As part of your healthcare team, our desire is to walk beside you and assist you during your journey. The Advocate Condell Medical Center nurse navigators act as a point person for the cancer patient from time of diagnosis throughout the continuum of care to survivorship in a lifelong partnership. As part of a multidisciplinary team of medical professionals, the navigator provides emotional support and helps to identify resources and potential challenges related to a cancer diagnosis. Navigators assist in linking the patient, family, and/or caregiver to physicians, medical care, transportation, financial, nutritional and supportive services. The new Cancer Resource Center is conveniently located off the main entrance to the hospital, near the women’s center, the outpatient cancer infusion center and radiation therapy. The Cancer Resource Center offers a wide array of services to educate and support patients, including: • • • • • • • • • • • Wigs and prosthetics fittings Exercise programs Reiki Therapy Nutritional education Lymphedema treatment and referral to Rehabilitation Services Support groups Psychosocial and spiritual counseling Genetic counseling Clinical trials Cancer management and prevention resources Space for relaxing and respite A computer in the lounge links users to online resources available to patients based on their individual diagnosis. The Center is open 24-hours a day for inpatient family members who may need a break from the cancer unit. A family conference room also is available to allow a physician or medical team to meet with an entire family to discuss the patient’s diagnosis, if desired. Genetic Counseling Condell Medical Center offers on-site cancer risk assessment and genetic counseling by certified genetic counselors who are licensed health care providers in the state of Illinois. The decision to pursue genetic testing is a very personal one. Cancer gene testing has significant medical, social and psychological implications and the advantages and disadvantages need to be considered before undergoing testing. Our genetic specialists will help facilitate the decision-making process, allowing patients to make informed choices about genetic testing. Individuals who do not pursue genetic testing can still learn valuable information about cancer risk management from a genetic counseling session. Knowing an individual has an increased risk for cancer can result in an improvement in their health and quality of life. Genetic cancer risk assessment and genetic testing may: • Identify one of the causes of cancer • Identify other family members, such as children and siblings, who are at an increased risk of cancer and may benefit from cancer screening • Identify individuals who are NOT at increased risk of cancer • Help develop an individualized cancer screening schedule that utilizes the appropriate screening and diagnostic tests, based on the latest genetic information • Aid in decision making about risk-reducing preventive surgery If you have questions or need assistance scheduling an appointment, please talk to your nurse navigator. Hospitals that engage in clinical research have better patient care, outcomes, patient safety, and lower death rates than hospitals that do not participate in clinical research. Why participate in a clinical trial? There are many reasons why patients choose to participate in a clinical trial. Here’s what you should know as a patient participating in a clinical trial: You are playing an important role in advancing medicine and improving care. You may have access to new medications or treatments not yet available. All patients are closely monitored throughout the study to determine both the safety and effectiveness of new therapies. How am I protected? The Advocate Institute Review Board (IRB) is an administrative body composed of scientists and non-scientists, established to protect the rights and welfare of human research subjects recruited to participate in research activities conducted under the auspices of Advocate Health Care and its affiliations. Legislation was passed in 1979 requiring that all investigators doing a "medical experiment" must offer their subjects a copy of the "Experimental Subject's Bill of Rights." An Advocate version of the Bill of Rights has been developed which includes all the elements required by this legislation, written in language that is meant to be understood by a wide range of people. Rehabilitation Services At Condell Medical Center, our Rehabilitation Services is made up of talented therapists who understand the holistic needs of the cancer patient. Services are offered at three convenient locations including the hospital campus, Vernon Hills, and Gurnee. Schedule through your nurse navigator or by calling 847.990.2300. We offer: • A multidisciplinary approach • One-to-one treatment sessions • Convenient early morning and evening appointments • Comprehensive evaluations and best practice standards • Pre-surgical intervention for breast cancer • Lymphedema evaluation and management What is Lymphedema? Lymphedema, or the accumulation of fluid in the arms and/or legs, is a common but often misunderstood condition that affects the quality of life of many cancer survivors. The condition can occur when lymphatic vessels are damaged or when lymph nodes are removed, as in the case of patients who receive surgery, radiation or chemotherapy for prostate, breast or other cancers located near lymph nodes. Our physical and occupational therapist have additional training in treating lymphedema. They can develop a comprehensive treatment plan that addresses all of your lymphedema needs. Some of the techniques we use to minimize lymphedema side effects are: • Performing massage techniques to increase fluid circulation • Applying and educating on compression bandages or garments • Educating on proper skin care • Exercises to improve muscle contraction and joint mobility A physician’s order is necessary to begin lymphedema and rehabilitation services. Cancer Care and Treatment Services Interventional Radiology The Interventional Radiology Department has several dedicated radiologists specially trained and board certified to support you during your course of care. Types of care provided in the Interventional Radiology Department include: Ports (or portacath) Some patients may need to have a special port (or portacath) used to inject medications such as chemotherapy and draw blood samples with less discomfort than a more typical "needle stick." A portacath is a small medical device that is placed under the skin, typically in the upper chest area, and connects a catheter from the port to a vein. The port resides under the skin and requires no special maintenance and is completely internal so swimming and bathing are not a problem. Specialty procedures performed in interventional radiology include: Yttrium 90 Spheres In this procedure, radioactive microspheres are injected into the vessel feeding primary and metastatic tumors of the liver from colon cancers. Chemoembolization A highly concentrated chemotherapy agent is injected into the arteries feeding the tumor. The arteries are then occluded to prevent the agent from flowing out. Other procedures that may be done by interventional radiology include biopsies and drainage or aspiration of internal fluids. Comprehensive Radiation Oncology The newly renovated Cancer Institute’s Radiation Oncology Center features some of the most comprehensive radiation therapy options available in Lake County, allowing radiation oncologists the ability to select the most effective treatment for their patients. In addition to board-certified radiation oncologists, our radiation oncology team includes medical physicists, dosimetrists, radiation therapists, nurses, and navigators. The Radiation Oncology Center treats hundreds of new patients every year and is conveniently located on the campus of Advocate Condell Medical Center. The Center has convenient access with a designated entrance and handicapped parking directly in front of the entrance as well as on-site Registration. During your radiation treatments you will have regular appointments with your radiation oncologist and radiation nurse navigator. Leading Edge Radiation Oncology Therapies 3-D Conformal Treatment Planning (3D) Using CT images, physicists can reconstruct body parts and customize treatment angles to spare healthy tissue. Intensity Modulated Radiotherapy (IMRT) One step beyond 3-D conformal treatment planning, this technology involves inverse treatment planning to break the radiation beam into numerous beamlettes. Exact locations of the tumor and healthy structures are used by the computer to determine the intensity for each individual beamlette, sparing as much healthy tissue as possible. Image Guided Radiation Therapy (IGRT) Image Guided Radiation Therapy or IGRT allows the radiation oncologist to accurately treat the areas with confidence and repeatability. The On-Board Imager (OBI) demonstrates patient anatomy in 2D radiographic format or in 3D cone-beam CT images. High Dose Iodine I-131 (I-131) High Dose I-131 treatments are given to patients with carcinoma of the thyroid gland. The treatment is administered on an outpatient basis and certain criteria must be met. The dose is given orally in a capsule form and is very well tolerated. Stereotactic Radiosurgery (SRS) and Stereotactic Body Radiotherapy (SBRT) Stereotactic radiosurgery (SRS) is a non-surgical radiation therapy used to treat functional abnormalities and small tumors of the brain. It can deliver preciselytargeted radiation in fewer high-dose treatments than traditional therapy, which can help preserve healthy tissue. When SRS is used to treat body tumors, it’s called stereotactic body radiotherapy (SBRT). Outpatient Infusion Center The Outpatient Infusion Center at Condell Medical Center provides blood transfusions, chemotherapy, antibiotics and other infusion medication injections and central line blood draws. Patients are treated in comfortable and warm surroundings, and our highly trained nurses are oncology certified by the Oncology Nursing Society. The Outpatient Infusion Center is conveniently located adjacent to the main lobby and appointments can be made by calling 847.990.5700 or your nurse navigator can assist you. Inpatient Oncology Unit Located on the second floor of Condell Medical Center, the inpatient unit includes 21private rooms focused on providing 24-hour primary nursing care to patients requiring diagnosis and treatment for cancer, disorders of the blood and medicalsurgical intervention. Each room is equipped to accommodate families who wish to stay overnight; free Wi-Fi, local telephone service and satellite TV are also available. As a multi-disciplinary patient centered team, our mission is to provide the most up to date therapies available for all forms of cancer. Dedicated to building lifelong relationships, the team collaborates with all the available resources of the Advocate Condell Oncology Institute for best patient outcomes. The inpatient oncology unit is a vital part of Condell’s Cancer Institute which earned re-designation from the Commission on Cancer in 2013 with commendations to nursing. Your primary care physician may partner with a specialty physician called a hospitalist to manage your care during your hospitalization. What is a Hospitalist? A hospitalist is a physician who specializes in the care of patients while they are in the hospital. Instead of dividing their time between their offices and the hospital, the physician focuses all of his or her attention on the care of hospitalized patients. How does the Hospitalist Program work? If your primary care physician refers your inpatient care to a hospitalist, the hospitalist will be in charge of your day-to-day care when you are in the hospital. Your hospitalist will talk to you in-depth about the issues that are important for your medical needs. He or she will take a complete medical history and perform a thorough examination. Detailed summaries of your hospitalization, lab and test results, changes and updates will be communicated to your primary care physician. The hospitalists will work closely with your primary care physician, other care providers and medical specialists to ensure that you get the best possible care. What are the benefits of the Hospitalist Program? The physicians associated with our Hospitalist program are physicians who are board-certified in Internal Medicine and Family Practice. They are not medical interns or residents, but highly trained and experienced in acute inpatient care. The Hospitalist Program increases the availability of our physicians within the hospital. Most important, high quality care is available around the clock, seven days a week. They are responsive and can rapidly modify treatment or follow-up on a test result. This hands-on approach not only helps prevent medical complications, but also results in reduced hospital stays. Additional Support Services and Resources Fertility Preservation before Cancer Treatment Egg or Embryo Freezing Advocate Condell fertility specialists are dedicated leaders in the field of reproduction who understand the time constraints and emotional stress of patients facing cancer treatment, as well as the concern over the potential negative impact on future fertility. Expedited, compassionate care is available for egg or embryo freezing prior to cancer treatment. Chemotherapy and radiation can damage eggs and possibly render a woman infertile. Freezing a woman’s eggs (egg vitrification), or embryos created with the sperm of her partner or an anonymous donor (embryo vitrification), allows her the chance to have a genetic child in the future. Who is a candidate for these procedures? Candidates for egg freezing are women up to the age of 40 who do not currently have a male partner or prefer not to use sperm from a partner or donor. Patients must have medical clearance from their oncologists to use injectable fertility medications and to undergo the egg retrieval procedure. Candidates for embryo freezing are women up to the age of 45 who wish to use the sperm of a male partner or anonymous donor to create embryos. These women must also have clearance from their oncologists to undergo the procedure and to use injectable fertility medications. All women going through egg or embryo freezing must be willing to take daily injections and attend frequent office appointments (sometimes daily) for approximately two weeks. Centre Club Fitness Centers and Continuum of Care As you progress towards improved health, a wide variety of programs to help you regain strength and endurance is available. Our two medically based fitness centers offer a wide variety of classes and exercises including certified cancer exercise instructors with a specialty in exercise while undergoing cancer treatment. There are 60-day membership privileges to both of the full service medically based fitness centers along with a 5 punch pass to bring a friend of family member to the program. Advocate Hospice: Making Every Day Count Patients don’t come to hospice, hospice comes to our patients. While 90 percent of hospice services are provided in the home, services can be provided in a longterm facility, retirement community or hospital. Services are customized to fit each patient’s health status and are focused on making the patient as comfortable as possible. American Cancer Society Services and Partnership with Advocate Condell Cancer Resource Center Condell Medical Center partners with the American Cancer Society to provide free services to all cancer patients. Free Wig Fittings Condell offers a wig fitting center, staffed by an experienced professional from the American Cancer Society. Fittings are held at various times every month and appointments may be made by calling HealthAdvisor at 1.800.3.ADVOCATE (1.800.323.8622) or through your nurse navigator. Look Good, Feel Better Look Good, Feel Better is a participative class provided by Tender Loving Care (T.L.C.), a division of the American Cancer Society. Trained volunteer cosmetologists teach women how to cope with skin changes and the use of cosmetics, especially during and after chemotherapy and radiation therapy treatments. Every woman taking part in the class is given a free cosmetic kit. Appointments may be made by calling HealthAdvisor at 1.800.3.ADVOCATE (1.800.323.8622) or through your nurse navigator. Road to Recovery American Cancer Society provides transportation at no cost to and from any oncology appointment. The program is called Road to Recovery and can be reached by calling their number 800.277.2345. They require 3 days’ notice prior to your appointment and will make every effort to find a volunteer driver in your area. Patient Financial Services Billing and Insurance Many employers, insurance companies, HMO’s and PPO’s require notification prior to admission. If you do not comply with this requirement, your medical benefits could be jeopardized or your insurance company could refuse to pay your bill in part or in full. By providing your complete insurance information, including policy number, group number and the correct mailing address and phone number of your insurance company, you will make it possible for us to address the insurance promptly. Coordination of Benefits (COB) Coordination of Benefits (COB) is a term used by insurance companies when you are covered under two or more insurance policies. Most insurance companies have COB provisions that determine who is the primary payer when medical expenses are incurred. This prevents duplicate payments. COB priority must be identified at admission in order to comply with insurance guidelines. Your insurance company may request a completed COB form before paying a claim and every attempt will be made to notify you if this occurs. The hospital cannot provide this information to your insurance company. You must resolve this issue with your insurance carrier in order for the claim to be paid. Your Hospital Bill Your hospital bill is made up of charges for room and board, as well as charges for procedures and other services ordered by your physician and performed by the hospital. Some of the services and procedures you may be billed for include nursing care, radiology, medications, laboratory tests, physical therapy, operating room, delivery room recovery room and intensive care. You will receive separate bill(s) from your physician and other consulting physician(s) for their professional services including radiologists, pathologists, anesthesiologists, and emergency room physicians. Patient Financial Services (continued) Financial Assistance In keeping with our value of compassionate care, Condell Medical Center has a financial assistance program available for qualified patients. If you anticipate problems paying your portion of your bill or you are uninsured, we encourage you to contact one of our financial counselors. Medicare This hospital is an approved Medicare provider. All services billed to Medicare follow federal guidelines and procedures. Medicare has a COB clause. At the time of service you will be asked to answer questions to help determine the primary insurance carrier paying for your visit. This is referred to as a Medicare Secondary Payer (MSP) questionnaire and is required by federal law. Your assistance in providing accurate information will allow us to bill the correct insurance company. Medicare deductibles and co-insurance are normally covered by your secondary insurance depending on the benefits you selected. If you do not have secondary insurance you will be asked to pay these amounts or establish a payment plan. If you are unable to pay these amounts, we will help you determine if you qualify for a state-funded program. One on One Financial Counseling for Cancer Patients One on one financial counseling is available at no charge in the Cancer Resource Center. Appointments may be made by calling HealthAdvisor at 1.800.3.ADVOCATE (1.800.323.8622) or through your nurse navigator. Our financial counselors are available to meet in person, by phone or via e-mail. All communications are handled confidentially and with the utmost respect.
