Fourth Street Ottawa, OH 45875 419-523-4121 Volume 38, Issue 3 3rd Quarter 2016 New Program Starting at the Putnam County Council on Aging S.A.F.E. Homes: Securing Safe Aging in Elderly Homes The Putnam County Council on Aging is starting a new program called S.A.F.E. Homes. The program is a home modification program that offers home safety evaluations, education, environmental modifications and adaptive equipment to older adults who are at risk for falling. The home safety evaluations are offered in partnership with a Certified Aging in Place Specialist, Mary Maas. The focus of the home assessment will be on safety, accessibility and fall prevention. What Does It Mean to Age in Place? Aging in Place simply means to be able to remain in your home as you grow older and your needs and your abilities change. Staying in your home may mean making home modifications that can help you live more comfortably and safely. Who is Eligible for the S.A.F.E. Homes Program? Putnam County residents, age 60 and better, who are at risk for falling. There is no cost for the program, but donations are graciously accepted to help support the program. How Does the S.A.F.E. Homes Program Work? Step 1: Contact the Council on Aging at 419-523-4121 to sign up for the program and to complete a fall efficacy scale. Don’t delay because there are only 30 spots available for the program. Step 2: A basic home safety evaluation will be completed by the Certified Aging in Place Specialist. The home safety evaluation will consist of an assessment for the entrance, kitchen and bath areas. The checklist will include items such as: Is walkway free of debris? Are handrails in place and secure? Is there proper lighting indoors and outdoors? Are grab bars needed? Is a raised toilet or shower chair needed? Is the home clutter free? And so much more. Step 3: Following the assessment, the findings will be shared with the client and the Council on Aging. Step 4: The Council on Aging chore workers will work with the client to make the necessary home modifications, which fall within the agency’s skill level. The client will be responsible for the cost of any equipment or supplies that may be needed, unless the item is available through the “Loan Closet”. Step 5: The Council on Aging’s outreach specialist will work with the client to see if adaptive equipment is needed through the Council on Aging “Loan Closet” or through a local medical equipment providers. The outreach specialist will also help access other local resources. The Putnam County Council on Aging Starts a Medical Equipment “Loan Closet” Beginning September 1, 2016 Has a hospital stay, recent injury, or just coming of age as a senior presented a need for medical equipment? If so, the Putnam County Council on Aging has a Medical Equipment Loan Closet with an assortment of new and used “durable medical equipment” on hand to lend to those in need for a few days, a few weeks, or a few months at no charge. The only eligibility is that you must be a Putnam County resident, age 60 and better. In order to loan equipment to others, there is a 6 month loan period. If you need an item longer, please contact the Outreach Specialist at 419-523-4121 to request an extension of the loan. Please note, there is an exception for all wheelchair loans. Wheelchair loans have a maximum 6 week loan time. Steps to Borrow Equipment To arrange to borrow an items from the Loan Closet, please call the Council on Aging Outreach Specialist at 419-523-4121. The Outreach Specialist will: 1. Assess your need and see if the needed equipment is available. 2. Fill out proper paperwork which includes a release of liability form signed by the client. 3. Arrange for pick up of the medical equipment. Steps to Return Equipment 1. Please clean equipment prior to returning. 2. Return equipment to the Council on Aging Outreach Specialist at 1425 E. Fourth Street, Ottawa, Ohio. 3. Please include client’s name when returning item so the return can be documented. 4. Donations are graciously accepted to help support the loan closet. Do You Have Items to Donate? The Council on Aging relies on generous donations from our community to keep our loan closet full. Our supply of items constantly changes depending on the needs of older adults as well as donations received. If you have an item to donate, call the Council on Aging and describe your item. We can take most small medical equipment, as long as it is in good repair. All donated items will be inspected to insure they are in good working order before being accepted for the Loan Closet. Examples of Items NOT Accepted: No hospital beds No power wheelchairs/scooters No hoyer lifts No lift chairs No medical supplies such as oxygen tubing, masks, wound care supplies, orthotics, braces, etc. No clothing or bedding No cloth items. Putnam Palooza Silent Auction Fundraiser Benefits the Council on Aging Saturday, July 16, 2016 from 1:00 to 9:00 p.m. Location: 1st Annual Putnam Palooza (formerly the OG Jaycees Rhythm & Rib Festival) in the Pandora-Gilboa Shelter House at the Putnam County Fairgrounds The Council on Aging will be holding our biggest fundraiser of the year, a silent auction fundraiser at the OG Jaycees Putnam Palooza (formerly the Rhythm and Rib Festival), which is scheduled for Saturday, July 16, 2016. Falls among older adults have reached epidemic proportions and rates continue to rise. Statistics show that 1 in 3 Ohio adults, age 65+, fall each year. The psychological consequences of any fall, whether injury causing or not, can be severe, resulting in fear, decreased quality of life, limited activity, social isolation and depression. Falls are not a normal part of aging. There are simple steps older adults can take to reduce their risk for a fall. One of those steps is to attend a Matter of Balance Program (MOB). The MOB program is an award winning, evidenced based fall prevention program that consists of 8, two hour sessions for groups of 5-12 participants. The program helps address the physical, social and cognitive factors affecting the fear of falling and teaches fall prevention strategies to reduce the fear of falling and increase activity levels in order to improve flexibility, balance and strength. Council on Aging Welcomes New Chore Worker, Stan Myers 2016 Outstanding Senior Citizens Recognized The Council on Aging would like to introduce Stan Myers as a new chore worker. Stan resides in Ottawa. He is married to Polly. Stan is a retired teacher. Please help the Council on Aging welcome Mr. Myers! Please visit us at: www.putnamcouncilonaging.org Enrollment in the Senior Dining Program Begins July 1, 2016 Enrollment into the Area Agency on Aging’s Senior Dining Program begins on July 1, 2016 and ends on July 31, 2016. During the month of July, all current participants and anyone else 60 and older that want to apply for the Senior Dining Program MUST fill out a new application. The applications will be processed in August and September of 2016 and a new list of participants will begin in October of 2016. No one is guaranteed to remain in the program from year to year. Therefore, everyone wanting to be considered MUST reapply in July of 2016. All applications must be received by July 31, 2016. Applications can be mailed to the AAA3 at 200 E. High Street, 2nd Floor, Lima, OH 45801 or dropped off at the AAA3 office in Lima. Any applications received after August 1, 2016 will be processed and put on the waiting list. On July 1, 2016 the Senior Dining applications will be available at the Council on Aging office and on the AAA3 website, www.aaa3.org. Current senior dining participants and those on the waiting list will receive an application in the mail from AAA3. On October 1, 2016 applicants will either receive their Senior Dining certificates by mail or they will receive a postcard stating their name has been put on the waiting list. Summer Crisis Program Walk to End Alzheimer’s The HHWP Community Action Commission’s (CAC) HEAP Summer Crisis Program (SCP) will begin on July 1, 2016 and continues through August 31, 2016, or until funding is exhausted. This program can provide a payment of up to $300 towards a regulated or up to $500 towards an unregulated electric bill and/or a window air conditioner and/or boxed fan or tower oscillating electric fan. CAC can assist income qualified customers who meet the following criteria: The End of Alzheimer’s Starts with You Individuals with a documented medical condition that would benefit from continued electric service and/or air conditioning or individuals age 60 and older. The electric bill must be in an adult’s name who is 18 years or older and resides in the home. Households must have an annual income that is at or below 175% of the 2016 federal poverty guidelines. Customers will need to bring the following information to their appointment: Proof of all household income for persons 18 or older for the last 90 days or 12 months. Social security numbers and birth dates for everyone in the household. A current heating or electric utility bill. Physicians documentation if applicable. Proof of age, if 60 or older. Proof of citizenship. Proof of disability, if applicable. Please contact the Community Action Commission at 1-800-423-4304 to schedule your appointment.
