Disclaimer
My son has permission to attend the New Berlin
West Football Camp. I certify that within the past
two years he has had a physical examination and that
he is now physically able to participate in football
camp activities without restriction. In the event of
illness or injury, I give my consent for medical
treatment and permission to the attending physician
to hospitalize, secure proper treatment and order
injections, anesthesia or surgery. I will be responsible
for any medical or other charges in connection with
my son’s attendance at camp.
I acknowledge that, by my son’s participation in the
New Berlin West Football Camp, he will be involved
in, among other things, physical contact of the body
with other persons or objects, including the ground.
I understand that my son, while in attendance at the
New Berlin West Football Camp, may incur the risk
of injury. I specifically release the New Berlin West
Football Camp and staff from all liability for a claim
for damages which my son or I may have for injuries
or illness that he may sustain at camp.

Welcome to our 2015 Viking Football AS ONE Summer Football Camps.
Our camps are designed to offer a positive learning experience and will play a key part in developing the skills necessary to becoming a quality football
player.
All camps will be run by our High School Football Staff, and will stress character, proper attitudes, teamwork, and skills development. These are the key
ingredients to your success as a football player.
We are excited to continue to build the winning tradition inside the Viking Program.
