you need. That’s why we have telephone
advice nurses available to assist you. Our advice
nurses are registered nurses specially trained to
help assess medical symptoms and provide advice
over the phone, when medically appropriate. Whether
you are calling for advice or to make an appointment,
you can speak to an advice nurse. They can often
answer questions about a minor concern, tell you
what to do if a network facility is closed, or advise
you about what to do next, including making a
same-day urgent care appointment for you if it’s
medically appropriate. To speak with an advice
nurse or make an appointment, please refer to
the medical facility listing starting on page 8 for
appointment and advice telephone numbers.
What if you are outside our service
area when you have an urgent need
for care?
When you are outside the service area and cannot
get care from a network provider, we will cover
urgently needed services that you get from any
provider. Our plan covers urgently needed services
anywhere in the world.

Emergency Services
What is the service area for
Kaiser Permanente?
Emergency care can always be obtained in or
out of the service area from the nearest available
provider. If you have a medical emergency:
The counties and parts of counties in our service
area are listed below.
• Get
Honolulu County, full county
help as quickly as possible. Call 911 for help
or go to the nearest emergency room, hospital,
or urgent care center. Call for an ambulance if you need it. You do not need to get approval or
a referral first from your PCP.
• As
soon as possible, make sure that your plan
has been told about your emergency. We
need to follow up on your emergency care. You
or someone else should call to tell us about your
emergency care, usually within 48 hours. The
number to call is listed on the back of your
membership card.
After the emergency is over, you are entitled to
follow-up care to be sure your condition continues
to be stable. We will cover your follow-up
post-stabilization care in accord with Medicare
guidelines. If your emergency care is provided by
out-of-network providers, we will try to arrange for
network providers to take over your care as soon as
your medical condition and the circumstances allow.
It is very important that your provider call us to get
authorization for post-stabilization care before you
receive the care from the out-of-network provider.
In most cases, you will only be held financially liable
if you are notified by the out-of-network provider or
us about your potential liability.
You must use plan providers except in emergency or
urgent care situations or for out-of-area renal dialysis
or other services. If you obtain routine care from
out-of-network providers, neither Medicare nor
Kaiser Permanente will be responsible for the costs.

pages 7, 43, 60 and 72). Then locate the medical
facility listing starting on page 8. You will find a
listing of primary care physicians and specialists
practicing at that medical facility. Behavioral health
providers are listed on pages 8 and 44 and hearing
aid providers are listed on page 83. Optical care
centers, pharmacies, and hospitals are listed
separately following the primary care physician and
specialists listings in this directory.

patient’s recovery. We welcome you 24 hours a day,
7 days a week.
Latex balloons (rubber type) are not allowed in the
medical center. Plants and flowers are not allowed
in all Intensive Care Units. For the protection and
security of everyone in our facility, any visitors after
8 p.m. will need to check in with the security officer
at the front desk.
