Depressed chronic kidney disease patients more

DALLAS — Patients with chronic kidney disease who have been diagnosed
with depression are twice as likely to be
hospitalized, progress to long-term dialysis treatments or die within a year as
those who are not depressed, UT Southwestern Medical Center researchers
have found.
In the study, appearing in the May 19
issue of the Journal of the American
Medical Association, researchers monitored for one year 267 patients with
chronic kidney disease - 56 of them with
a diagnosis of a current major depressive
episode, referred to here as depression,
based on the Diagnostic and Statistical
Manual of Mental Disorders 4th edition
(DSM-IV).
Nearly 61 percent of patients with depression compared to 44 percent without
depression either died, progressed to
long-term dialysis, or were hospitalized
within a year of observation; 55 percent
of depressed patients were hospitalized
compared to 40 percent of patients who
were not depressed; 27 percent of depressed patients needed to start regular
dialysis treatments compared to 11 percent without depression; and 9 percent of
depressed individuals died compared to
6 percent without depression.
“Chronic kidney disease patients with
depression have poorer health outcomes
than those without depression, even after
adjusting for other factors that determine
poor outcomes in these patients, such as
other medical diseases, anemia and low
albumin levels,” said Dr. Susan
Hedayati, assistant professor of internal
medicine at UT Southwestern, staff
nephrologist at the Dallas VA Medical
Center and lead author of the study. “Clinicians should consider screening
chronic kidney disease patients for depression, especially since depression is
also associated with poor quality of life.”
Twenty-six million people in America
have chronic kidney disease and millions
more are at an increased risk, according
to the National Kidney Foundation. If
treatment does not begin early, the condition progresses to end-stage renal disease. At that point, a patient’s kidneys
have failed to the point where dialysis - a
filtering of toxic chemicals in the blood
and removal of fluid to help control
blood pressure - or a kidney transplant is
needed.
In 2007 an estimated $24 billion, about
6 percent of the entire Medicare budget,
was spent on dialysis-related health care
in patients with chronic kidney disease,
according to the U.S. Renal Data System
2009 Annual Report.
This JAMA study is the first to look at
outcomes as they relate to an association
between chronic kidney disease and depression in patients prior to starting regular dialysis. In previous reports, Dr.
Hedayati has found that one in five
chronic kidney disease patients is depressed before beginning long-term dialysis therapy and that patients already on
dialysis who have been diagnosed with
depression are nearly twice as likely to
be hospitalized or die within a year than
those who are not depressed.
For the current study from May 2005 to
November 2006, researchers invited patients at the Dallas VA Medical Center
who were visiting the clinic for chronic
kidney disease appointments to join the
study.
Patients who agreed to participate then
underwent a structured clinical interview
to determine if they had a current major
depressive episode, based on the
DSM-IV definition which is considered
the gold-standard in evaluating depression.
Fifty-six patients were found to be de-
pressed. The mean age of patients was
about 65; two were women; and nearly
56 percent were white. All patients were
veterans.
The association between chronic kidney disease, depression and poorer
health outcomes held after adjusting for
age, race and other current medical conditions.
“Our results support the need for
well-designed clinical trials to investigate if antidepressant treatment is efficacious and safe in this vulnerable
population. Patients with advanced kidney disease have been previously excluded from larger randomized trials of
antidepressant medication treatment,”
said Dr. Hedayati, who is now conducting the Chronic Kidney Disease Antidepressant Sertraline Trial (CAST) to
determine whether antidepressant medication would be tolerated in kidney-disease patients and whether such
treatments can improve depression and
quality of life.
Other UT Southwestern researchers
participating in this study were Dr. Abu
Minhajuddin, assistant professor of
clinical sciences; Dr. Masoud Afshar, a
postdoctoral fellow in nephrology; Dr.
Robert Toto, professor of internal medicine and clinical sciences; and Dr.
Madhukar Trivedi, professor of psychiatry.

Research Core Center, the National Institutes of Health and the Department of
Veterans Affairs.

Fort Worth detective Jose Hernandez
was named Officer of the Year for 2009
at a special VIP dinner on Tuesday, May
18, 2010. The dinner was hosted by the
Fort Worth Police Officers’Award Foundation and was held at the City Club of
Fort Worth.

Detective Jose Hernandez has been a
police officer with the City of Fort Worth
for 22 years. His arrest and clearance
rate on murder cases is at the top of his
unit. Because of his many achievements,
Detective Hernandez was chosen to fill a
Homicide Cold Case Unit position sponsored by a Department of Justice federal
grant entitled “Solving Cold Cases with
DNA.” He had weekly meetings with
Crime Lab personnel and soon had 40
murder cases targeted for evidence pro-
cessing. He worked to become educated
in DNA and the biological sciences and
kept constantly updated in new techniques, increasing his ability to recognize and submit DNA evidence.
On September 22, 1986, a Fort Worth
housewife was bound with duct tape,
sexually assaulted, and shot in the head
in the master bedroom of her residence
while her small children played in another part of the house. Detective
Hernandez reviewed the case and observed that latent fingerprints had been
taken from the duct tape wrapped around
the head of the victim. Over the course of
several years, the fingerprints had been
submitted into the AFIS system with no
identifications made. Then, Detective
Hernandez hand carried the fingerprints
to the Texas Department of Public Safety
in Austin for another attempt to enter
them into AFIS. A suspect was identified. The extra effort taken by Detective
Hernandez resulted in an arrest in a case
that already had been investigated by
many detectives.
Detective Hernandez also re-opened a
1978 case involving a 17-year-old female who was strangled and placed in the
trunk of her car and a 1984 case involving an 18-year-old female who was
found in her apartment stabbed multiple
times. These cases have developed suspects and biological evidence linking the
suspects to the crimes. During his time
assigned to the cold case DNA grant, Detective Hernandez also took the existing
FWPD cold case website, re-organized it
and added numerous victims, photographs, case summaries, wanted persons, and successful Fort Worth cold
case arrests. His involvement and many
hours working on the website resulted in
it being one of the most comprehensive
in the country.
Because of his outstanding work ethic,
Detective Hernandez’s work on the projects throughout 2009 will have an effect
on unsolved homicide cases for years to
come.
on proposed TRE fare increase
FORT WORTH — The T will make a
presentation and hear public comments
on proposed fare increases for the Trinity
Railway Express at three public meetings on Wed. May 26, at 7 a.m., noon and
6 p.m. at the Intermodal Transportation
Center at Jones & 9th Streets (MAPSCO
77A).
The proposed fares would be the second phase of a two-part increase. The
first phase occurred in October 2009.
The second part would increase the regional day pass from $7.50 to $10 in October of this year. TRE weekly, monthly
and annual passes also would increase.
Reduced TRE fares would not go up under the proposed new fare structure, and
The T’s fares for local bus, local reduced
and MITS would not be affected.
The T will provide comments from the
public meeting to its board prior to its
June 16 meeting when it will present the
proposed new fares for board action.
“The TRE has been operating consid-
erably below the fare level of other commuter railroads for many years, and this
increase would bring it closer to the national average for commuter railroads,”
said T President Dick Ruddell.
Ruddell explained that the board of
Dallas Area Rapid Transit (DART), with
whom it co-owns and operates TRE, had
approved both segments of the
two-phase increase in May 2009. The T
had postponed consideration of the second phase in case it could have been
postponed.
“As a more expensive operation, The
TRE continues to require a greater share
of The T’s operating revenues than its
bus system,” said Ruddell. “TRE fares
were set lower than average for passenger rail in order to introduce commuter
rail service to our area, but with the decline in our sales tax revenue, it is necessary that The T consider this phase of the
increase,” he said.
Ruddell also pointed out that keeping
TRE fares the same as DART’s to keep
travel seamless across the train corridor
is an important consideration.
The T’s sales tax revenue is down
more than $2 million year-to-date and
similar funding short-falls are facing
transit agencies across the country.
More than 95 percent of U.S. transit
agencies have had to increase fares, reduce service or both within the past year,
according to a survey by the American
Public Transportation Association
(APTA).
