Kaiser Permanente uses the same quality, member experience, or cost-related measures to select
practitioners in Marketplace Silver-tier plans as it does for all other Kaiser Foundation Health Plan
(KFHP) products and lines of business. Members enrolled in KFHP Marketplace plans have access to all
professional, institutional and ancillary health care providers who participate in KFHP plans’ contracted
provider network, in accordance with the terms of the members’ KFHP plan of coverage. All Kaiser
Permanente Medical Group physicians and network physicians are subject to the same quality review
processes and certifications.
A narrow network is defined as a tailored, tiered or high-performance network that has 80 percent or less
overlap with a health plan’s broadest network offered to a member in any type of plan offered by that
health plan. Kaiser Permanente does not have a narrow network with respect to its Marketplace plans.
Members enrolled in Kaiser Foundation Health Plan (KFHP) Marketplace plans have access to the same
provider networks as members enrolled in all other KFHP plans.

to all Kaiser Permanente facilities, and your source
for referrals to specialists.
Select your personal physician from any of our
available providers. You can change your personal
physician at any time and for any reason.
To choose a provider:
1. Select a medical location where you plan to
receive services. Most members select a medical
facility that is convenient to home or work.
2. Choose a doctor from one of three primary care
options:
• Family Medicine cares for members of all
ages and specializes in caring for entire
families.
• Internal Medicine specializes in medical and
preventive care for adults.

Paying for services
Two easy ways to schedule your appointments:
When it’s time for your appointment, be prepared
to pay for your services. Your portion may include a
copayment, coinsurance, or deductible.
•Visit kp.org/appointments on your computer or
use the Kaiser Permanente mobile app, and tap

appointment cancellation line of your Kaiser
Permanente medical location. If you’re bringing
in a child that is not your own, please get an
authorization form from Member Services
or have a notarized Health Care Power of
Attorney form.
Pay with cash, personal check, debit or credit card
(Visa®, MasterCard®, Discover®, and American
Express®).
You may be billed supplemental charges for
services performed after you’ve paid and left the
clinic. For example, your doctor may need to send
tissue samples or specimens for further testing.

patient’s recovery. We welcome you 24 hours a day,
7 days a week.
Latex balloons (rubber type) are not allowed in the
medical center. Plants and flowers are not allowed
in all Intensive Care Units. For the protection and
security of everyone in our facility, any visitors after
8 p.m. will need to check in with the security officer
at the front desk.

For member convenience, our pharmacies are located in most Kaiser Permanente clinics and are
open during clinic hours. Additionally, we contract with many community pharmacies throughout
the islands to provide prescription drug services. Any member can get prescriptions filled and
buy over-the-counter medications and supplies at a Kaiser Permanente pharmacy or select from a
contracted pharmacy listed below. Members also have the option of refilling their prescriptions by
mail, online, or over the phone.
