We Believe Our #1 Mission1 to help individuals access and navigate current complex support systems to get the same excellent supports already available to many. navCare will create 2 common-copyright learning resources (Our Goal) (1) generic learning resources applicable anywhere, and (2) templates for provincial-territorial, regional and local resources that will help people learn 3 practical skills (1) to navigate service and support systems (2) to negotiate the best services and supports possible in specific circumstances and (3) to mediate problems as they arise through 4 entry points (1) technology (online, telephone, media) (2) existing personal communities (e.g., families & friends, cultural, faith, geographic) (3) new communities (e.g., support and community groups, condition specific organizations, alternative support groups) (4) community, health, legal and social services and institutions. using their own (of 5) learning style preferences (1) self-directed learning, (4) coaching, and (2) self-instructional learning, (5) person-to-person health care advisor (3) instructed learning, in a 3-phase project (Phase 1) research, draft, revise content in generic and template formats and launch website (12-18 months) (Phase 2) use, evaluate and revise content for 4 entry points and within 5 learning style methods (24 months) (Phase 3) final revision and evaluation of collaborative project with reports to all funders (6-12 months). (After Phase 3) the common copyright material will be universally available to individuals and organizations and they can customize and adapt the material for their own uses and share their improvements with others. The navCare Textbook [Audience: advisors, professionals, volunteers, patients, family caregivers] Core training resource materials for people who want to learn more in-depth navigation, negotiation and mediation skills. Can be used for staff and volunteer training, college or university courses. 3. ThenavCare Hospital and Homecare Notebook [Audience: family caregivers, advisors] Notebook to keep track of hospital visits as well as discharge planning and homecare services. 4. The navCare Developing a Comprehensive Personal Profile [Audience: family caregivers, advisors professionals, volunteers] To help family caregivers and advisors provide specific and practical information on how to best support a person at risk. 5. The navCare Personal and Family Information Binder [Audience: patients and family caregivers] Binder to keep all your personal and family information up to date. 6. The navCare Systems Information Binder [Audience: patients, family caregivers, advisors, professionals, volunteers] Personal file system to keep track of information collected on the various systems. 7. The navCare Templates Binder [Audience: advisors, professionals, volunteers] Aid to help organizations customize the navCare Series for their own use. For more information contact: Harry van Bommel 416.264.4665 or [email protected] navCare Harry van Bommel Biographical Sketch Harry van Bommel is the author of over 50 books in the fields of home and hospice care, family caregiving, management and staff development, adult learning, and writing family histories. He is an international speaker primarily in the fields of family caregiving, navigating systems and celebrating Canada’s 150th birthday. He is the Executive Director of the non-profit organization, Legacies Inc. and President of PSD Consultants. Harry is the cofounder of the navCare Collaborative Project to help individuals, families, professionals and volunteers better navigate health care and other systems, negotiate the best care and services possible and mediate problems as they arise. He is also the founder of the Canada 150: Canada’s Untold Stories project to encourage Canadians to record their life stories as their gift to Canada on its 150th birthday in 2017. Harry has a Masters of Adult Education Degree from St. Francis Xavier University in Antigonish, Nova Scotia. Harry began his work in hospice care 35 years ago when he cared for his parents and grandfather at home until they died. His writings, speaking and recorded songs all come from a patient-family perspective. She launched the Solo Traveler project two years after her husband passed away from a rare neurological disorder and one year after co-founding navCare with Harry van Bommel. Janice’s background as a birth doula and frustration navigating the health care system for her late husband prompted a discussion with Harry and their search for partners in an as yet unknown project that turned into navCare. Janice has a Masters Degree in History from the University of Toronto and a long entrepreneurial career.
