Full Time Nurse Practitioner
This is an exciting full time opportunity for an experienced Nurse Practitioner to work full scope of practice in a
new model of primary care delivery being piloted by the MOHLTC in Muskoka. The Nurse Practitioner will work
in a satellite office of Cottage Country Family Health Team (CCFHT) called a Health Hub. The Health Hub will be
integrated with CCFHT through a shared EMR, and will have access to the FHTs health care disciplines to serve the
community’s needs ‘close to home’.
As the Health Hub Nurse Practitioner, you will demonstrate strong organizational and leadership skills, and as the
main Primary Care provider, you will work independently to deliver personalized care within the full scope of your
practice. Administrative support will be available. The Port Carling Community Health Hub will serve the primary
care needs of both the permanent and semi-permanent populations in that community. To serve these needs the
Nurse Practitioner’s care delivery model will include home visits, service connection to specialists via OTN, and
flexible clinic hours.
Key Qualifications:
 Minimum masters level nurse practitioner preparation, or 3-5 years prior NP experience in a progressive
clinical setting
 Current registration in the Extended Class with College of Nurses of Ontario
 Advanced assessment and diagnostic reasoning skills
 Strong problem solving and negotiation skills
 Must be able to practice independently and interdependently
 Demonstrated leadership to advance the practice of nursing and inter-professional practice to achieve
established goals
 Excellent communication skills and the ability to develop positive relationships with patients, families,
physicians and inter-professional team members
 Demonstrated use of theory and research to enable evidence based practice
 Experience or knowledge of research methodology, program planning, and evaluation
 A strong understanding of rural communities and the determinants of health
 Exceptional performance and attendance required
