Personal Health & Information
Forms
Forms are mandatory for each child registered
in a day camp program and are kept on file
during the course of the summer session.
Forms can be updated at any time by simply
advising the Program Recreationist or the
Summer Programs Coordinators throughout the
course of the summer if your child is attending
more than one program, or even throughout the
week that your child(ren) attends a day camp.
The form is available for pick up in hard copy at
the reception desk at the Peterborough Sport
& Wellness Centre, or at the reception desk at
the Recreation Division, City Hall. It may also
be emailed or faxed upon request to families.
The form may also be downloaded online
and completed and emailed back, or printed
and completed manually. Please submit all
forms to the Program Recreationist or Summer
Programs Coordinators attention one week prior
to the start of your child’s camp program. If you
have any questions regarding the forms, or
would like to discuss information pertaining to
your child(ren) in further detail, please contact
705-742-0050 ext. 2206 for assistance.

Administration of Medications
Camp staff cannot administer prescribed
medications unless an Authorization of
Administration form has been completed.
Forms may be completed upon arrival at
your child’s first day of camp to allow further
discussion with Senior Camp Staff. When
medication is provided to staff, it must be
in its original container and bear a current
prescription label, which will be crossreferenced with the completed form. When a
unit of measurement is required for medication,
please supply a dosage implement with the
medication (i.e. a tsp). The implement must
be labeled with the child’s name. Staff will
not administer over-the-counter medications
unless prescribed by a physician. For selfadministered medications, such as asthma
inhalers and epi-pens, children are required to
bring the medication to camp daily to eliminate
any chance of forgetting these lifesaving items
at home. Please do not hesitate to contact the
Program Recreationist or the Senior Camp
Staff in advance of, or during the program, to
review your child’s needs.
Extended Care Service for
PSWC Summer Day Camp
Programs
Extended Care Service provides additional
care at the beginning and/ or the end of
each camp day to better accommodate your
family’s schedule. This service is available
for the majority of our day camp programs
and operates from 8:00 to 9:00 am and from
4:00 to 5:30 pm This service is included in
your camp registration fees. Parents may
choose to utilize the morning or afternoon or
both for the week.
Drop-Off and Pick-Up
Procedure
Upon drop-off in the mornings, staff require
parents to check-in with camp attendance staff
daily in person with their child(ren). Staff take
attendance and direct the child(ren) to their
appropriate group and counsellors. At this
time, any special instructions or information
may be exchanged and documented. At
pick-up time in the afternoons, staff also
require parents to check-in with sign-out staff
in person daily to collect their children. At this
time, parents and counsellors are provided
with the opportunity to discuss the day. If you
are picking up prior to the normal pick-up
time at the end of the day, please report to the
Summer Programs Coordinators to sign your
child out, receive a sign-out slip and receive
directions for your child’s location. If an adult
other than those indicated on your personal
health and information form requests to pick
up your child, he/she will not be released to
the individual without contact from the parent.
Please ensure that all information is provided
to staff in advance via verbal or written
communication. If your child would like to
walk/bike/utilize public or taxi transportation
to travel to and from camp, detailed written
permission and instructions are required from
the parent prior to the start of the program.
