Athlete’sRelease:Athlete's Release: I know that I am running, walking, and /or volunteering in a road race that is potentially
hazardous. I realize that I should not participate in the Forget Me Not 5k unless I am medically able and properly trained. I
agree to abide by any decision of a race official relative to my ability to safely complete the run/walk. I assume all risks
associated with running, walking, or volunteering in the Forget Me Not 5K including but not limited to falls, contact with other
participants, the effects of weather, and the condition of the road and traffic on the course. Having read this waiver and
knowing these facts, and in consideration of your acceptance of my application, I for myself, and anyone entitled to act on my
behalf, waive and release Forget Me Not 5K, Knox County, the City of Knoxville, University of Tennessee, and all further
sponsors, their representatives and successors from all claims or liabilities of any kind arising from my participation in the
aforementioned event even though that liability may arise out of negligence on the part of the persons named in this waiver. I
also grant the foregoing to use any photograph, motion pictures, recordings, or record of my participation in the event for any
purpose they deem appropriate.
