(if you didn't register your child last year) For all new registrations, we invite you to create your family file online no later than April 12, 2016. Please note that the health form and parent authorizations must be completed for each member of your family in order to receive confirmation of your registrations, which will take place starting at noon on April 13. What you need in creating your family file • Avantages Laval card for each participant* • The payer's social insurance number (SIN) (for Relevé 24 income tax slip) • Each participant's health insurance number • Valid email address for registration confirmation and receipt * With proof of residence and one piece of identification, you may obtain a free Avantages Laval card at municipal libraries, BMLs (bureaux municipaux lavallois) or the Multiservice Counter. In this way, you will be able to complete your online registration quickly and more easily. Method of payment You may make your payment online. To do so, you must have a credit card (VISA or MasterCard). If you don't or if you wish to pay later, just select the "pay later" option when registering online. You will then have 3 working days to make your payment in person at 1333 boulevard Chomedey, suite 104, Monday – Friday, 9 a.m. – 8 p.m. After that deadline, your registration will be automatically cancelled. Through its Family Policy, Ville de Laval wishes to make it easier for Laval families to take part in leisure activities. Registered residents will therefore have the option of paying half the amount at time of registration and the other half on June 10. Family discount (for day camps and Campgourou only) A 10% discount is offered for the 2nd registration. For the 3rd registration or more, the discount is 50%. These discounts apply to children in the same family, registered for the same weeks. CANCELLATIONS, REFUNDS AND CHANGES • All requests for cancellations or changes will have to be made in writing and be sent to 1333 boulevard Chomedey, Case postale 422, Succursale Saint-Martin, Laval (Québec), H7V 3Z4 or by clicking on "Contact Us" at the top right-hand side of the Home page on Ville de Laval's Internet site and informing us accordingly, at least 5 working days before the start of the activity. • If Ville de Laval cancels the activity, the registration amount will be fully refunded. • For all other cancellations, the City will charge $15 per week (day camps) or $10 per season (tennis) in order to cover administration costs related to the planned, reserved activities. • No refunds will be granted once the activities have begun. In joining the Cadre de reference pour les camps de jour municipaux of the Association des camps du Québec (ACQ), Ville de Laval is focusing on the quality of children's experience and their safety. To ensure the campers are well supervised, each group of 15 children has its own camp counsellor. All counsellors are trained in leadership skills and first aid. The courses are given by Ville de Laval, in cooperation with its partners. Once again this year, all camps are using the Shape Up program as a springboard to healthy habits. This program offers loads of ideas and tips to get children to be more active and encourage them to eat healthy. Developing good habits is fun, too! Visit www.goshapeup.ca/ to inspire you. For all new applications, we therefore encourage you to create your online family file and register your child on the 2016 waiting list starting March 23, 2016. You will have to pay the counsellor in cash for these charges. ? 450 662-5000 The Camper's Kit EVERY DAY, your child's clothing and shoes must be suitable for taking part in sports and recreational activities. Campers must bring: • a swimsuit • a towel • a cap • sunscreen • a bottle of water • lunch • two snacks PLEASE NOTE: Children who don't know how to swim must bring a life jacket approved by Transport Canada. Please read! • Consult the parents' guide (available on Ville de Laval's Internet site): It provides full information so that your child can enjoy a very pleasant experience at camp. • To find out the activities planned, take a look at the weekly schedule, which you will receive on the Friday before your child's week of day camp. Important note: In order to ensure that the activities are carried out smoothly, the Administration will inform the parents or guardian if the child disturbs the group in an unacceptable way. In such cases, the child will be refused access to the service (regular day camp, Campgourou or Integration programs). Registration: www.formation-animation.qc.ca Integration program & Campgourou Integration program (locations indicated on page 8) The Integration program is offered to 6-12-year-olds with an intellectual or slight physical disability or autism spectrum disorder. The youngster must be able to follow the regular day camp group's activities and travels, with the help of a companion. Campgourou JUNE 27 – AUGUST 19 Campgourou is a specialized day camp for disabled 6-17-year-olds and requires special logistics for the municipality. The registration period runs from February to mid-March in order to be able to carefully assess the applications, the youngsters' needs and the necessary resources. Each youngster is welcomed and placed in the care of trained staff that is aware of the disabled person's needs. Campgourou provides the disabled with a safe service in an environment where each one can grow and develop while having fun. Parents wanting to use adapted transit services although their youngster has not yet applied for it will have to submit a duly completed application to the Service du transport adapté de la Société de transport de Laval (STL) no later than April 29, 2016. Information: www.stl.laval.qc.ca or 450 662-8356 Please note: Each youngster's needs are assessed in order to be able to properly integrate that camper and ensure that the youngster enjoys the best possible, positive experience. If a child displays any actions that are considered dangerous, showing that this camper is unable to integrate the regular activities of his or her group, the Administration will inform the camper's parents. Similarly, if it comes to the conclusion that the youngster's needs and abilities are beyond the services and resources set up by the City and the systems in place to accommodate the camper, the Administration may be obliged to refuse that youngster's access to Campgourou or the Integration program. The number of tickets is limited. Tickets1 ($25) will be on sale at the Parents' Meeting on Wednesday, June 15. The player can get his opponent to run for the ball or can return easy balls with more power.
