CEO Message Kim Hosking Volume 2 Issue 3 March 2016 One of the expectations of the Commonwealth for us, is the commissioning of Commonwealth funded programs and we are well advanced on an ‘approach to the market’ for mental health services. CSAPHN will be in receipt of funds that have in the past been dedicated to the ATAPS program. We are also now holding funds that relate to the Mental Health Nurse Incentive Program. In recent weeks we have had considerable engagement with SA Health principles in mental health, with a view to how we can better integrate state and Commonwealth funded activities to improve access and quality of services. These funds are now included in a mental health flexible fund, for us to apply locally and allowing us to tailor the activities to our various local community and regional needs. Our approach to the market will provide opportunity for existing and new potential providers to progress mental health support across the region. Our expectation is; the use of learnings from past service provision, a need for continuing quality of service and innovation to potentially improve and expand access to mental health services and ensure equity of opportunity for people to access the services across our entire area. Keep an eye on our website www.countrysaphn.com.au for further news, as the commissioning work progresses. Some Commonwealth funds for Alcohol and Drug support activities will also be directed to the PHN in the 2016/17 financial year. Guidelines for PHN use of these funds are under development by the Commonwealth and now in consultation with the PHNs. We are now embarking on a lengthy process of community and provider engagement (refer to page 4) to ensure we can allocate these funds to best affect. The views and needs of local communities are critical in this activity. There is also change on the way for Close the Gap activities, with the Commonwealth forecasting changes, that at this early stage seem to integrate activity and enhance the capacity of the PHN to support Aboriginal communities and ACCHOs. Another key expected activity of the PHN has been to undertake a comprehensive needs analysis of our region. This may seem like an exercise in bureaucracy, but the reality is that it is a much needed activity that provides a real analysis of what is happening in primary health in our region. Not only is it a good base for the PHN to consider and potentially advocate for services and community needs, but it also information that others can access to help their own advocacy for local and community needs. Sadly even powerful anecdote does not ‘cut the mustard’ in modern advocacy, but real time quality data does and is very hard to argue with. We have a very strong team that is providing good quality analysis of our region and we are using same in our relationship activities widely. This team is our local providers of support to communities, general practice and community allied health. Suzanne Mann has been appointed the new manager for this team and the team is undergoing changes that will see new local support provided in areas we see to be under supported, such as the mid north and south east. This team supports our ‘Cluster’ activities aimed at building local capacity and resilience in health services provision in small communities and Suzanne and her team are seeking to invigorate existing ‘Clusters’ and create new ones in other areas. Finally, with election bells starting to sound, we are assured that very soon, there will be a number of announcements to be made that will relate to the various Commonwealth level inquiries into aspects of health. Far West Women's Council Country SA PHN has recently signed a service agreement with the Far West Aboriginal Women’s Council based in Ceduna. General practice research grant and award opportunities. The ‘Pathways to Healing’ project is aimed at implementation of a community specific response and support model that address episodic mental health occurrences in the after-hours period. The RACGP Foundation is pleased to announce the opening of the 2016 research grants and awards round. The project will build capacity within individuals, families and the community to respond to mental health needs through the delivery of Aboriginal mental health first aid training and education in the Ceduna, Yalata and Oak Valley communities. The RACGP is committed to supporting general practitioners conducting research into primary healthcare and general practice. These research grants acknowledge the vital contribution general practitioners make in furthering medical advancements within the community by building the evidence that is necessary to deliver the highest quality care and best health outcomes to all Australians. The General Practice Mental Health Standards Collaboration (GPMHSC) Mental health training for GPs The GPMHSC is responsible for establishing standards of mental health education and training for GPs and formally accrediting GP mental health training programs. Completion of accredited GP mental health training provides GPs with access to higher schedule and additional MBS item numbers for mental health consultations. The GPMHSC is responsible for reporting GPs who have completed Mental Health Skills Training (MHST) and/or Focussed Psychological Strategies Skills Training (FPS ST) to Medicare Australia in order for GPs to access the relevant MBS item numbers. Healthdirect Australia has extended the National Health Services Directory (NHSD) database and redeveloped the Management Interface (MI) to better support national e-health reforms and standards, to make it easier for service providers to update their information and to meet security and privacy legislative requirements. The NHSD now supports the National Endpoint Proxy Service (NEPS), which enables the data captured in the directory to be used to facilitate Secure Messaging Delivery and support the My Health Record system. For more information go to http://www.healthdirect.gov.au/ national-health-services-directory. 2 Nursing and Midwifery Board of Australia The Nursing and Midwifery Board of Australia (NMBA) is releasing the revised registration standards for continuing professional development (CPD). A Gift for Living – National Bowel Cancer Screening Program 2016 campaign The National Bowel Cancer Screening Program saves lives but it can only work with the support of Australia’s health professionals. All nurses and midwives must meet the continuing CPD standards. This standard sets out the minimum requirements for CPD. CPD must be directly relevant to the nurse or midwife’s context of practice. The second phase of the National Bowel Cancer Screening Program (the Program) campaign is launched on 20 March 2016, with the goal of increasing participation in the Program. This will include print, digital and radio media as well as targeted communications for Aboriginal and Torres Strait Islander and culturally and linguistically diverse audiences. The revised registration standards come into effect on 1 June 2016 and will replace the registration standards that are currently in place. All nurses and midwives will need to meet the obligations of the revised registration standards by the next registration renewal period in May 2017. They do not apply to renewals of registration in 2016. Bowel cancer is the second biggest cancer killer in Australia, with 80 Australians dying each week from this disease. But, if detected early, up to 90% of cases can be successfully treated. Key changes: Clarity has been provided about the requirement for nurses and midwives who are registered but not working, to complete CPD. Despite the evidence, participation in the Program is still low at 36%. Endorsement of bowel screening from a GP or other trusted health professional can significantly improve participation and prevent more deaths from bowel cancer in Australia. The Commonwealth Department of Health is working with the Royal Australian College of General Practitioners, the Australian College of Rural and Remote Medicine and the Australian Primary Health Care Nurses Association to develop a range of strategies to better engage primary health professionals in the Program. The requirement for midwives with an endorsement for scheduled medicines to complete an additional 20 hours of CPD has been revised to 10 hours of additional CPD, related to prescribing and ordering of diagnostics. Women Want to Know Women Want to Know is a national campaign funded by the Commonwealth to encourage health professionals to discuss alcohol consumption during pregnancy with pregnant women or women planning pregnancy, highlighting the change to the NHMRC guidelines in 2009. More information on the campaign, including posters and flyers, can be found at www.australia.gov.au/ bowelscreening. Translated campaign materials are available in Arabic, Simplified Chinese, Traditional Chinese, Greek, Italian and Vietnamese. Materials have also been adapted for Aboriginal and Torres Strait Islander audiences and are available in the Alyawarra, Arrernte, Luritja, Pitjantjatjara, TSI Creole and Warlpiri languages. Women Want to Know was developed by Foundation for Alcohol Research and Education (FARE) in collaboration with leading health professional bodies including the Royal Australian College of General Practitioners (RACGP), the Royal Australian and New Zealand College of Obstetricians and Gynaecologists (RANZCOG), the Australian College of Midwives (ACM), the Australian Medical Association (AMA), the former Australian Medicare Locals Alliance (AMLA) and Maternity Choices (formerly Maternity Coalition). To download the handbook click on the publication below or go to http://www.racgp.org.au/education/gpmhsc/ training/. 3 A handbook for GPs A handbook for training providers 4 Country SA PHN in translation… Cardiovascular Disease From Country SA PHN current Baseline Needs Assessment regional health profiling, we can confirm that Cardiovascular disease (CVD) has a major impact on country people, with markedly increasing higher rates of disease prevalence the further you get from the metropolitan region. This is not new news but does require action. Country SA PHN is working with our partners in Country Health SA LHN’s iCCnet unit and the Wardliparingga unit in SAHMRI to improve access to care and appropriate care strategies across the region. We are also pleased to have accepted a membership to the South Australian Academic Health Science and Translation Centre Cardiac Rehabilitation and Secondary Prevention working group. Funding Open for Application National Disability Conference Initiative 2016-17 The Australian Government is inviting providers to apply in an open process to deliver services under the Disability, Mental Health and Carers Programme – Disability and Carer Support - National Disability Conference Initiative (NDCI) 2016-17. Total funding of $315,000 is available for NDCI 2016-17 conferences held between 1 July 2016 and 30 June 2017. Within this funding, up to $10,000 is available for each individual conference. Improving Health Care Access to older Australians’ living in Residential Aged Care Facilities Access to health care for older Australians is a National PHN priority and continues to be an important part of our work flowing on from the previous Medicare Local space. In 2014 the Silver Connections pilot project was established under the Commonwealth Better Health Care Connections program with the aim to inform the Commonwealth on the use of video consultations by GPs to Residential Aged Care Facilities (RACFs) as a viable and clinically sound business case for an MBS item. The project, which involves four general practices and three RACFs in the Barossa, has demonstrated that timely consultations between a resident in an aged care facility and GP via video, during or immediately following an illness event reduces the number of avoidable hospitalisations for residents in participating RACFs. GPs have also noted improvement in clinical safety and decision making using video consultations as opposed to a phone conversation with the registered nurse. Credentialed Mental Health Nurse The Angaston Medical Centre (AMC) is a modern 14 Doctor practice in a tranquil county setting, located in the heart of Australia’s premier wine country, the Barossa Valley. We are looking for an energetic and caring Mental Health Nurse who can provide additional skills to our existing mental health services. You would be working closely with our GPs, other allied health professionals and psychiatrists to facilitate care and treatment, establish therapeutic relationships with patients, liaise closely with family and carers, medication management, patient assessment, case planning, clinical briefings and to assist the GPs with the development of care plans. Our Mental Health systems are well established and you would receive plenty of peer support. Country SA PHN is now in the process of expanding the Silver Connections project into After Hours. The GP will have the ability to undertake a supported assessment and provide approval for drug alterations or other treatment where appropriate or make recommendation for transfer of the resident to an acute facility, all via video consult from another location in the after hours period. Where RACFs have imprest drug stocks available, treatment is able to be initiated sooner and prevent a further decline in health of the resident. At this point in time, the Silver Connections After Hours project will run for approximately 18 months. Rural GP - Kangaroo Island Medical Practice We are looking for a VR GP with emergency medicine skills to join our team at the Kangaroo Island (KI) Medical Clinic. The clinic is currently staffed by 5 GPs (who also provide emergency on-call services to the KI Health Service), supported by 2 practice nurses, a practice manager and administration team, all with a commitment to providing high quality care to our patients. Our Service Design team have been building a strong evaluation process into the Silver Connections After Hours project, including ethics approval. Suicide is a time of immense crisis and the tragedy of suicide loss can create trauma – deeply affecting individuals, families and entire communities. From 1 April 2016, all children aged 18 months are scheduled to receive a booster Diphtheria Tetanus and Pertussis (DTPa) vaccine as part of the National Immunisation Program (NIP) schedule. This vaccine is recommended to be given at the same visit as the 18 month MMRV. All children born from 1 October 2014 will be considered due to receive the 18 month DTPa vaccine from April 1 2016 The 4 year old vaccines should now be administered from 4 year of age and not from 3 ½ as previously recommended. Appointment Reminder Translation Tool NSW Refugee Health Service's on-line Appointment Reminder Translation Tool allows you to translate appointment details into your client's language. The Translation Tool allows the practitioner (and/or admin staff) to produce the details of the patient's next appointment in the patient's own language - including a list of what to bring to their next appointment (Medicare cards, relevant x-rays etc.). It can be given to the patient at the surgery, sent, or emailed to them as a pdf. The Tool is available in 30 languages and is very easy to use. Clinical practice guidelines and principles of care for people with dementia in Australia Indigenous Eye Health Unit (IEHU) The Indigenous Eye Health Unit (IEHU) has produced some very useful resource kits about the importance of eye checks for people with diabetes. To download the standards click here or go to www.anmf.org.au. 6 Practice Toolbox Chronic Hepatitis C Pathway Campaign to raise awareness of coeliac disease As from Tuesday 1 March 2016, GPs can prescribe four newly PBS subsided direct-acting antiviral agents (DAAs) for their patients with hepatitis C. These DAAs- sofosbuvir (Sovaldi), ledipasvir/sofosbuvir (Harvoni), daclatasvir (Daklinza) and ribavirin (Ibavyr) can successfully cure hepatitis C in over 90% of cases. Treatment can take as little as 8-12 weeks, without many of the serious side-effects associated with older, interferon based regimens. Coeliac Australia has developed new resources for general practitioners as part of its campaign to help increase the awareness and diagnosis of coeliac disease in Australia. The Consider Coeliac Disease campaign encourages GPs to think of coeliac disease if patients present with at-risk clinical features.
