Employee Health Fair
Make plans to join us at the annual Conroe ISD Employee Health Fair on Saturday,
January 9, 2016, from 9 a.m. to 12 p.m.! This is a free family event with something
for everyone. Receive information, giveaways, and product samples from more than
70 health and wellness organizations in our community. All employees are eligible for
door prizes and free onsite chair massages. Members of the Conroe ISD Aetna medical
plan can get a free flu shot ($25 for non-Aetna members) and a free biometric wellness
screening (employees and spouses only). We are also pleased to provide onsite screening
mammogram services by three different providers. Please note appointments for
biometric wellness screenings and mammograms should be made in advance. Employees
will need to bring their Conroe ISD ID badges to the fair in order to collect their entry

New IRS Form 1095-C
Conroe ISD is committed to providing you and your family with affordable, substantial
health benefits that meet the requirements of “minimum essential coverage” under the
Affordable Care Act (ACA). In January, Form 1095-C will be provided to you through the
Employee Access Center as proof of the health coverage we offer you and your dependents.
It contains information about who provides the health insurance, as well as other information
about the health coverage that was offered to you. It also lists the lowest monthly
premium that you could have elected for the self-only health benefits offered to you.
If you did not elect to receive Form 1095-C electronically during your most recent
enrollment period, then a paper copy will also be mailed to you. To help ensure delivery,
make certain your current address is listed in the Employee Access Center. Use the “update”
button to make any changes by January 4, 2016.
You can use Form 1095-C to help you report your insurance coverage when filing your tax
return. Only one form will be provided by Conroe ISD for all the individuals listed on your
policy, so you may need to provide copies to your spouse or dependents, as necessary.

Flexible Spending Account Online Services
First Financial Administrators began offering an improved online experience for managing
flexible spending accounts in September. The enhanced portal at www.ffga.com allows
you to receive email alerts about your account activity, upload receipts and documentation,
submit claims electronically, sign up for text alerts, and much more. You can also manage
your account from the new FF Flex Mobile App for AndroidTM and Apple® devices.* If you have

Walk-In Clinics and Urgent Care Centers
Medical emergencies happen, but not every health need is a medical emergency. If you need
care for minor illnesses like strep throat, seasonal allergies, and ear or eye infections, and
you cannot get in to see your primary care physician, you may want to seek care from a
walk-in clinic as an affordable alternative. If your situation is more than minor, such as
a sprain, fracture, or other urgent injury, you may want to visit an urgent care center.
Always use your best judgment when seeking treatment for you and your family. If you
have chest pain, trouble breathing, extensive bleeding, or other symptoms you believe
put your life at risk, you should go to the emergency room (ER).
