IS YOUR INSURANCE CURRENT?
As we enter a new year, it is essential that Holy Redeemer Lafayette has the most
up-to-date insurance and financial information for our residents. Please be aware
of the following important information:
*If there are any changes in residents’ insurance coverage, please bring a copy of
new insurance cards to the Admissions Office to ensure that up-to-date information is on file. The most recent information is needed to guarantee appropriate
coverage for services.
*The Business Office would also like to request that families send in any updated
insurance plans, updated insurance premium information and any other
updated financial information.
*If you have an HMO plan, please make sure the correct physician is listed on your
loved ones insurance card. If you need more information contact Melinda Lee at
