New Collaborations Benefit MHC Patients Through our recent collaborations, Mercy Health Clinic is able to provide patients with access to the wonderful services and benefits of two great non-profit organizations in our neighborhood. Montgomery Hospice is a nonprofit serving residents of Montgomery County. Their team of doctors, nurse practitioners, nurses, spiritual counselors, social workers, certified nursing assistants, bereavement counselors and volunteers works with patients and their families to provide quality end-of-life care in homes, extended care facilities and at Casey House, their acute care facility. They also provide support to the family for a year after the death of their loved one. Through our collaboration with Montgomery Hospice, we refer patients with terminal diagnoses to receive end of life care. Pain Connection is a non-profit organization dedicated to empower people with chronic pain to improve their quality of life, their sense of isolation and take a more active role in their treatment. Through our collaboration with the Pain Connection, we refer our patients who suffer from chronic pain to receive care and support. They do this by providing support groups, specialized classes, a speaker series on treatment modalities, training and supervision of mental health professionals on chronic pain, a newsletter on chronic pain issues, information and referrals, and community outreach and education. 1 WE THANK OUR ADVERTISERS!
