It is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote quality cost effective outcomes. (CMSA, 2010) How do we help our patients? By improving the delivery of care, enhancing access to care, and achieving optimal outcomes while ensuring a uniform patient experience for all beneficiaries. We help to plan the care for the patient by facilitating communication and education for them and their Family Members, and advocating for more positive outcomes, while also empowering and encouraging them. Does every patient need a Case Manager? No, not usually. Case management is a unique service that is provided to more complex patients and determined on a case by case base, after an initial assessment, but anyone can be referred by any medical provider or can self-refer for initial assessment and determination. Patients should contact their PCM if interested in being self-referred. Where are we located? Each clinic/medical home has an assigned Nurse Case Manager.
