4/15/16. To be eligible to receive a mail-in rebate under this offer you must: (1) make a purchase of 4 select
Goodyear, Cooper or other select tires. (2) Complete and submit a rebate form together with a copy of the
original invoice that is postmarked and mailed by 5/30/16. Customers who use the Midas Credit Card for their
qualifying purchase will be eligible to receive an additional rebate of up to $70 on 4 select Cooper or Goodyear
tires, or up to an additional $40 rebate on 4 other select brand tires.
*Purchases made with a Midas Credit Card are subject to credit approval. One rebate per purchase valid at
participating U.S. locations. Plus shop fee of up to 15% based on non-discounted retail price, not to exceed
$35.00, where permitted. See store for details. Limit three redemptions per household. Disposal fee, if
applicable. Original equipment manufacturers, fleets, wholesalers, commercial accounts and purchases for resale
are not eligible for this promotion. Most vehicles. Prepaid Cards will be sent to customer’s address provided
