SO WHAT IS PERSONALIZED CARE ? IT IS A PATIENT CENTERED QUALITY CARE THE FOCUS IS THE CLIENT PERSONALIZED CARE Many benefits can be gained from personalised care planning. International evidence shows best outcomes are achieved when there are: systematic proactive services; people engaged in their own care; and healthcare professionals and people with longterm conditions working in partnership (Wagner et al, 1996) Midwifery services is about the care of mother and child. Personalised care is the ART of midwifery services. The midwifery KNOWLEGDE and SKILLS is the SCIENCE of provide comprehensive health midwifery services. THE OBJECTIVE IS : To services where there is Continuity Of Care from the time of registration followed up by appointments and visits to the clients home by the same personnel. Therefore there is consistency. There is no continuity of care because different nurses may differ in knowledge, experiences , treatment plans and instruction which may cause their clients to be confused, and thus affect the trust of the clients to our services. ADVANTAGES OF PERSONALIZED CARE Save time on client care, as nurses know their clients especially the high risk cases. Strengthen the rapport and trust with clients and thus enhance the compliance and reduce the incidence of defaulter Less closeness with client due to frequent change in nurses and this can cause dissatisfaction to the clients especially when questioned repeatedly on the same matter by different nurses. No long term plans for the clients because nurses see clients on rotation basis and there is a tendency for short term plan only. ADVANTAGES OF PERSONALIZED CARE Nurses are more focused on cases under their own supervision, thus ensuring continuity of care. Nurses are more thoughtful and serious in client care, because they care from the beginning of pregnancy till end of pregnancy and so forth – ‘encouraging professional and accountability. ADVANTAGES OF PERSONALIZED CARE Providing clients with information such as test results or prompting them to think about questions to ask before the care planning consultation allows clients to reflect and prepare to get the most out of the discussion. It also establishes an equal relationship with their nurses.
